The Short Term Effects of Ankle Strengthening Emphasis with Jumping on Strength, Mechanical Properties, and Balance with and without Wearing High Heel in Ankle Instability

  • Purpose: The purpose of this study was to investigate the short term effects of ASEJ (ankle strengthening with emphasis on jumping) for 3weeks on strength, mechanical properties, and balance and to compare the balance with and without HH(high-heel) condition. Methods: ASEJ (a combined exercise of squat, heel raise up, and jumping) were performed for the subjects in 11 female ankle instability young females ($21.7{\pm}2.0yrs$ Cumberland ankle instability score $19{\pm}6.5$). To investigate the effect of ASEJ, investigator used dynamometer for measuring strength, MyotonPRO for measuring mechanical properties(tone, stiffness, and elasticity of the muscles), I-Balance test for static balance, and Y-balance test for dynamic balance between the condition with and without HH condition. All data were normally distributed and analyzed using the SPSS 22.0 statistical program. Comparing pre- and post-intervention and the condition with and without HH conditions data were examined using the paired t-test. The level of significance was chosen as 0.05 for all the analyses. Results: 3wks of ASEJ would strengthen leg muscles and increasing muscle tone and stiffness in most muscles however there was decreasing muscle elasticity of gastrocnemious. In addition, the ASEJ improves the static balance for ankle instability young females and increases the dynamic balance when wearing the heels especially. Conclusions: the ASEJ could recommend to improve the strength and balance for ankle instability young females. Also, measuring the balance with HH conditions well represents the risk of ankle damage in female.

Glenohumeral versus subacromial steroid injections for impingement syndrome with mild stiffness: a randomized controlled trial

  • Background: The subacromial (SA) space is a commonly used injection site for treatment of impingement syndrome. For shoulder stiffness, glenohumeral (GH) injections are commonly performed. However, in cases of impingement syndrome with mild shoulder stiffness, the optimal site of steroid injection has yet to be identified. Methods: This prospective, randomized study compared the short-term outcomes of ultrasound-guided GH and SA steroid injections in patients who were diagnosed with impingement syndrome and mild stiffness. Each group comprised 24 patients who received either a GH or SA injection of 40 mg of triamcinolone. Range of motion and clinical scores were assessed before and 3, 7, and 13 weeks after the injection. Results: GH and SA injections significantly improved the range of motion and clinical scores after 13 weeks of follow-up. Notably, targeting the GH joint resulted in an earlier gain of forward elevation, external rotation, and internal rotation in 3 weeks (P<0.001, P=0.012, and P=0.002, respectively) and of internal rotation and a Constant-Murley score in 7 weeks (P<0.001 and P=0.046). Subsequent measurements were similar between the groups and showed a steady improvement in all ranges of motion and clinical scores. Conclusions: GH injections may be more favorable than SA injections for treatment of impingement syndrome with mild stiffness, especially in improving the range of motion in the early period. However, the procedures showed similar outcomes after 3 months. Level of evidence: I.

Effective Length of Reinforced Concrete Columns in Braced Frames

  • The American Concrete Institute (ACI) 318-11 permits the use of the moment magnifier method for computing the design ultimate strength of slender reinforced concrete columns that are part of braced frames. This computed strength is influenced by the column effective length factor K, the equivalent uniform bending moment diagram factor $C_m$ and the effective flexural stiffness EI among other factors. For this study, 2,960 simple braced frames subjected to short-term loads were simulated to investigate the effect of using different methods of calculating the effective length factor K when computing the strength of columns in these frames. The theoretically computed column ultimate strengths were compared to the ultimate strengths of the same columns computed from the ACI moment magnifier method using different combinations of equations for K and EI. This study shows that for computing the column ultimate strength, the current practice of using the Jackson-Moreland Alignment Chart is the most accurate method for determining the effective length factor. The study also shows that for computing the column ultimate strength, the accuracy of the moment magnifier method can be further improved by replacing the current ACI equation for EI with a nonlinear equation for EI that includes variables affecting the column stiffness and proposed in an earlier investigation.

