Purpose: The objective of this study were to examine the determinants of self-rated health, specially focused on the effect of functional capacity of community dwelling sedentary older adults on self-rated health. Method: The data has been collected from 654 community-dwelling sedentary older adults (mean age: 75 years) during the period from April to June in 2007. The data were collected by the in-person interview and direct measurement of functional capacity. The data were analyzed using chi-square test and multiple regression analysis with the SPSS 9.1 program. Result: The elderly rated their health as very good (3%), good (28%), fair (38%), poor (29%0, and very poor (2%). The higher average daily walk minutes ($\beta$=0.12, p<.01), number of chair stand ($\beta$=0.10, p<.05), scores of self-efficacy ($\beta$=0.16, p<.001) and the lower number of disease ($\beta$=-0.44, p<.001) show better self-rated health. Conclusion: Self-rated health is the most commonly used indicators in social epidemiology and geriatric research because it has been known as the good predictor of mortality and reflects health related disability. The finding suggested that daily walking habits, lower body strength, physical self-efficacy should be considered to improve the senior's self-perception of health. The community-based intervention associate increase these factors should be considered.
Purpose: This study aimed to investigate objectively measured physical activity (PA) in institutionalized older adults with mild cognitive impairment (MCI) and to elucidate the influence of autonomic nervous function, salivary cortisol, and PA on cognitive functions based on neurovisceral integration model. Methods: Overall cognitive function was evaluated using the mini-mental state examination (MMSE) and executive function was evaluated using semantic verbal fluency test and clock drawing test. Actigraph for PA, HRV and sAA for autonomous function, and the geriatric depression scale for depression were used. Saliva specimens were collected in the morning for sAA and cortisol. Results: Ninety-eight older adults from four regional geriatric hospitals participated in the study. They took 4,499 steps per day on average. They spent 753.93 minutes and 23.12 minutes on average in sedentary and moderate-to-vigorous activity, respectively. In the multiple regression analysis, lower salivary cortisol level (β = - .33, p = .041) and greater step counts (β = .37, p = .029) significantly improved MMSE score. Greater step count (β = .27, p = .016) also exerted a significant influence on verbal fluency, and greater sAA (β= .35, p = .026) was significantly associated with a better clock drawing test result. Conclusion: Salivary cortisol, sAA and physical activity were significantly associated with cognitive functions. To prevent older adults from developing dementia, strategies are needed to increase their overall PA amount by decreasing sedentary time and to decrease salivary cortisol for cognitive function, and to maintain their sympathetic nervous activity for executive function.
우리나라 고령인구의 급격한 증가로 인해 '건강한 노화'에 대한 관심이 높아지고 있으며, 이러한 흐름에서 노인들의 생활습관이 건강관련 삶의 질과 어떠한 관계를 가지는지 규명하는 것은 중요하다. 본 연구의 목적은 한국 노인에게서 신체활동 수준 및 좌식시간과 건강관련 삶의 질의 연관성을 알아보는 것이며, 2014~2016년 국민건강영양조사 데이터를 이용하여 한국 노인 4,589명을 분석하였다. 분석결과 활동적인 여성 노인에게서 더 좋은 건강관련 삶의 질이 나타났으며, 이는 복부비만일 경우 두드러지게 나타났다. 이러한 관계는 남성 노인에서 관찰되지 않았다. 반면에 좌식시간이 긴 남녀 노인에게서 삶의 질이 낮게 나타났으며, 특히 여성 노인은 복부비만 유무에 따라 좌식시간과 건강관련 삶의 질 간의 관계에 차이를 보였다. 추가적으로 신체활동 유형과 건강관련 삶의 질 하위항목 간의 관계를 조사한 결과, 노인들은 장소이동 관련 신체활동량이 증가할수록 건강관련 문제가 있을 가능성이 낮고, 좌식시간이 증가할수록 문제가 있을 가능성이 높은 것으로 나타났다. 본 연구결과는 노인에게서 신체활동 수준, 좌식시간, 복부비만 유무가 건강관련 삶의 질과 관련 있음을 보여주며, 좌식시간을 줄이는 것이 삶의 질 향상과 보다 밀접한 연관이 있음을 시사하고 있다.
