General Pharmacology of DWQ-013, A New Synthetic Quinolone Antibiotics (Effects on the Central Nervous System) (신규 합성 퀴놀론계 항생물질(DWQ-013)의 일반 약리 작용 -중추신경계에 대한 작용-)

  • The general pharmacological effects of DWQ-013, a new synthetic quinolone antibacterial agent, were examined on the central nervous system in experimentral animals and the following results were obtained. Drug interaction of DWQ-013 with theophylline, fenbufen and nonsteroidal antiinflammatory drugs was also examined. DWQ-013 decreased touch escape effect on the general behavior and decreased body temperature at a concentration of 1000 mg/kg in mice. But DWQ 013 had no effect on the locomotor activity, rotarod perfomance and traction test in mice. Furthermore, DWQ-013 increased pentobarbital-induced sleeping time and affected the onset time in acetic acid-induced writhing test in mice. DWQ-013 reduced onset time and death time on strychnine-induced convulsions and death time on pentylenetetrazole-induced convulsions at a concentration of 1000 mg/kg in mice. But, the drug had no effect on the electroshock. DWQ-013 did not interact with fenbufen and any other NSAIDs but it did interact with theophylline. From these results, it could be suggested that DWQ-013 had less pharmacological effect than other quinolones on the central nervous system.

Sleep Deprivation and Fatigue among Nurses Working Consecutive Night Shifts: A Prospective Observational Study

  • Purpose: This study aimed to identify changes in sleep patterns and fatigue levels during consecutive night shifts among shift nurses and to determine the association between sleep parameters and increased fatigue levels during work. Methods: This prospective observational study employing ecological momentary assessments was conducted using data collected from 98 shift nurses working in Korean hospitals between June 2019 and February 2021. The sleep patterns were recorded using actigraphy. The participants reported their fatigue levels at the beginning and end of each night shift in real time via a mobile link. Linear mixed models were used for the analysis. Results: Nurses spent significantly less time in bed and had shorter sleep durations during consecutive night shifts than on off-duty days, whereas their wake times after sleep onset were much longer on off-duty days than on on-duty days. Fatigue levels were higher on the second and third night-shift days than on the first night-shift days. A shorter time spent in bed and asleep was associated with a greater increase in fatigue levels at the end of the shift than at the beginning. Conclusion: Nurses experience significant sleep deprivation during consecutive night shifts compared with off-duty days, and this sleep shortage is associated with a considerable increase in fatigue levels at the end of shifts. Nurse managers and administrators must ensure sufficient intershift recovery time during consecutive night shifts to increase the time spent in bed and sleeping.

Sleep Quality and Sleep Patterns of Patients Treated by the Hemopoietic Stem Cell Transplantation (조혈모세포 이식환자의 입원 후 수면의 질과 수면양상의 변화)

  • Purpose: To provide the basic data in developing the nursing intervention for patients who have symptoms of insomnia after undergoing hemopoietic stem cell transplantation (HSCT). This was achieved through investigating sleep quality and sleep patterns according to admission time, and discharge time. Methods: Data was put together by studying 52 HSCT patients who have been admitted to the department of HSCT from August 2002 to August 2003, in a university hospital. Research instruments used were, PSQI for sleep quality and a specified questionnaire for sleep patterns. Results: The PSQI regarding the past mono which was measured at the last day of hospitalization, was 11.8. This was significantly higher than 5.3, which was a measurement for the past month before the hospitalization (t=11.41, P=0.000). Looking at it with 7 categories-quality of sleep, consistency of sleep, sleep period, effectiveness of sleep, sleeping disorder, usage of sleeping pills, impediment of daily life- the PSQI for a month after admission increased significantly compared to the rate measured for a month before admission. Comparing the subjects sleep pattern before and after admission, it showed a significant difference regarding time attending sleep, time it takes to sleep, wake-up time, total time of sleep, day time sleep, number of times waking up during sleep, number of usage of sleeping pills, actions that are taken during sleep disorder, reasons for insomnia. Conclusion: The PSQI score of patients who undergo hemopoietic stem cell transplantation have increased significantly after hospitalization. Compared to the change of sleep patterns when hospitalized, significant changes were observed. Therefore nursing interventions addressing sleep are needed.

