TinyOS 기반의 센서 노드 제어 알고리즘 (Sensor Node Control Algorithm Based on TinyOS)

  • 최근에 대표적인 센서 노드 운영체계인 TinyOS를 이용하여 센서 네트워크를 개발하여 다양한 유비쿼터스 응용 서비스를 개발하고 있다. 이들 TinyOS 기반의 센서 네트워크에서는 상황 정보를 획득하기 위해 센서로부터 센싱된 정보의 전달과 수집을 집중적으로 수행한다. 이에 본 논문에서는 센서 노드의 전력 상태를 파악하여 이를 토대로 센서 노드를 수면, 활동, power off 모드로 전환하는 센서 노드 제어 알고리즘을 제시한다. 그리고 이 알고리즘을 토대로 센서 네트워크의 센서 노드, 싱크, 서버에서 센서 제어 모듈을 설계하고 구현한다. 이를 위하여 센서 노드의 센서 전력제어 모듈과, USN 서버의 센싱 데이터 수신 및 도시 모듈과 센서 제어 모듈을 설계하고 TinyOS와 자바 언어를 이용하여 구현한다. 이를 통하여 센서 노드의 전력 상태를 확인하여 데이터 수집이 어려울 경우 수면이나 power off 모드로 전환하여 전력 손실을 방지하고, 주변 환경이 정상적일 경우 활동 모드로 변경함으로써 효과적으로 센서 노드의 전력을 제어할 수 있을 것으로 사료된다.

규칙적인 신체활동과 운동중재가 노인의 우울증에 미치는 영향분석 (Combined Impacts of Physical Activity and Exercise on Depression in Elderly People)

  • 본 연구는 노인의 우울증 예방 및 치료를 위해 신체활동과 운동이 어떠한 효과가 있는지를 확인하는 것이다. 전 세계적으로 우울증은 지속적으로 증가하는 추세이며 특히 가속화되어 가는 고령화 사회에서 노인의 우울증은 더욱 심각한 문제라고 볼 수 있다. 노인의 우울증은 젊은 층에 비해 예후가 좋지 않으며 노화로 인한 신체적 질병의 증가와 신체적 장애에 의한 자존감 저하로 더욱 취약하다고 할 수 있다. 그러나 노인들의 우울증 관리, 노쇠로 인한 우울증 등의 문제에도 불구하고 지금까지 예방 및 치료에 관한 연구는 미미하다고 판단된다. 따라서 본 연구는 중요한 매개 변인인 수면장애와 우울증, 우울증에 대한 운동과 해마의 연관성, 우울증 관리를 위한 신체활동 및 운동의 과학적 효과를 분석하였다. 추후 노인의 우울증 개선과 자살예방에 관한 다양한 연구들이 수행되어져야 할 것으로 여겨진다.

치과보철치료를 요하는 환자에서의 수면질과 스트레스 반응 평가 (Evaluation of sleep quality and stress response in patients requiring dental prosthetic treatment)