본 연구는 만 65세 이상의 노인 49명을 대상으로 가속도계와 신체활동일기를 함께 사용하여 신체활동량과 좌식행동패턴을 성별에 따라 비교하고 WHO 신체활동지침의 실천율을 평가하였다. 1. CPM과 METs는 성별에 따른 유의한 차이가 없었으나 PAEE는 남자가 여자보다 유의하게 높았다. 활동 강도별 1일 평균 소요시간을 살펴보면, 본 연구대상자들은 좌식행동에 소요되는 시간 (남녀 각각 60.6%, 57.6%)이 가장 많았으며 좌식행동 및 중 고강도 활동의 소요시간에서는 남녀간의 유의한 차이를 보이지 않았으나 저강도 활동의 소요시간은 여자가 $364.2{\pm}74.5$분으로 남자 ($313.7{\pm}70.8$분)보다 유의하게 많았다. 2. 남녀 모두 수면시간을 제외한 좌식행동 (2 ~ 5단계) 중 2단계의 '휴식, 담화 및 TV보기' 활동으로 가장 많은 시간 (남자 26.9%, 여자 22.0%)을 보내고 있었으며, 남자가 $388.0{\pm}134.3$분으로 여자 ($316.1{\pm}65.2$분)보다 유의하게 많았다. 저강도 활동에 포함되는 활동 중에서는 7단계의 '미용 및 옷 갈아입기' 활동과 9단계의 '음식 준비 및 정리하기' 활동의 소요시간에서 여자가 남자보다 유의하게 많았다. 중 고강도 활동에서는 15단계의 '체조, 탁구 및 자전거타기' 활동의 소요시간에서 남자가 여자보다 유의하게 많게 나타났다. 3. 좌식행동패턴을 성별에 따라 비교한 결과, 좌식행동을 30분과 40분 이상 지속한 횟수에서 남자가 각각 $3.10{\pm}1.34$회, $1.78{\pm}1.09$회로 여자 ($2.34{\pm}1.22$회, $1.32{\pm}1.07$회)보다 유의하게 많았으나 시간당 좌식행동의 지속을 끊어주는 횟수는 남자 ($5.74{\pm}0.89$회)가 여자 ($6.44{\pm}0.71$회) 보다 유의하게 적었다. 좌식행동을 30분 이상 지속한 횟수 및 시간 당 좌식행동을 끊어주는 횟수는 2단계의 '휴식, 담화 및 TV보기' 활동의 소요시간과 유의한 상관관계를 보였으며 주로 저강도 활동을 통해서 좌식행동의 지속이 끊어지는 것으로 나타났다. 4. 일주일 동안 중 고강도 활동을 각각 2분과 5분 이상 지속한 시간에서는 남자가 $319.8{\pm}255.2$분과 $247.3{\pm}228.2$분으로 여자 ($193.3{\pm}130.6$분, $137.2{\pm}101.9$분)보다 유의하게 많은 것으로 나타났다. 또한 본 연구대상자의 경우 중 고강도 활동의 짧은 지속시간 (10분 미만)으로 인하여 WHO 신체활동지침의 실천율이 22.4% (남자 30.8%, 여자 13.0%)으로 매우 낮았다. 본 연구 결과를 통하여 노인의 경우 장시간 지속되는 좌식행동 패턴을 줄이기 위하여 노력해야함을 알 수 있었다. 이를 위하여 무엇보다도 앉아서 TV를 시청하는 시간을 줄이는 것이 중요함이 지적되었다. 앞으로 노인의 만성질환 예방을 위하여, 노인의 신체활동량 및 좌식행동 패턴과 건강 관련 지표와의 관련성 분석 평가 연구가 필요하다. 특별히 본 연구에서 노인만을 위한 별도의 신체활동지침 마련의 필요성이 제시된 바, 노인에게 적합한 신체활동의 적정 강도 및 적정 유지 시간 등이 반영된 새로운 지침이 개발되어야 할 것이다.
Background: Physical activity and quality of life (QOL) influence the health status of older adults. Recently, the use of wearable devices to monitor physical activity has increased. Objects: This study examined the relationship between the amount of physical activity, measured using a wearable device, and QOL among older adults. Methods: In total, 71 older adults (aged ≥ 65 years) were enrolled. The amount of physical activity was measured using a wearable device with a wrist strap, and daily physical activity was classified according to intensity (sedentary, light, moderate, or very active). Self-reported QOL was evaluated using the Short Form 36 (SF-36) questionnaire. Pearson and Spearman correlation analyses were conducted to analyze parametric and non-parametric variables, respectively. The relationship between amount of daily physical activity and SF-36 scores was assessed. Results: The correlation analyses revealed positive correlations between the amount of moderate-intensity and very active physical activity (minutes/day) and SF-36 scores (p < 0.05). Conclusion: Physical activity of at least moderate intensity is associated with better QOL in older adults. Further studies are required to verify the effects of increased physical activity on QOL in older adults.
