Purpose: This study aimed to find the factors affecting medical sequelae management service satisfaction among injured workers. Method: This study population were 200 randomised samples of 619 medical sequelae management beneficiaries from April to June 2007 among occupational accident treatment ending workers in 2006. Data were collected through the telephone survey from November 28 to December 7 in 2007. Data were analyzed by ${\chi}^2$ test and multiple logistic regression using SAS 9.1 version. Results: According to the finding of this study, positive perceptions for the purpose and the effect of medical sequelae management were the factors influencing the satisfaction of medical sequelae management service. Conclusions: We recommend key issues to take into account for enhancing medical sequelae management service satisfaction in workers' compensation as follows; explaining the purpose and effect of medical sequelae management to client, evaluating medical sequelae management effect, and the policy participation of medical profession.
Objectives : In order to apply useful data to clinical practice, we undertook this study and tried to find factors related with sequelae of facial palsy in relation with patients' age and gender distribution, past history with the disease, condition before onset, duration of recovery time, existence of remaining sequelae, types of the sequelae, and duration for sequelae to disappear. Methods : We evaluated patients' condition (fatigue, stress, chills, cold and so on), past history (diabetes mellitus, hypertension, stroke, herpes zoster, cancer and so on), duration of recovery time, types of the sequelae, age and sequelae distribution as to when the treatments were started as we examined 473 patients who were diagnosed with facial palsy, and visited the Out-patient Department of Acupuncture and Moxibustion of Bundang Oriental Medicine Hospital of Dongguk University through 2003 and 2004. Results : The sequelae of facial palsy were not significantly relevant to the signs found before facial palsy occurred(fatigue, stress, chills, and cold), or to patients' past history (hypertension, diabetes mellitus, facial palsy, herpes zoster). The duration of recovery time was within 30 days for 45.3% of the patients examined in this study, and within 90 days for 72.6%. Evaluating the existence of sequelae in relation to age, we found more in the group comprised of patients aged 50 and over than under 50. We found more sequelae in the group which is consisted of patients who had not been treated until 6 days after than within 5 days from the onset. Conclusion: Attention to sequelae will be needed for patients aged 50 and over and who were not treated until 6 days after the onset, as they had more sequelae.
Objectives : The purpose of this study is research on facial palsy sequelae and evaluating scale that have studied insufficiently until now. Methods : We researched on the symptoms, epidemiology and evaluating scale of facial palsy sequelae. For this, we searched the research papers on facial palsy sequelae and the clinical papers that find out the effect of treatment by evaluating facial palsy sequelae. Results : The symptoms of facial palsy sequelae are synkinesis, contracture, spasm, crocodile tears syndrome, tearing decrease, gustation impairment, hearing impairment, tinnitus, hyperacusis, etc. Among these, synkinesis, contracture, spasm and crocodile tears syndrome are the most frequently observed broadly. The poor prognosis factor of facial palsy can be the risk factor of facial palsy sequelae. For example, severe degeneration of facial nerve can be the risk factor of facial palsy sequelae. Most of clinical papers on facial palsy sequelae have used NRS(numeric rating scale) as evaluating scale. But NRS is very subjective scale. The scales of Stennert, Peitersen, Murata et al. can evaluate facial palsy sequelae grossly. Sunnybrook scale, Sydney scale, SAQ(synkinesis assessment questionnaire), the scale of Kim, the scale of Scott, HFS-7(hemi facial spasm), HFS-36 and Schirmer's test can evaluate the respective symptoms of facial palsy sequelae. Conclusions : The symptoms of facial palsy sequelae are synkinesis, contracture, spasm, crocodile tears syndrome, etc. Most of clinical papers on facial palsy sequelae have used NRS as evaluating scale. There were some scales that can evaluate facial palsy sequelae grossly and respectively. In future, we will need more progressed study of facial palsy sequelae and evaluating scale.
Objectives : This study was performed to review articles referring to facial palsy sequelae evaluating scale and propose how to assess facial palsy sequelae. Methods : We searched for the medical literature to find out method of evaluating facial palsy sequelae. Then, we researched strengths and weaknesses of the scale and considered the way to appraise the facial palsy sequelae. Results : There were subjective and objective scale for evaluating facial palsy sequelae. Subjective scale could offer detailed assessment but had low objectivity and reproducibility. Objective scale had consistency and detachment but needed for specific equipments with time for measuring and calculating. Conclusions : We should evaluate the facial palsy sequelae with the strengths of subjective and objective scale and assess facial nerve function, symptoms of facial palsy sequelae and quality of life entirely. Moreover, the scale has to contain the reliability with static and dynamic assessment.
Objectives : The aim of this study was to observe the effect of Needle-embedding therapy on sequelae of peripheral facial palsy. Methods : 27 patients with sequelae of peripheral facial palsy were treated with Needle-embedding therapy. Yanagihara score, number of symptoms, symptoms score were compared between before and after treatment to evaluate the effect of Needle-embedding therapy. Results : After treatment, Yanagihara score, number of symptoms, symptom score were significantly decreased(p<0.05). Conclusions : Needle-embedding therapy could be effective to improve symptoms of sequelae of peripheral facial palsy patients.