Short-term Low-dose Oral Corticosteroid Therapy of Impingement Syndrome of the Shoulder: A Comparison of the Clinical Outcomes to Intra-articular Corticosteroid Injection

  • Background: To assess the clinical outcomes of short-term oral corticosteroid therapy for impingement syndrome of the shoulder and determine whether it can be substituted as an alternative to the intra-articular injection. Methods: The clinical outcomes of the 173 patients, the oral steroid group (n=88) and the injection group (n=85), were measured at 3 weeks, 2, 4, and 6 months postoperatively. The clinical outcomes were assessed by measuring the the University of California at Los Angeles (UCLA) score, visual analog scale (VAS) and range of motion (ROM) at every follow-up. Any complications and recurrence rate were noted. A relationship between the treatment outcomes and factors such as demographic factors, clinical symptoms and radiographic findings were determined. Results: No difference was observed in VAS and UCLA scores between the two groups, but forward flexion and internal rotation of ROM were significantly improved in the injection group at the 2nd and 4th postoperative month (p < 0.05). At 6th postoperative month, recurrence rate of symptoms was 26% in the oral steroid group and 22% in the injection group. No major adverse effects were observed. When the clinical outcomes of the oral steroid group were compared to either demographic, clinical symptoms, or radiographic findings, UCLA score was found to be significantly low (p < 0.05) in patients with joint stiffness and UCLA score, whereas VAS score was significantly improved in patients with night pain (p < 0.05). Conclusions: Short-term low-dose oral corticosteroid therapy of impingement syndrome showed comparable clinical outcomes to intra-articular injection without any remarkable adverse effects. Low-dose oral steroids can be regarded as a partial alternative to intra-articular injection for the initial therapy of impingement syndrome of the shoulder.

  • Damage and overbreak of the remaining rock induced by blasting can not be avoided during tunnel construction which may result in either short-term or long-term tunnel instability. Therefore, in this paper, a methodology to take into account the effect of blast-induced damage in tunnel stability assessment is proposed. Dynamic numerical analysis was executed to evaluate damage and overbreak of the remaining rock for the most common blasting pattern in road tunnel. Rock damage was quantified by utilizing the damage variable factor which is adopted proposed in continuum damage mechanics. The damaged rock stiffness and the damaged failure criteria are used to consider the effect of rock damage in tunnel stability analysis. The damaged geological strength index of the damaged rock was newly proposed from the relationship between deformation modulus and geological strength index. Also the Hoek-Brown failure criteria of the damaged rock was obtained using the damaged geological strength index. Analysing the tunnel stability with the consideration of the blast-induced damage of remaining rock, it was found that the extend of plastic zone and deformation increased compared to the undamaged rock. Therefore the short-term or long-term tunnel stability will be threatened when the rock damage from blasting is ignored in the tunnel stability analysis.

Continuous Half Passive Motion under Distracted External Fixation for the Treatment of Distal Tibial Pilon Fractures (신연 외고정 및 지속적 반수동 운동을 이용한 경골 원위부 필론 골절의 치료)

  • Purpose: Pilon fracture has several serious complications such as joint stiffness, arthrosis and delayed angular deformity. We report short-term results of new treatment modality using distracted dynamic external fixators and early controlled ankle motion. Materials and Methods: Eight cases of severe pilon fractures for which we tried small plate fixation and additional distracted dynamic external fixators from July 2007 to June 2009 were included. Half passive continuous ankle joint motion was allowed under free hinged ring fixators after the operation. The external fixators were removed after two or three months from the surgery. We investigated joint space by radiograph, joint pain, range of motion, patient's satisfaction of treatment protocol. Results: Joints were distracted when external fixators were applied and mean 28% of space loss developed after removal of external fixators. In most of cases, satisfactory alignments were maintained. Regarding range of joint motion, mean dorsiflexion angle was 15 degrees and mean plantarflexion angle was 32 degree in the condition of wearing external fixators. There was mean 8% reduction of range of motion but no further progression of ankle stiffness after removal of external fixators. Dorsiflexion was not improved after that, but plantarflexion angle was improved 10% even after removal of external fixators. Patients were generally in compliance with the treatment protocols with high level of satisfaction. Conclusion: We got good results with distracted dynamic external fixators and early continuous half-passive joint motion for pilon fractures in terms of joint pain and range of motion. Therefore we suggest this new protocol as an alternative modality for severe pilon fractures.

