Efficacy and Safety of Bolus 5-Fluorouracil and L-Leucovorin as Salvage Chemotherapy for Oral Fluoropyrimidine-Resistant Unresectable or Recurrent Gastric Cancer: A Single Center Experience

  • Purpose: The International Organization for Standardization-5fluorouracil (FU) 10 trial found that bolus 5-FU and l-leucovorin was not inferior to S-1 in the treatment of gastric cancer (GC). Continuous 5-FU and the rapid injection of 5-FU have different anti-cancer effects. Thus, bolus 5-FU and l-leucovorin treatment might be useful for oral FU-resistant GC. Materials and Methods: We retrospectively analyzed the medical records of all patients with S-1 or capecitabine-resistant, unresectable, or recurrent GC treated with bolus 5-FU and l-leucovorin between January 2010 and December 2015 at Hokkaido University Hospital. The bolus 5-FU and l-leucovorin regimen consisted of intravenous l-leucovorin ($250mg/m^2/2h$) and bolus 5-FU ($600mg/m^2$) administered once weekly followed by a 2-week rest period; each cycle was repeated every 8 weeks. Results: A total of 14 patients were identified. The disease control rate was 35.7%. The median progression-free survival was 1.6 months (95% confidence interval [CI], 1.3~2.0 months), and the median overall survival was 6.3 months (95% CI, 4.7~7.9 months). No patient died from treatment-related causes. The most common severe adverse event associated with bolus 5-FU and l-leucovorin was neutropenia, which occurred in 21.4% of patients. Conclusions: Bolus 5-FU and l-leucovorin treatment might be useful for oral FU-resistant GC. We are planning a multi-center prospective phase II trial to evaluate the efficacy and safety of bolus 5-FU and l-leucovorin treatment for pre-treated unresectable or recurrent GC to confirm the results of this limited, retrospective study.

  • 1992년 8월에 발사된 한국의 첫번째 위성인 우리별 1호에는 저궤도 고에너지 입자환경를 관측하는 탑재체 cosmic ray experiment(CRE)가 실렸다. CRE는 크게 cosmic particle experiment(CPE)와 total dose esperiment(TDE) 두 시스템으로 구성된다. 이와 더불어 실험 탑재물은 아니지만, 프로그램 메모리와 램디스크(RAM disk)의 비트바뀜현상(SEU) 결과를 통해서 고에너지 입자환경에 대한 간접적인 정보를 얻을 수 있었다. 1993년 9월에 발사된 두 번재 위성 우리별 2호에는 OBCI인 KASCOM에서 프로그램 메모리와 램디스크 메모리에 대한 비트바뀜현상실험을 수행하였다. 우리별 1호와 2호는 그 구조는 비슷하지만 서로의 궤도가 다르기 때문에 두 위성의 실험결과를 분석해본다면 각 궤도에서의 고에너지 입자환경에 대한 비교연구할 수 있는 좋은 기회가 된다.

Investigating the effects of ultra-rapid, rapid vs. final precise orbit and clock products on high-rate GNSS-PPP for capturing dynamic displacements

  • The use of final IGS precise orbit and clock products for high-rate GNSS-PPP proved its effectiveness in capturing dynamic displacement of engineering structures caused by earthquakes. However, the main drawback of using the final products is that they are available after approximately two weeks of data collection, which is not suitable for timely measures after an event. In this study, the use of ultra-rapid products (observed part), which are available after a few hours of data collection, and rapid products, which are available in less than 24 hrs, are investigated and their results are compared to the more precise final products. The tests are designed such that harmonic oscillations with different frequencies and amplitudes and ground motion of a simulated real earthquake are generated using a single axis shake table and the PPP was used to capture these movements by monitoring time-change of the table positions. To evaluate the accuracy of PPP using ultra-rapid, rapid and final products, their results were compared with relative GNSS positioning and LVDT (Linear Variable Differential Transformer) data, treated as reference. The results show that the high-rate GNSS-PPP solutions based on the three products can capture frequencies of harmonic oscillations and dynamic displacement with good accuracy. There were slight differences between ultra-rapid, rapid and final products, where some of the tested events indicated that the latter two produced are more accurate and provide better results compared to the ultra-rapid product for monitoring short-term dynamic displacements.

  • Space Physics Sensor (SPS) on-board the KOMPSAT-1 consists of the High Energy Particle Detector (HEPD) and the Ionospheric Measurement Sensor (IMS). The HEPD is to characterize the low altitude high energy particle environment and the effects on the microelectronics due to these high energy particles. It is composed of four sensors: Proton and Electron Spectrometer(PES), Linear Energy Transfer Spectrometer (LET), Total Dose Monitor (TDM), and Single Event Monitor (SEM). 35 MeV proton beam from the medical KCCH cyclotron, at Korea Cancer Center Hospital in Seoul, is used to calibrate the PES. Primary proton beam of 35MeV scattered by polypropylene target is converted to various energy protons according to the elastic collision kinematics. In this calibration, the threshold level of the proton in the PES can be determined and the energy ranges of PES channels are also calibrated.

