전주지역 복지관 노인대상 영양교육 및 맞춤형 점심 급식 프로그램 효과 (Effects of Nutrition Education and Personalized Lunch Service Program for Elderly at Senior Welfare Center in Jeonju)

  • The purpose of this study was to investigate the effects of a nutrition education and personalized lunch service program in a senior welfare center. A total of 30 elderly (14 males, 16 females) aged 62~89 years participated in this study. Nutrition education lessons (2 hour/lesson/week) were provided for 4 weeks. Also, ten weeks from week 3rd to week 12th, personalized lunch providing 1/3 personal needed energy was served 5 times for a week. After the nutrition intervention program, we compared anthropometric characteristics, blood biochemical characteristics, nutrition knowledge, dietary attitude and dietary intake using 24 hr recall with those before the intervention. The body weight (p < 0.001) and body mass index (BMI) (p < 0.001) were decreased. There were significant increases in score of nutrition knowledge (p < 0.01) and consumption of milk & milk products for snacks. There was a positive effect on fasting blood sugar (FBS) showing significant decreases in portion of impaired fasting glucose and diabetes mellitus (p < 0.05). Also, serum triglyceride (TG) was significantly decreased (p < 0.05). In evaluation of nutrient intake by Dietary Reference Intakes for Koreans (KDRIs), riboflavin (p < 0.01), vitamin C (p < 0.001), calcium (p < 0.05) were positively improved. The index of nutritional quality (INQ) and intakes of vitamin C (p < 0.001), riboflavin (p < 0.05), Ca (p < 0.01) and Fe (p < 0.05) were increased. In conclusion, this nutrition education and lunch service program providing 1/3 personal needed energy can be used to develop and implement a tailored nutritional intervention programs in the setting of a community senior welfare center to improve health and nutritional status of Korean elderly.

위암 및 대장암의 유발에 영향을 미치는 식이요인에 관한 연구 (A Study on Dietary Factors Related to the Incidence of Stomach Cancer and Colon Cancer in Korea)

  • This study was undertaken to investigate the dietary factors related to the incidence of stomach and colon cancers in Korean. The subjects were 139 stomach and 52 colon cancer patients recruited from 3 general hospitals in Seoul. Food intake, anthropometric measurement, and blood compositions were studied through personal interview and using medical records. Body weight, body mass index, triceps skinfold thickness, body muscle mass of the subjects were lower than reference values. The body weight was reduced after the onset of the illness, which suggests body waste due to the cancers. The patients showed the lower valuies of hemoglobin and hematocrit. Serum protein and calcium were at lower limit of the normal range. Therefore the untritional status assessed by anthropometry and blood composition should be said to be marginal. The average intake of most of the nutrients except Ca of the subjects met the RDA, but the range was wide and the nutrient intake of large proportion of the subjects feel below 75% of RDA. The food intake of egg, milk, legumes, and fruts were lower than national average, on the other hand the subjects had higher intake of meat, vegetables, and fats. The subjects drank and smoked heavily, stomach cancers being more severs. From this results, dietary risk factors for the stomach and colon cancers in Korean did not agree with the reports of western societies. Even though the intakes of meat and animal food of colon cancer patients can not be classified as high, it was higher than stomach cancer and national average. Therefore it could be concluded that with increasing consumption of animal food, the incidence of colon cancer would be increased in Korea.

간외담도에 발생한 신경내분비암종 1예 (Large Cell Neuroendocrine Carcinoma of the Extrahepatic Bile Duct)

