Orbital Abscess from Odontogenic Infection of Maxillary Molar: Case Report (상악 대구치 치성감염으로 인한 안와농양: 증례보고)

  • Orbital infection is typically caused by spread of inflammation from the paranasal sinuses. Less common causes can be skin infections, trauma, and hematogenous spread from other infections located elsewhere in the body. Odontogenic orbital infections account for 2~5% of all orbital infections, and occur as a result of periodontitis, odontogenic abscess from caries, tooth extraction, and oral surgery. Orbital infections can be divided into preseptal infection, orbital subperiosteal abscess, orbital abscess, and postorbital abscess. Symptoms which can be observed are swelling of the eyelids and erythema, orbital edema, displacement of the eyeball, exophthalmos, ophthalmoplegia, and even impairment of the optic nerve. Here we present the case of a patient who had an orbital abscess secondary to an abscess of the right maxillary third molar. Rapid recovery occurred following surgical treatment and antibiotic therapy. In addition a brief review of the literature is included.

  • Chronic wounds, pressure sores, lesions, and infections of microbial origin in bedridden, paralyzed, or malnutrition patients remain the object of study of many researchers. A variety of factors behind the development of these disorders are related to the patient's immune system, making it unable to respond effectively to the treatment of the wound. These factors can be properly controlled, giving particular importance to the ethiology and stage of the wound, as well as the time periods corresponding to the replacement of the dressings. The present research reports a novel foam/soft material, ${{\small}L}$-Cys-g-PCL, with an application for decubitus/pressure ulcers, especially for wounds with a difficult healing process due to infections and constant oxidation of the soft tissues. During this work, the interactions between S. aureus and ${{\small}L}$-Cys-g-PCL foam were studied under conditions that simulate decubitus ulcers; namely, pH and exudate. The effects of duration of grafting (1 or 8 h) and pH (7.0 and 8.9) on wettability, surface energy, swelling, and porosity were also evaluated. Results showed an effective microbicidal activity exhibiting an inhibition ratio of 99.73% against S. aureus. This new ${{\small}L}$-Cys-g-PCL soft material showed saftey to contact skin, ability to be shaped to fill in sunken holes (craters) - pressure ulcers stage III - and to act as a smart material responsive to pH, which can be tailored to develop better swelling properties at alkaline pH where exudates are normally higher, so as to address exudate self-cleaning and prevention of desiccation.

  • Distraction osteogenesis is applied to correct mandible or maxilla deformities. Owing to the distractor being away from the skin, it is not aesthetic. Infection can arise due to manual part. And therefore, it is necessary to make the distractor using a new mechanism. 3D software was used for the intraoral distractor modeling. 3D meshes were used to analyze the stress distribution of the distractor was analyzed using Finite element analysis software. This research developed a intraoral distractor for continuous mandible distraction through convergence study. Base on the results, it provide helpful data for future version of the wireless intraoral distractor in other fields of bio and medical engineering.

Development and Preliminary Validation of Mibyeong Questionnaire(MQ) Based on Deficiency-Stagnation pattern (허울(虛鬱) 기반 미병 평가도구 개발 및 예비타당성 검증)

  • Objective : This study aimed to develop and preliminary validate the Mibyeong Questionnaire (MQ) based on Deficiency-Stagnation pattern, which was report of a person's health condition that comes directly from the person. Method : The first phage of developing a MQ was to generate and exhaustive list of all MQ issue that are relevant to the domain of interest, using literature research and expert group discussions. Through those steps, we established MQ with 33 items divided into two parts: 21-item deficiency pattern and 12-item stagnation pattern. The second phage, we examined the preliminary tests of reliability and validity including the 16-item Deficiency of MQ (16D-MQ), with data (n=1,890) already collected on the Korean medicine data center in KIOM. Results : Exploratory factor analysis revealed three factors of the 16D-MQ. These factors were fatigue(Qi, 氣); psychic and physical elements(Shen, 神); and skin and hair(Jing, 精). Cronbach's coefficient alpha was 0.876 and the intraclass correlation coefficients was 0.368-0.538. In support of criteria validity, the 16D-MQ was weakly correlated with EQ-5D and physicians's opinion, but it was acceptable. Conclusion : The MQ shows that it has an appropriate level of internal consistency and validity. We think further study to reveal its reliability and validity, including stagnation pattern as well as deficiency pattern, is needed.