The Clinical study of Su-Gi therapy's Effects on Insomnia by observing of ISI and PSQI (불면환자에 대한 수기치료 효과의 임상 사례 보고)

  • Objectives : The purpose of this study is to investigate the effects of Su-Gi therapy for Insomnia by using ISI scores and PSQI test. Methods : We investigated 4 patients on condition of Insomnia who was hospitalized in the M Korean medicine hospital in Gyeonggi Province from 8th June, 2015 to 21st October, 2015. The Su-Gi therapy was treated on patients once a day for 10 minutes. We figured out the outcome by using ISI scores and PSQI test. Results : There were no differences on average sleeping time and the time to falling a sleep. Meaningful outcomes in values of ISI scores and PSQI test was not deduced after Su-Gi therapy. There were some progress on disturbance of daytime due to insomnia, ISI scores and PSQI test. In the Correlation analysis of outcomes of before and after the Su-Gi therapy, there were some improvements on ISI scores and PSQI tests and some questionnaire but were not enough to reach meaningful outcomes. We concluded that the more treatment time is needed to improve the condition of patients on insomnia as we see in PSQI test, one month is minimum period time to draw the conclusion. Conclusions : Theses outcomes suggest a possibilities that improvements who are suffering from insomnia can be get well when the Su-Gi therapy is performed for sufficient moments.

Investigation of Dietary Behaviors According to Residence Status and Ethnicity of University Students in Yanbian, China (중국 연변지역 대학생들의 거주형태와 민족에 따른 식생활 연구)

  • This study was conducted to examine dietary behaviors according to residence status and ethnicity of university students in Yanbian, China. For the subjects, 334 university students (Male=141, Female=193) answered a questionnaire about perception of weight control, dietary patterns, health habits, residence status, and ethnicity. Perception of weight, meal frequency, favorite kind of food, meal finishing time, skipping breakfast, type of breakfast, snack frequency, late-night snack frequency, exercise frequency, regular life, and sleeping time were all significantly associated with residence status. Motivation of weight control, meal frequency, regularity of meal time, meal volume, favorite kind of food, exercise time, and regular life were all significantly associated with ethnicity. University students who lived with their parents perceived their weight more properly, exhibited a more positive dietary pattern, and lived a more regular life than those students who lived in a dormitory with their friends. Chinese students exhibited a more regular dietary pattern and lived a more regular life than the Korean-Chinese students. As a result, both environmental and inherent factors are related with the dietary behaviors of university students in Yanbian, China. These data could be used to help university students in Yanbian, China attain a healthy diet.

A Study on the Temporomandibular Joint Disorder and School Life Stress of High School Student by Department (계열별 남자고등학생의 학교생활스트레스와 측두하악장애에 관한 연구)

  • The purpose of this study targeted on high school student in the department of liberal arts, industry in Daegu metropolitan city, is to get basic data necessary for the development of dental educational program, to discern prevention and treatment of temporomandibular joint disorder by observing the situation temporomandibular joint disorder and contribution element, of relationship of school life stress The results are as follows.: 1. The percentage of occurring temporomandibular joint disorder in the high school resulted in a joint noise at 61.8% and joint dislocation 6.9%, sharp pain 47.5% at time of chewing. 29.8% at the time of the non-chewing, lockjaw 11.3%, a headache appeared at 40.4%.2. In the contribution factor of occurring temporomandibular joint disorder, the cause of joint noise was the clench one's teeth, lip and cheek clench, For the pain at the time of chewing clench one's teeth, one side chewing, over-chewing, lip clench, sideways sleeping showed the difference. (P < 0.01) For the pain at the time of non-chewing, clench one's teeth, bruxism, one side chewing, lip and cheek clench were similar, and for the lockjaw, clench one's teeth, bruxism, sideways sleeping showed the difference. The plum evil thing period at time of the fault writing that statistically showed the difference. For the headache, the contribution factors were the all bad habits mentioned above excluding one side sleeping.(P < 0.01, P < 0.05). 3. The rate of experiencing temporomandibular joint disorder by oral and maxillofacia was 13.4% in industrial department, and 19.6% in liberal arts. And for the factor of wound was that exercise 26.8%, others 24.4%, fall-down 19.5%. And for the industrial, exercise 44.4%, fall-down 22.2%, others 14.9%. The treatment experience appeared at 5.0% in industrial department, 2.9% in liberal arts. And for the medical institutions, liberal arts were dental clinic 50%, orthopedics 50%, and the industrial department orthopedics 40%, oriental medicine clinic 30%, dental clinic 30%. 4. In case of temporomandibular joint disorder, there were no difference by grades or educational background. And at the time of chewing or non-chewing showed similar difference.(P < 0.01). 5. Compared to stress in the high school, it generally showed higher in liberal arts than in industrial department due to school record. Its scope was $3.75{\pm}1.14$ in liberal arts, $3.01{\pm}1.23$ in industrial department. 6. The school record, school life, stress problems by teachers, chewing/non-chewing pain of temporomandibular joint disorder, joint noise had a similar correlation.(P < 0.01, < 0.05).

