Clinical study of the vasocreactivity and blood flow velocity of t he cerebral infarctions using Transcranial Doppler Sonography (Transcranial Doppler를 利用한 腦硬塞 患者의 血流速度와 血管 反應性에 대한 臨床的 考察)

  • Background : The vasoreactivity of cerebral artery is currently the subject of increasing interest. Transcranial Doppler Sonography(TCD) is an accurate method of monitoring the blood flow velocities of the cerebral artery. We wished to assess the vasoreactivity between age-matched normal and cerebral infarction group. Method : We performed TCD findings in 40 normals and 20 cb-inf. subjects who arrived at hospital during 48 hours after attack. The former were devided into twenties normal group and fifties normal group. Result : 1. Fibrinogen levels showed significant changes between age-matched normal and cb-inf. group. 2. $V^{MEAN}$ of the both side Carotid Siphon during rest state increased significantly in cb-inf. group as compared with the age-matched normal group. 3. $V^{MEAN}and\;V^{MAX}$ of the both side carotid siphon during breath-hold state increased significantly in cb-inf. group as compared with the age-matched normal group. 4. Vasoreactivity of cerebral artery increased significantly in cb-inf. group as compared with the age-matched normal group. 5. $V^{MEAN}and\;V^{MAX}$ of the both side Radial artery during heat-stimuration state increased significantly in cb-inf. group as compared with the age-matched normal group. 6. Vasoreactivity of Radial artery increased significantly in cb-inf. group as compared with the age-matched normal group.

Nutrient Components in the Siphon of the Surf Clam Tresus keenae

  • We evaluated the nutritional composition of the siphon of the surf clam Tresus keenae in regard to the presence of nitrogenous [amino acids, nucleotides and their related compounds, total creatinine, betaine, trimethylamine oxide (TMAO), and trimethylamine (TMA)] and non­nitrogenous compounds (sugars and organic acids), lipid fatty-acid composition, and occurrence of minerals. The content of total free amino acids was 660.27 $\pm$ 7.94 mg/100 g, and the predominant amino acids were arginine, alanine, sarcosine, glycine, and glutamic acid. These amino acids accounted for $71\;\%$ of the total free amino acids. Among the nucleotides and their related compounds, inosine was the major component and comprised 40.38 $\pm$ 0.02 mg/100 g. Free amino acids were the largest contributor to total extracted nitrogen, comprising $49.94\%$, followed by total creatinine, betaine, nucleotides, and ammonia; the contribution of TMAO and TMA was small. For the non-nitrogenous compounds, malic acid, propionic acid, and succinic acid comprised the major portion of the ten kinds of organic acids detected, and the sugars found were glucose, maltose, and arabinose, which were estimated to be $147.0\pm7.15,\;34.45\pm1.09,\;and\;1.21\pm0.02\;mg/100\;g,$ respectively. The predominant minerals were Na and K, which comprised $11.43\pm1.06\;and\;9.46\pm1.02\;mg/100\;g,$ respectively. The major fatty acids were C22:6, C20:5, C23:0, C18:3, and C16:0 in the lipid fractions. The 23:0 level of glycolipid (GL) was the highest of any other lipid fraction. The amount of total polyunsaturated fatty acids (PUFA) in the lipid fractions was higher, ranging from $58.22\%\;in\;GL\;to\;77.1\%$ in phospholipid (PL), compared to the saturated and monounsaturated fatty acids. Of the n-3 fatty acids, C20:5 and C22:6 contributed $35.30-64.44\%$ of PUFA in the lipid fractions. The ratios of n-3 to n-6 PUFA in total lipid (TL), neutral lipid (NL), PL, and GL were 4.35, 4.26, 6.69, and 2.04, respectively.

