DENTAL TREATMENT OF A PEDIATRIC PATIENT WITH HOMOZYGOUS PROTEIN C DEFICIENCY: A CASE REPORT (동형접합성 단백 C 결핍 환아의 치과적 치험례)

  • Protein C deficiency increases the risk of thrombosis due to the lack of anticoagulant factor protein C. Among the numerous congenital protein C deficiencies, homozygous protein C deficiency has an especially low protein C activity level, that it is almost undetectable. It is a rare disease with a probability of 1:250000~500000. The signs and symptoms of homozygous protein C deficiency include purpuric, necrotic dermatosis, ecchymosis, blindness, and thrombosis in central nervous system. A 4-year-old girl was brought to the clinic with a chief complaint of extensive caries. The child was under warfarin medication in order to prevent possible complications during dental treatment. We consulted the pediatric department. Without warfarin intake, serious complications may occur due to thrombosis during dental treatment. Therefore, certain warfarin dosage (INR 3~5) and fresh frozen plasma as a backup for excessive hemorrhage were recommended. This child was a severely disabled child with the loss of vision, and it was difficult to manage her behavior effectively. Thus, dental treatment was carried out under general anesthesia, where bleeding control would be also easier to achieve.This report presents the case of a 4-year-old girl with protein C deficiency, who has received dental treatment for extensive caries under general anesthesia.

A Clinical Study on 61 Cases of Tuberculous Tracheobronchitis (기관 및 기관지 결핵 61예의 임상적 고찰)

  • Tuberculous tracheobronchitis is defined as a specific inflammation of the trachea or major bronchi caused by the tubercle bacillus and recognized as one of the most common and serious complication of pulmonary tuberculosis. It had been a diagnostic challenge in prebronchoscopic era and since 1968, fiberoptic bronchoscopy has been accepted as a safe and valuable diagnostic procedure of tuberculous tracheobronchitis. Now, it remains a troublesome therapeutic problem due to its sequelae such as bronchostenosis, bronchiectasis and bronchial deformity. The authors analyzed the clinical features, radiological findings and bronchoscopic findings with pathologic and bacteriologic study on 61 cases of tuberculous tracheobronchitis and following results were obtained. 1) The peak incidence was in the fourth decade and male to female ratio was 1:3.4. 2) The most common symptom was cough (86.9%) and followed by sputum (49.2%), dyspnea (27.9%), fever (19.8%), weight loss (11.5%), hemoptysis (6.6%), hoarseness (6.6%) and chest discomfort (3.3%) and localized wheezing was heard in 18%. 3) In chest X-ray, consolidation with collapse was observed in 70.5%, and followed by consolidation only (18.0%), mediastinal node enlargement (8.2%), cavitary lesion (6.6%), suspicious hilar mass (3.3%) and miliary lesion (1.6%) and there was no abnormal findings in 4.9%. 4) Bronchoscopy showed hyperplastic lesion in 67.2%, mucosal lesion (18.0%), ulcerative lesion (9.8%) and stenotic lesion (4.9%). The most common site of bronchial lesion was right upper bronchus (36.1%) and followed by right main bronchus (34.4%), left main bronchus (29.5%), left upper bronchus (16.4%), right middle bronchus (8.2%), right lower bronchus (6.6%) and left lower bronchus (3.3%). 5) Chronic granulomatous inflammation with or without caseation necrosis on microscopic examination was confirmed in 69.7%, bronchial washing AFB stain was positive in 34.1%, prebronchoscopic sputum AFB stain was positive in 88.1% and postbronchoscopic sputum AFB stain was positive in 30.1%.

  • Currently, according to the climate change, serious damage by typhoon has been occurred in the world. In this respect, the research on the prediction model to minimize the damage from various natural disaster has been conducted in several developed countries. In the case of U.S, various models to predict building damage costs have been used widely in many organizations such as insurance companies and governments. In South Korea, although studies regarding damage prediction model according to typhoon have been conducted, the scope has been only limited to consider the property of typhoon. However, it is necessary to consider various factors such as typhoon information, geography, construction environment, and socio-economy factors to predict the damages. Therefore, to address this issue, first, correlation analysis is conducted between various variables based on the data of typhoon from 2003 to 2012. Second, the damage prediction model by using regression analysis is developed based on suggested influence factors. The findings of this study can be utilized to develop the model for predicting the damage costs of buildings by typhoon like HAZUS-MH of US.

