The Prognostic Factors in Progression of Pneumoconiosis (진폐증의 예후에 영향을 미치는 인자)

  • Backround : Pneumoconiosis is the parenchymal lung disease that results from the inhalation and deposition of dust, usually mineral dust of occupational or environmental origin. Most of the pneumoconiosis can be categorized to coal workers' pneumoconiosis (CWP) in Korea. No effective treatement is currently available, and the therapy for symptomatic CWP is limited to treatment of complication. Therefore authors analyzed and reviewed clinical features and radiological findings of 95 patients with pneumoconiosis for assessing the prognostic factors in disease progression. Method: We reviewed medical records of 95 cases with pneumoconiosis including history, chest X-ray, pulmonary function test, electrocardiography, AFB stain and culture of sputum, and routine blood examination between June 1995 and June 1997 in Seonam University Namkwang Hospital. Results: All of cases are male(mean age, 57.4 years), 91 cases out of them are miners. The mean duration of exposure to dust is 18.8 years. Major clinical symptoms are dyspnea (100%), sputum (71.6%), chest pain (55.8%), cough (23.2%), and hemoptysis (6.3%). 82 % of cases are over Morgan-Seaton Grade 2 in the degree of dyspnea. Small opacity on chest x-ray is 82.1 % and large opacity is 17.9%. Small opacity has tit type (37.2%), q/q type (25.6%) and r/r type (11.5%). B type is 42.2% in large opacity. For the pulmonary function test, restrictive type is 40.3%, mixed type 19.5% and obstructive type 8.3%. The more increasing chest Xray density, the more decreasing $FEV_1$ (p<0.01). 38% of patients show tuberculosis in chest X-ray, 15.8% positive smear of acid fast bacilli in sputum. The prevalence of pulmonary tuberculosis is high in patients with poor clinical condition. The cases with the active pulmonary tuberculosis have severe dyspnea. Expired cases show 100% and 75% of positive pulmonary tuberculosis in chest X-ray and sputum examination, respectively. 75% of expired cases show the chronic cor pulmonale, who died of acute respiratory failure. Conclusion: These findings indicate that tuberculoois infection has a decisive influence on the progress and prognoois of pneumoconioois.

Effect of Gamma Irradiation and Cichorium Products on Oxidative Damage and Lipid Metabolism in Streptozotocin-Induced Diabetic Rats (감마선 전신 조사와 치커리 가공물 식이가 Streptozotocin 유발 당뇨쥐의 산화적 손상과 지질대사에 미치는 영향)

  • The increased occurrence of hyperglycemia and oxidative stress in streptozotocin (STZ) induced type I diabetes has been implicated in the etiology and pathology of disease complication. STZ has known to be genotoxic in a variety of assays including tests for microbial mutagenesis and unscheduled DNA synthesis in rat kidney. Diabetes mellitus (DM) is a pathologic condition, resulting in severe metabolic imbalances and non-physiologic changes in many tissues. We examined the effect of gamma radiation and KWNP on preventing the development of insulin dependent diabetes mellitus using streptozotocin-induced Fisher 344 diabetic rats. The hematological values (red blood cell and white blood cell), serum biochemical constituents-alkaline phosphatase (ALP), total cholesterol, triglycerides and insulin-were checked and the organs (testis, spleen and kidney) were weighed. The gonad indices of the STZ treated groups were much lower than the value of the control group. But the gonad indices of the KWNP treated groups were higher than those of the treated groups. The ratio of the weight of kidney to the body weight of the STZ treated groups was higher than that of the control group. The value of the diabetic group treated with KWNP after irradiation (F group) was lower than the other STZ treated groups. The white blood cell and ALP values of the F group were lower than the other STZ groups, as well. The cholesterol and triglyceride values of all the KWNP treated groups were significantly lower than the other groups. A significant increase (about 10 times) of insulin was detected in the F group. The results of hematological assay showed the distinctive damage in the irradiated and STZ treated groups. The quantity of apoptotic cells in seminiferous tubule of testis confirmed a serious damage as assessed in the STZ treated groups. These experimental results have revealed that treatment of the products of KWNP after irradiation has the antidiabetic effect in the STZ-induced diabetic rats. But the F group showed higher recuperative power. These experimental results have revealed that treatment of the gamma irradiation and KWNP have the recovering effect in the STZ-induced diabetic rats.

