  • Objective : As prevalence of atopic dermatitis(AD) rises and medicine of atopic dematitis develops, the methods of measurement for atopic dermatitis have developed. Therefore, we aimed to establish a new sconring system in oriental medicine for a childhood atopic dematitis, by comparison, analysis of various sconing systems. Method : We searched for the data related to severity sconing systems of atopic dermatitis in Entrez PubMed on abroad thesis and KISS(Korenstdued Information Service System) on domestic thesis from 1990 to 2005. Result : Severity sconing systems for atopic dermatitis satisfy validity, reliability, sensitivity of change and verification of this. The essential items of severity sconing systems are extent, intensity and subjective symptoms. The lesion is divided into 9 areas(head and neck, upper limbs, elbow, hands, trunk, hip and inguinal, lower limbs, knee, feet). The items of intensity are erythema, oedema and induration and papulation, oozing and weeping and erosion, excorition, scaling, lichenification. The subjective symptom is pruritis, evaluated according to sleep loss and obstruction of play and study. The extent and intensity is evaluated in combination. We suggested that extent and intensity occupied for 70% and subject symptoms accounted for 30% of total score.

  • This study proposes CAOPI(Computer Aided Organ Prediction Index) system based on APACHE II(Acute Physiology And Chronic Health Evaluation) for classifying disease severity and predicting the conditions of patients' major organs. The existing ICU disease severity evaluation is mostly about calculating risk scores using patients' data at certain points, which has limitations on making precise treatments. CAOPI system is designed to provide personalized treatments by classifying accurate severity degrees of emergency patients, predicting patients' mortality rate and scoring the conditions of certain organs.

  • Purpose: Many patients are injured by trauma. And some of them expire due to severity of trauma. Various scoring systems have been introduced in grading severity and predicting mortality of trauma patients. This study is to evaluation the usefulness of factors for determining the severity and predicting the prognosis of the trauma victims. Methods: Data on the patients who visited our Emergency departments from January 2010 to December 2011 were retrospectively reviewed using electronic medical records. The patients were activated severe trauma team calling system. The patients were categorized as survivors and non-survivors. Univariated associations were calculated, and a multiple logistic regression analysis was used to determine variables associated with hospital mortality. Results: Two hundred sixty two(262) patients were enrolled, and the mortality rate was 25.6%. By multivariate analysis, lower respiration rate, lower Glasgow Coma Score, higher International Normalized Ratio and emergency transfusion within 6 hours were expected as severity and prognosis predict factors (each of odds ratio were 24.907, 14.282, 2.667 and 16.144). Conclusion: As predict factors, respiration rate, Glasgow Coma Score, International Normalized Ratio and emergency transfusion, are useful determining the severity and predicting prognosis of trauma victims.

  • In this paper, we proposed the DFMEA Implementation Method for safety design of Weapon System. First, we presented the process for DFMEA. And then, the case analysis of OOO missile was performed in accordance with the process presented. After defining the system requirements of OOO missile, failure definition scoring criteria was set. In order to clarify the definition of failure, the failure was classified into safety, reliability, maintainability and others. After performing the function analysis, the relationship matrix analysis was performed to identify the failure mode according to the function without omission. After clarifying the failure classification, mode of failure, cause of failure and effect were analyzed to calculate the severity, occurrence and detection values. After the action priority was judged, the recommended action according to the failure classification was identified for the determined action priority. The results of this study can be used as a relevant basis for the design reflection and resource re-allocation of stakeholders.

  • Purpose: We compared the Korean Triage and Acuity Scale (KTAS), Injury Severity Score (ISS), and Revised Trauma Score (RTS) determined the validity of KTAS for classifying trauma patients. Methods: A retrospective chart review of 10,865 trauma patients (aged ≥15 years) who visited a single regional trauma and emergency medical center from January 1, 2016, to December 31, 2020, was conducted. Data were collected from the Korean Trauma Data Bank. Based on KTAS classification, the rates of intensive care unit admission, surgery and intervention, transfusion, emergency room (ER) and hospital mortality, and ER stay time were investigated. Data were analyzed using Chi-square test, Pearson's correlation coefficient, receiver operating characteristic curve, and area under the ROC curve. Results: In the KTAS, severe trauma patients (ISS ≥16) were classified as Level 1 (79.6%), 2 (44.8%), 3 (15.5%), 4 (4.0%) and 5 (7.6%). The following were the predictive powers of KTAS, ISS, and RTS for different parameters: surgery and intervention rate, KTAS (.71), ISS (.70), and RTS (.63); transfusion rate within 4h, KTAS (.82), ISS (.82), and RTS (.74); ER stay time within 90 min, KTAS (.72), ISS (.62), and RTS (.56); and ER mortality, KTAS (.84), ISS (.72), and RTS (.88). These findings were statistically significant (p<.001). The sensitivity and specificity of KTAS for trauma patients were .88 (.87~.90), and .38 (.37~.39), respectively. Conclusion: KTAS is a useful classification system that can predict the clinical outcomes of patients with trauma, and effectively triage acutely ill trauma patients, thus provide appropriate treatment.

  • Objectives To evaluate the effectiveness of allergic rhinitis treatment, and to set the ground in clinical studies, adequate outcome measures must be established. Thus, this study was designed to develop useful criteria based on those that have been suggested, and those that we are using today. Methods To assess outcome measures used in allergic rhinitis in Korean medicine, related articles found in Korea Traditional Knowledge Portal from 2000-2015 were evaluated. Then pattern identification and outcome measures were reviewed in each study. Results Among 27 studies, we have found 24 studies used subjective symptoms as outcome measures, 9 studies used quality of life, and most of allergic rhinitis were caused by Spleen-Gi deficiency. Conclusions It is necessary to develop outcome measures that directly reflect pattern identifications in Korean medicine, and those measures need to be further evaluated for their applicability, validity and sensitivity.