A Study on Improvement of Field Implementation of Asphalt Surface Reinforcement Method (아스팔트 표면 강화공법의 현장 적용성 개선 방안 연구)

  • A study was carried out to improve the field implementation of asphalt surface reinforcement method which is a preventive maintenance. Mean Texture Depth (MTD) was measured to quantify the surface condition and used to determine the optimum application rate. Determining the application rate using MTD can reduce the material loss from 30% to 15%. In order to reduce the curing time to 30 minutes, the heat capacity of $317kcal/m^2$ is required. Therefore, the design capacity of the heating curing device requires more than 380,000 kcal/hr. The asphalt surface reinforcement method is preferably applied at a time when slight cracking occurs before the permanent deformation becomes serious. Through the analysis of the pavement survey data, it was decided to apply the surface reinforcement method at the crack rate of 3~4%. Heating the surface reinforcement agent to $50^{\circ}C$ improves workability and ensures sufficient penetration depth even at a crack width of 1 mm. The results will be utilized as basic data for the development of automated construction equipment for efficiency improvement.

  • Efficiency estimation of toxicity free resistance cable experiments were conducts to understand toxicity free fire ersistance polyolefin insulation material and smoke density characteristic and combustion gas corrosion analysis. A main cause of fire-growth and generating toxic gas when it burns, should be dealt with great care in life safety design. Similar patterned fire incidents such as, Inchon Live-Hof Pub Restaurant as, Sea-land Children Resort have proven that serious loss of lives were caused by hazardous gas generated fire resistance cable materials. In this paper, Referenced documents were ASTM E662 standard test method for specific Ds genalated by solid materials. The furnace control system shall maintain the required irradiance level under steady-state condition with the chamber door closed of 2.5$\pm40.04〔w/$\textrm{cm}^2$〕for 20 min. According to the results of the smoke density analysis of NFR-8 and FR-PVC the highest decomposition flaming smoke density range of NFR-8 and FR-PVC were 25.2 to 37.5 and 51.1 respectively. Nonflaming smoke density range of NFR-8 and FR-PVC were 100.4 to 112.2 and 126.5 to 398.8. Also, the fire gases was occurred carbon monoxide and decomposition than in polyolefin due to incomplete combustion of PVC which has high content of carbon in chemical compound.

Evaluation of Runoff and Sediment Yield Reduction with Diversion Ditch and Vegetated Swale Using WEPP Model (WEPP 모형을 이용한 우회수로 및 식생수로의 유출 및 토사유출 저감 평가)

  • It has been known that soil erosion caused by water has been a serious problem worldwide. Thus various modeling techniques for conservationists, farmers, and other land users have been developed and utilized to estimate effects of numerous site-specific Best Management Practices on soil erosion reduction. The physical process-based WEPP model would provide both temporal and spatial estimates of soil loss within small watersheds and for hillslope profiles within small watersheds. Thus, the WEPP watershed version was applied to study watershed, located at Jawoon-ri, Gangwon to simulate diversion ditch and vegetated swale with detailed input data set. The sediment yield and runoff reduction rates reduced by 5.8% and 29.6% with diversion ditch and 9.8% and 14.5% with vegetated swale. With vegetated diversion ditch, runoff and sediment yield could be reduced by 11.8% and 40.4%, respectively. Based on the results obtained in this study, the WEPP model would be an useful tool to measure runoff and sediment yield reduction and establish site-specific sediment reduction best management plan.

  • Network is changed from voice-based network into multimedia-based network by development of communication technology and multimedia service. We need a large bandwidth for multimedia service. The optical fiber is a more suitable medium than existing copper-based cable for large bandwidth. But, it is so expensive than copper-based cable. So, Minimizing total cost becomes a more important concept. In order to construct a minimum cost network, we have to consider existing conduits in network. On the other hand, optical fiber network allows that larger amount of traffic can be transmitted than copper-based network does. However, a failure of a node or link can make a serious damage to the network service. Thus, we have to get multiple paths to support continuous service even if a loss of failure occurs in some point of the network. The network survivability problem is to design the network that can provide reliable service to customers anytime with minimum total cost. In an existing solution of the network survivability problem with conduits, a conduit is considered only one time. But, the conduit is reusable if the network satisfies the required survivability. Proposed algorithm can more effectively considered already existed conduit. Network survivability and edge cost is predetermined. The proposed algorithm finds the best solution by conduit sharing within the limits of network survivability. According to the simulation result, the proposed method can decrease 7% of total cost than an existing method by effective conduits adaption.