Objectives : This study was designed to evaluate the effect of Jung-an acupuncture on the sequelae of peripheral facial palsy. Methods : Four patients suffering from sequelae of peripheral facial palsy were treated with Jung-an acupuncture. Yanagihara's total scores, the scale of Peitersen grades and picture, before and after the Jung-an acupuncture treatment were compared to evaluate its treatment efficacy. Results : Yanagihara's total score of all subjects were increased. Peitersen grades of all subjects were decreased. Conclusions : As indicated by unequivocal increase in all subjects' Yanagihara and Peitersen scores, Jung-an Acupuncture treatment is effective in alleviating the sequelae of peripheral facial palsy. Randomized-controlled trial for further evaluation is necessary to verify the results and findings of this study.
There has been an immense need for elaborate studies on the complications and the neuological sequelae generated by acute carbon monoxide (CO) poisoning which is highly prevalent in Korea due to widespread adoption of the anthracite coal briquette as domestic fuel for heating and for cooking. For this epidemiological study, a total of 444 subjects who received hospital emergency care for acute CO poisoning during the period of March 1982 to February 1983 were randomly selected from the emergency patients's lists of 13 general hospitals in Seoul area. Informations on the neurological sequelae were elucidated by means of home visiting with prearranged questionnaire consisting questions and concise neurological examination. The findings obtained were summarized as follows; 1. The complications were found in 18% of the surveyed and acute decubitus was comprised 67.5% of the complications. 2. The total cumulative incidence of the neurological sequelae was 41.2 per 100 patients and the absolute incidence rate regardless of the duration after poisoning was 40.8%. 3. The incidence of the neurological sequelae was higher in the older age than in the younger and also higher in female than in male. Twice higher incidence was observed in the admitted patients than in the non-admitted patients and the incidence became higher in proportion to the duration of CO exposure, coma and admission. The poorer the consciousness level of patients found, at emergency room and at discharge, the higher the incidence. The incidence of the neurological sequelae by emergency care was higher in hyperbaric oxygen therapy group(51.9%) than in 100% $O_2$ group(38.0%) 4. A total of five variables significantly associated with the occurrence of the neurological sequelae were selected by the stepwise discriminant analysis. The variables were following course of emergency care, age, consciousness level at discharge, admission duration, and consciousness level at emergency room in their sequence of discriminant power. Eight variables were selected as those associated with the degree of the neurological sequelae through the stepwise multiple regression analysis. Of these variables, the acute decubitus alone explained 21.1% of the total variation ana all the eight variables could explain 36.5% of the same. The remaining seven variables listed in the order of their relative importance were: age, consciousness level at discharge, admission duration, coma duration and consciousness level at emergency room. 5. It was postulated that unexpectedly high incidence of the neurological sequelae of the CO poisoning in this epidemiological study was mainly due to the inadequate emergency care and the lack of efficient and sophisticated treatment measure. In the effort to minimize the incidence of grave neurological sequelae of acute CO poisoning, new guidelines for the emergency care and treatment should be pursued with efficient ways.
Objectives : The purpose of this study is to confirm and to suggest the Korean medical treatment is effective treatments to patients with facial paralysis sequelae. Methods : We conducted a survey on patients with facial paralysis sequelae who visited the facial paralysis center from August 2017 to November 2018. We then evaluated House-Brackmann Grading System(HBGS), Sunnybrook Scale(S-Scale), Visual analog scale(VAS) against those who agreed and analyzed the information through Electronic Medical Record(EMR) and Order Communication System(OCS). Results : Clinically, Korean medical treatment such as embedding therapy are effective for the facial paralysis sequelae. For the purpose of raising the level of evidence against this, research will be needed to confirm the treatment effects by comparing accurate assessment indicators that are conducted before and after the treatment. Conclusions : In order to confirm the progress of treatment of facial paralysis sequelae, evaluation indicators such as House-Brackmann Grading System and Sunnybrook Scale should be performed by experts. And after 3-4 weeks, if patients have any sequelae symptoms, it may be helpful to take treatments such as pharmacopuncture treatment and embedding therapy at intervals of 1-2 weeks depending on the symptoms or areas.
Background : Sequelae symptoms of peripheral facial palsy include not only partial recovery of the paretic muscles but contracture, spasm, synkinesis and atrophy which cause significant functional, esthetic, and psychosocial disturbances to the lives of patients. Objective : This study is to investigate the prevalence, time of onset, patient's self assessment on the degree of understanding of the sequelae and the association of the degree of palsy with the appearance of sequelae. Methods : 106 patients with peripheral facial palsy were sequentially interviewed and examined. Results : 29 patients(27.4%) of 106 patients showed sequelae symptoms, of whom 19 patients(65.5%) showed sequelae symptoms during 4 to 6 months after the onset of palsy. The degree of understanding on the sequelae at the time of interview showed improvement compared to that at the time of first consultation to any physician but not to reach an sufficient understanding. No relation between the degree of palsy and the appearance of sequelae was observed.
Objectives : The purpose of this study is to report the effect of Jung-an acupuncture on the sequelae of peripheral facial palsy.Methods : Five patients suffering from sequelae of peripheral facial palsy were treated with Jung-an acupuncture.
