Objectives : To investigate the effects of electroacupuncture on parameters related to obesity in adults with abdominal obesity. Methods : A three arm randomized single blind pilot study was conducted from Jan 4 to March 25, 2010 in Kyung Hee Oriental Medical Hospital. The subjects were 39 adults with abdominal obesity and were randomly divided by computer generated random table into 3 groups; EA(electroacupuncture), sham EA(sham electroacupuncture) and waitlist groups. Acupuncture points located at abdomen($CV_{12}$, $CV_6$, $ST_{25}$, $SP_{15}$, $SP_{14}$) and extremities($LI_4$, $LI_{11}$, $ST_{36}$, $ST_{44}$) were inserted by disposable stainless steel needles and were stimulated 30 minutes with 24Hz, 0.27~1.3mA(tolerable strength), asymmetric biphasic continuous pulse wave form by STN-111 Stratek device in EA group. Two treatment sessions per week for 5 weeks(10 sessions in total) were done in EA and sham EA groups. The primary outcome measurement was WC(waist circumference), and the secondary outcome measurements included WHR(waist hip ratio), ASF(thickness of abdominal subcutaneous fat), and inbody measurements of BW(body weight), BMI(body mass index), BFR(body fat ratio) and VFA(visceral fat area), and also scores of BULIT-R(bulimia test revised), KoQoL(Korean obesity of QoL) and BSQ(body shape questionnaire). Results : All of 39 subjects were included in ITT(intention-to-treat) analysis. There were significant reductions in WC, WHR and ASF after 5-week electroacupuncture treatments and the percentage reductions were significantly greater than sham EA or waitlist group. There were no significant differences between groups in percentage reductions of other parameters(BW, BMI, BFR, VFA, BULIT-R, KoQoL and BSQ). But, there were continuous reductions in BW, BMI, BFR and VFA at 3 weeks after the end of treatment and there was significant reduction in BW compared with the baseline value in EA group. No seriously adverse effects were reported during the period. Conclusions : Electroacupuncture was more effective than sham electroacupuncture or no intervention on the reduction of WC, WHR and ASF in adults with abdominal obesity.

Machine foundations with impact loads are common powerful sources of industrial vibrations. These foundations are generally transferring vertical dynamic loads to the soil and generate ground vibrations which may harmfully affect the surrounding structures or buildings. Dynamic effects range from severe trouble of working conditions for some sensitive instruments or devices to visible structural damage. This work includes an experimental study on the behavior of dry dense sand under the action of a single impulsive load. The objective of this research is to predict the dry sand response under impact loads. Emphasis will be made on attenuation of waves induced by impact loads through the soil. The research also includes studying the effect of footing embedment, and footing area on the soil behavior and its dynamic response. Different falling masses from different heights were conducted using the falling weight deflectometer (FWD) to provide the single pulse energy. The responses of different soils were evaluated at different locations (vertically below the impact plate and horizontally away from it). These responses include; displacements, velocities, and accelerations that are developed due to the impact acting at top and different depths within the soil using the falling weight deflectometer (FWD) and accelerometers (ARH-500A Waterproof, and Low capacity Acceleration Transducer) that are embedded in the soil in addition to soil pressure gauges. It was concluded that increasing the footing embedment depth results in increase in the amplitude of the force-time history by about 10-30% due to increase in the degree of confinement. This is accompanied by a decrease in the displacement response of the soil by about 40-50% due to increase in the overburden pressure when the embedment depth increased which leads to increasing the stiffness of sandy soil. There is also increase in the natural frequency of the soil-foundation system by about 20-45%. For surface foundation, the foundation is free to oscillate in vertical, horizontal and rocking modes. But, when embedding a footing, the surrounding soil restricts oscillation due to confinement which leads to increasing the natural frequency. Moreover, the soil density increases with depth because of compaction, which makes the soil behave as a solid medium. Increasing the footing embedment depth results in an increase in the damping ratio by about 50-150% due to the increase of soil density as D/B increases, hence the soil tends to behave as a solid medium which activates both viscous and strain damping.