Comparison of Health Status, Sleep and Depression by the Employment Status in the Elderly (직업 유무에 따른 노인의 건강상태, 수면 및 우울의 비교연구)

  • The purpose of this study was to compare the health status, sleep and depression and to examine relationships between health status, sleep and depression by employment status in the elderly. This study used the data from the "2008 Korean National Survey on Older Adults" by Ministry of Health, Welfare and Family. Out of the total 15,146 Korean elderly surveyed, 5,204 subjects with job and 9,942 subjects without job at the present were used in a final analysis. There were significant differences in health status, sleep and depression by employment status. As for old adults with job, job satisfaction had the greatest effect on depression, followed by perceived health status, difficulty of daily activities by disease, and deep-sleeping days in the order. These variables accounted for 29% of depression. As for old adults without job, perceived health status had the greatest effect on depression, followed by difficulty of daily activities by disease, deep-sleeping days and total sleeping time in the order. These variables accounted for 32% of depression. Therefore, the health-promotion intervention to improve the health status, sleep and depression of the elderly should be planed and provided considering such differences by employment status.

A Phenomenological Study for the Inquiry into Long Life Factors in Persons of One Hundred and Above on Cheju Island (제주도 100세이상 노인의 장수요인 규명을 위한 현상학적 연구)

  • A long life is the desire of many people. The purpose of this study was to describe long life factors for people aged one hundred and above on Chju island. The research was conducted using a phenomenological method to add understanding to this subject. The field work for this study was done from July, 1997 to February, 1998 on Cheju island. The sample consisted of 10 persons of one hundred or above and 12 families. They were audio-recorded and analyzed using Colaizzi's (1978) method. The results of the present study shows the following: The thoughts of the persons one hundred and above showed ten important concepts; 'dietary regimen', 'mental regimen', 'sleep regimen', 'residence regimen', 'health regimen', 'labor regimen', 'exercise regimen', 'being born', 'filial piety and posterity', 'attachment to life'. Concerning 'Dietary Regimen': Dietary regulation, Preferential food intake, Living on vegetables, Light eating, and concern for longevity in food intake were important. Concerning 'Mental Regimen' : Harmony, Clearheadedness, Mildness, Abstinence, Generosity, Relaxation, Gaiety, Inspiration and Strength were important. Concerning 'Sleep Regimen'; Enough sleeping hours, Sound sleep, Curved sleeping posture, and Comfortable bedding were important. Concerning 'Residence Regimen' ; Clean dwelling, Pleasant surroundings, Thatched house life, Living in the village with good-hearted people, and Regular life were important. Concerning 'Health Regimen'; Temperance of drug, use Proper temperature, Proper humidity, and a Clean body were important. Concerning 'Labor Regimen'; Agricultural labor, Diligence, and Domestic labor were important. Concerning Exercise Regimen': Proper exercise, and Sunbathing were important. Concerning 'Being Born' ; Strong physical constitution, Longevity lineage, and Destiny were important. Concerning 'Filial Piety and Posterity' : Showing respect to a long-lived elder and Reducing anxiety were important. Concerning 'Attachment to Life' ; The desire to live long was important. The following conclusion was made from the above results. Human beings cannot avoid death, but the span of life can be prolonged to the maximum span of human life by wisdom, health care, and proper environmental conditions. As a result, a health regimen must be imposed for longevity such as controling food intake, having positive thoughts, being relaxed, sleeping enough, clean dwelling, taking care of oneself which can improve immunity and resistance to decrease, as well as proper labor and exercise. In addition, when filial piety is served, the desire of longevity and retaining one's youthfulness for a long time can be achieved. These should be done to live long and lead a qualitative life. By the results of this research, the following is suggesed for nursing. To satisfy the basic nursing principle that nursing must be practiced to help people live long, education must be spread to people who are nursed and their family so that they can lead a healthy life. And in nursing elders, nurses must recognize that elders have a strong desire for life, even though they may have negative thoughts about life at times. Therefore nurses have to respect the elders' value of life, and help them improve their self-esteem and self -fulfilment.