Effects of Higher Dietary Cation with or without Protected Fat and Niacin on the Milk Yield and Thermoregulatory Ability in Holsteins During Summer Heat Stress (여름철 고온스트레스 기간에 고 양이온 사료와 반추위 보호지방과 나이아신의 추가공급시 착유우의 유생산 및 체온조절에 미치는 영향)

  • The objective of this study was to evaluate the effects of dietary cation-anion difference (DCAD) with or without ruminally protected fat and niacin on the thermoregulatory ability, milk yield and milk composition of lactating dairy cows during summer in Korea. Thirty mid-lactating Holstein cows (134±12.4 DIM and 23.4±2.3kg/d of milk yield) were divided into three groups (10 animals/group). Cows were housed in a free-stall barn and were provided with forced- air ventilation (wind velocity = 4 m/s) using 41 cm diameter fans. Diet one was formulated to contain low DCAD (+15 DCAD) while the remaining two diets were higher in DCAD (+30 DCAD). One higher DCAD diet was formulated to contain by-pass fat and the second higher DCAD diet contained the niacin along with by-pass fat. The maximum ambient temperature during July was 28.5℃ which could be seen as a period of mild heat stress. As summer progressed, August was characterized as a severe heat stress condition with maximum ambient temperature (32.4℃) and THI (74.0). Dry matter, crude protein and total digestible nutrients intake was not affected by the DCAD level and supplementation of ruminally protected fat or niacin. Milk production was higher in cows fed diets supplemented with fat and niacin than those fed un-supplemented diet. No difference in milk yield was observed in cows fed diets supplemented with fat or niacin plus fat. Milk fat and rectal temperature were not affected by the DCAD level and supplementation of ruminally protected fat or niacin. However, respiration rate was decreased in cows fed diets supplemented with either fat or fat and niacin compared to those fed. The results of the present study indicated that higher DCAD (+30) and supplementation of fat along with niacin can somehow mitigate the negative effects of heat stress on milk yield and physiology of lactating Holsteins during July and August in Korea. In present study reduced respiration rate and increased milk yield in lactating cows may be attributed to the cooling effect of supplemented fat along with vasodilatory functions of niacin. Effects of Exogenous Thyroid Hormone $(T_3)$ on Skeletal Development and Physiological Conditions of Juvenile Black Seabream (Acanthopagrus schlegeli) (감성돔(Acanthopagrus schlegeli) 치어의 골격발달 및 생리적 조건에 미치는 외인성 갑상선호르몬$(T_3)$의 영향)

  • Juveniles of black seabream, Acanthopagrus schlegeli were fed with the diets containing 0, 10, 20, 50 and 100 ppm of 3,5,3'-triiodo-1-thyronine $(T_3)$ respectively to assess the effect of this hormone on skeletal development and the change of physiological conditions for 50 days. $(T_3)$ treatment lasted for initial 40 days. Fish were fed the prescribed diet by hand to satiation in $2\~4$ times per day. After an initial 40 days period, skeletal development and abnormality were examined, and after a 50 days period, food intake, hepatosomatic index (HSI), thyroid cell height (TCH) and body proximate composition were also examined. Although toed intake was not different among 0, 10 and 20 ppm, the food intake of black seabream fed with the diets containing 50 and 100 ppm of $T_3$was significantly lower than those of 10 ppm. After the initial 40 days of $T_3$ administration, $T_3$ increased the relative growth of operculum, head, caudal fin and pectoral fin to body length, resulting in severe morphological abnormalities at the highest dose. Black seabream treated with 50 and 100 ppm of $T_3$ had abnormal shapes such as lordosis and opercular curl. The HSI parameters were reversely correlation with the dietary concentration of $T_3$. After the initial 40 days of this experiment, atrophy of thyroid gland was observed in fish administered with 50 and 100 ppm of $T_3$. On the 50th day of this experiment, atrophy of thyroid gland was observed only in the group administered with 100 ppm of $T_3$, and no difference was observed on TCH among the rest fours of experimental groups. At the end of the experiment the whore body proximate analyses indicated that there were significant effects of $T_3$ level on moisture, protein, lipid and ash contents.