  • Primary neuroendocrine tumors originating from the extrahepatic bile duct are rare. Among these tumors, large cell neuroendocrine carcinomas (NECs) are extremely rare. A 59-year-old man was admitted to Sanggye Paik Hospital with jaundice that started 10 days previously. He had a history of laparoscopic cholecystectomy, which he had undergone 12 years previously due to chronic calculous cholecystitis. Laboratory data showed abnormally elevated levels of total bilirubin 15.3 mg/dL (normal 0.2-1.2 mg/dL), AST 200 IU (normal 0-40 IU), ALT 390 IU (normal 0-40 IU), and gamma-glutamyl transferase 1,288 U/L (normal 0-60 U/L). Serum CEA was normal, but CA 19-9 was elevated 5,863 U/mL (normal 0-37 U/mL). Abdominal CT revealed a 4.5 cm sized mass involving the common bile duct and liver hilum and dilatation of both intrahepatic ducts. Percutaneous transhepatic drainage in the left hepatic duct was performed for preoperative biliary drainage. The patient underwent radical common bile duct and Roux-en-Y hepaticojejunostomy for histopathological diagnosis and surgical excision. On histopathological examination, the tumor exhibited large cell NEC (mitotic index >20/10 high-power field, Ki-67 index >20%, CD56 [+], synaptophysin [+], chromogranin [+]). Adjuvant concurrent chemotherapy and radiotherapy were started because the tumor had invaded the proximal resection margin. No recurrence was detected at 10 months by follow-up CT.

이중탕, 육군자탕, 보중익기탕의 이상지질혈증 및 고혈압과 관련된 일과성 수용체 전압 바닐로이드 4 이온통로 조절에 관한 연구 (Effects of Leejung-tang, Rikkunshito, and Bojungikgi-tang on Transient Receptor Potential Vanilloid 4 Channels)

  • Objectives: Metabolic syndrome is defined by a cluster of major cardiovascular risk factors: obesity, insulin resistance, dyslipidemia, and arterial hypertension. Several members of a large family of nonselective cation entry channels, e.g., transient receptor potential vanilloid 4 (TRPV4) channels have been associated with the development of dyslipidemia and hypertension. The purpose of this study was to investigate the effects of Leejung-tang (Lizhong-tang), Rikkunshito, and Bojungikgi-tang (Buzhongyiqi-tang) on TRPV4 channel. Methods: Human embryonic kidney 293 cells stably transfected with the TRPV4 expression vectors were maintained in Dulbecco's modified Eagle's medium supplemented with 10% fetal bovine serum, 1% penicillin/streptomycin, $5{\mu}g/mL$ blasticidin, and 0.4 mg/mL zeocin in a humidified 20% $O_2/10%$ $CO_2$ atmosphere at $37^{\circ}C$. Whole-cell patch clamp recordings were obtained using an Axopatch 700B amplifier and pClamp v.10.4 software (Molecular Devices, San Jose, CA, USA), and signals were digitalized at 5 kHz using Digidata 1422A. Results: Leejung-tang and Rikkunshito (10, 30 and 50 mg/mL) had no effects on the TRPV4 whole-cell currents at dose dependent manner. However, Bojungikgi-tang (10, 30, and 50 mg/mL) inhibited the TRPV4 whole-cell currents in a dose dependent manner and the half maximal inhibitory concentration (IC50) of Bojungikgi-tang was 18.2 mg/mL. Conclusions: These results suggest that Bojungikgi-tang plays an important roles to inhibit the TRPV4 channel, suggesting that Bojungikgi-tang is considered one of the candidate agents for the treatment of metabolic syndrome such as dyslipidemia and hypertension.

Synergistic effect of exogenous multi-enzyme and phytase on growth performance, nutrients digestibility, blood metabolites, intestinal microflora and morphology in broilers fed corn-wheat-soybean meal diets

  • Objective: This study was conducted to investigate the synergistic effect of exogenous multienzyme and phytase on growth performance, nutrients digestibility, blood metabolites, intestinal microflora, and morphology in broilers fed corn-wheat-soybean meal diets. Methods: A 2×2 factorial design was used in this study. Four dietary treatments consisted of i) basal diets (corn-wheat-soybean meal based diets without multi-enzyme and phytase), ii) basal diets with phytase (0.05%), iii) basal diets with exogenous multi-enzyme (0.05%), and iv) basal diets with exogenous multi-enzyme including phytase (0.05%). A total of 480 broiler chickens (Ross 308 - one day old) were weighed and allotted to thirty-two cages (15 birds per cage), and chicks were randomly allocated to four dietary treatments. Results: The body weight gain and feed conversion rate were improved by supplementation of exogenous multi-enzyme containing phytase during the finisher period (p<0.05). The birds fed diets with exogenous multi-enzyme containing phytase had a significantly greater digestibility of dry matter, gross energy, crude protein, calcium, and phosphorus compared with birds fed non-supplemented diets (p<0.05). The chickens fed diets with exogenous multi-enzyme containing phytase showed a higher concentration of Ca and P in the serum (p<0.05). The population of Lactobacillus spp., Escherichia coli, and Clostridium were not affected in the ileum and cecum of chickens fed enzyme-supplemented diets. The dietary supplemental exogenous multi-enzyme containing phytase showed a significant improvement in villus height, crypt depth, and villus height and crypt depth ratio, compared to basal diets or dietary supplemental phytase (p<0.05). Conclusion: The supplementation of the exogenous multi-enzyme containing phytase synergistically improved the growth performance, nutrients digestibility, and villus height of the small intestine of broiler chickens fed a corn-wheat-soybean meal based diets.