  • 목적: 치아를 상실하여 치료를 받기 위해 내원하는 보철과 환자를 대상으로 처음 내원할 당시 스트레스에 대한 반응과 수면의 상태가 어떤지 살펴보고자 하였다. 대상 및 방법: 2018년부터 2년간 부산대학교병원 보철과에 신환으로 접수한 236명의 성인 환자(남자 94명, 여자 142명, 24 - 86세)를 대상으로 검사 및 상담, 설문조사를 시행하였다. 잔존치 수, 무치악 여부, 저작 가능 여부, 향후 보철치료 방향을 기록하였으며, 시각화 아날로그형 척도(Visual analogue scale, VAS), 스트레스 반응 척도(Stress reponse inventory, SRI) 및 불면증 심각도 지수(Insomnia severity index, ISI)를 도구로 사용하여 환자의 주관적 구내불편감, 스트레스 반응, 수면 상태를 측정하였다. 결과: 전체 보철과 신환에서 SRI 의 7개 항목 중 '피로'가 유의하게 높은 평균값을 나타냈고 '공격성'이 유의하게 낮은 값을 보였다 (P < .001). 무치악자와 완전유치악자는 VAS로 측정한 구내 불편감 값에 유의한 차이가 있었다 (P = .004). 20개 미만의 치아가 잔존한 경우, 스스로 저작이 불가능한 경우, 가철성 보철물을 치료계획을 잡게 되는 환자의 경우는 그 반대의 경우에 비하여 VAS 값, SRI 중 '우울'과 '피로' 값이 유의하게 높게 드러났다 (P < .05). VAS 수치가 심각에 해당하는 환자의 경우 심각에 해당하지 않는 환자에 비하여 SRI 항목 중 '긴장', '신체화', '우울', '피로', '좌절'의 값이 유의하게 높았다 (P < .05). 임상적 불면증은 전체 환자 중 11.4%에서 나타났고, 여자가 남자보다 수면의 질이 유의하게 낮았다 (P = .044). 불면증을 가진 사람은 정상수면자에 비해 VAS값과 7가지 모든 SRI 항목에 대해 유의하게 높은 점수가 나타났다 (P < .05). 결론: 보철 신환에서 스트레스에 대한 반응 검사(SRI)를 시행하였을 때 7가지 스트레스 반응 항목 중 '피로' 항목이 높게 나타났으며, 잔존치 수와 저작 기능으로 나타나는 구강 상태, 불면증 유무는 주관적 구내불편감, 스트레스 반응 항목 및 정도에 영향을 주었다.

한의원에 내원한 탈모증 환자 80명의 임상적 고찰 (A Clinical study on 80 Cases of Alopecia Patients in the Oriental Medicine Clinic)

  • Objectives : This study was planned to evaluate clinical status of the alopecia patients who had visited oriental medicine clinic. Methods : 80 patients with alopecia, who had visited oriental medicine clinic from January 2004 to August 2004, were examined. Results & Conclusions : 1. The kinds and incidences of Alopecia : androgenetic alopecia 57.5%(46/80), alopecia areatea 20.0%(16/80), telogen effluvium 16.3%(13/80), seborrheic alopecia 6.3%(5/80). 2. In distribution of sex, the rate of male to female was 1:1 and most patients belonged to 20-30 year old group. 3. The most common onset of age was adolescence(63.8%) and the most common duration of hair loss was 1 to 3 years. 4. Among the 30 male androgenetic alopecia patients, Hamilton's type 4 was most common and among the 16 female androgenetic alopecia patients, Ludwig's type II and III were most common. 5. Family history of baldness in Androgenetic alopecia and alopecia areata were 56.5% and 25.0%, respectively. 6. Self-conscious causes of hair loss : stress(48.8%), irregular eating habits(21.3%), and lack of sleep(20.0%). 7. Associated diseases with alopecia patients : chronic neck pain(58.8%), temporomandibular disorders(55.0%) and seborrheic dermatitis(20.0%). 8. Associated symptoms with alopecia Patients : stress(78.8%), sleep disorders(68.8%), irregular eating habits(55.0%), Hot flush(43.8%), Stool disorders(43.8%), cold hands and feet(37.5%) and menstruation disorders(31.3%). 9. The most common scalp type was oily scalp(70.0%) and the symptoms of scalp were iching, dandruff, pain, inflammation. 10. $Bi{\grave{a}}n$ $zh{\grave{e}}ng$(辨證) of alopecia patients : $xu{\grave{e}}-r{\grave{e}}-f{\bar{e}}ng-z{\grave{a}}u$(血熱風燥) (25.0%), $g{\bar{a}}n-sh{\grave{e}}n-b{\grave{u}}-z{\acute{u}}$(肝腎不足) (23.8%), $shi-r{\grave{e}}-sh{\grave{a}}ng-zh{\bar{e}}ng$(濕熱上蒸) (16.3%), $xu{\grave{e}}-r{\grave{e}}-sh{\bar{e}}ng-f{\bar{e}}ng$(血熱生風) (13.8%), $xu{\grave{e}}-x{\bar{u}}-f{\bar{e}}ng-z{\grave{a}}u$(血熱風燥) (13.8%), $y{\bar{u}}-xu{\grave{e}}-z{\check{u}}-lu{\grave{o}}$(瘀血阻絡) (7.5%).

