Effect of Lepidium spp. (Maca) Extract on the Improvement of Sexual Function in Rats Induced Erectile Dysfunction with Ethanol (에탄올로 발기부전을 유도한 흰쥐의 성기능 개선에 마카추출물이 미치는 영향)

  • The purpose of this study was to examine the effects of Maca water and/or ethanol extract on the nitric oxide (NO) production in human umbilical vein endothelial cells HUVAC and on erectile dysfunction in rats. Maca was extracted due to both solutions, which are water and ethanol. Each Maca extract was applied to HUVAC, and NO production was checked. Additionally, three different dosages (250, 500 and 1,000 mg/kg) of Maca ethanol extract was administered to Sprague-Dawley rats for 4 weeks. All rats were sacrificed and each sample was collected for analysis. The control rats received only the saline vehicle. The NO production of HUVAC was significantly increased by domestic and homemade Maca water extracted at $60^{\circ}C$ group. Both NO generation and testosterone release were not influenced due to the oral administration of Maca. In the EtOH group rats, the number of sperm was reduced compared to that of the control group. All Maca groups had a high number of sperm and each sperm count had increased as a result of the Maca extract dose. The results of this research suggest that Maca has a positive effect on male erectile dysfunction, which need to be examined further in future studies.

Clincal Efficacy of Korean Red Ginseng for Erectile Dysfunction -Multi-national Approach

  • Ginseng has been used in maintaining physical vitality all over the East Asian countries and recently its metabolism and actions on neurologic, cardiovascular and endocrinologic systems are being elucidated. Korean red ginseng (KRG) has been used in various aliments, and to prove its efficacy icy erectile dysfunction an international study on Asians other than Korean was performed. Patients with borderline organic and psychogenic erectile dysfunction were included. KRG were given daily, and placebos were given as controls. Treatment lasted a total of 3 months. Surveys including libido, erectile potency, the sexual satisfactions were given. Serum testosterone and erectile function study were taken. Among the 23 patients with KRG, 18 patients were followed. Four had diabetes, 2 hypertension, 3 hyperchole, iterolemia, 1 low testosterone,4 psychogenic, and 4 idiopathic. In 10 patients with placebo, 7 were followed for more than three months. The clinical efficacy of KRG was 66.7% on objective Questionnaire and 72.2% on subjective analysis. When KRG were given, all parameters surveyed have shown improvements compared to the controls. Serum testosterone Bevels were normalized in 2 patients with KRG, whose serum testosterone levels were reduced from prestudy. When the erectile functions after audiovisual stimuli evaluated using Rigiscan in 6 patients with KRG,4 showed rigidity move than 70oyc. One patient reported constipation, and 2 gastric up-sets in the KRG group. In conclusion, KRG has beneficiary action on male erectile capabilities with minimal side effects. Thus KRG has been proven effective in Koreans, and a result on other Asians is pending. The exact action mechanism and the active ingredients in KRC need to be itudied.

Multiple Endocrinologic Complications in Thalassemia Major

  • Thalassemia major is a genetic disorder with a defective synthesis of either the alpha or the beta chain of hemoglobin A. Blood transfusion is crucial for the survival in these patients. Unfortunately, endocrine dysfunction is a very common complication in these patients and is principally due to excessive iron overload as a result of frequent blood transfusions. Although regular blood transfusion may increase life expectancy, disturbances in growth and pubertal development, abnormal gonadal functions, impaired thyroid, parathyroid and adrenal functions, diabetes, and disorderly bone growth are common side effects. We hereby present a case of a 23-year-old, unmarried woman with beta thalassemia major presenting with primary amenorrhea, poor development of secondary sexual character, and short stature. Thorough history, clinical examination, and laboratory investigation, including dynamic function test (insulin tolerance test) were conducted. These tests confirmed that she had multiple endocrinopathies, including hypogonadotropic hypogonadism, growth hormone deficiency, and subclinical adrenal insufficiency, which were caused by iron overload. She required hormone replacement therapy. Early recognition of possible deficiencies in hypothalamo-pituitary-end organ hormones caused by iron overload in thalassemia patients that undergo frequent blood transfusion procedures is essential. Appropriate treatments, including transfusion regimen and chelation therapy, as well as specific treatment of each complication are the crucial for the successful management and improvement of quality of life these patients.

  • Piriformis Syndrome is characterized by pain of the low back, groin, perineum, buttock, hip, posterior thigh, leg and foot. Symptoms are aggravated by sitting, prolonged combination of hip flexion, adduction, the medial rotation, or by activity. In addition, patient may complain of painful swelling of the limb and sexual dysfunction-dyspareunia in female, and impotence in male. It currently appears that three specific conditions may contribute to the piriformis syndrome: (a) myofascial pain; (b) nerve and vascular entrapment; (c) dysfunction of the sacroiliac joint. The important keys of diagnosis are history and physical examination. There is no known objective diagnostic method. We described the clinical features of four cases of piriformis syndrome and reviewed foreign literature.

  • Background : Recent discoveries on the physiology of an erection have demonstrated that the organic causes of impotence are more common, and psychogenic impotence is correspondingly less common than was formally believed. The incidence of sexual dysfunctions in chronic obstructive pulmonary disease (COPD) patients is largely unknown or may be perfunctorily attributed to the associated illness or to aging. This study investigated whether or not the impotence was related to the COPD itself as well as whether or not it nay stem from organic causes in a notable proportion of such patients. Methods : The sexual function was evaluated in 10 COPD patients and 10 normal control subjects. A nocturnal Rigi Scan was performed to evaluate the erectile function of each group. The level of hormones such as the free testosterone, prolactin and thyroid stimulating hormone (TSH) was measured, and a pulmonary function test and arterial blood gas analysis was performed. Results : The time duration and frequency of a penile erection were significantly lower in COPD patients than the controls (p<0.05). In addition, the $PaO_2$ levels correlated with the time duration of the penile erection. Conclusion : These results suggest that COPD is one of the causes of organic erectile dysfunction.

