Treatment of Acute Suppurative Thyroiditis Caused by Pyriform Sinus Fistula : 1 Case Report (이상와 누공으로 인한 급성 화농성 갑상선염 치료 1예)

  • Acute suppurative thyroiditis is rare, infectious thyroid disorder because the thyroid gland is resistant to infection. We report a 26-year-old man with acute suppurative thyroiditis due to a pyriform sinus fistula. He presented with anterior neck swelling and tenderness for 2 weeks. Antibiotic treatment failed to improve his symptoms and signs. Diagnosis was made by bariums studies, computed tomography scan and endoscopic examination. The pyriform siuns fistula was successfully treated by chemical cauterization, partial thyroidectomy and ligation of fistula tract.

  • Aneurysm of the sinus of Valsalva on mostly congenital disease that develops more frequently in Orientals, is very low in incidence. In most cases, aneurysm of the sinus of Valsalva extends to intracardiac and results in ruptures into the right ventricle or atrium. The likelihood of extracardiac aneurysm of the sinus of Valsalva is very low. Cases of extracardiac aneurysm are usually accompanied by aortic regurgitation and can cause right ventricle outflow tract obstruction, myocardial ischemia, and myocardial infarction due to compression by aneurysm. Since the aneurysm can rupture in the intrapericardium and cause cardiogenic shock or sudden death, definite diagnosis and management are important. If confirmed, it is preferable operform a surgical correction. We report here, with a literature review, a case where myocardial ischemia and aortic egurgitation caused by aneurysm developed in the left coronary and noncoronary sinus, and were surgically corrected with satisfactory esults.

  • Sinus histiocytosis with massive lymphadenopathy or Rosai-Dorfman disease(RDD) is a rare type of histiocytosis syndrome, that presents in its most typical form as massive, painless, bilateral lymph node enlargement in the neck during the first or second decade of life. The disease involves extranodal site in over 25% to 43% of the cases, however cases of extranodal RDD without nodal disease have rarely been reported. The involved sites of extranodal RDD have been reported various that were orbit, ocular adnexae, head and neck, upper respiratory tract, breast, gastrointestinal tract, CNS, etc. A 35 year-old man was present with pleural pain on left anterior chest and anterior mediastinal mass, that underwent an excision to remove the suspected invasive thymoma, and the diagnosis was confirmed to extranodal Rosai-Dorfman disease.

  • Actinomycosis is a chronic suppurative and granulomatous bacterial infection characterized by contiguous spread, abcess formation and sinus tract formation. There are four clinical forms according to the lesional site, as 1) cervicofacial, 2) thoracic, 3) abdominal, and 4) disseminated form. Recently, we experienced a case of 54 year-old patient with left mandibular actinomycosis. The pathognomonic findings of actinomycosis is sulfur granule with multiple filaments in Gram-stain and the treatment of actinomycosis is surgical excision of mass or sinus tract with massive antibiotics(esp. Penicillin) therapy for 6 to 12 months.

  • Actinomycosis is a rare subacute-to-chronic infection that causes sinus fistula, tract, or abscess due to the invasion surrounding the soft tissue. Actinomyces colonize the mouth, colon, and vagina. Mucosal disruption may lead to infection at virtually any site in the body. Cervicofacial infection accounts for 50-60% of all actinomycosis cases. The mandible and nasopharynx are the sites of predilection, but maxillary sinus infection is rare. Reported herein is a case involving a 57-year-old female with acute myeloid leukemia who had simultaneous actinomycosis with mucormycosis in the maxillary sinus.

  • Management of maxillofacial trauma includes primary care, in which diagnosis and management of dentoalveolar injury play a vital role. Due to the impact sustained during a maxillofacial injury (whether direct or indirect), dentoalveolar injuries can occur, leading to fracture and displacement of teeth and associated alveolar bone into the surrounding soft tissues and associated structures, such as the maxillary sinus, nasal cavity, upper respiratory tract, tracheobronchial tree, or gastrointestinal tract. Undiagnosed displaced teeth may cause complications such as airway obstruction. This paper reports a case of displaced teeth in the nasal cavity and gastrointestinal tract and highlights the management protocol for displaced teeth secondary to maxillofacial trauma.

  • Basaloid squamous cell carcinoma(BSCC) is uncommon and distinct variant of squamous cell carcinoma that arises mostly in the upper aerodigestive tract and aggressive, high grade tumor with an increased tendency to be deeply invasive, multifocal, and metastatic even at the initial presentation. The typical microscopic features of carcinoma with a basaloid pattern in intimate association with a squamous component helps in diagnosis of this tumour. Since Wain's report in 1986, BSCC of oral cavity, the palate, floor of the mouth, nasopharynx, oropharynx and mastoid region have been reported. However, BSCC in the nasal cavity or in the paranasal sinuses is rare and there are few reports in the Korean literature. We had experienced a case of basaloid squamous cell carcinoma that occurred in the left maxillary sinus of 72-year-old woman and reported with review of the clinical and pathologic features from the literature.

The Free Musculocutaneous Flap in the Treatment of Chronic Osteomyelitis (유리 근피부판을 이용한 만성 골수염의 치료)

  • Purpose : To evaluate the efficacy of free musculocutaneous flap in the treatment of chronic osteomyelitis with drain sinus. Materials and Methods : Eighteen patients (sixteen male and two female) whose average age was 42.6 years were followed for an average of 3.5 years after free musculocutaneous flap for chronic osteomyelitis with drain sinus tract. The period of time between the initial injury and this procedure was average 10.6 years. The patients had an average of 4.1 prior surgical procedure before it. Results : Sixteen(88.9%) of the eighteen free musculocutaneous flap survived, including thirteen of latisimus dorsi flap, three of scapular free flap with muscle, two of medial plantar flap with abductor muscle. Sixteen cases had no evidence of recurrence at follow up. Conclusion : Free musculocutaneous flap is an extremely successful and reliable procedure for the treatment of chronic osteomyelitis with drain sinus. This flap with microvascular anastomses is highly vascularized through its own intrinsic blood vessels, which has the advantage of obliterating dead space and providing improved vascularity to the relatively ischemic recipient site and covering the skin defect.