Evaluation of Stent Apposition in the LVIS Blue Stent-Assisted Coiling of Distal Internal Carotid Artery Aneurysms : Correlation with Clinical and Angiographic Outcomes

  • Objective : To evaluate the stent apposition of a low-profile visualized intraluminal support (LVIS) device in distal internal carotid artery (ICA) aneurysms, examine its correlation with clinical and angiographic outcomes, and determine the predictive factors of ischemic adverse events (IAEs) related to stent-assisted coiling. Methods : We retrospectively analyzed a prospectively maintained database of 183 patients between January 2017 and February 2020. The carotid siphon from the cavernous ICA to the ICA terminus was divided into posterior, anterior, and superior bends. The anterior bends were categorized into angled (V) and non-angled (C, U, and S) types depending on the morphology and measured angles. Complete stent apposition (CSA) and incomplete stent apposition (ISA) were evaluated using unsubtracted angiography and flat-panel detector computed tomography. Dual antiplatelet therapy with aspirin 200 mg and clopidogrel 75 mg was administered. Clopidogrel resistance was defined as fewer responders (≥10%, <40%) and non-responders (<10%) based on the percent inhibition (%INH) of the VerifyNow system. These were counteracted by a dose escalation to 150 mg for fewer responders or substitution with cilostazol 200 mg for non-responders. IAEs included intraoperative in-stent thrombosis, transient ischemic attack, cerebral infarction, and delayed in-stent stenosis. A multivariate logistic regression analysis was used to determine the predictive factors for ISA and IAEs. Results : There were 33 ISAs (18.0%) and 27 IAEs (14.8%). The anterior bend angle was narrower in ISA (-4.16°±25.18°) than in CSA (23.52°±23.13°) (p<0.001). The V- and S-types were independently correlated with the ISA (p<0.001). However, treatment outcomes, including IAEs (15.3% vs. 12.1%), aneurysmal complete occlusion (91.3% vs. 88.6%), and recanalization (none of them), did not differ between CSA and ISA (p>0.05). The %INH of 27 IAEs (13.78%±14.78%) was significantly lower than that of 156 non-IAEs (26.82%±20.23%) (p<0.001). Non-responders to clopidogrel were the only significant predictive factor for IAEs (p=0.001). Conclusion : The angled and tortuous anatomical peculiarity of the carotid siphon caused ISA of the LVIS device; however, it did not affect clinical and angiographic outcomes, while the non-responders to clopidogrel affected the IAEs related to stent-assisted coiling.

  • Spongicola tropicanus, a new genus and species of the family Eunicicolidae, and Asterocheres brevisurculus, a new species of the family Asterocheridae, are described as copepods associated with the sponge Phyllospongia foliascens (Pallas) from the Moluccas. Spongicola is the second genus of the family and may be characterized by a two-segmented prosome, the absence of leg 3, and the presence of leg 5. Asterocheres brevisurculus may be charecterized by one-segmented mandibular palp, short oral siphon, and 19-segmented female antennule.

Development of Mathematical Model for the Cherepnov Water Lifter (CHEREPNOV송수기의 수학적모델 개발)

  • This paper presents a mathematical model which simulates the Cherepnov water lifter, that can lift water without the use of external energy such as electricity. A theoritical study was conducted to reveal the characteristics of the Cherepnov water lifter that was continuously operated with the aid of the siphon. The mathematical model wal composed of the continuity equation and energy equation, and was coded using FORTRAN language. In this study, the govering flow equation of the lifter were derived and the computer programs of the equations were worked out. The accuracy of the theoretical equations and their solutions was checked by laboratory experimentation. The mathematical model that simulate the Cherepnov water lifter is appeared to be good to predict the behavior of Cherepnov water lifter. Therefore, the mathematical model and the simulation method used in this study could be used in designing of the Cherepnov water lifter.