Protective Effect of Kefir Grain Against Dextran Sodium Sulfate-Induced Colitis in Rats (Dextran Sodium Sulfate로 대장염을 유도한 흰쥐에서 캐피어 원말의 장보호 효과)

  • Probiotics have emerged as a potential treatment modality for numerous gastrointestinal disorders, including IBD. However, few probiotics have undergone appropriate preclinical screening in vivo. Kefir is considered a probiotic, benefiting the host through its effects in the intestinal tract. Despite numerous studies examining the action of probiotics on the host organism, few have analyzed the effects on intestinal environment. We assessed the protective effect of kefir for three weeks before inducing colitis with 2% dextran sodium sulfate for five days. The DSS loads were similar in all DSS treatment group. The results of the experiment are as follows. Food intake and FER of experimental groups were not significantly different each other, but water consumption tended to be higher in all DSS treatment groups as compared with the normal control. And visual inspection of feces revealed mild diarrhea in rat given 2% DSS. The anti-inflammatory activity of kefir was determined by myeloperoxidase activity during the DSS treatment, and there was no significant difference in any group. The levels of thiobarbituric acid reactive substances (TBARS) as a colonic lipid peroxidation were significantly lower in the kefir intake groups than in rats treated with 2% DSS alone. The DNA % in tail and tail moment values as a DNA damage level of the blood lymphocytes in kefir intake groups tended to be lower than 2% DSS treatment alone, especially tail lengths were significantly diminished. According to the colonic histopathological assay, there were a severe inflammation of lamina propria and submucosa and mild edema in mucosa and sub mucosa in DSS alone treated group. We found a slight regenerative change in kefir treatment groups. In our experiments, this means that ulcerative colitis related to oxidative injury might be prevented by kefir as a probiotic. Further studies of the potential benefits of kefir as a probiotic in inflammatory condition are encouraged.

Study of the Effects of Ambient Temperature and Car Heater Power on the Train Cabin Temperature (외기 온도와 난방 출력의 철도차량 객실 온도에 대한 영향 연구)

  • Recently, abnormally cold weather has been reported more frequently in winter due to the climate change and abnormal weather changes. On the other hand, the heating capacity of a railcar may be not enough to warm the cabin under severe cold climatic conditions, which is one of the reasons for the passengers' complaints about heating. In this study, the effects of ambient temperature and heater power on the cabin temperature was investigated to obtain the minimum ambient temperature for the tested railcar. The test railcar was placed in a large-climatic chamber, and various ambient temperature conditions were simulated. The effects of the heater output were investigated by monitoring the cabin temperature under a range of heater output conditions. The mean cabin temperature was $14.0^{\circ}C$, which was far lower than the required minimum temperature of $18^{\circ}C$, under a $-10^{\circ}C$ ambient temperature condition with the maximum heat power. When the ambient temperature was set to $0^{\circ}C$ and $10^{\circ}C$, the maximum achievable cabin temperature was $26.1^{\circ}C$ and $34.0^{\circ}C$. Through calculations using the interpolation method, the minimum ambient temperature to maintain an $18^{\circ}C$ cabin temperature was $-6.7^{\circ}C$ for this car. The vertical temperature difference was higher with a higher power output and higher ambient temperature. The maximum vertical temperature difference was higher than $10^{\circ}C$ in some cases. However, the horizontal temperature difference vs. low temperature (< $2^{\circ}C$) was independent of the power output and ambient temperature. As a result, it is very important to reduce the vertical temperature difference to achieve good heating performance.

