A Study on the Appropriateness of Health Insurance Fee in Main Nursing Practices (주요 간호활동의 건강보험 수가 적절성에 관한 연구)

  • Purpose: This study was to present improvement strategy and the problems of the nursing fee in national health insurance system. Methods: A total of 23 nursing activities performed by nurses were selected. Data were collected the relative value score and criteria of the Health Insurance Review & Assessment Agency. Sixty clinical nursing experts panels were composed and nursing time surveyed self-reported method. The actual fee was calculated through the nursing time, relative value score and actual labor costs. Nextly, the labor costs analyzed was compared with that in the national health insurance. Results: Although the practices were mainly performed by the nurse, other occupations have been recorded as main practitioners and the time of the nursing activity is partially improperly reflected. Additionally, although the nurse practiced mainly in glucose (semi-quantitative) test, it was confirmed that the principal practitioner was described as a clinical pathologist. The the labor cost gap was estimated that is 9.3 times (median) and 11.9 times (average) in this analysis. Conclusion: This study suggests that it is necessary to legislate a policy that can improve the quality of clinical nursing by reinforcing the appropriateness and improving nursing fee through reflection of the actual time spent for nursing care.

Association of Supplementary Private Health Insurance Type with Unmet Health Care Needs (민간의료보험 유형과 미충족 의료와의 관련성)

  • Background: The extent of coverage rate of the public health insurance is still insufficient to meet healthcare needs. Private health insurance (PHI) plays a role to supplement coverage level of national health insurance in Korea. It is expected that reduce unmet need healthcare. This study was aimed to identify relationship between PHI type and the unmet healthcare need and its associated factors. Methods: Data were obtained from the 2014 Korea Health Panel Survey using nationally representative sample was analyzed. Respondents were 8,667 who were adults over 20 years covered by PHI but have not changed their contract. According to the enrollment form, PHI was classified into three types: fixed-benefit, indemnity, and mixed-type. To identify factors associated with unmet needs, multiple logistic regression conducted using the Andersen model factors, which are predisposing factors, enabling factors, and need factors. Results: Our analysis found that subjects who had PHI with mixed-type were less likely to experience unmet health care needs compared than those who did not have it (odds ratio, 0.80; 95% confidence interval, 0.66-0.98). As a result of analyzing what affected their unmet healthcare needs, the significant factors associated with unmet medical need were gender, marital status, residence in a metropolitan area, low household income, economic activity participation, self-employed insured, physically disabled, low subjective health status, and health-risk factors such as current smoking and drinking. Conclusion: The results of this study suggest that having PHI may reduce experience of unmet healthcare needs. Findings unmet healthcare needs factors according to various subjects may be useful in consideration of setting policies for improving accessibility to healthcare in Korea.

Health Problems and Coping of Workers under Special Employment Relationships: Home-visit Tutors, Insurance Salespersons, and Credit Card Recruiters (특수고용형태근로종사자들의 건강문제와 대처: 학습지 교사, 보험설계사, 신용카드회원모집인을 중심으로)

  • Purpose: This study aimed to determine health problems experienced by workers in special employment relationships (WSER) and identify coping strategies used when such problems occur. Methods: This qualitative study used the focus group interview method. Thirteen study participants included five home-visit tutors, five insurance salespersons, and three credit card recruiters. The interviews were conducted from November 2018 through January 2019, with each occupational group interview lasting about 2 hours. Analysis based on phenomenological research was independently performed by two researchers. Results: Most participants had common health problems involving vocal cord symptoms, and stress related to emotional labor and traffic accidents. The unique health problems included cystitis, musculoskeletal, and digestive symptoms in home-visit tutors; reduced vision and hearing in insurance salespersons; and mental distress in credit card recruiters. There was no protection system for their health coverage, and the company emphasized their self-employed status to avoid taking responsibility for them. Twelve participants did not purchase occupational accident insurance owing to both not having adequate information and economic burden concerning premium status. Conclusion: WSER experienced both physical and mental health problems. These problems were caused by their unstable employment status, and the social security system for their coverage being non-functioning.