  • Antioxidants of sesame have been reported to cure and prevent various diseases by means of diverse physiological activities, prevention of acidification in organisms, prevention of acidification and decay of lipids, cholesterol depression, preventive effects on chemical breast cancer, skin beauty and senescence inhibition, and so on. Recognizing their significance to health and disease prevention, researchers in Japan and America have given so much importance to study antioxidants in the last decade. In addition, they are actively pursuing studies on production, processing for food use and development of new varieties that have high antioxidant content. Recently, researchers in Korea have shown the same interest and have conducted similar studies, however, the importance of the following basic issues must be recognized to guide in future activites : First, improvement of sesame quality must be done to raise the contents of not only the fat and fatty acid but also sesamin, sesamolin and sesaminol glucoside. For the use of these components it is necessary to study the gentic pattern and individual selections developed from minimum sample size and fast lipid analysis techniques. Second, sesaminol of sesame has a remarkable function in preventing acidification and so sesame can be utilized as a food that prevents or delays aging caused by automatic acidification of fat. Therefore, for maximum medicinal benefit from sesame oil there is a need to develop food materials having new medicinal functions. Third, the sesamin and sesamolin content of sesame germplasms collected in Korea showed lower ranges of $0.04\~0.68$ percent and $0.08\~0.68$ percent respectively, while Japanese germ-plasm showed 1.9 percent maximum content of seasmin. Thus, germplasm collection and analysis of worldwide genetic resources are urgently needed.

  • Palpation of the pulse has been used in Korean traditional medicine since ancient times to assess physical health. Pulse wave contour may be obtained by measuring arterial pressure or blood volume change of skin. The latter is called as Photoplethysmography(PPG) or digital volume pulse(DVP). The PPG signal is measured by a device comprising an infrared light sourece and a photodetector. Although less widely used, this technique deserves further consideration because of its simplicity and ease of use. The contour of the PPG is formed as a result of a complex interaction between the left ventricle and the systemic circulation. It usually exhibits an early systolic peak and an early diastolic peak. the first peak is formed mainly by pressure trasmitted along a direct path from the left ventricle to the finger. The second peak is formed in part by pressure transmitted along the aorta and large arteries to sites of impedance mismatch in the lower body. The contour of the PPG is sensitive to changes in arterial tone and is influenced by ageing and large artery stiffness. Measurements taken directly from the PPG or from its second derivative can be used to assess these properties. In some mathematical approaches, the extraction of periodic components using frequency analysis was tried to analysis of the PPG. But we don't understand yet what kind of factor in the cardiovascular system or human body is related with the respective specific Fourier components of PPG. This review describes the background to measurement principles, representative contour, contour analysis and frequency domain analysis of PPG, and current and future.

  • As sports infiltrates each field of modern society and becomes familiar, sports wear was changed functionally and was introduced and enveloped into everyday dress. Modern sports wear has been the settled in everyday life deeply and become the clothes of life which are worn regardless of place, time and age. The aim of this paper was to clarify what kind of fashion of sports style would be given to the modern people. In this study, sports wear which has become everyday dress classified from a gender point of view. It was divided into masculine image, feminine image, and neutral image. Sports wear of masculine image generated a silhouette which emphasized the shoulder with the aspiration for youth and health. Wide shoulder was considered as the symbol of masculine beauty and the expression of healthy beauty. It was reflected well in body conscious look. And owing to the development of up-to-data materials, innovation of design, and the study of human body technology, the functional character was settled in the sports wear which showed masculine image. Sports wear of feminine image was represented fashion of body exposure, body feet with body conscious look, and romantic mode. This image was expressed fashion as comforts, pleasant, active design, materials, color, and romantic feminine beauty. Sports wear of neutral image was expressed into unisex clothes. This cloths have no difference in gender, age, and class. It was used as casual sports wear. In the 1960s, young generation participated in such street sports as street basketball and skate. They usually sore the sports wears of neutral image such as cycling, skating, and ski. In the materials of sports, the development of up-to-data material like lycra made the sayings lifelike, “up-to-data material is the second skin” It show that glamorous feminine image and strong masculine image coexisted. The contemporary concept of sportswear is no longer limited to those clothes for sports found in such places like tennis court or swimming pool. Now, the sports wear become more like casual activity wear all classes of people can enjoy in their life regardless of where they are, when they wear, and even how old they are.

