Is Diabetes Mellitus a Prognostic Factor for Survival in Patients with Small Cell Lung Cancer?

  • Background: Previous studies have pointed to many different prognostic factors for small cell lung cancer (SCLC) but diabetes mellitus (DM) has not been clearly or consistently identified as of prognostic value. The aim of this study was to investigate the prognostic significance of the characteristics of patients and clinical laboratory tests in SCLC. Specifically, we investigated that the impact of DM for survival in the patients receiving first-line etoposide plus cisplatin (EP) chemotherapy. Methods: We retrospectively reviewed 161 patients with SCLC with a focus on DM and other potential prognostic variables were chosen for univariate and multivariate analyses with respect to survival. Result: Among the sixteen variables of univariate analysis, five were identified to have prognostic significance: performance status (PS) (p<0.001), stage (p=0.001), DM (p=0.005), serum albumin (p<0.001) and hemoglobin levels (p=0.03). Multivariate analysis showed PS, stage and serum albumin level to be independent prognostic factors for survival (p=0.02, p=0.02 and p=0.009 respectively), but DM was not an independnet factor. Conclusion: In conclusion, PS, stage and serum albumin level were identified as important prognostic factors, while DM at the time of diagnosis of SCLC did not have prognostic importance for survival.

Changes of Serum Chemistry Values on Size and Cultured Method in Korean Rockfish, Sebastes schlegeli (사육방법 및 크기에 따른 조피볼락의 혈청 화학성분 변화)

  • The purpose of this study was to obtain reference values of serum chemistry in Korean rockfish (Sebastes schlegeli), 5 juvenile (n=25) and 3 broodstock (n=15) subpopulations which were differentiated by cultured method. Serum samples were analyzed for the following commercial clinical kits: total protein, albumin, glucose, AST (aspartate aminotransferase), ALT (alanine aminotransferase), total cholesterol, free cholesterol, LDH (lactate dehydrogenase) and ALP (alkaline phosphatase). Significant differences were observed in the values of total protein, albumin, total cholesterol, AST, ALT, LDH and ALP between juvenile and broodstock subpopulations. In the comparison of total protein, albumin, total cholesterol and ALP, broodstock subpopulations showed higher levels. On the other hand, increased AST, ALT and LDH were observed in juvenile subpopulations. These results indicate that cultured method are major contributors to health-related physiology of Korean rockfish, juveniles and broodstocks.

Sentinel lymph node mapping using tri-modal human serum albumin conjugated with visible dye, near infrared fluorescent dye and radioisotope

  • We developed an evans blue-indocyanine green-$^{99m}Tc$-human serum albumin conjugate for sentinel lymph node mapping and we describe its unique potential usage for clinical implications. This conjugate has combined the strengths of visible blue dye, near-infrared fluorescence and radioisotope into one single conjugate without any additional weakness/disadvantage. All the components of evans blue-indocyanine green-$^{99m}Tc$-human serum albumin are safe and of low cost, and they have already been clinically used. This conjugate was stable in the serum, it showed a long retention time in the lymphatic system and the lymph nodes showed a much higher signal-to-noise ratio after the conjugate was injected intradermally into the paw of mice. Both the single-photon emission computed tomography and near-infrared fluorescent images of the mice were successfully obtained at the same time as the excised sentinel lymph nodes showed blue color. The visual color, near-infrared fluorescence and gamma ray from this agent could be complementary for each other in all the steps of sentinel lymph node sampling: exploring and planning sentinel lymph node before excision with visualization of the exact sentinel lymph node location during an operation. Therefore, the triple modal agent will possibly be very ideal for sentinel lymph node mapping because of the high signal-to-noise ratio for non-invasive imaging and its complementary multimodal nature, easy preparation and safety. It is promising for clinical applications and it may have great advantages over the traditional single modal methods.

A Study of Nutritional Improvement in the Patients with Neurologic Disorders by Changing Enteral Feeding Methods (경장영양 공급방식 변경에 따른 신경계질환자의 영양개선 효과 연구)

  • Protein-calories malnutrition is common among patients in the hospital. In particular, elderly patients with neurologic disorders has more risk of nutritional deficiency due to swallowing difficulty. Enteral tube feeding is more economical, physiological and immunological than parenteral nutrition for patients who have adequate gastrointestinal function. This study was conducted patients with neurologic disorders who received enteral nutrition at Asan Medical Center from February 1 to October 10, 2002. The control group (48 patients) were given traditional feeding methods 4 times a day while the treatment group (45 patients) were given improved feeding methods 3 times a day. We assessed nutritional status of patients and compared to both groups. We investigated body weight, serum albumin, hemoglobin, total lymphocyte count by means of nutrition markers. The objectives of this study is to reduce the time needed for nutritional requirement of patients without an increase in gastrointestinal intolerances. The results of this study are as follows: 1. Nutritional status of many patients in both groups were either malnourished or at risk for malnutrition. 2. The time to arrive to the nutritional requirements were 6.21 $\pm$ 0.35 days for the control group and 4.24 $\pm$ 0.52 days for the treatment group. The treatment group showed a significantly shorter amount of time. 3. The changes of the nutritional marker in the control group showed a significant drop in body weight, serum albumin and serum hemoglobin while the treatment group experienced a significant increase in body weight, serum albumin and total lymphocyte count. 4. Feeding intolerane such as diarrhea, high residual volume, ileus, nausea and vomiting were investigated. Diarrhea found in 25.1% (12 patients) of the control group and 22.2% (10 patients) of the treatment group and these findings are not significant.