혈액투석 환자들이 경험한 건강문제 분석 (An Analysis of Health Problems Experienced by the Clients Receiving Hemodialysis)

  • The purpose of this study was to discover practical health problems which hemodialysis clients experienced so as to present basic data for development of a health assessment tool. The research subjects were 70 clients receiving hemodialysis in Seoul and Inchon from Mar. 1996 to Sep. 1996. Data were collected by researcher's informal indepth interview and nurses' open ended question. Content analysis was applied to collect similar contents and common experiences in order to derivate concepts and categories for better understanding of hemodialysis clients' experiences. As a result, 9 categories derivated to identify the health problems of clients receiving hemodialysis were as follows : 1) They experienced 'the decreased digestive function' which contained the changed appetite, nausea, vomiting, constipation and diarrhea. 2) They experienced 'the decreased respiratory and circulatory function' which contained dyspnea, changed blood pressure, tingling sensation and the fear of aggrevated vascular condition. 3) They experienced 'the aggrevated oral condition' which contained dry mouth and destruction of teeth and their soft tissue. 4) They experienced 'the decreased sensory function' which contained visual disturbances, sensation difficulty, and hearing loss. 5) They experienced 'the aggrevated skin condition' which contained dark brown skin color, dry skin (and hyperpigmentatic freckle, seborrheric keratosis, scale), itching sense, and alopethia. 6) They experienced 'the decreased urinary reproductive function' which contained anuria or oliguria, dysmenorrhea, sterility and decreased libido. 7) They experienced 'the restricted activity' which contained decreased activity, muscle cramp and stiffness of joint. 8) They experienced 'the changed mental status' which contained memory disturbance, decreased cognition, disorientation, neurosis and psychosis. 9) They experienced 'the aggrevated general condition' which contained kyphosis, weight loss, fatigue, sleep disturbance, bleeding tendency, inflammation, generalized edema and foul oder of uremia.

Honokiol Potentiates Pentobarbital-Induced Sleeping Behaviors through GABAA Receptor Cl- Channel Activation

  • This study was undertaken to investigate whether honokiol could enhance the pentobarbitalinduced sleeping behaviors through $\gamma$-aminobutyric acid (GABA) receptor $Cl^-$ channel activation. Thirty minutes after the oral administration of honokiol, mice were received sodium pentobarbital (42 mg/kg, i.p.). The time elapsed from pentobarbital injection to the loss of the righting reflex was taken as sleeping latency. The time elapsed between the loss and voluntary recovery of the righting reflex was considered as the total sleeping time. Western blot technique and $Cl^-$ sensitive fluorescence probe were used to detect the expression of $GABA_A$ receptor subunits and $Cl^-$ influx in the primary cultured cerebellar granule cells. Honokiol (0.1 and 0.2 mg/kg) prolonged the sleeping time induced by pentobarbital (42 mg/kg) in a dosage-dependent manner. Honokiol (20 and 50 ${\mu}M$) increased $Cl^-$ influx in primary cultured cerebellar granule cells, and selectively increased the $GABA_A$ receptor $\alpha$-subunit expression, but had no effect on the abundance of $\beta$ or $\gamma$-subunits. Chronic treatment with 20 ${\mu}M$ honokiol in primary cultured cerebellar neurons did not affect the abundance of GAD65/67. The results suggested that honokiol could potentiate pentobarbital-induced sleeping through $GABA_A$ receptor $Cl^-$ channel activation.