The Preliminary Analyses on Damage Types of Stone Hertage induced by Natural Hazard, Korea (석조문화재의 자연재해 피해양상 예비분석)

  • The severe damage of cultural heritages induced by natural hazards like heavy rain has been dramatically increased since 1990. The number of the repair works of stone heritage of 2005 was six times as many as those of 1986 year. Especially the ratio of the repair works of Gyeongsang Province and Jeolla Province stood 63% of those of all over the country. Since 1990, the typhoons usually struck the southern part of Korea and went northward. The heavy damage of stone heritages in two provinces was caused by them. We made a preliminary survey the stone heritages that exposed to the natural hazards on the basis of repair works of them and a field survey. The analysis results indicate that the natural hazards such as landslide and soil disaster of the stone heritages related to a sloping surface stood 58% of all kind of natural hazards. The reasons are caused by the 59 % of all the stone heritages distributed in a sloping surface resulted in natural hazards like landslide and soil disaster. The bases of stone heritages can be easily eroded by the surface water with high energy induced by heavy rainfall. Most of the stone heritages like Maebul were engraved on a natural rock wall(outcrop). But some of them engraved on rolling stones are very vulnerable in a change of a base condition caused by erosion and ground subsidence and they can be tilted or fell down. The distribution of the stone heritages vulnerable in natural hazard is related to that of the rainfall distribution compounded five typhoons after 1990. Most of them are included in level two on the rainfall distribution map except those of Taean peninsula and some of Gyeonggi Province. They seem to be rather related to the rainfall distribution of the Typhoon Olga.

Health status of children in low socioeconomic conditions (공부방을 이용하는 저소득층 소아들의 건강상태에 대한 조사)

  • Purpose : The purpose of this study was to investigate the health status and nutritional condition of children living in a low-income community through anthropometric, laboratory evaluation. Methods : A community-based survey identified children below 15 years living in a low-income community. Their weight, height, visual acuity, hearing level and dental status were measured. Blood sample were obtained on June and July, 2004. Hemoglobin, serum cholesterol, Hepatitis B antigen/antibody, AST and ALT were measured. Results : A total of 285 students(M : F=141 : 144) aged 6 to 14 years were included in this study. The heights and weights in some grades were smaller than controls. The prevalence of obesity was 10.6 percent in males and 10.7 percent in females. The prevalence of abnormal visual acuity, hearing impairments and dental carries were 20.5 percent, 0.3 percent and 69.4 percent. The prevalence of anemia was 10.1 percent. Serum total cholesterol was over 200 mg/dL in 7 percent. They complained of abdominal pain(22.1 percent) and headache(17.1 percent). Hyperthyroidism, cataract, neurofibromatosis, severe atopic dermatitis, ventricular septal defect, strabismus and inguinal hernia were newly diagnosed. Conclusion : Mean heights and weights of children in the low-income community were smaller than controls. The prevalence of abnormal visual acuity, hearing impairment and dental carries were higher than in the 2003 national health survey. Additional research is needed to evaluate the health status of the low-income community.

Usefulness of modified ambu® in patients who need artificial ventilation (인공 환기가 필요한 환자에서 변형된 수동식 인공호흡기(Ambu®)의 유용성)

  • Purpose : The comatose mentality can be catastrophic, especially if the condition is severe or the duration is prolonged. Therefore, delayed diagnosis can result in a poor outcome or death. The best radiologic modality to differentiate from cerebral lesions in patients suffering from cerebral diseases is magnetic resonance imaging (MRI) rather than computed tomography (CT). Special apparatuses with metal materials such as ventilators, and cardiac pacemakers belonging to patients cannot be located in the magnetic field. We aimed to exhibit the possibility of examining MRI, maintaining ventilation at a relative long distance by means of modified $Ambu^{(R)}$. Methods : Self-inflating bags as a sort of a manual ventilator, connected with relatively long extension tubes instead of mechanical ventilators, were adopted to obtain MRI. PVC (polyvinyl chloride) extension tubes had different lengths and diameters. Lengths were 1, 2, and 3 cm and diameters were 15, and 25 mm. The work of breathing and expiratory changes of expiratory tidal volume (TVe), minute volume of expiration (MVe), peak inspiratory pressure (PIP) were measured by use of the mechanical ventilator, $Servoi^{(R)}$, as the alteration of TVi (inspiratory tidal volume), extension tube lengths and diameters with other values fixed. Results : Measured TVe and MVe by ventilator were the same values with control at every TVi, regardless of extension tube lengths and diameters, but PIP were increased with the rise of TVi, tube lengths, with decline of tube diameters, these were statistically significant. Conclusion : MRI examination can be carried out with a self-inflating bag connected with an extension tube at a long distance in patients who need artificial ventilation.

