Subcutaneous Forehead Lift (피부밑이마당김술)

  • Purpose: The purpose is to present an useful and simple surgical method to improve the aging of upper third face in patients with high frontal hairline as well as low frontal hairline. Methods: Forty eight female patients were treated with subcutaneous forehead lift using an anterior hairline incision over 14 years. This surgical technique is performed under direct vision utilizing a beveled incision made 4 to 5 mm into the anterior hairline with subcutaneous dissection, which is continued near to eyebrow, sometimes extended to supraorbital rim to remove corrugator and procerus muscles. In patients with high frontal hairline, excess forehead skin anterior to incision line is removed. On the contrary in the patients with low frontal hairline, scalp posterior to incision line is removed. Results: This technique provided constant and good results with the forty six patients, who were satisfied with eyebrow elevation and removal of wrinkles in forehead and glabellar region. However two patients were undercorrected, and focal alopecia developed in another two patients. One patient complained of pruritus over one year, but subsided spontaneously without any treatment. Temporary paresthesia developed in the forehead and frontal scalp of all cases after operation but permanent sensory loss never occurred in all the patients. Conclusion: Subcutaneous forehead lift using an anterior hairline incision is suggested to be one of the effective surgical methods to improve the aging of upper third face in the patients with high frontal hairline as well as low frontal hairline.

Anti-inflammatory Effects of Extracts of Duchesnea chrysantha in Human Monocytic THP-1 Cells and Human Eosinophilic EoL-1 Cells

  • Atopic dermatitis is a recurrent or chronic eczematous skin disease with severe pruritus and has annually increased in Korea. In this study, we investigated whether Duchesnea chrysantha (Dc) extracts have an anti-inflammatory effect in human monocytic THP-1 cells and human eosinophilic EoL-1 cells. The dried and powdered whole plants of Dc were extracted with 80% EtOH (Dc-1). The residue was diluted with water, and then successively partitioned with n-hexane, EtOAc, and BuOH to produce the n-hexane (Dc-2), EtOAc (Dc-3), BuOH (Dc-4), and the water-soluble fractions (Dc-5), respectively. The mite extract and LPS increased the production of IL-6, IL-8 and MCP-1 in THP-1 cells and the increase was strongly suppressed by Dc-3 extract, as compare with other extracts. Dc-3 also inhibited the release of IL-6 increased by mite extract and LPS in EoL-1 cells. However, Dc-3 extract increased IL-8 production induced by the mite extract and LPS in EoL-1 cells. These results suggest that Dc extract may be used as anti-inflammatory agents in treating allergic disorders such as asthma and atopic dermatitis.

Clinical Study of Atopic Dermatitis ; the Classification of Oriental Medical Clinical type and Treatment (아토피 피부염 환자에 대한 임상적 연구; 한의학적인 임상유형분류 및 치료)

  • Objectives : Atopic dermatitis(AD) assume an remarkable clinical aspect and it s diagnosis almost depends on clinical symptoms. Therefore, we aimed to study the clinical diagnostic standard of AD for more accurate treatment. We repert as follows; Methods : For 6 months from March to August in 2000 we selected fifty outpatients who were prognosis of AD in the department of dermatology, Oriental medical hospitol, Dong-eui University. Results and Conclusions : 1. We classified of the grade, the condition of AD patient was slight and severe, by the sum of total by the clinical index of AD (diagnostic features). 2. By consulting previous oriental medical theories, we divided symptom-complex of AD into two type ; one was damp-heat type and the other was deficiency of blood- wind-dryness type. 3. Male to female ratio was 17 : 33 and the third stage, more than half of the patients were adolescents. 4. According to the results of symptom-complex of AD patients, on the first examination damp-heat type was more than deficiency of blood-wind-dryness type and in progressing treatment, the condition has been change to deficiency of blood-wind-dryness type. 5. In the lesions of AD, arm and knee were most serious skin lesions and in symptoms of AD, pruritus was most complained, and in progressing treatment, erosion and erythema were greatly improved. 6. When we measured the levels of serum Total IgE, that of thirty eight patientswere higher than that of normal, but elevation of serum IgE levels was not correlated with the severity of AD.

Sorafenib-triggered radiation recall dermatitis with a disseminated exanthematous reaction

  • Sorafenib is a multi-targeted kinase inhibitor, which is the current standard treatment for advanced hepatocellular carcinoma (HCC). Only one case of radiation recall dermatitis (RRD) associated with sorafenib has been reported so far. Our patient with recurrent HCC was treated with palliative radiotherapy (RT) for the chest wall mass. Sorafenib at 400 mg twice daily was begun on the day following RT. On the 14th day post-RT, an erythematous patch was observed on right chest wall which matched area previously irradiated. It was consistent with RRD. Ten days later, a disseminated exanthematous rash and severe pruritus occurred. Sorafenib was stopped and an oral antihistamine was prescribed to relieve symptoms. At the 1-week follow-up after the cessation of sorafenib, all symptoms were resolved. Physicians should be alert to this recall phenomenon as it can occur both in the skin and elsewhere and the occurrence of RRD may be unpredictable.