The Impacts of Social Support on Industrial Injured Workers' Self-rated Health (산업재해 근로자의 사회적 지지가 주관적 건강에 미치는 영향)

  • Background: Social support contributes directly and indirectly to maintaining physical, mental, and social well-being. The aim of the study was to identify the impact of social support on self-rated health among Korean industrial accident workers. Methods: This study used data from the panel study of workers' compensation insurance (PSWCI). The final subjects were 2,759 workers who responded to a 2018 to 2020 PSWCI. Social support was defined as social contact with friends, neighbors, family, and social participation activities like religious activity, social activity, and club activity. Multivariate logistic regression analysis was performed to investigate causal relationships between social support and self-rated health using a generalized estimating equation model. Results: Proportion of workers' good self-rated health steadily increased (2018: n=1,447, 63.2%; 2019: n=1,542, 66.2%; 2020: n=1,653, 67.3%). Higher levels of social contacts with friend (worse: reference; same: β=0.442) and higher levels of social activity (yes: reference; no: β=-0.173) were especially associated with good self-rated health. Conclusion: This study confirmed social support positively influenced self-rated health among the self-rated health of industrial injured workers. The results of this study suggested that recovery policies that the government served should include programs enhancing social support for improving health among industrial injured workers.

The Association between Household Type and Self-rated Health of the Elderly in Korea: Analysis of the National Survey of Older Koreans 2017 (우리나라 노인의 가구형태와 주관적 건강상태의 관련성: 2017년 노인실태조사 자료를 이용하여)

  • Background: In Korea, the population is rapidly aging, and the types of households for the elderly are also diversifying. The self-rated health of the elderly is a valuable health indicator that can comprehensively represent the overall quality of life along with physical, mental, and functional health. On the other hand, studies on the association between household type and self-rated health of the elderly are still insufficient. Thus, this study analyzed the association between household type and self-rated health by gender in Korean older adults. Methods: Using data from the analysis of the National Survey of Older Koreans 2017, 10,299 elderly people aged 65 and over were targeted. For the accuracy of the analysis data, 9,910 people were selected as the study sample by excluding proxy responses, those diagnosed with dementia, and non-response. And technical analysis, univariate analysis using the Rao-Scott chi-square test, and logical regression analysis involving survey characteristics were conducted by gender. Results: According to the adjusted model with all variables, in both men and women, the odds ratio of self-rated health 'bad' in 'couple (with ill spouse)' was significantly higher than 'couple (with spouse)'. It was 2.54 (95% confidence interval [CI], 2.05-3.15) for men and 2.11 (95% CI, 1.70-2.62) for women. In addition, the odds ratio of self-rated health 'bad' in 'living with adult children' was 1.43 (95% CI, 1.09-1.87) for men and 1.42 (95% CI, 1.15-1.75) for women, which was more significant in women than men. Conclusion: This study states that there is an association between gender, household type, and self-rated health of the elderly, and the health of a spouse and cohabitation with children have a significant effect on self-rated health. As a result, in order to improve the health status of the elderly, health promotion and health care policies involving the characteristics of the elderly's gender and household type are needed.

  • Data Envelopment Analysis(DEA), a non-parametric productivity analysis tool, has become an accepted approach for assessing efficiency in a wide range of fields. Despite of its extensive applications and merits, some features of DEA remain bothersome. DEA offers no guideline about to which direction relatively inefficient DMUs improve since a reference set of an inefficient DMU, several efficient DMUs, hardly provides a stepwise path for improving the efficiency of the inefficient DMU. In this paper, we aim to show that DEA can be used to evaluate the efficiency of life insurance companies while overcoming its limitation with the aids of machine learning methods.