As the basic research for the application of a parametric acoustic source to fish finding, the characteristics of beam patterns and parametric gains of the acoustic source were investigated and target strengths of fish, grey mullet, with the acoustic source were measured. The mean primary frequency of the acoustic source was 200KHz and the produced sounds by difference-frequencies were 5KHz, 10KHz, 16KHz and 20KHz. For measurement of target strength in yaw (coronal) plane of fish the to be target was 34cm in length, the pulse duration of the source was 0.3m/sec and the difference frequency was 10KHz in consideration of the length of fish and of parametric gain of the acoustic source. The results obtained are as follow: 1. Beam widths(down 3 dB) of the parametric acoustic source excited at frequencies of 5KHz, 10KHz, 16KHz, and 20KHz were 4.3$^{\circ}$, 2.2$^{\circ}$, 3.0$^{\circ}$ and 2.5$^{\circ}$ respectively. 2. Parametric gains of the parametric acoustic source excited at frequencies of 5KHz, 10KHz, 16KHz and 20KHz were -41 dB, -45 dB, -60 dB and -68 dB respectively. 3. Target strengths of a fish in head and tail aspect using the parametric acoustic source were 5 dB lower than those using 200KHz single frequency sound, but those in side aspect were similar. 4. Target strengths of two or three fish with the parametric acoustic source were 1-3 dB lower than those in head and tail aspect using 200KHz single frequency sound.
The vertical profiles and optical properties of Asian dust are investigated using ground-based measurements from 1998 to 2002. Vertical profiles of aerosol extinction coefficient are evaluated using MPL (Micro Pulse Lidar) data. Optical parameters such as aerosol optical thickness ($\tau$), ${\AA}ngstr\ddot{o}m$ exponent ($\alpha$), single scattering albedo ($\omega$), refractive index, and volume size distribution are analyzed with sun/sky radiometer data for the same period. We can separate aerosol vertical profiles into three categories. First category named as 'Asian dust case', which aerosol extinction coefficient is larger than $0.15km^{-1}$ and dust layer exists from surface up to 3-4km. Second category named as 'Elevated aerosol case', which aerosol layer exists between 2 and 6km with 1-2.5km thickness, and extinction coefficient is smaller than $0.15km^{-1}$. Third category named as 'Clear sky case', which aerosol extinction coefficient appears smaller than $0.15km^{-1}$. and shows that diurnal variation of background aerosol in urban area. While optical parameters for first category indicate that $\tau$ and $\alpha$ are $0.63{\pm}0.14$, $0.48{\pm}0.19$, respectively. Also, aerosol volume concentration is increased for range of 1 and $4{\mu}m$, in coarse mode. Optical parameters for second category can be separated into two different types. Optical properties of first type are very close to Asian dust cases. Also, dust reports of source region and backward trajectory analyses assure that these type is much related with Asian dust event. However, optical properties of the other type are similar to those of urban aerosol. For clear sky case, $\tau$ is relatively smaller and $\alpha$ is larger compare with other cases. Each case shows distinct characteristics in aerosol optical parameters.
Purpose : Recently, clinical trials of steroid add-on therapy were reported with variable results in Kawasaki disease. We analyzed the clinical outcomes of patients at high risk of with Kawasaki disease(${\geq}4$ points of Harada score) treated by three commonly used different treatment regimens, with or without corticosteroids. Methods : Medical records of 96 children with Kawasaki disease treated with one of the threee regimens were reviewed retrospectively. Regimen 1 was aspirin(100 mg/kg/day) plus intravenous gamma globulin 2 g/kg single dose; regimen 2, aspirin(100 mg/kg/day) plus intravenous gamma globulin 1 g/kg single dose; regimen 3, regimen 2 plus prednisolone(2 mg/kg/day), followed by tapering two weeks and pulse therapy of methyl prednisolone performed in cases of retreatment. Also low dose aspirin was given in all three regimens for eight weeks after the acute phase. The cardiovascular and laboratory evaluations were performed on acute phase, immediate after acute phase, and subacute phase, eight weeks after treatment. Results : The frequency of coronary artery lesions and laboratory findings in the three different regimens were similar. The more rapid control of fever after treatment was noted in regimen 3. Furthermore the frequency of retreatment was decreased in regimen 3 compared to the other two regimens. Conclusion : Steroid add-on therapy showed some beneficial outcome compared to conventional treatment regimens. The role of steroid in the treatment of Kawasaki disease should be reassessed in systemic manner.