  • Purpose: The aim of this study was to determine which factors contribute to the surgical treatment outcomes of acetabular fractures. Simultaneously, we aim to report on the treatment results after our hospital was designated as the focused training center for trauma. Methods: We conducted a retrospective review of all patients who experienced acetabular fractures from January 1, 2014 to May 1, 2017 and visited our hospital. Patients who had associated pelvic ring fractures or were lost to the one-year follow-up were excluded; a total of 37 fractures were evaluated. We evaluated the clinical results using the scoring system of Merle $d^{\prime}Aubign\acute{e}$ (MDA) and grade of Brooker for heterotopic ossification. Results: Thirty-seven patients (31 men and 6 women) were identified. The mean injury severity score (ISS) was 8.7, with 32.4% of patients having a score >15. The average blood transfusion in the first 24 hours was 0.54 pints. Falling was the most common injury mechanism (32.4%). Chest injury was the most common associated injury (16.2%), followed by head injury (13.5%). The posterior wall and both column fracture were the most common (37.8%) fracture patterns. Excellent and good clinical grades of MDA included 28 patients (75.6%) and fair and poor grades included nine (24.3%), respectively. Four patients were diagnosed with a post-operative infection (10.8%); one out of four patients who had co-morbidity died (2.7%), and another patient underwent a replacement surgery (2.7%). Multivariate analysis showed that age and operation time were associated with MDA. In addition, operation time and ISS were significant co-factors of the Brooker grade. Conclusions: Korea University Guro Hospital showed similar treatment results of acetabular fractures compared to other publications. The age and operation time were co-factors of the clinical outcome of this fracture. Additionally, increased operation time and injury severity score were suggested to increase the Brooker grade.

  • Purpose: To evaluate compliance by analyzing and comparing treatment duration, degree of improvement after treatment and treatment response of oral lichen planus (OLP) patients according to characteristics of them and the severity of the lesion. Methods: According to treatment process, 132 subjects with OLP who first visited the Department of Oral Medicine at the Pusan National University Dental Hospital from January 2017 to December 2020were classified into three groups: Treatment completed (CT) group, Under treatment (UT) group, and Dropped out during follow-up (DT) group. The reticulation/keratosis, erythema, and ulceration (REU) scoring system was used to assess the severity of OLP. The degree of improvement after treatment was evaluated in CT group. Results: There were 53 (40.15%) CT, 27 (20.45%) UT and 52 (39.39%) DT. In CT group, according to initial REU score there was a statistical difference in the degree of improvement, but not in the length of time to complete treatment. There was no statistical difference between the days it took for patients to feel symptom relief, and the days of entire treatment among three groups. However, there was a positive correlation between the REU score of gingiva and duration of treatment in DT group. In the CT and DT groups, there was a correlation between the length of time taken to relieve symptoms and the duration of treatment. Conclusions: The severity of the gingival lesion and the initial response to treatment have a large effect on the entire treatment period and prognosis, so it should be considered when explaining the disease prognosis and treatment period to patients, and the clinician needs to focus on initial symptom relief.

  • Objectives : The purpose of this study is to establish the basis of the assessment tools that can be used for the diagnosis and assessment of skin diseases by reviewing the present status of the assessment tools used in domestic and overseas traditional medicine papers for the past five years. Methods : This study is based on analysis of papers on clinical trials about atopic dermatitis, psoriasis, acne, urticaria published from 1st July 2013 to 30th June 2018. The papers were searched from Oriental Medicine Advanced Searching Integrated System(OASIS), Korean Traditional Knowledge Portal(KTKP), Pubmed, MEDLINE. Results : 40 articles on atopic dermatitis, 34 articles on psoriasis, 26 articles on acne and 10 articles on urticaria were selected. Among them, 38 articles(90%), 20 articles(85%), 22 articles(85%) and 5 articles(50%) used assessment tools in order. Conclusions : SCoring Atopic Dermatitis(SCORAD), objective SCORing of Atopic Dermatitis index(OSI), Eczema Area and Severity Index(EASI) were used in studies on atopic dermatitis. In the case of psoriasis, The Psoriasis Area and Severity Index(PASI) was used in most papers. In the study on acne, most of Korean papers used Korean Acne Grading System(KAGS), but in the overseas papers, various assessment tools were used. In the case of urticaria, the rate of use of the assessment tool was the lowest, but the assessment tool used was unified by Urticaria Activity Score(UAS). Since skin diseases can cause stress and a decrease in quality of life, the quality of life should be taken into account when evaluating treatment effects.

  • This study was conducted to establish scores according to the importance levels of each HACCP evaluation item in cattle farm. The importance levels and score of each HACCP evaluation item was derived through the non-compliance rate and severity levels of hazard. In order to change the score criteria according to the importance of each HACCP evaluation item, we analyzed the importance of each HACCP evaluation item by using the portfolio mapping method according to the occurrence frequency and severity levels of hazard. The scores were classified into 3 point, 2 point, and 1 point, respectively, by classifying the importance of each category as 'high', 'middle' and 'lower'. Accordingly, we have established a now scoring system of each HACCP evaluation item through this study. Through the result of this study, the objectivity of the comparative evaluation was verified by implementing the currently used HACCP evaluation item to the cattle farm. In conclusion, Implementation of the result of this study to cattle farm may help to increase the objectivity and also improve more safety and hygienic cattle management and raw milk production.