Factors Determining Cesarean Section Frequency Rates of the OBGY Clinics in Metropolitan Area (수도권 소재 산부인과의원의 제왕절개율과 관련 요인분석)

  • This study aims to find factors that affect variations in cesarean section frequency rates among OBGY clinics in Metropolitan areas. The factors include patient, medical supplier characteristics and economic factors. This study is a cross-sectional analysis using health insurance delivery claims from July to December 2000 and files of the NHIC(national health insurance corporation). Multiple regression was used to analyze the dependent variable of cesarean section frequency rate at each clinic. The results are as follows : Cesarean section frequency rate is increasing in proportion to the number of the following patients : repeated caesarean section, disproportion, obstructed labour, fetal distress, emergency caesarean section and self-employed patients. There are geographic variations as well. Cesarean section frequency rates are higher in Inchon and Gyonggi province than in Seoul. The higher number of total delivery the clinic has, the lower rate of cesarean section it has. Clinics with high frequency rates in 1999 showed higher rates the next year. Further research is required to develop evidence based delivery modes and change strategies for increasing normal delivery and activating midwife clinics.

A Study on Symptoms of VDT Syndrome among Health Insurance Review Nurses (보험심사간호사의 VDT 증후군 증상에 관한 연구)

  • Purpose: The present study examines symptoms of VDT syndrome perceived by those nurses reviewing the application of health insurance at hospitals. This study also investigates those factor influencing on the experience of VDT syndrome among those nurses. Method: Data were collected through mail questionnaire survey in April, 2003. Of the 250 questionnaires disseminated, 214 questionnaires were finally analyzed using descriptive statistics and multiple regression analysis through SPSS/PC version 10.0. Result: Of the VDT syndrome, the musculoskeletal symptom score was highest, followed by ophthalmologic symptoms. The musculoskeletal symptoms were higher among younger nurses, having smaller space under the desk, and feeling uncomfortableness of the chair. The ophthalmologic symptoms were higher among those with a bachelor's degree, younger nurses, and those having a light reflection on the computer monitor, and those maintaining less than 40cm between eyes and the monitor. Higher dermatologic symptoms were observed among younger nurses and those using unclear monitor. The psychological symptoms were higher among younger nurses, those having a light reflection on the monitor, those unable to control the height of their chair, and those using uncomfortable chair. The overall symptoms were higher among younger nurses, those having a light reflection on the monitor, and those smaller space under the desk. Conclusion: Primary intervention should be given to those nurses with symptoms of VDT syndrome in order to regain health and prevent further aggravation of the symptoms. In particular, self-care behaviors of the nurses using VDT should be promoted with administrative support.

A study on the production environment of crown prosthesis for National Health Insurance benefit (건강보험 급여화 관련 크라운보철물 제작실태 연구)

  • Purpose: The purpose of this study was to investigate the production environment of crown prosthesis for National Health Insurance(NHI) benefit. Methods: This study carried out self-administered questionnaire survey from September 1, 2016 to October 31 by having research subjects as 261 dental technician. Except 100 copies with incomplete response, 161 copies were used as the materials of final analysis. Results: Unlike gold crowns, the material cost of metal crowns was paid at the dental laboratory(86.3%). Total material consumption for making metal crown was more than gold crown(63.4%), especially for the finishing and polishing processes(78.3%). The subjects responded that a routine dental laboratory fee of crown prosthesis is unreasonable, and it is necessary to adjust and improve it(metal crown 96.2%, gold crown 96.9%). NHI coverage dental prosthesis was not marked on the order form(46.0%), and the dental laboratory fee of that was nor received(64.0%). Conclusion : It is necessary to estimate the NHI cost level of the crown prosthesis by reflecting the production environment and engineering process in dental laboratories. In addition, institutional arrangement should be backed up so that dental laboratories can receive appropriate dental laboratory fee.

The relations of the Long-term care insurance awareness and the aged parent-supporting (장기요양보험제도 안식과 노부모 돌봄과의 관계)

  • The purpose of this research was to examine the whether to recognition of the long-term care insurance have affected on aged parent-supporting. The research method was used self-report questionnaire to 331 persons who was supported their parents. And I used the Frequency, simple linear regression as an analysis methods. The result demonstrated that Educational background, the emotional intimacy of the subjects, and the recognition degree of the long-term care service have significant influence on their physically parent supporting consciousness. The researchers suggest the need of family support system based on the result for decrease the parent-supporting burden in middle age generation.