Elevation of heart-femoral pulse wave velocity by short-term low sodium diet followed by high sodium diet in hypertensive patients with sodium sensitivity

  • BACKGROUND/OBJECTIVES: We compared changes in heart-femoral pulse wave velocity (hfPWV) in response to low sodium and high sodium diet between individuals with sodium sensitivity (SS) and resistance (SR) to evaluate the influence of sodium intake on arterial stiffness. SUBJECTS/METHODS: Thirty-one hypertensive and 70 normotensive individuals were given 7 days of low sodium dietary approach to stop hypertension (DASH) diet (LSD, 100 mmol NaCl/day) followed by 7 days of high sodium DASH diet (HSD, 300 mmol NaCl/day) during 2 weeks of hospitalization. The hfPWV was measured and compared after the LSD and HSD. RESULTS: The hfPWV was significantly elevated from LSD to HSD in individuals with SS (P = 0.001) independently of changes in mean arterial pressure (P = 0.037). Conversely, there was no significant elevation of hfPWV from LSD to HSD in individuals with SR. The percent change in hfPWV from the LSD to the HSD in individuals with SS was higher than that in individuals with SR. Subgroup analysis revealed that individuals with both SS and hypertension showed significant elevation of hfPWV from LSD to HSD upon adjusted analysis using changes of the means arterial pressure (P = 0.040). However, there was no significant elevation of hfPWV in individuals with SS and normotension. CONCLUSION: High sodium intake elevated hfPWV in hypertensive individuals with SS, suggesting that high sodium intake increases aortic stiffness, and may contribute to enhanced cardiovascular risk in hypertensive individuals with SS.

Effect of Manual Therapy on a Patient With Atlantoaxial Rotatory Subluxation (환축추 회전 아탈구 환자에 대한 도수치료 효과)

  • Background: An 8-year-old girl had severe neck pain and stiffness after trauma. CT scan showed atlantoaxial rotatory subluxation (AARS). She had conservative treatment because she did not have neurological symptoms and spinal basilar artery dysfunction. Conservative therapy was halter traction twice for 4 weeks. However, pain and stiffness persisted. She had been recommended to have surgery from her physician, but she received manual therapy for non-surgical procedures. Methods: The joint mobilization, muscle energy technique, motor control exercise, and deep neck flexor (DNF) endurance exercise were applied as manual therapy and 10 session for 2weeks. Results: Clinical outcomes were measured at initial baseline, after 2 weeks, and after 6weeks. Active range of motion was completely restored after 6weeks and numeric pain rating scale was completely reduced after 2 weeks. The strength of neck flexor muscle recovered to normal after 2 weeks, and the DNF endurance was improved to 25 seconds after 2 weeks and to 42 seconds after 6weeks. Motor control capacity recovered to 30 ㎜Hg after 2 weeks. Conclusions: This case report describes the immediate and short-term clinical outcomes for a patient presenting with symptoms of neck pain following AARS. Clinical rationale and patient preference aided the decision to incorporate manual therapy as a treatment for this patient. Manual therapy has shown a successful recovery in AARS patients, more research is needed to validate the inference of this case report.

  • In this study, we introduced the spiral pipe nailing system (refer as SPN system) with self drilling method, can apply to ground which is hard to keep shape of bore hole, and performed limit equilibrium analysis with simplilied trial wedge method while length ratio and bond ratio were altered to evaluate slope stability considered of tensile strength and bending stiffness. A newly soil nailing system named as the SPN system is respected to reduce displacement of nail and increase global slope stability. And effects of various factors related to the design of the SPN system, such as the type of drilling method and the bit, are examined throughout a series of the displacement-controlled field pull-out tests. 6 displacement-controlled field pull-out tests are performed in the present study and the volume of grouting arc also evaluated based on the measurements. In addition, short-term characteristics of pull-out deformations of the newly proposed SPN system are analyzed and compared with those of the general soil nailing system by carrying out the displacement-controlled field pull-out tests.

  • Objective : The aims of this study were to show the clinical state and to investigate depression and anxiety by BDI and STAI scale in tension headache patients. Method : The patient group was consisted of the 20 patients with Tension Headache who were treated in Department of Neuropsychiatry, Daejeon University Oriental Hospital from 3 March 2003 to 30 September 2003. The control group was consisted of the 20 patients on physical therapy with cerebaral vascular disorder in the Daejeon University Cheonan Oriental Hospital. The patient group was investigated by various characteristics, and compared with the control group by BDI and STAI scores. Result : 1. The ratio of female was higher, the 40 aged were higher frequence, in distribution of the period of the clinical history, short term within 1 week was the most.. 2. Tension headache was most frequent at whole portion, neck stiffness, shoulder pain, general body weakness were mainly coexited, and the prescription invigorating spleen supplementing qi and tonifying qi and blood. 3. The BDI score was higher in patient group, but there was no significant difference 4. The STAI score was higher in patient group, but there was no significant difference.