난소 적출 마우스 모델에서 Oryza sativa L.의 뿌리 추출물에서 분리된 화합물의 골다공증 개선 효과에 관한 예비연구 (Preliminary Study on the Osteoporosis Improvement Effect of Compounds Isolated from Oryza sativa L. root Extract in the Ovariectomized Mouse Model)

  • Bisphosphonates, estrogen, and calcium supplements are commonly used medications for postmenopausal osteoporosis, but they are associated with various side effects such as vaginal bleeding, deep vein thromembolism, and breast cancer. In this study, we aimed to investigate the potential of a compound isolated from the roots of Oryza sativa L. to improve osteoporosis using an ovariectomized mouse model. We isolated and identified oryzativol A, a lignan compound, through chemical analysis of an ethanol extract using a bioassay-guided fractionation protocol. We also examined the metabolism, clearance, and CYP enzyme activity of oryzativol A, and found that it showed plasma stability of over 80% at all analysis times, and indicating a low likelihood of inactivation or excretion by the CYP3A4 enzyme. Our results showed that the high-dose group of oryzativol A exhibited a significant increase in bone mineral density compared to the control group. Although the ALP concentration did not differ significantly compared to the control group, it showed a tendency to increase in the high-dose group of oryzativol A. Furthermore, the abnormal ratio of serum Ca/P, caused by osteoporosis, was improved to a level closer to that of the normal group as the dosage of oryzativol A increased. Taken together, these findings suggest that oryzativol A is stable in vivo and has potential as a therapeutic agent for osteoporosis, particularly when administered in high doses.

DaVinci SP-based simultaneous bilateral partial nephrectomy from the midline transperitoneal approach: a case report

  • While simultaneous bilateral partial nephrectomy with a conventional multiport robot has been consistently reported since the 2010s, the introduction of the DaVinci SP system (Intuitive Surgical, Sunnyvale, CA, USA) could provide a novel way to perform surgery on bilateral kidneys while innovatively reducing the number of incisions. In our first report worldwide, the patient with bilateral small renal mass (2.0 cm for the left and 1.5 cm for the right side) and preoperative normal renal function was placed in the lateral decubitus position on an inverted bed. After tilting the bed to be as horizontal as possible, a 4-cm incision was made in the lower part of the umbilicus for the floating trocar technique. The partial nephrectomy was performed reliably as with the conventional transperitoneal approach, and then the patient could be repositioned to the contralateral side for the same procedure, maintaining all trocars. Total operation time (skin to skin), total console time, and the left- and right-side warm ischemic times were 260, 164, 27, and 23 minutes, respectively, without applying the early declamping technique. The estimated blood loss was 200 mL. The serum creatinine right after the operation, on the first day, 3 days, and 90 days after surgery were 0.92, 0.77, 0.79, and 0.81 mg/dL, respectively. For 90 days after the procedure, no complications or radiologic recurrence were observed. Further clinical studies will reveal the advantages of using the DaVinci SP device for this procedure over traditional multiport surgery, maximizing the benefit of a single port-based approach.