Appraisal of the Special Production Area Development Project in Rural Area and Countermeasures for Off-farm Income Increase (The Case of Chungnam Province) (농어촌(農漁村) 특산단지개발사업(特産團地開發事業)의 평가(評價)와 농외소득증대방안(農外所得增大方案) (충청남도(忠淸南道)를 중심(中心)으로))

  • Korean agriculture has encountered two problems. One is internal income disparity between rural and urbarn area and the other is external Uruguay Round trade problems as an abolition of direct and indirect import barriers, reduction in export subsidies and to reduce internal price supports. These problems will be brought severe farm problems such as decreasing farm household income and repressing agricultural growth in the near future. Considering the above inevitable facts Korean government has implemented several development projects such as rural industrial area development project, rural special production area development project, leisuresight seeing farm development project, traditional food development project, unskilled labor training project for off-farm employment and so on, to increase farm household income through off-farm income increase. This study was mainly concentrated on the identification of operational problems and post evaluation of the rural special production area development projects which aimed at increasing non-farm incomes and giving employment opportunity for rural farmers in small factories processing regional special farm products and mine products. The main findings and problems to be solved for the successful project implementation are as followed ; 1. Total number of the special production area development projects as of the end of 1991 was amount to 138, and total number of farm household participated were estimated at 2,079, and total amount of off-farm income per farm household was reached to 3,011 thousand won. 2. The total number of processed special products have increased from 21 items in 1981 to 56 items in 1991. On the other hand the total number of farm household participated in the projects have decreased from 2,518 to 2,079 during same period. 3. Total amount of investment for the projects has increased from 1,429 million won in 1981 to 24,760 million won in 1991 but the rate of G'T loan of the total investment has reduced from 24.5% to 5.2% during same period. 4. 138 special production area development project are classified into 6 kinds of commodity groups such as 19 of general industrial good production areas, 52 of folks-industrial art objects production areas, 39 of food processing areas, 9 of fiber and texstile processing areas, 18 of agricultural and fishery inputs processing areas and 1 of stone processing area. 5. The total production value in 1990 was estimated 20,169 million won of which export was amount to 2,627 million won. 6. The finacial rate of return of the UNGOK KUGIJA Tea processing Project operated by UNGOK coops and BAKSAN ginseng tea processing project were estimated at 45.4% (B/C Ratio=1.17, NPV=152.5 million won) and 17.7% (B/C Ratio=1.12, NPV=120.2 million won) respectively. 7. More favorite terms and condition of the loan including collateral problems have to be given to farmers participated. Heavy investment and G'T subsidy policies should be started for the successful project implementation anf farm household income increase. 8. To expand market demand of the rural special goods G'T have to provide special program of TV or other mass media for commodity propaganda and the total cost concerned must be supported by G'T subsidy. 9. The special farm products as GUGUJA,MOSI'Ramie', Ginseng. SOGOKJU,HEMP,Mushroom.DUGYUNJU and Chesnut processing projects have to be propelled and expanded for off-farm income increase in Chung Nam Province. 10. Direct operational pattern of the special production area by coops is more favorable to farmers and recommendable considering with off-farm income increase and market demand creation throughout Korea. 11. In rural area, special organizations for project appraisal are not exist. Accordingly special training program, project appraisal, formulation and preparation for civil servants concerned have to be prepared for project selection and sound implementation under limited budget and financial support.