Premature Ejaculation and Erectile Dysfunction in Iranian Prostate Cancer Patients

  • Background: To investigate the prevalence of premature ejaculation (PE) and erectile dysfunction (ED) in a sample of patients with prostate cancer and to determine the utility of the previously suggested cutoffs of the Premature Ejaculation Diagnostic Tool (PEDT) for the diagnosis of PE and that of International Index of Erectile Function (IIEF-5) for ED. Materials and Methods: A total of 1,202 men with prostate cancer were invited from urology clinics at the universities of Iran, Tehran, Qazvin, Ahvaz, Guilan and Tabriz. Clinical characteristics were collected through medical records. PE and ED diagnoses were made by trained urologists. In addition to the clinical diagnoses, PE and ED were measured through self-report using the PEDT and the IIEF-5. Questionnaire cutoff scores were determined using receiver operating characteristic (ROC) curves and confirmed by predictive ability using logistic regression. Results: The prevalence of PE was 63.7% and that of ED was 66.2%. Prevalences of PE decreased and that of ED increased with advanced TNM stages. According to ROC, the suggested cutoff for the PEDT to diagnose a PE was ${\geq}11$ (sensitivity=0.988, 1-specificity=0.084, and predictive ability=0.914) and ${\leq}17$ for the IIEF-5 (sensitivity=0.966, 1-specificity=0.031, and predictive ability=0.967). Conclusions: Prevalence of sexual problems was high in prostate cancer patients in Iran, therefore oncologists should take into account these potential problems when deciding on treatment modalities.

Association between Perceived Periodontal Status and Sexual Function in Adult Men (성인 남성의 주관적 치주건강상태와 성기능의 관련성)

  • The aim of this study was to investigate the association between perceived periodontal status and sexual function among adult men. Three hundred thirty adult male participated in this study living at Changwon and Jinju. All the participants surveyed the socio-demographic characteristics, perceived periodontal status, stress, and international index of erectile function (IIEF) using self-administrated questionnaire. The univariate and multivariate analyze were adapted to access the crude and adjusted associations using PASW Statistics 18.0. In univariate analysis, there were significant differences between perceived periodontal status and IIEF and most of socio-demographic characteristics (p<0.05), but no significant differences between stress and all of socio-demographic characteristics (p>0.05). The sexual function was negatively correlated with perceived periodontal status (r=-0.228, p<0.001) and stress (r=-0.257, p<0.001), respectively. In multivariate analysis, perceived periodontal status was significantly associated with sexual function (b=-0.889, p=0.030) after adjusting for socio-demographic characteristics and stress. Therefore, it is found that perceived periodontal status negatively influenced on sexual function in adult men.

Depression, Anxiety and Sexual Satisfaction in Breast Cancer Patients and their Partners-Izmir Oncology Group Study

  • Background: We aimed to investigate anxiety, depression and sexual satisfaction levels and the effects of depression and anxiety upon the sexual satisfaction of Turkish breast cancer patients and their partners. Materials and Methods: Data were collected from one hundred breast cancer patients and their partners, using three forms: one covering information about socio-demographic characteristics of the patients, the Hospital Anxiety and Depression Scale (HADs) and the Golombok-Rust Inventory of Sexual Satisfaction (GRISS). Results: The frequencies, avoidance and touch subscores were statistically significantly high in the patients. Among those with high anxiety scores, the frequency, communication, satisfaction, touch, and anorgasmic subscale scores of GRISS were found to be significantly high. Among the partners whose anxiety scores were high, only the premature ejaculation subscale was statistically significant. It was determined that for partners with higher depression scores, the communication, satisfaction, avoidance, premature ejaculation and erectile dysfunction subscores of GRISS were statistically higher compared to partners with lower depression scores. Conclusions: Patients' quality of life may be increased by taking precautions to reduce their and their partners' psychosocial and psychosexual concerns.

Pharmalogical Effects and Toxicity of Licorice (감초의 효능과 독성)

  • Licorice has been wed in clinical medicine for thousands of years. However it is only in recent times that we have been able to employ scientific methods to prove its efficacy and to give us a better understanding of its mechanism of action. One of important mechanisms for its efficacy is related to mineralocorticoid activity increased by glycyrrhizic acid, the active ingredient in licorice. Also the main undesirable side-effects of Licorice relate to is mineralocorticoid activity resulting in a state of apparent mineralocorticoid excess (AME). These therapeutic and undesirable effects are explained by the inhibition of 11-$\beta$-hydroxysteroid dehydrogenase (11-$\beta$-HSD) activity. Recently, the reduction of serum testosterone in men by licorice was reported which would have important health implications in the context of fertility and sexual dysfunction. Here, health implication of licorice were reviewed In term of its pharmacodynamic and toxicodynamic mechanism.

The Needs of Sex Education for Recovering Male Alcoholics (회복중인 남성알코올중독자의 성교육 요구)

  • Purpose: This study was conducted in order to identify the needs of sex education for recovering male alcoholics. Methods: The participants were 17 male alcoholics. Data were collected through focus group interviews from August 2010 to February 2011, and the data were analyzed using thematic content analysis. Results: The results of the analysis were 4 categories and 10 sub-categories. The 4 categories were 'Lack of sex knowledge', 'Effects of alcohol on sex life', 'Strategies to cope with sexual dysfunction', and 'Relationship and conflict with women'. Conclusion: The findings of this study described that education for sex knowledge and safe sex life is necessary for preventing the relapse of alcoholism and supporting recovery of male alcoholics.