일개 보건지소에 내원한 급성기 후 코로나-19 환자들의 특성 및 한의 치료 효과 분석: 사례군 연구 (Effects of Korean Medicine Treatment at Public Health Center for Post-acute COVID-19 Patients: A Retrospective Case-series of 11 patients)

  • Objectives: The purpose of this study is to determine the clinical features of post-acute COVID-19 syndrome and the effectiveness and safety of Korean medicine treatment at the Public health center. Methods: The study was conducted among 11 patients with post-acute COVID-19 syndrome who attended a single public health center from January to December 2022. We retrospectively analyzed the charts of 11 patients and collected clinical characteristics, previous treatments, Korean medicine treatments, outcome variables (Numeral Rating Scale (NRS), Leicester Cough Questionnaire (LCQ), Visual Analog Scale (VAS)), adverse events, etc. Results: Of the 11 patients, six were women, and the average age of all patients was 68.27±12.31 years. The most common symptom were cough(n=9, 81.82%) and sore throat(n=9, 81.82%), followed by sputum, fatigue, rhinorrhea, and loss of appetite, etc. All 11 patients were treated with herbal medicine, with Samso-eum(n=6, 54.55%), Yeonkyopaedok-san(n=5, 45.45%), and Haengso-tang(n=3, 27.27%) being the most commonly used. After herbal medicine treatment, the median cough NRS decreased from 5 to 1, and the median sore throat NRS decreased from 4 to 1, both of which were statistically significant. One patient reported adverse event of dyspepsia and heartburn, but it was mild. Conclusions: The study presented the clinical features of the post-acute COVID-19 syndrome and suggested that Korean medicine treatment at public health centers may be effective and safe in alleviating associated symptoms.

Seismic behavior and design method of socket self-centering bridge pier with hybrid energy dissipation system

  • Seismic resisting self-centering bridge piers with high energy dissipation and negligible residual displacement after an earthquake event are focus topics of current structural engineering. The energy dissipation components of typical bridge piers are often relatively single; and exhibit a certain level of damage under earthquakes, leading to large residual displacements and low cumulative energy dissipation. In this paper, a novel socket self-centering bridge pier with a hybrid energy dissipation system is proposed. The seismic resilience of bridge piers can be improved through the rational design of annular grooves and rubber cushions. The seismic response was evaluated through the finite element method. The effects of rubber cushion thickness, annular groove depth, axial compression ratio, and lateral strength contribution ratio of rubber cushion on the seismic behavior of bridge piers are systematically studied. The results show that the annular groove depth has the greatest influence on the seismic performance of the bridge pier. Especially, the lateral strength contribution ratio of the rubber cushion mainly depends on the depth of the annular groove. The axial compression ratio has a significant effect on the ultimate bearing capacity. Finally, the seismic design method is proposed according to the influence of the above research parameters on the seismic performance of bridge piers, and the method is validated by an example. It is suggested that the range of lateral strength contribution ratio of rubber cushion is 0.028 ~ 0.053.

롤러신발과 일반신발의 착용 후 보행 시 하지근의 근전도 비교 (The Effects of Wearing Roller Shoes on Muscle Activity in The Lower Extremity During Walking)

  • The purpose of this study was to compare muscle activity in the lower extremity during walking wearing jogging and roller shoes. Twelve male middle school students (age: 15.0 yrs, height 173.7 cm, weight 587.7 N) who have no known musculoskeletal disorders were recruited as the subjects. Seven pairs of surface electrodes (QEMG8, Laxtha Korea, gain = 1,000, input impedance >$1012{\Omega}$, CMMR >100 dB) were attached to the right-hand side of the body to monitor the rectus femoris (RF), vastus medialis (VM), vastus lateralis (VL), biceps femoris (BF), tibialis anterior (TA), and medial (GM) and lateral gastrocnemius (GL) while subjects walked wearing roller and jogging shoes in random order at a speed of 1.1 m/s. An event sync unit with a bright LED light was used to synchronize the video and EMG recordings. EMG data were filtered using a 10 Hz to 350 Hz Butterworth band-passdigital filter and further normalized to the respective maximum voluntary isometric contraction EMG levels. For each trial being analyzed, five critical instants and four phases were identified from the recording. Averaged IEMG and peak IEMG were determined for each trial. For each dependent variable, paired t-test was performed to test if significant difference existed between shoe conditions (p<.05). The VM, TA, BF, and GM activities during the initial double limb stance and the initial single limb stance reduced significantly when going from jogging shoe to roller shoe condition. The decrease in EMG levels in those muscles indicated that the subjects locked the ankle and knee joints in an awkward fashion to compensate for the imbalance. Muscle activity in the GM for the roller shoe condition was significantly greater than the corresponding value for the jogging shoe condition during the terminal double limb stance and the terminal single limb stance. Because the subjects tried to keep their upper body weight in front of the hip to prevent falling backward, the GM activity for the roller shoe condition increased. It seems that there are differences in muscle activity between roller shoe and jogging shoe conditions. The differences in EMG pattern may be caused primarily by the altered position of ankle, knee, and center of mass throughout the walking cycle. Future studies should examine joint kinematics during walking with roller shoes.