Study on the Association of DSOM Pathogenic Factor in the Insomnia Patients (불면환자의 수면의 질 척도와 한방진단시스템 병기(病機)와의 연관성 연구)

  • Objectives: This study was to investigate the relation of Pittsburgh Sleep Quality Index(PSQI) and Diagnos system of oriental medicine(DSOM) in the Insomnia Patients. Methods: For this study, we carried out PSQI and DSOM of 92 patients with insomnia who have come to Kijang public health center and Donguei oriental hospital of Donguei university from November 2008 to October 2009. We verified the association of PSQI and DSOM using crosstabulation analysis, independent sample T-test, one-way ANOVA and correlation analysis. Results: 1. Blood-deficiency(血虛) is the most frequent pathogenic factor and frequency of nightmare has positive correlation with that factor including heart(心) in total insomnia patients. 2. Female patients have a higher percentage of chronic insomnia than male patients. 3. As patients aged, maintaining sleep gets harder, sleep time is lessening, taking hypnotics is increasing and quality of sleep gets worse. 4. There is high frequency of chronic insomnia in 40~50 years and over 60 years old. Also, the people who are over 60 years of age tend to sleepless and urinated frequently during the middle of the night. The people with the age of 20~30 years tend to take less sleeping pills and frequency of sleeping during daytime is higher than the other age groups but the quality of sleep is quite good. 5. The patient with over 6 months of chronic insomnia tend to be older in age than the patient with less than 6 months. And the patient with chronic sleeping disorder has frequent pain and the quality of sleep is very poor. 6. The patient who took hypnotics tend to be older than hypnotics non-user and frequency of urination was relatively high. 7. The patient who visited oriental medical hospital were tend to be older and get difficult in maintaining sleep. And frequency of taking hypnotics and urinating during night were tend to be higher and the quality of sleep was poor compared with the patient who visited public health center. Conclusions: This study provides insights into the complicated relations of the associated symptom of insomnia with Diagnos system of oriental medicine. And especially this study showed apparent correlation between nightmare in insomnia patients and pathogenic factor of blood-deficiency and heart. Specific correlations and characteristics in this study could be connect develop of new questionaire of insomnia.

The Relationship between Sleep Quality and Clinical Features of Adolescents with Temporomandibular Disorder

  • Purpose: Many researchers have established a connection between sleep disturbances and the symptoms of temporomandibular disorder (TMD) in adults, but there are a few studies targeted at adolescents with TMD. The aim of this study was to analyze the sleep pattern of adolescents with TMD and to determine the effect of poor sleep quality on their clinical symptoms. Methods: The subjects were composed of 47 adolescents with TMD. The sleep pattern and preliminary information of patients were measured by self-reported questionnaires; Pittsburgh Sleep Quality Index (PSQI) and Questionnaire for TMD analysis. TMD pain was scored using the Numerical Rating Scale. The Kruskal-Wallis test, Mann-Whitney U test, Fisher's exact test and logistic regression were used for statistical analysis. Results: The poor sleeping group had statistically more females (25.00% males, 75.00% females) than the good sleeping group. As compared with that of good sleepers, sleep quality of poor sleepers was significantly worse in the items of subjective sleep quality, sleep latency, sleep duration, sleep disturbances, daytime dysfunction and global PSQI score. The sleep time of adolescents was much longer during vacation ($7.20{\pm}1.38hours$) than during school days ($6.10{\pm}1.26hours$). Poor sleep of patients was associated with the pain in the ear or in front of the ear, in the face, jaw, throat or temple and in neck or back. Adolescents with pain in those areas had significantly higher proportion of poor sleepers than adolescents with no pain in those areas. Conclusions: The high ratio of girls in poor sleeping group could reflect the greater prevalence of TMD in women than in men. Pain in peri-temporomandibular joint areas, neck or back could negatively influence sleep quality of adolescents with TMD.