콩 이소플라본이 난소를 절제한 성장기 흰쥐의 골격대사에 미치는 영향 (Effects of Soy Isoflavone on the Bone Metabolism in Ovariectomized Growing Rats)

  • 본 연구에서는 대두 이소플라본이 난소를 절제한 성장기 흰쥐의 골격 발달에 미치는 영향을 알아보기 위하여 3주령 된 Wister 암컷 쥐를 난소절제한 후 8주 동안 genistein을 경구 투여하여 골대사에 미치는 일반지표 및 생화학지표를 관찰하였다. 1. 체중 증가는 OVX군$(230.2{\pm}25.15\;g)$이 SH군$(170.6{\pm}20.51\;g)$에 비해 유의적으로 높게 나타났으나 (p<0.05), OVX+GEN군과 OVX+ES군의 체중 증가는 OVX군에 비해 유의적인 차이를 보이지 않았다. 2. 체중 당 간과 자궁의 무게는 난소를 절제한 모든군에서 SH군에 비해 유의적으로 낮았으며(p<0.05), 신장의 무게 또한 SH군에 비해 난소를 절제한 모든 군에서 유의적으로(p<0.05) 감소하였으나 난소절제군 사이에서 유의적인 차이는 없었다. 3. 대퇴골의 무게는 모든 군간에서 유의적인 차이는 없었으나 대퇴골의 길이는 SH군에 비해 OVX군에서 길게 나타났다. 4. 뇨 중 creatinine의 양은 난소 절제군에서 모두 증가하였으나 유의적인 차이는 나타나지 않았다. 또한 hydroxyproline 함량도 모든 군간에서 차이가 나타나지 않았다. 5. 혈청 ALP 활성은 모든 군에서 차이가 없었으며 대퇴골, 변, 혈청, 뇨에서의 Ca의 함량도 모든 군간에서 유의적인 차이가 없었다. 6. 대퇴골의 골밀도는 SH군에 비해 난소를 절제한 군에서 낮았으며 genistein 투여로 다시 증가하는 경향을 보였으나 유의적인 차이는 없었다. 파단력과 stiffiness의 경우 난소 절제 및 genistein 처리에 의한 영향을 받지 않았다. 본 결과를 통하여 genistein에 의한 투여는 난소를 절제한 성장기의 흰쥐에서도 최대골질량을 유지하고 골질량의 손실을 억제할 수 있는 효과가 있어 성장기 동안의 섭취가 골다공증 예방에 효과가 있을 것으로 예상된다.

  • 본 연구는 유산균의 급여가 육계의 생산성, 영양소 이용성, 장내미생물, 계사내 환경 요인 및 혈청내 생화학적인 변화에 대한 영향을 구명하고자 실시하였다. 사양실험은 1일령 육계 수평아리 480수를 평사로 7주간 실행하였다. 첨가된 유산균 배양물은 육계 맹장에서 유래한 Lactobacillus reuteri 였으며, 첨가수준은 0, 0.1, 0.2, 0.4%였다. 체중은 1주령부터 유산균 배양물 급여구에서 대조구에 비하여 높았으며, 7주령에는 82~212g이 현저하게 높았다 (P<0.05). 사료섭취 량은 0.1% 첨가구가 가장 높았으며 및 사료요구율은 처리구간에 통계적인 차이가 없었다. 소화기관내 미생물 변화에서 유산균 배양물의 첨가구가 무첨가 대비유산균의 수는 회장과 맹장에서 증가하는 경향을 보였으며, 7주령에 맹장에서는 유의적으로 높게 나타났으며(P<0.05), 혐기성 미생물도 비슷한 경향을 보였다. E. coli와 Salmonella 늘 처리구간에 차이가 없었다 혈청내 total protein과 albumin 의 함량은 LR 0.1%구가 각각 3주령과 7주령에 가장 높게 나타났으며(P<0.05), total cholesterol은 처리구간에 일관적인 차이가 나타나지 않았다. Triglyceride는 3주령과 5주령에서 LR급여구가 대조구에 비하여 높게 나타났으며(P<0.05), glucose, blood urea nitrogen 그리고 calcium은 전 기간에 걸쳐 처리간 비슷하게 나타났다. inorganic phosphate는 7주령에서 LR 0.2%와LR 0.4% 급여구가 대조구에 비하여 함량이 높은 것으로 나타났다(P<0.05). 영양소 이용율에서 건물소화율은 LR 0.4% 급여구가 대조구에 비하여 증가하였으며(P<0.05), 단백질, Ca 이용율도 전체적으로 향상되었다(P<0.05). 계분내, NH$_3$ gas 발생량은 LR급여구에서 현저하게 감소하여 1/3~l/2의 수준을 유지하였다(P<0.05). 자리깃(왕겨)의 수분함량은 유산균 급여구가 무첨가 대비 감소되는 경향을 보였다. 이상의 결과에서 유산균 배양물의 급여는 육계의 생산성 향상과 사육환경 개선에 효과적으로 작용하였다고 판단된다.

