Objective: A case report on the improvement of sleep disorders using Ortho-Cellular Nutrition Therapy (OCNT) Methods: The study subject is a Korean woman in her 40s who is very sensitive to caffeine and suffers from sleep disorders. Results: She no longer complained of discomfort due to sleep disorders after undergoing OCNT. Conclusion: OCNT can be helpful in the treatment of patients with sleep disorders.
Sleep disorders such as insomnia, obstructive sleep apnea (OSA), and restless legs syndrome (RLS) are very common disorders and may cause significant burden in terms of individual as well as societal aspects. Sleep insufficiency from such sleep disorders may cause deleterious effects on daily work life and may be associated with other major medical or psychiatric disorders including cardiovascular disease, diabetes mellitus, depression, and anxiety disorder. Various motor or occupational accident may result from the sleep problems. In addition, recent researches provide the method to evaluate the lost productivity time in terms of absenteeism and presenteeism. Moreover, several studies on cost-effectiveness of treatment of sleep disorders show that it is cost-effective.
It is very difficult to evaluate sleep disorders by simple history taking, because which covers very comprehensive areas such as psychobiosocial fields. Although polysomnography is used for the method of final diagnosis, systemic history taking and sleep question-aires are still critically important especially in evaluation of insomnia. Proper informations through sleep questionnaires can provide very precise data for effective treatment as well as exact diagnosis. Sleep questionnaires consist of largely four kinds of questionnaires, which are screening questionnaire of sleep disorders, sleep diary and questionnaire of sleep hygine, diagnostic questionnaire for specific sleep disorder and questionnaire of special symptoms of sleep disorders including insomnia, daytime sleepiness, cognitive function, mental symptom and personality, parasomnia, physical illness and sexual function. However, for more conclusive diagnosis especially in excessive daytime sleepiness nocturnal polysomnography and multiple sleep latency test should be performed.
Sleep disorders, increasingly prevalent in the general population, induce impairment in daytime functioning and other clinical problems. As changes in cortical excitability have been reported as potential pathophysiological mechanisms underlying sleep disorders, multiple studies have explored clinical effects of modulating cortical excitability through non-invasive brain stimulation in treating sleep disorders. In this study, we critically reviewed clinical studies using non-invasive brain stimulation, particularly transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), for treatment of sleep disorders. Previous studies have reported inconsistent therapeutic effects of TMS and tDCS for various kinds of sleep disorders. Specifically, low-frequency repetitive TMS (rTMS) and cathodal tDCS, both of which exert an inhibitory effect on cortical excitability, have shown inconsistent therapeutic effects for insomnia. On the other hand, high-frequency rTMS and anodal tDCS, both of which facilitate cortical excitability, have improved the symptoms of hypersomnia. In studies of restless legs syndrome, high-frequency rTMS and anodal tDCS induced inconsistent therapeutic effects. Single TMS and rTMS have shown differential therapeutic effects for obstructive sleep apnea. These inconsistent findings indicate that the distinctive characteristics of each non-invasive brain stimulation method and specific pathophysiological mechanisms underlying particular sleep disorders should be considered in an integrated manner for treatment of various sleep disorders. Future studies are needed to provide optimized TMS and tDCS protocols for each sleep disorder, considering distinctive effects of non-invasive brain stimulation and pathophysiology of each sleep disorder.
Bipolar disorders are a group of mood disorders characterised by relapsing mood episodes throughout the course of illness. Patients with bipolar disorders commonly present with various sleep problems. Patients in a manic episode generally show decreased need of sleep and those in a depressed episode frequently complain about hypersomnia. Current literature even points to evidence that patients with bipolar disorder in euthymic state may still show signs of sleep disturbances when compared to the general population. Clinicians may also note intricate interactions between changes of circadian rhythm and evolution of mood episodes in patients with bipolar disorder. Also, commonly prescribed medications which plays a crucial role in treatment of bipolar disorders including mood stabilisers and antipsychotic medications often cause significant weight gain over time. Being a risk factor of sleep apnoea, weight gain can predispose the patient to develop sleep apnoea. In this narrative review, we summarised current evidence and literature regarding characteristics of circadian rhythm and comorbid sleep apnoea in patients with bipolar disorder. We also present literature regarding implications of circadian disturbance and comorbid sleep apnoea in managing patients with bipolar disorder.