Purpose: Damage to pulse crops by wild birds is a serious problem. The damage is to such an extent that the rate of damage during the period between seeding and cotyledon stages reaches 54.6% on an average. In this study, a crop-position detection method was developed wherein infrared (IR) sensors were used to determine the cotyledon position under a vinyl mulch. Methods: IR sensors that helped measure the temperature were used to locate the cotyledons below the vinyl mulch. A single IR sensor module was installed at three locations of the crops (peanut, red lettuce, and crown daisy) in the cotyledon stage. The representative thermal response of a $16{\times}4$ pixel area was detected using this sensor in the case where the distance from the target was 25 cm. A spatial image was applied to the two-dimensional temperature distribution using a non-integral moving-average method. The collected data were first processed by taking the moving average via interpolation to determine the frame where the variance was the lowest for a resolution unit of 1.02 cm. Results: The temperature distribution was plotted corresponding to a distance of 10 cm between the crops. A clear leaf pattern of the crop was visually confirmed. However, the temperature distribution after the normalization was unclear. The image conversion and frequency-conversion graphs were obtained based on the moving average by averaging the points corresponding to a frequency of 40 Hz for 8 pixels. The most optimized resolutions at locations 1, 2, and 3 were found on 3.4, 4.1, and 5.6 Pixels, respectively. Conclusions: In this study, to solve the problem of damage caused by birds to crops in the cotyledon stage after seeding, the vinyl mulch is punched after seeding. The crops in the cotyledon stage could be accurately located using the proposed method. By conducting the experiments using the single IR sensor and a sliding mechanical device with the help of a non-integral interpolation method, the crops in the cotyledon stage could be precisely located.

The objective of this study was to determine the effectiveness of topical 5% EMLA cream versus 20% Benzocaine gel in reducing pain from intra oral needle insertion alone as well as injection of anesthetic. The 2 topical anesthetics were tested against each other bilaterally using a randomized, controlled, single blinded, split mouth design. Phase I was conducted to find out the rapidity of onset action of the two agents on anterior/posterior vestibules and anterior/posterior palatal mucosa. Phase II was conducted to evaluate the efficacy of the two topical anaesthetic agents in reducing the pain of intraoral injections. The agents were left in anterior/posterior vestibules and anterior/posterior palatal mucosa for the amount of time recorded in phase I. Subjects recorded pain on a 100-mm modified visual analog scale(VAS). A pulse oximeter was used to recorded the preoperative and postoperative pulse rates. In phase I of the study, two topical agents showed the longer onset of action at anterior part and vestibules than posterior part and palatal mucosa. EMLA cream showed the rapidest onset of action compared to benzocaine gel except on anterior palatal mucosa. In phase II of the study, the VAS grading of the pain for anesthetic administration showed EMLA cream was significantly(P<.05) better in elimination or reducing the pain on the anterior/posterior palatal mucosa. In conclusion, EMLA cream showed the rapidest onset of action compared to benzocaine gel except on anterior palatal mucosa. EMLA cream was found to be superior to Benzocaine gel with regards to pain reduction for anesthetic administration especially on anterior and posterior palatal mucosa.