Analysis of Heavy Metals in Dyes and Pigments Used in Make up Tattoo (화장용 문신으로 사용되는 염료 및 색소의 중금속 분석)

  • This study was attempted to provide data for establishing appropriate guidelines and control standards for accurate information and production of dyes and pigments through the analysis of representative heavy metals and other metals used in cosmetic tattoos. A total of 30 tattoo products for cosmetics were selected from among the representative heavy metals such as arsenic, antimony, cobalt, cadmium, copper, lead, zinc, barium, tin, selenium, chromium and magnesium, manganese(15kinds) were analyzed. As a result of examining the contents of heavy metals in dyes and pigments used in cosmetic tattoo products, it was found that the total of 20products(66.7%) exceeded the reference value and found to be unsuitable for safety standards. It is suggested that the guidance of the specialized agency for safe procedures, appropriate control controls and standards for the dyes and pigments used, and the potential risks of heavy metals are needed.

  • We have experienced a case of sarcoidosis appearing asymmetrical diffuse interstitial lung lesions with multiple lymphadenopathy. This patient was a 57 year-old female who had been in good health until 2 months ago. At that time she noted the onset of an exertional dyspnea and weakness. Pulmonary function tests showed moderate obstructive pattern with mild decreased DLco. In the chest CT, multiple lymphadenopathy with small nodular lesions are scattered, and the impression was a metastatic lymph nodes with lymphangitic carcinomatosis. In bronchofiberscopy, we noted luminal narrowing by extrinsic compression in the right middle and lower lobe bronchi. And microscopic examination of by bronchofiberscopic biopsy showed chronic inflammation. Thus we performed subcarinal and tight supraclaviclar lymph nodes aspiration biopsy cytology, and that revealed class 0 and class 1, respectively. Finally, we performed an excisional biopsy for the right scalene lymph node, which revealed the specimen as a noncaseating granuloma. The angiotensin convecting enzyme level was overt two folds compared to normal value. And the patient had negative PPD skin test and hyperglobulinemia. After 18 weeks treatment with prednisone, the signs and symptoms which the patient clad suffered from, disappeared.

The Effect of Sponge Padding on Edema Reduction (스펀지 패딩을 이용한 붕대법이 부종감소에 미치는 영향)

  • The purpose of this study was to determine the effects On the lymphedema patients of sponge padding and the correlation between the rate of edema reduction and downward shifting of bandages. Fifty secondary lymphedema patients with no infection, active cancer, limitation of movement, pain or drugs were randomly assigned to two groups: a sponge padding group, and a cotton padding group. Complex decongestive therapy was applied for 2 weeks. In each group the volume of the ankle, mid-calf, knee, and mid-thigh and downward shifting of bandages at the thigh was measured with measuring tape. The reduction of volume and downward shifting of bandages were analyzed by the unrelated t-test and the Pearson Correlation Test. The results were as follows: 1. In comparison between the sponge padding group and the cotton padding group, there was no significant difference in volume except in the thigh area. The edema volume was reduced significantly to $37.77{\pm}31.15%$ at the thigh with sponge padding after treatment (p<.05). 2. Bandage shifted 10.12 cm with cotton padding, but only shifted 2.85 cm when sponge padding was used. Sponge padding had an excellent effect in preventing the shifting of bandages. There was a significant difference in the amount of shifting that occurred with each type of padding (p<.05). 3. There was also a strong correlation between the reduction of edema and the shifting of bandage. These results suggest that short-stretch bandaging with sponge padding is more effective than bandaging with cotton padding. Further study on skin irritation and the comfort of sponge padding is needed to use padding materials safely and effectively.