차 음료가 카드뮴 섭취 흰쥐의 카드뮴 축적과 배설에 미치는 영향 (Effects of Tea Beverages on Cadmium Accumulation and Excretion in Rats Given Cadmium)

  • 말차, 오룡차, 홍차 및 보이차로 제조한 음료가 Cd에 중독된 SD계 흰쥐의 해독에 미치는 영향을 조사하였다. 관능검사 결과 일반 물 추출물에 비하여 색상, 향, 맛에 대한 기호도가 현저하게 향상되었으며, 종합적 기호도는 오룡차 음료가 4.18점, 홍차음료가 4.14점으로 말차음료나 보이차 음료의 $3.74{\sim}3.88$점에 비하여 높았다. SD계 흰쥐를 정상군 (NC), Cd 대조군(Cd-Co), Cd투여 말차음료 15% 급여군 (Cd-Gt), Cd투여 오룡차음료 15% 급여군(Cd-Ot), Cd투여 홍차음료 15% 급여군(Cd-Bt) 및 Cd투여 보이차음료 15% 급여군 식이군(Cd-R)의 6군으로 구분하여 5주간 사육하였다. 차 음료 식이군의 체중증가량은 NC군에 미치지는 못하였으나 Cd에 의하여 감소된 현상을 상당히 개선하였다. 식이효율, 뇨량 및 분변량은 각 실험군간의 유의적인 차이가 없었다. Cd의 투여는 간과 신장의 중량, 경골의 중량 및 대퇴골의 길이를 감소시켰으나 차 음료의 섭취로 다소 개선되었다. 혈청 AST 및 ALT 활성은 모두 Cd 투여군에서 현저하게 증가하는 경향을 보였으나 차 음료 급여군에서는 5%수준에서 유의적으로 감소되었다. 대퇴골의 조회분 함량은 Cd-Co군에서 40.72%, Cd 투여-차 음료 급여군에서는 $35.31{\sim}37.02%$로 차음료의 급여에 의하여 5%수준에서 유의적으로 감소하였다. 대퇴골의 조회분 함량은Cd-Co군에서 40.72%, Cd 투여-차 음료 급여군에서는 $35.3{\sim}37.02%$로 차음료의 급여에 의하여 유의적으로 감소하였다 대퇴골의 Cd함량은 NC군에서는 0.08 ppm이었으나 Cd-Co군은 123.28 ppm, 실험식이군에서는 $23.07{\sim}28.82 ppm$으로 차음료의 급여로 현저하게 감소하였으며, Ca 함량은 NC군에서 6700 ppm, Cd-Co군에서 3760 ppm, 실험식이군에서는 $4040{\sim}5420 ppm$이었다. 분변의 회분함량은 Cd-Bt군이 14.80%로 높았으나 기타 음료군은 Cd-Co군과의 유의적인 차이를 보이지 않았다. 분변의 Cd 함량은 NC군 0.02 ppm, Cd-Co군 2.06ppm, 실험식이군 $4.38{\sim}5.35ppm$로 차 음료 급여군에서 높았으며, 이와 대조적으로 Ca함량은 차음료 급여군에서 낮았다. 이상의 실험결과, 말차, 오룡차, 홍차 및 보이차로 제조한 음료는 상호간의 뚜렷한 차이는 없으나 모두 섭취한 Cd를 체외로 배설시킴으로서 Cd에 의한 장기 손상을 경감시켜 줄 수 있을 것으로 생각된다.