Effects of Dietary Protein Levels on Organ Growth and Protein Metabolism in Early and Normally Weaned Rats (단백질 섭취수준이 조기 이유 및 정상이유 흰쥐의 기관성장과 단백질 대사에 미치는 영향)

  • This study was designed to examine how dietary protein levels affect organ growth and protein metabolism in early and normally weaned rats. Early and normally weaned rats separated fro the dam on the 15th and 121st day postpartum, respectively. were fed diets containing three levels of protein : low(10%) , normal (20%),and high(40%) . On the 35th day, the weight and DNA, RNA and protein contents in brain , liver, and kidney were determined to ascertain organ and cellular growth. Furthermore, serum total protein , albumin , $\alpha$-amino N and creatine and urinary urea N, and creatinine were determined in order to ascertain protein metabolism and renal functions. Dietary protein levels were not observed to significantly affect total DNA content, which may represent an index of cell number in the liver, brain and kidney. Fresh weight and protein/DNA ratio, which may represent indices of cell size, significantly increased in proportion to dietary protein in the kidney. As for the early weaned rats , the liver cell size significantly decreased. Dietary protein levels and weaning periods did not affect serum total protein and albumin . However, serum urea-N significantly increased in proportion to dietary protein levels whereas serum $\alpha$-amino N was decreased by early weaning . Nitrogen retention was lower in early weaned rats fed low or high levels of protein than in normally weaned rats. The results demonstrate that low or high levels of dietary protein have less desirable effects on protein metabolism in prematurely weaned rats.

Predictive Factors for Neutropenia after Docetaxel-Based Systemic Chemotherapy in Korean Patients with Castration-Resistant Prostate Cancer

  • The aim of this study was to determine predictive factors for neutropenia after docetaxel-based systemic chemotherapy in patients with castration-resistant prostate cancer (CRPC). The study included 40 Korean CRPC patients who were treated with several cycles of docetaxel plus prednisolone from May 2005 to May 2012. Patients were evaluated for neutropenia risk factors and for the incidence of neutropenia. In this study, nine out of forty patients (22.5%) developed neutropenia during the first cycle of docetaxel-based systemic chemotherapy. Four experienced grade 2, three grade 3, and one grade 4 neutropenia. Multivariate analysis showed that pretreatment white blood cell (WBC) count (p=0.042), pretreatment neutrophil count (p=0.015), pretreatment serum creatinine level (p=0.027), and pretreatment serum albumin level (p=0.017) were significant predictive factors for neutropenia. In conclusion, pretreatment WBC counts, neutrophil counts, serum creatinine levels, and serum albumin levels proved to be significant independent risk factors for the development of neutropenia induced by docetaxel-based systemic chemotherapy in patients with CRPC.

Effect of Family Type on the Nutrient Intake and Nutritional Status in Elderly Women (가족형태에 따른 여자 노인의 영양소 섭취 및 영양 상태에 대한 연구)

  • This study was performed to investigate the effect of family type on the nutrient intake and nutritional status in elderly women. Blood samples and anthropometric data of elderly women aged from 60 to 78 years were collected in 1996, who visited a health examination center in Seoul to screen their health status. They were divided into two groups by their family type, one was extended family(extended family elderly, n=37) and the other was independently living(independently living, n=29). Nutrients intake was measured by interview using a simplified questionaire. Albumin, cholesterol, triglyceride(TG), hemoglobin (Hb), hematocrit, Ca, Mg, and alkaline phosphatase activity of serum were analyzed. Intakes of energy, protein, carbohydrate, Fe, vitamin A, niacin, thiamin, riboflavin, and vitamin C were not different between the two groups. However, fat and Ca intakes were higher(p<0.05) in extended family elderly than indepen dently living elderly. There are no significant difference in hematocrit value and serum concentrations of Hb, TG, and Ca. Serum Mg and cholesterol levels tended to be higher in extended family elderly. However, serum albumin level was lower(p<0.05) in extended family elderly. These results show that the indepen dently living elderly women had intakes of daily energy, fat and Ca compared to the extended family elderly. Therefore, the nutritional education for the independently living elderly will be focused on the health risk resulted from less intakes of Ca and total energy.