Effects of total body irradiation-based conditioning on allogeneic stem cell transplantation for pediatric acute leukemia: a single-institution study

  • Purpose: To evaluate the effects of total body irradiation (TBI), as a conditioning regimen prior to allogeneic stem cell transplantation (allo-SCT), in pediatric acute leukemia patients. Materials and Methods: From January 2001 to December 2011, 28 patients, aged less than 18 years, were treated with TBI-based conditioning for allo-SCT in our institution. Of the 28 patients, 21 patients were diagnosed with acute lymphoblastic leukemia (ALL, 75%) and 7 were diagnosed with acute myeloid leukemia (AML, 25%). TBI was completed 4 days or 1 day before stem cell infusion. Patients underwent radiation therapy with bilateral parallel opposing fields and 6-MV X-rays. The Kaplan-Meier method was used to calculate survival outcomes. Results: The 2-year event-free survival and overall survival rates were 66% and 56%, respectively (71.4% and 60.0% in AML patients vs. 64.3% and 52.4% in ALL patients, respectively). Treatment related mortality rate were 25%. Acute and chronic graft-versus-host disease was a major complication; other complications included endocrine dysfunction and pulmonary complications. Common complications from TBI were nausea (89%) and cataracts (7.1%). Conclusion: The efficacy and toxicity data in this study of TBI-based conditioning to pediatric acute leukemia patients were comparable with previous studies. However, clinicians need to focus on the acute and chronic complications related to allo-SCT.

처오름의 신뢰성 해석에 대한 파고_주기결합분포의 영향 (Influence of Joint Distribution of Wave Heights and Periods on Reliability Analysis of Wave Run-up)

  • 경사식 방파제에서 파랑과 구조물의 상호작용에 의하여 발생되는 처오름에 대한 파고i주기결합분포의 영향을 해석할 수 있는 신뢰성 해석 모형이 제시되었다 처오름과 관련된 파괴모드의 정의에 따라 수면의 불확실성에 따른 영향을 해석할 수 있는 신뢰함수가 유도되었다. 특히 신뢰함수에 주기가 하나의 확률변수로 포함되어, 주기의 통계적 특성과 분포함수가 직접적으로 고려될 수 있을 뿐만 아니라 평균주기에 따른 파고분포의 거동특성이 조건부파고분포에 의해 올바로 고려될 수 있었다. 주기의 영향을 받지 않는 유의파고의 극치분포를 이용한 신뢰성 해석의 결과와 비교하여, 파고-주기결합분포를 이용한 신뢰성 해석이 극치분포를 이용한 신뢰성 해석보다 더 큰 파괴확률을 추정한다는 사실을 확인할 수 있었다. 따라서 중요한 구조물인 경우, 극치분포를 이용하여 경사식 방파제의 마루높이가 결정되더라도 단일 폭풍사상에 대하여는 파고-주기결합분포를 이용한 추가적인 해석이 필요하다. 한편 주기의 분포함수에 영향을 주는 계수에 따른 신뢰지수의 거동특성이 해석되었는데, 이에 따른 영향은 매우 작은 것으로 나타났다. 그러나 평균주기에 의한 파고분포의 거동에 따른 신뢰지수의 차이는 큰 것으로 나타났다 이는 파괴확률에 미치는 주기의 영향이 파고분포를 통하여 주로 발생된다는 것을 의미하는 것이다. 마지막으로 수면 변동 효과를 고려한 합리적인 마루높이 산정을 위해 마루높이의 변화에 따른 파괴확률을 산정하였다.

일강우를 고려한 SATEEC R 모듈 적용성 평가 (Evaluation of SATEEC Daily R Module using Daily Rainfall)

  • Soil erosion is an natural phenomenon. However accelerated soil erosion has caused many environmental problems. To reduce soil loss from a watershed, many management practices have been proposed worldwide. To develop proper and efficient soil erosion best management practices, soil erosion rates should be estimated spatially and temporarily. The Universal Soil Loss Equation (USLE) and USLE-based soil erosion and sediment modelling systems have been developed and tested in many countries. The Sediment Assessment Tool for Effective Erosion Control (SATEEC) system has been developed and enhanced to provide ease-of-use interface to the USLE users. However many researchers and decision makers have requested to enhance the SATEEC system for simulation of soil erosion and sediment reflecting effects of single storm event. Thus, the SATEEC R factors were estimated based on 5 day antecedent rainfall data. The SATEEC 2.1 daily R factor was applied to the study watershed and it was found that the R2 and EI values (0.776 and 0.776 for calibration and 0.927 and 0.911 for validation) with the daily R were greater than those (0.721 and 0.720 for calibration and 0.906 and 0.881 for validation) with monthly R, which was available in the SATEEC 2.0 system. As shown in this study, the SATEEC with daily R can be used to estimate soil erosion and sediment yield at a watershed scale with higher accuracy. Thus the SATEEC with daily R can be efficiently used to develop site-specific soil erosion best management practices based on spatial and temporal analysis of soil erosion and sediment yield at a daily-time step, which was not possible with USLE-based soil erosion modeling system.