Purpose : Be aware of clinical possibilities on image quality by comparison of contrast-enhanced dynamic CT and MR imaging applied of MIP technique after the experimentally induced clonorchasis infection in dogs. Materials and Method : Twenty mongrel dogs prepared in zoo-laboratory were followed up with serial CT scans and MR imaging for 13 weeks after the experimental infection in liver. Two-phase helical CT was acquired in the supine position with the following scanning parameters. After the injection of contrast material, the arterial phase was initiated using a bolus-racking method. The portal phase scan was started 15 seconds after the arterial phase scan. CT protocol was determined after single level dynamic scans. MR imaging used the CP body coil and images get a 2D image using HASTE, FLASH, TSE pulse sequence. Bile duct MR imaging were obtained in three plans. Then each image was post processed by using target MIP algorithm. Extracorporeal life support (ECLS) system is a device for respiratory and/or heart failure treatment, and there have been many trials for development and clinical application in the world. Currently, a non-pulsatile blood pump is a standard for ECLS system. Although a pulsatile blood pump is advantageous in physiologic aspects, high pressure generated in the circuits and resultant blood cell trauma remain major concerns which make one reluctant to use a pulsatile blood pump in artificial lung circuits containing a membrane oxygenator. The study was designed to evaluate the hypothesis that placement of a pressure-relieving compliance chamber between a pulsatile pump and a membrane oxygenator might reduce the above mentioned side effects while providing physiologic pulsatile blood flow. The study was performed in a canine model of oleic acid induced acute lung injury (N=16). The animals were divided into three groups according to the type of pump used and the presence of the compliance chamber, In group 1, a non-pulsatile centrifugal pump was used as a control (n=6). In group 2 (n=4), a single-pulsatile pump was used. In group 3 (n=6), a single-pulsatile pump equipped with a compliance chamber was used. The experimental model was a partial bypass between the right atrium and the aorta at a pump flow of 1.8∼2 L/min for 2 hours. The observed parameters were focused on hemodynamic changes, intra-circuit pressure, laboratory studies for blood profile, and the effect on blood cell trauma. In hemodynamics, the pulsatile group II & III generated higher arterial pulse pressure (47$\pm$ 10 and 41 $\pm$ 9 mmHg) than the nonpulsatile group 1 (17 $\pm$ 7 mmHg, p<0.001). The intra-circuit pressure at membrane oxygenator were 222 $\pm$ 8 mmHg in group 1, 739 $\pm$ 35 mmHg in group 2, and 470 $\pm$ 17 mmHg in group 3 (p<0.001). At 2 hour bypass, arterial oxygen partial pressures were significantly higher in the pulsatile group 2 & 3 than in the non-pulsatile group 1 (77 $\pm$ 41 mmHg in group 1, 96 $\pm$ 48 mmHg in group 2, and 97 $\pm$ 25 mmHg in group 3: p<0.05). The levels of plasma free hemoglobin which was an indicator of blood cell trauma were lowest in group 1, highest in group 2, and significantly decreased in group 3 (55.7 $\pm$ 43.3, 162.8 $\pm$ 113.6, 82.5 $\pm$ 25.1 mg%, respectively; p<0.05). Other laboratory findings for blood profile were not different. The above results imply that the pulsatile blood pump is beneficial in oxygenation while deleterious in the aspects to high pressure generation in the circuits and blood cell trauma. However, when a pressure-relieving compliance chamber is applied between the pulsatile pump and a membrane oxygenator, it can significantly reduce the high circuit pressure and result in low blood cell trauma.
Objectives : This study aimed to clarify the relationship between the Oriental medical diagnosis and arteriosclerosis by measuring carotid-ankle vascular index(CAVI) in acute cerebral infarction patients. Method : One hundred thirty-one subjects were recruited from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center from April 2007 to August 2008. We sorted cerebral infarction patients and assessed one hundred fourteen patients' CAVI data. We diagnosed dampness-phlegm by Oriental medical diagnosis and evaluated stroke type by single or multiple infarctions. then, we analyzed their characteristics with type of stroke, risk factor, lifestyle, metabolic syndrome and dampness-phlegm diagnosis. Result : 1. On the demographic variables of the patients, age, hypertension, hyperlipidemia, multiple infarction group and metabolic syndrome and dampness-phlegm group were significantly higher in the high CAVI score group than in the control. 2. According to the significant difference in the dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by CAVI score. As a result, dark circles, insomnia, headache, white coating tongue. slippery pulse, and rough pulse were significantly higher in the high CAVI score group then in the control. 3. In multivariate analysis, age, hypertension, multiple infarction and dampness-phlegm groups showed a close relationship with the high CAVI score group. Conclusions : According to the analysis, significance between dampness-phlegm diagnosed patients group and high CAVI score were clarified. Moreover, multiple location infarctions also have a relationship with high CAVI score in cerebral infarction patients. These results can be utilized in the future as a basis material.