Relationship Between Interleukin-6 Production and Inflammatory Response during Cardiopulmonary Bypass (체외순환기 인터루킨-6의 생성과 염증반응간의 상관관계에 관한 연구)

  • Background: With open heart surgery(OHS), it has been recognized that many postoperative complications and postperfusion syndrome are associated with the activations of complements and leulocytes. Recently, some investigators also demonstrated that interlukin-6(IL-6) linked highly with postperfusion syndrome. The puropose of this study was to investigate the sequential changes of the IL-6 and to clarify each IL-6 relationship to the complements(C3, C4) and inflammatory response following cardiopulmonary bypass(CPB). Material and Method: To determine serum levels of IL-6, complements, leukocytes, and biochemistric markers of liver and renal function, blood samples were taken from th radial artery in 30 adult patients undergoing OHS with CPB. Result: Serum IL-6 levels incrased significantly at 10 minutes after CPB-on(CPB-10) in comparison with the control levels and reached the peak at CPB-off(p<0.05). Serum complement levels declined rapidly at CPB-10 and remained at the lower levels during CPB(p<0.01). Sequential changes of IL-6 levels had positive correlations with the changes of total leukocytes and neutrophil fractions(p<0.05), but had negative correlations with lymphocyte fractions(p<0.05). Changes of C3 related postively to monocyte fractions(p<0.05). Postoperative levels of total protein and albumin, decreased significantly in comparison with the control levels(p<0.01), while the postoperative levels of AST(aspartate transaminase) and bilirubin increased (p<0.01). At CPB-off, IL-6 levels had negative correlations with total protein and albumin levels(r=-0.60, -0.47 respectively, p<0.05), whereas C3 levels had positive correlations with albumin levels(r=0.40, p<0.05). IL-6 levels, as well as neutrophil fractions, had positive correlations with aortic clamp time(ACT) and total bypass time(TBT) (IL-6; r=0.82, 0.79 respectively, neutrophil fractions; r=0.50, 0.56 respectively, p<0.05), wheres lymphocyte frations and albumin levels had negative correlations whith ACT and TBT(lymphocyte fractions; r=-0.52, -0.58 respectively, albumin; r=-0.58, -0.55 respectively, p<0.05). Conclusion: These data showed that elevated production of serum IL-6 during CPB may play a pivotal role in systemic inflammatory responses and prologed CPB period may be assosiated with more sever postperfusion syndromes.

  • Total amount of Calories and protein intakes of fishing villagers show higher level than those of farming and mountain villagers, however, the proportion of animal protein (meat protein) in total amount of protein intakes of mountain villagers is much lesser than those of other two villagers. Blood specific gravity, serum protein content and serum tryptophan content were low in inhabitants of mountainous area than those of other two villagers, and in the serum protein fractions, the Albumin Globulin ratio (A/G ratio) show also lower value in inhabitants of mountainous area than those of other two villagers. It is interesting result that serum tryptophan content (serum protein tryptophan) presented a significant positive correlation with that of serum gamma globulin and a significant negative correlation with that of serum albumin. The fact of farming villagers under twenty year old female showing plasma vitamine deficiency phenomenon, however, the plasma carotene value show higher level indicates that the most of the plasma carotene being hardly transfered to the plasma vitamine A in blood. The thiamine value in urine of mountain villagers show higher level than others, it indicates that there is correlation between their poor protein intakes and amount of thiamine ia urine. And the data obtained in the present study could be established by the result of animal experiment reported by Koyanagi et al. and the result of previous paper of auther.

Effects of Mulberry-Leaf Powder Tofu Consumption on Carpal Bone Mineral Density, Biochemical Bone Turnover Markers and Serum Lipid Profiles in Smoking Male Adults Living in Choongnam (뽕잎 분말 첨가 두부 섭취가 충남 일부 지역에 거주하는 흡연 남자 성인의 손목 골밀도, 생화학 골대사 지표 및 혈청 지질 성상에 미친 영향)

  • The effects of mulberry-leaf powder Tofu(MPT) on anthropometric measurements, including bone mineral density(BMD) in the right carpus, biochemical bone turnover markers, serum levels of lipids and macrominerals, were investigated in 30 smoking male adults who lived in Choongnam were given MPT(100 g/day) for 4 weeks. The average ages, number of smoked cigarettes and packyear were 22.38 years, 15.12/day and 3.54 years, respectively. The nutrient contents per 100 g MPT were 86.10 kcal energy, 8.98 g protein, 0.53 mg fiber, 211.33 mg Ca and 1.59 g fat. Anthropometric measurements, including dietary intake using the 24-hours recall method, carpal BMD using DEXA, serum levels of protein, albumin and glucose, lipid profiles (cholesterol, triglyceride, HDL-cholesterol, LDL-cholesterol) with Al(atherosclerosis index), HTR, CRF, LHBt, some biomarkers of BMD(serum alkaline phosphatase activity, osteocalcin, urinary DPD), and serum macrominerals(Ca, Ca/P ratio, Mg) and Pb were analyzed before and after consumption of MPT. After MPT consumption, dietary intakes of plant protein, total Ca and plant Ca increased significantly, but there were no significant differences in anthropometric measurements, BMD with bone metabolism markers, serum levels of protein, albumin or glucose, lipid profiles with AI, HTR, LHR and CRF.
