IMMEDIATE IMPLANT PLACEMENT AFTER EXTRACTION OF RETAINED DECIDUOUS TEETH AND IMPACTED CANINES: REPORT OF A CASE (상악 전치부 잔존 유치와 매복 견치 발치 후 즉시 임플란트 식립: 증례 보고)

  • Alveolar bone resorption after extraction impairs the necessary bone volume and complicates the case for implant surgery and aesthetic implant prosthesis. Immediate implant surgery after extraction decreases the number of surgical procedures and the duration of treatment, while allowing minimum alveolar bone resorption and preserving the residual bone volume. Although immediate implant holds many advantages such as preservation of hard and soft tissue around the extraction socket, greater implant survival rate and higher patient satisfaction, various complications and high failure rate are discouraging factors for the clinicians. In this case report, severe alveolar bone resorption with soft tissue changes were predicted after the extraction of prolonged retained deciduous incisors and impacted maxillary canines and thus decided on immediate implant procedure. Immediate implant surgery after extraction was carried out with minimal bone reduction and tapered wide-neck implant to establish initial stability. Simultaneous bone graft was done by filling the defect area with iliac cancellous bone with additional onlay-type bone graft and absorbable membrane on the labial bone for upper lip support. A stable and esthetic result was obtained with shortened treatment period.

Technique for Placing Continuous Media on a Disk Array under Fault-Tolerance and Arbitrary-Rate Search (결함허용과 임의 속도 탐색을 고려한 연속 매체 디스크 배치 기법)

  • 연속 매체, 특히 비디오 데이타에 대한 일반 사용자 연산에는 재생뿐만 아니라 임의 속도 탐색 연산, 정지 연산, 그리고 그 외 다양한 연산이 있다. 이 연산 중에서 원하는 화면을 빨리 찾는 데에 유용한 고속 전진(FF: fast-forward)과 고속 후진(FB: fast-backward)은 재생 연산과는 달리 비순차적인 디스크 접근을 요구한다. 이러한 경우에 디스크 부하가 균등하지 않으면 일부 디스크에 접근이 편중되어 서비스 품질이 떨어진다. 본 논문에서는 디스크 배열을 이용한 저장 시스템에서 디스크 접근을 고르게 분산시키기 위하여 '소수 라운드 로빈(PRR: Prime Round Robin)' 방식으로 연속 매체를 디스크에 배치하는 기법에서 문제가 됐던 낭비된 디스크 저장 공간을 신뢰도 향상을 위해서 사용하는 '그룹화된 패리티를 갖는 소수 라운드 로빈(PRRgp: PRR with Grouped Parities)' 방식을 제안한다. 이 기법은 PRR 기법처럼 임의 속도 검색 연산에 있어서 디스크 배열을 구성하는 모든 디스크의 부하를 균등하게 할뿐만 아니라 낭비됐던 디스크 저장 공간에 신뢰도를 높이기 위한 패리티 정보를 저장함으로서 신뢰도를 향상시킬 수 있다. 신뢰도 모델링 방법으로 조합 모델과 마르코프 모델을 이용해서 결함발생율과 결함복구율을 고려한 신뢰도를 산출하고 비교.분석한다. PRR 기법으로 연속 매체를 저장하고 낭비되는 공간에 패리티 정보를 저장할 경우에 동시에 두 개 이상의 결함 발생 시에 그 결함으로부터 복구가 불가능하지만 PRRgp 기법에서는 약 30% 이상의경우에 대해서 동시에 두 개의 결함 발생 시에 저장한 패리티 정보를 이용한 복구가 가능할 뿐만 아니라 패리티 그룹의 수가 두 개 이상인 경우에는 두 개 이상의 결함에 대해서도 복구가 가능하다.Abstract End-user operations on continuous media (say video data) consist of arbitrary-rate search, pause, and others as well as normal-rate play. FF(fast-forward) / FB(fast-backward) among those operations are desirable to find out the scene of interest but they require non-sequential access of disks. When accesses are clustered to several disks without considering load balance, high quality services in playback may not be available. In this paper, we propose a new disk placement scheme, called PRRgp(Prime Round Robin with Grouped Parities), with enhanced reliability by using the wasted disk storage space in an old one(PRR: Prime Round Robin), in which continuous media are placed on a disk array based storage systems to distribute disk accesses uniformly. The PRRgp can not only achieve load balance of disks consisting of a disk array under arbitrary-rate search like PRR, but also improve reliability by storing parity information on the wasted disk space appropriately. We use combinatorial and Markov models to evaluate the reliability for a disk array and to analyze the results. When continuous media like PRR are placed and parity information on the wasted disk space is stored, we cannot tolerate more than two simultaneous faults. But they can be recovered by using stored parity information for about 30 percent as a whole in case of PRRgp presented in this paper. In addition, more than two faults can be tolerated in case there are more than two parity groups.