A Study on Trends for Reforming the Rule of Warranty in English Insurance Contract Law (영국 보험계약법 상 담보법원칙의 개혁동향에 관한 연구)

  • Since the age of Lord Mansfield, who laid the foundation of the modern English insurance contract law in the second part of the 18th century, English insurance law has developed a unique rule of warranty. Lord Mansfield adopted very different approach and afforded such a strict legal character to insurance warranty, because the promise, given by the insured, played an important role for the insurer to assess the scope of the risk insured at that time. It is still important that the insured keep his promises strictly to the insurer under the insurance contract, but legal environments have changed dramatically since the times of Lord Mansfield. English Law Commission proposed some proposals for reforming the warranty regime to reflect the changes of legal environment in CP 2007. This article is, therefore, designed to examine the proposals and consider their legal and practical implications. The proposals of Law Commission is summarized as following. First, in CP 2007, Law Commission made two principal proposals for reform of the law on warranty. The first is that the insurer should not be entitled to rely on a breach of warranty unless the insured has been provided with a witten statement of what they have undertaken under warranty. The second is that the insurer should not be entitled to reject a claim on the ground that the insured has breached a warranty unless there was a causal connection between the breach and the loss. Secondly, for consumer insurance, the rule requiring a causal connection would be mandatory, whereas for business insurance, it would be possible for the parties to agree on the effect a breach of warranty should have, provided they use clear language to express their intentions. Thirdly, where the insured contracted on the insurer's written standard terms of business, some statutory controls would be afforded to the contract to ensure that the cover was not substantially different from what the insured reasonably expected. Finally, Law Commission propose that a breach of warranty give the insurer the right to terminate the contract, rather than automatically discharging it from liability, but (unless otherwise agreed) only if the breach has sufficiently serious consequences to justify termination under the general law of contract. Having evaluated the proposals of the Law Commission and considered their legal and practical implications, it is quite clear that the proposed rule interfere with freedom of contract and create legal uncertainty. But change can not made without any victims, so Law Commission's attempt to change severe and injust aspects of the warranty regime would be very welcomed and respected.

Understanding to Enhance Efficiency of Nitrogen Uses in a Reclaimed Tidal Soil

  • In most agricultural soils, ammonium ($NH_4{^+}$) from fertilizer is quickly converted to nitrate ($NO_3{^-}$) by the process of nitrification which is crucial to the efficiency of N fertilizers and their impact on the environment. The salinity significantly affects efficiency of N fertilizer in reclaimed tidal soil, and the soil pH may influence the conversion rate of ammonium to nitrate and ultimately affect nitrogen losses from the soil profile. Several results suggest that pH has important effects on recovery of fall-applied N in the spring if field conditions are favorable for leaching and denitrification except that effects of soil pH are not serious under unfavorable conditions for N loss by these mechanisms. Soil pH, therefore, deserves attention as an important factor in the newly reclaimed tidal soils with applying N. However, fate of N studies in a newly reclaimed tidal soils have been rarely studied, especially under the conditions of saline-sodic and high pH. Therefore, understanding the fate of nitrogen species transformed from urea treated into the reclaimed tidal soil is important for nutrient management and environmental quality. In this article, we reviewed yields of rice and fate of nitrogen with respect to the properties of reclaimed tidal soils.

Evaluating the Policy of Transfer System to Promote a Use of the Busan Subway (지하철 이용 활성화를 위한 환승체계의 정책대안 평가)

  • The purpose of this paper presents realistic policy alternative about recent tendency to decrease of subway-users and diminution of use efficiency which are serious problems of Busan Subway. Several policy alternatives have been studied until now, such as subway transfer impedance solution plan, introduction of subway to transfer fare discounting policy, and etc.. But, those policy alternatives are difficult to carried out, because they are less effective and overburden to financial aspect. Therefore, I made use of research on subway utilization to presuppose service improvement, as an alternative, in the transfer fare discounting system between bus and subway which might be powerful influence over subway-users. To verify this proposed study, I took advantage of Stated Preference(SP) where I estimated fare revenue and effects on fluctuation of subway-users with nested logit model based on research results. Suitable alternatives are as follows: First, If municipal government carries out transfer fare discounting policy without shortening in-vehicle time and out-of-vehicle time transfer fare, it is reasonable to discount transfer fare 50% off on the assumption of financial support as much as \6.700 million annually. Secondly, in case of application of multi-factors at a time, transfer fare discounting and in & out vehicle time, it is preferred to have no charge for transfer option with financial support as much as expected income-loss \5,600 million.