  • Transcranial doppler ultrasonography (TCD) is a new, non-invasive and easily applicable method to evaluate cerebral hemodynamics. Last 10 years, its use in Korea has been dramatically expanded, but the qualification of TCD laboratory has yet to be settled. Since duplex sonography is seldom used in Korea, we have to depend totally on TCD to evaluate cerebral hemodynamic changes. Thus, all of the available data from every detectabler cerebral arteries has to be obtained for accurate interpretation of TCD measurements. Moreover, flow direction and wave form should be concerned in addition to the flow velocity. In this article, I present technique to measure the anterior, meddle and posterior cerebral arteries, the internal carotid artery siphon and at cervical level, and the vertebral and the basilar artery, and normal values for these measurements which is essential for the adequate interpretation.

  • A small-scale solar collector and tracking system, using a Fresnel lens of $0.5m^2$, and novel compact fluidized-bed solar receiver[FBR] of closed type has been developed for high temperature solar gas heating. The FBR was improved in carrying over of SiC powder and thermo-siphon effect. The maximum outlet air temperature of 1140K and the maximum thermal efficiency of 64% were obtained. The present FBR's operated efficiently at extremely high temperatures in comparison with conventional solar receivers, composed of flat or tubular solid surfaces.

  • 본 연구에서는 유연생산시스템에서의 교착상태 확인 및 회피 알고리즘을 추이적 행렬을 이용하여 제안한다. 추이적행렬은 플레이스와 플레이스간의 관계를 표현함으로 마킹의 흐름을 플레이스와 트랜지션간의 관계형에서 상태와 상태간의 관계를 표현함으로 상태의 변화 검증에 편리하다. 교착상태 확인 및 회피 알고리즘을 제시하고, 기존에 발표되어진 siphon과 DAPN알고리즘간의 비료 검토를 통하여 제안한 알고리즘의 유용성을 검증하였다.

  • 당뇨병은 고대 에집트시대때부터 다뇨증상을 가진 질병으로 알러져 왔으며 희랍의 의사인 Aretaeus (of Cappadocia)가 이 질병을 희랍어로 siphon을 의미하는 diabetes라 명명하였다. 17세기에는 꿀을 뜻하는 mellitus가 diabetes에 더해져 지금까지 쓰이고 있으며 18세기에 들어서야 sweetness가 sugar에 기인한다는 걸 밝혀내었다. 당뇨병은 이와 같이 오래되고 흔한 질병이지만 그에 대한 치료제는 insulin, sulfonylurea제, biguanide제 그리고 최근에 개발되어 시판되었으나 간독성으로 인하여 영국에서는 판매가 금지된 thiazolidinedione제 뿐이기 때문에 많은 제약회사에서 새로운 치료제를 개발하기 위하여 치열한 경쟁을 하고 있다. 이는 전세계적으로 1억명 이상 그리고 미국에만 1,400만명 이상의 당뇨병 환자가 있다고 추정되며 이의 치료에 들어가는 경비가 $105억에 달하기 때문이기도 하다. 또한 주로 선진국에서 흔하던 이 질병이 요즈음엔 풍족해진 음식, 문명의 서구화(소위 말하는 "Coca-colonization")와 운동 부족 등의 이유로 개발국과 개발도상국에서도 발병율이 높아 가는 질병이기 때문에 획기적인 치료제의 개발이 절실히 필요한 때이기도 하다.한 때이기도 하다.

Inquiry into the Laboratory Diagnostic Tests in Larygopharyngeal Reflux Disease (인후두역류질환의 실험실 검사의 재평가)

  • Laryngopharyngeal reflux disease (LPRD) is the result of retrograde flow of gastric contents to the laryngopharynx. Laryngoscopic findings and special questionnaires are first step of diagnosis of LPRD. Empiric trials of Proton pump inhibitor' test (PPI test) is recommended as treatment and diagnosis. However confirmation of reflux is then recommended primarily in patients with persistent symptoms despite acid-suppressive therapy. The 24 hour ambulatory double pH monitoring has been a gold standard method in diagnosis of LPRD even though it has some limitation. The combined multichannel intraluminal impedance and pH monitoring is a new-rising test tool. It can detect acid/non-acid, liquid/gaseous reflux and clearance of refluxate. The water siphon test is also used for diagnosis of LPRD.