Purpose : This study aims to systematically review the methods of occupational therapy intervention in children with autism spectrum disorders having sleep disorders and evaluate the improvements with intervention effects, based on experimental studies of children with sleep disorders. Methods : Studies published overseas from January 2011 to June 2021 were searched from a total of two databases. Science Direct and OTseeker. The five selected studies were analyzed by dividing them into age, number, intervention period, intervention type, intervention effect, evaluation tool, research design type, and evidence level. Results : The total number of subjects was 182, 95 subjects in ther experimental group, and 87 in the control group. The interventions included weighted blankets, swimming, and sleep education for parents. Ther interventions were found to increase total sleep time, improve mood when waking up, reduce sleep anxiety, reduce sleep time, reduce the number of waking up of during sleep, and reduce sleep resistance behavior. Conclusion : Many people have sleep disorders, with or without disabilities, and the number is gradually increasing. Consequently, research on occupational therapy intervention in children with autism spectrum disorder are actively conducted in foreign countries, and these interventions have a positive effect. Based on the results of this study, it can be concluded that such occupational therapy intervention studies are necessary for children with autism spectrum disorder with sleep disorders in Korea. In addition, further research on the quality of life of parents of children with autism spectrum disorders due to sleep disorders and their methods are required.
Periodic limb movements during sleep (PLMS) are prevalent in various sleep disorders, such as restless legs syndrome (RLS), periodic limb movements disorder, obstructive sleep apnea (OSA), REM sleep behavior disorder (RBD), and narcolepsy. PLMS has been hypothesized to be related to the decline of dopaminergic transmission. In RLS, PLMS is suggested to be related to iron deficiency and symptom severity. PLMD is a rare sleep disorder, and the role of PLMS in PLMD has not been clearly investigated yet. PLMS in OSA, which remain after proper PAP therapy, may need further management. The clinical relevance of PLMS in RBD and narcolepsy have not been investigated thoroughly and need further studies. Whether PLMS are to be considered as a mere symptom of individual sleep disorders or not can be elucidated through studies investigating the efficacy of therapeutic approaches to reduce PLMS in various sleep disorders.
Purpose: The aim of this study was to develop and test an explanatory model for sleep disorders in people with cancer. A hypothetical model was constructed on the basis of a review of previous studies, literature, and sleep models, and 10 latent variables were used to construct a hypothetical model. Methods: Data were collected from April 19 to June 25, 2010, using self-report questionnaires. The sample was 291 outpatients with cancer who visited the oncology cancer center at a university hospital. Collected data were analyzed using SPSS Win 15.0 program for descriptive statistics and correlation analysis and AMOS 7.0 program for covariance structural analysis. Results: It appeared that overall fit index was good as ${\chi}^2/df=1.162$, GFI=.969, AGFI=.944, SRMR=.052, NFI=.881, NNFI=.969, CFI=.980, RMSEA=.024, CN=337 in the modified model. The explanatory power of this model for sleep disorders in people with cancer was 62%. Further, sleep disorders were influenced directly by cancer symptom experience, dysfunctional beliefs and attitudes about sleep, and past sleep pattern. Conclusion: Findings suggest that nurses should assess past sleep pattern and consider the development of a comprehensive nursing intervention program to minimize the cancer symptom experience, dysfunctional beliefs and attitudes about sleep, and thus, reduce sleep disorders in people with cancer.

Sleep is an essential physiological process, especially for proper brain function through the formation of new pathways and processing information and cognition. Therefore, when sleep is insufficient, this can result in pathophysiologic conditions. Sleep deficiency is a risk factor for various conditions, including dementia, diabetes, and obesity. Recent studies have shown that there are differences in the prevalence of sleep disorders between genders. Insomnia, the most common type of sleep disorder, has been reported to have a higher incidence in females than in males. However, sex/gender differences in other sleep disorder subtypes are not thoroughly understood. Currently, increasing evidence suggests that gender issues should be considered important when prescribing medicine. Therefore, an investigation of the gender-dependent differences in sleep disorders is required. In this review, we first describe sex/gender differences not only in the prevalence of sleep disorders by category but in the efficacy of sleep medications. In addition, we summarize sex/gender differences in the impact of sleep disorders on incident dementia. This may help understand gender-dependent pathogenesis of sleep disorders and develop therapeutic strategies in men and women.
Purpose: The purpose of this study was to identify the effects of aromatherapy on sleep disorders, satisfaction of sleep and fatigue in hemodialysis patients. Methods: The study design was a quasi-experimental study with nonequivalent control group pretest-posttest design. The subjects were assigned an experimental group (N=17) and a control group (N=17). The experimental group had inhaled lavender and sweet orange for 2 minutes three times a day for 7 days. Sleep disorders were measured using Sleep Measuring Instrument and satisfaction of sleep was gauged using Visual Analogue Scale. Fatigue was checked using Fatigue Measuring instrument. Collected data were analyzed by ${\chi}^2$-test, t-test, Fisher's exact test, and paired t-test using SPSS/WIN 17.0 program. Results: There are significant differences in the sleep disorders, satisfactions of sleep and fatigue between experimental group and control group. Conclusion: Aromatherapy reduced the sleep disorders and the fatigue and increased satisfactions of sleep in hemodialysis patients. Based on the results of this study, aromatherapy can be used at the any setting as a safe and effective independent nursing intervention to sleep disorders and fatigue in hemodialysis patients.