  • 1. 목적 Osteotome 상악동거상술(Bone Added Odteotome Sinus Floor Elevation ; 이하 BAOSFE) 과 동시에 식립한 임프란트($Br{\aa}nemark$, ITI)의 예상 생존율에 대해 현재까지 정확히 알려진 바는 없었으며, $Br{\aa}nemark$ Ti-Unite 과 ITI SLA 임프란트의 표면에 대한 비교 연구 또한없었다. 이번 연구는 BAOSFE 술식과 동시에 식립한 $Br{\aa}nemark$ Ti-Unite 과 ITI SLA 임프란트의 임상 결과를 비교, 평가하고 초기 치유기간 동안의 이식골 높이의 변화를 방사선학적으로 관찰하여 두 가지 임프란트 시스템을 비교해 보고자 한다. 2. 방법 위축된 상악 구치부를 갖는 22명의 환자를 대상으로, BAOSFE술식과동시에 $Br{\aa}nemark$ Ti-Unite(11명, 13 임프란트)임프란트와 ITI SLA(11명, 18 임프란트)임프란트를 식립하였다. 수술 전, 임프란트 식립 직후, 술후 6개월의 파노라마 방사선 사진을 촬영하여 비교 및 평가에 사용하였다. 각 임프란트 시스템의 생존율을 측정하고, 술전 상악동저 높이와 식립된 임프란트 길이를 참고하여 이식골 높이의 방사선학적 변화를 평가하였다. 3. 결과 평균12개월의 추적기간 결과, $Br{\aa}nemark$ Ti-Unite 임프란트의 생존율은 100%(13/13 임프란트)이었으며, ITI SLA 임프란트의 생존율은 94.4%(17/18 임프란트)이었다. 초기 치유 기간인 6개월 동안 평균 이식골 높이의 감소는 $Br{\aa}nemark$ Ti-Unite 임프란트에서 0.67mm(10.73%), ITI SLA 임프란트에서는 0.55mm(8.18%)로 나타났다. 두 가지 임프란트 시스템 간의 유의성 있는 차이는 보이지 않았다. 4. 고찰 BAOSFE 술식과 동시식립한 $Br{\aa}nemark$ Ti-Unite 과 ITl SLA 임프란트는 위축된 상악 구치부를 갖는 환자에서 효과적인 치료방법이 될 수 있으며, 임프란트 표면에 따른 이식골의 치유 반응은 두 가지 임프란트 시스템에서 유사한 양상으로 일어남을 알 수 있었다.

Autogenous Shrinkage of Very-Early Strength Latex-Modified Concrete with Latex Contents (라텍스함량 변화에 따른 VES-LMC의 자기수축)

  • Very-early strength latex-modified concrete (VES-LMC) was developed for the purpose of fast-track overlay of a concrete bridge deck under heavy traffic, concentrated on the workability, durability, and strength gain so that it can be opened to the traffic only three hours after its placement. The mixture of VES-LMC might accompany very high heat of hydration at early-age because of its inherent rapid hardening property and could have susceptibility to autogenous shrinkage because of its relatively low water-cement ratio. This study evaluated the effect of the latex-cement ratio(L/C) both of the constant and variable slumps on the autogenous shrinkage of VES-LMC by carrying out simple temperature rise test and early-age shrinkage experiment. Test results are as follows: The latex contributes on the enhancement of the concrete durability but has little effect on its hydration and the accompanied heat of hydration in VES-LMC. Autogenous shrinkage increased with the increase in latex-cement ratio at variable slumps and its pattern followed regularly a logarithmic increase. However, the influence of water-cement ratio and latex-cement ratios for the test specimens at constant slump on early-age autogenous shrinkage property was found to be minor due to the simultaneous effect of the two experimental variables.

Posterior Lumbar Interbody Fusion Using Posterolateral Placement of A Single Cylindrical Threaded Cage and Two Regular Cages : A Biomechanical Study (단일 나사형 Cage를 이용한 후방 요추체간 융합술과 두개의 나사형 Cage를 이용한 PLIF의 생체 역학적 비교)

  • Objectives : An in vitro biomechanical study of posterior lumbar interbody fusion(PLIF) with threaded cage using two different approaches was performed on eighteen functional spinal units of bovine lumbar spines. The purpose of this study was to compare the segmental stiffnesses among PLIF with one long posterolateral cage, PLIF with one long posterolateral cage and simultaneous facet joint fixation, and PLIF with two posterior cages. Methods : Eighteen bovine lumbar functional spinal units were divided into three groups. All specimens were tested intact and with cage insertion. Group 1(n=12) had a long threaded cage($15{\times}36mm$) inserted posterolaterally and oriented counter anterolaterally on the left side by posterior approach with left unilateral facetectomy. Group 2(n=6) had two regular length cages($15{\times}24mm$) inserted posteriorly with bilateral facetectomy. Six specimens from group 1 were then retested after unilateral facet joint screw fixation in neutral(group 3). Likewise, the other six specimens from group 1 were retested after fixation with a facet joint screw in an extended position(group 4). Nondestructive tests were performed in pure compression, flexion, extension, lateral bending, and torsion. Results : PLIF with a single cage, group 1, had a significantly higher stiffnesses than PLIF with two cages, group 2, in left and right torsion(p<0.05). Group 1 showed higher stiffness values than group 2 in pure compression, flexion, left and right bending but were not significantly different. Group 3 showed a significant increase in stiffness in comparison to group 1 for pure compression, extension, left bending and right torsion(p<0.05). For group 4, the stiffness significantly increased in comparison to group 1 for extension, flexion and right torsion(p<0.05). Although there was no significant difference between groups 3 and 4, group 4 had increased stiffness in extension, flexion, right bending and torsion. Conclusion : Posterior lumbar interbody fusion with a single long threaded cage inserted posterolaterally with unilateral facetectomy enables sufficient decompression while maintaining a majority of the posterior elements. In combination with a facet joint screw fixation, adequate postoperative stability can be achieved. We suggest that posterolateral insertion of a long threaded cage is biomechanically an ideal alternative to PLIF.