악액질(cachexia) 환자에게 적용한 운동치료가 피로와 불안에 미치는 영향(단일사례 보고) (Effects of Therapeutic Exercise on Fatigue and Anxiety of Cachexia Patients(Case Report))

  • Purpose : Cachexia, is a complex metabolic syndrome associated with underlying illness and characterized by loss of muscle with or without loss of fat mass. Patients with cachxia shows various symptoms including fatigue, anxiety, pain, sleep disorders, and poor appetite. Medications therapy, dietary and exercise therapy, and emotional support are recommended to treat patients with cachexia. However, evidence-based research verifying the role of exercise therapy in patients with cachexia is limited. The purpose of this study was to investigate the effects of therapeutic exercise on fatigue and anxiety in patients with cachexia. Methods : Case report. A 29-year-old woman was diagnosed with cachexia. Following 2 weeks of inpatient and 4 weeks of out-patient treatment. we assessed her weight, as well as pain, fatigue, and anxiety level. As an the patient exercised for an hour 5 times a week for 2 weeks,-and during the outpatient visit, she exercised for an hour twice a week for 4 weeks. Her weight was measured using a weighting scale. Pain was assessed using the visual analog scale, and fatigue and anxiety levels were assessed using questionnaires. Results : Following 6 weeks of treatment, exercise therapy a positively affected the patient's weight, as well as pain, fatigue, and anxiety levels. We observed a weight gain of 4.5 kg, pain reduction of 5.1 points. Fatigue reduction of 43 points, anxiety reduction of total 41 points. Conclusion : Exercise therapy positively affects weight, as well as pain, fatigue, and anxiety levels in patients with cachexia. However, generalization of this observation is inappropriate based on this single case study.

Congenital LQT Syndromes: From Gene to Torsade de Pointes

  • Congenital Long QT syndrome (LQTs) is a relatively rare pathologic disorder but results frequently in sudden cardiac death. Of the six LQTs that have been clinically described, five have been worked out for their genetic and biophysical profile. Most are generated by mutations which cause a loss of function in two delayed $K^+$ currents, $i_{Ks}\;and\;i_{Kr}.$ One syndrome is generated by mutations in the $Na^+$ channel which causes essentially a gain of function in the channel. Clinically the syndromes are characterized by slowed repolarization of the cardiac ventricular action potential and the occurrence of typical arrhythmias with undulating peaks in the electrocardiogram, called Torsade de Pointes. Arrhythmias are initiated by early or delayed afterdepolarizations and continue as reentry. Triggers for cardiac events are exercise (swimming; LQT1), emotion (arousal; LQT2) and rest/sleep (LQT3). ${\beta}-blockers$ have a high efficacy in the treatment of LQT1 and LQT2. In LQT3 their use is questionable. The study of congenital LQTsyndromes is a remarkable example of how basic and clinical science converge and take profit of each other's contribution.

복강경 담낭절제술 후 발생한 난치성 딸꾹질 환자 증례 (A Case Report of Intractable Hiccups after Laparoscopic Cholecystectomy)

  • Hiccups are defined as abrupt involuntary contractions of diaphragm and intercostal muscles with sudden closure of the glottis. Hiccups are one of the common transient symptoms but when hiccups last more than 48 hours, or have no response to treatment, they are defined as intractable hiccups. Intractable hiccups can induce multiple problems such as weight loss, dehydration and sleep disturbance. We report a 76-year-old male patient who suffered from intractable hiccups for 6 days after laparoscopic cholecystectomy. Hiccupping was so severe as to continue all day long and limit sleeping and eating. We succeeded in cure intractable hiccups with acupuncture and herbal formula; Gyulyeohwadam-tang.

  • Persistent and intractable hiccups (with respective durations of more than 48 hours and 1 month) can result in depression, fatigue, impaired sleep, dehydration, weight loss, malnutrition, and aspiration syndromes. The conventional treatments for hiccups are either non-pharmacological, pharmacological or a nerve block treatment. Pulsed radiofrequency lesioning (PRFL) has been proposed for the modulation of the excited nervous system pathway of pain as a safe and nondestructive treatment method. As placement of the electrode in close proximity to the targeted nerve is very important for the success of PRFL, ultrasound appears to be well suited for this technique. A 74-year-old man suffering from intractable hiccups that had developed after a coronary artery bypass graft and had continued for 7 years was referred to our pain clinic. He had not been treated with conventional methods or medications. We performed PRFL of the phrenic nerve guided by ultrasound and the hiccups disappeared.