Adult-onset Demodicosis Caused by Demodex canis and Demodex cornei in a Yorkshire Terrier Dog

  • An 11-year-old female Yorkshire terrier presented with alopecia, hyperpigmentation, and pruritus. Dermatological examination revealed a large number of Demodex mites including D. cornei and D. canis. Mean length of total body length of D. cornei was one third shorter than that of D. canis. In addition, sequential analysis of DNA extracted from the skin lesions showed a 97% and 99% identity with the two sequences of D. canis and D. cornei. This is the first report of a combined infestation of D. cornei and D. canis in Korea, along with its morphological and molecular characterization.

  • Objectives This study investigated the effect of camping style Hospitalization program for atopic dermatitis female adolescent patients. 6 patients, 12-14 year old female adolescent, admitted for camping style hospitalization program.Methods 6 patients were admitted for camping style hospitalization program (2014.08.13~2014.08.16). The program included acupuncture, herbal medicine, examination, education, psychotherapy, dressing, exercise. The patients were analyzed by Severity Scoring Atopic Dermatitis(SCORAD) index and measured skin pH value(pH). Also, patients evaluated themselves by writing an atopic dermatitis diary which consists of diet, emotion, severity of pruritus, and sleep loss, and checked questionnaires which consist of atopic dermatitis, self treatment methods and degree of satisfaction of each program.Results Most of subjective symptoms which patients complained were improved and also SCORAD index total scores were decreased by sasang constitutional medication.Conclusions We expect that the camping style hospitalization program based on sasang constitutional medicine could be helpful for female adolescent patient of atopic dermatitis.

A Case of Alternative Therapy for obese patient (대체의학(代體醫學)을 이용(利用)한 비만치료(肥滿治療)의 실제(實際))

  • This case study was done on a 37 year old female patient at the department for rehabilitation in Bundang Cha Oriental hospital who was treated for 70 days by traditional Acupuncture and Herbal medicine therapy as well as fasting with saengshig(生食), air bath(風浴) and cold-hot-bath(冷溫浴) for skin revitalisation, furthermore by Doinangyo and exercise for improvement of general condition and colon-hydrocleansing. The results are as following: 1. The body weigth was reduced by 14.5kg from 87.4kg to 72.9kg 2. The obesity degree reduced from 159% to 133% 3. The BMI reduced from 33.3 to 27.8 4. The WHR reduced from 0.96 to 0.88 5. The amount of body fat reduced by 7.7kg from 27.2kg to 19.5kg 6. The fat distribution of the abdomen reduced from 0.92 to 0.85 7. The percent body fat was reduced from 31.1% to 26.7 % 8. Except of a mild systemic pruritus the clinical symtoms like kneepain, fatigue, chilling, headache, indigestion, menorrhalaia improved significantly.

A pilot test on the effects of improvement on Tinea pedis by using functional soap Gaia 1400, containing medicinal herbs (한약 성분 함유 기능성 비누 가이아 1400의 족부 백선 개선에 대한 임상 연구)

  • Objectives : This clinical research was conducted to test Patients with Tinea Pedis by using functional soap Gaia 1400, containing medicinal herbs Methods : Total 24 patients who visited Oriental Medical Ophthalmology & Otolaryngology & Dermatology of Dongguk Ilsan Hospital from June 17th, 2008 to November 11th, 2008. They used functional soap daily during 8 weeks. We Observed change of erythema, scaling, vesicle, pustule, exudate, crust and pruritus after 4weeks and 8 weeks. Also Clinical evaluation I, II were used to evaluate the effects of Gaia 1400. Results : 1. There were no significant differences on sex, age and onset. 2. Statistically. After treatment(4,8weekes) Tinea Pedis symptoms score showed significant effect on Tinea Pedis. And the score of using Gala 1400 after 8 weeks is lower than 4 weeks. 3. After 8weeks of treatment, Gaia 1400 showed meaningful effect on Clinical global evaluation I, II. 4. Tinea Pedis Exacerbated in a case after using Gaia 1400 and in the 4 cases Skin dryness Incerased. Conclusion : Considering the above results, we can speculate that functional soap Gaia 1400, containing medicinal herb has some therapeutical effects in mitigating the symptoms of Tenia Pedis.

Seven Cases of the Seborrheic Dermatitis Patients Treated with Sihochunggan-tang gagambang and Gagamchengyoung-tang (시호청간탕가감방(柴胡淸肝湯加減方)과 가감청영탕(加減淸營湯)을 병행하여 치료한 지루성피부염 환자 7례 증례 보고)

  • Objectives : The purpose of this study is to report the effectiveness of Sihochunggan-tang gagambang and Gagamchengyoung-tang in the seven patients with seborrheic dermatitis. Methods : This case study was conducted for seven seborrheic dermatitis patients, who have visited Korean Medicine Clinic from 2013.10.01 to 2017.09.30. All seven patients were treated with Sihochunggan-tang gagambang and Gagamchengyoung-tang and acupucture. During taking medicine, we let them avoid bad food like fatty food and alcohol. We evaluated the symptom change through photographs and used Visual Analogue Scale(VAS) as the assessment method. Results : VAS such as skin severity of symptoms, sleep disorder, pruritus was decreased as a result of Sihochunggan-tang gagambang and Gagamchengyoung-tang and acupucture treatment in seven seborrheic dermatitis patients. Conclusions : After the treatment, the grade of VAS was decreased and most symptoms of seven patients were improved. As a result of examining VAS scores and photographs, Sihochunggan-tang gagambang and Gagamchengyoung-tang can be effective on the seborrheic dermatitis in seven cases in our study.