Background: Growing evidence suggests that beta-hydroxy-beta-methylbutyrate (HMB), arginine (Arg), and glutamine (Gln) positively affect wound recovery. This study investigated the effects of long-term administration of HMB/Arg/Gln on pressure ulcer (PU) healing in sedentary older adults admitted to geriatric and rehabilitation care facilities. Methods: This was a pilot retrospective case (standard of care and HMB/Arg/Gln)-control (standard of care alone) clinical study. Outcome measures were relative healing rates and Pressure Ulcer Scale for Healing (PUSH) scores (calculated after 4, 8, 12, 16, and 20 weeks) and time to healing. Results: The study subpopulation was comprised of 14 participants (four males, 28.6%) with the median age of 85.5 years (interquartile range [IQR], 82.0-90.2 years). The control subpopulation was comprised of 31 participants (18 males, 58.1%) with the median age of 84.0 years (IQR, 78.0-90.0 years). At the beginning of follow-up, there were no statistically significant demographic (sex and age) and clinical (main diagnosis, baseline area, and PU perimeter) differences between the groups. During the study period, there were no significant differences in the relative healing rates and PUSH scores between the subpopulations. The median time to complete healing in the study and control populations was 170.0 days (95% confidence interval [CI], 85.7-254.3) and 218.0 days (95% CI, 149.2- 286.7) (log-rank, chi-square=3.99; p<0.046), respectively. Conclusion: More than 20 weeks of HMB/Arg/Gln supplementation had a positive effect on difficult PU healing in older adults with multiple comorbidities.
[Purpose] Aerobic exercise training (AT) reverses aging-induced deterioration of arterial stiffness via increased arterial nitric oxide (NO) production. Asymmetric dimethylarginine (ADMA), an endogenous inhibitor of NO synthase, was decreased by AT. However, whether AT-induced changes in ADMA levels are related to changes in nitrite/nitrate (NOx) levels remains unclear. Accordingly, we aimed to clarify whether the relationship between plasma ADMA and NOx levels afected the AT-induced reduction of arterial stifness in middle-aged and older adults. [Methods] Thirty-one healthy middle-aged and older male and female subjects (66.4 ± 1.3 years) were randomly divided into two groups: exercise intervention and sedentary controls. Subjects in the training group completed an 8-week AT (60%-70% peak oxygen uptake [${\dot{V}}O_{2peak}$] for 45 min, 3 days/week). [Results] AT signifcantly increased ${\dot{V}}O_{2peak}$ (P < 0.05) and decreased carotid β-stifness (P < 0.01). Moreover, plasma ADMA levels were significantly decreased while plasma NOx levels and NOx/ADMA ratio were significantly increased by AT (P < 0.01). Additionally, no sex diferences in AT-induced changes of circulating ADMA and NOx levels, NOx/ADMA ratio, and carotid β-stifness were observed. Furthermore, the AT-induced increase in circulating ADMA levels was negatively correlated with an increase in circulating NOx levels (r = -0.414, P < 0.05), and the AT-induced increase in NOx/ADMA ratio was negatively correlated with a decrease in carotid β-stifness (r = -0.514, P < 0.01). [Conclusion] These results suggest that the increase in circulating NOx with reduction of ADMA elicited by AT is associated with a decrease in arterial stiffness regardless of sex in middle-aged and older adults.
Objectives: A physically active lifestyle is important for cancer survivors. Therefore, this study was conducted to 1) provide population-based estimates of the prevalence of physical activity and sitting time, and 2) their correlates in Korean cancer survivors. Materials and Methods: This study analyzed a cancer survivor subsample (N=1,482) from 2008-2013 Korea National Health and Nutrition Examination Survey (KNHANES), data selected with a complex sampling design. Overall and subgroup-specific prevalences of physical activity and sitting time were estimated. Correlates of moderate- to vigorous-intensity physical activity ( MVPA) and sitting time were tested using age-group-specific hierarchical multiple regression models. Results: Overall adherence rate to physical activity guidelines was 34.9% (95% CI=31.5-38.4). Age-group-specific adherence rates were 41.1% (95% CI=36.3-45.9) in adults (30-64 years old), and 25.3% (95% CI=21.0-25.3) in older adults (65 years or older). Adults spent 213.33 minutes (95% CI=172.4-254.3) per week on MVPA and 55.3 minutes (95% CI=36.4-64.6) on sitting time per day. In adults, sitting time was significantly associated with employed status (B=28.0, p=0.046), smoking (B=-47.4, p=0.020), and number of comorbidity conditions (B=-13, p=.037). MVPA was significantly associated with marital status (B=134.9, p<0.001), employment status (B=98.12, p=.046), and years since cancer diagnosis (B=104.7, p=0.015). Older adults spent 162.2 minutes (95% CI=119.5-204.8) per week on MVPA and 63.0 minutes (95% CI=45.0-89.5) on sitting time per day. Their significant correlates were sex (B= -45.2, p=0.014), smoking (B=-70.14, p<0.001), and years since cancer diagnosis (B=37.0, p=0.024). Age (B=5.8, p=0.042) and marital status (B=83.8, p=0.033) were also significantly associated with MVPA in older adults. Conclusion: A majority of Korean cancer survivors do not sufficiently participate in physical activity. In general, older, unhealthier, non-working, and being unmarried were risk factors for physical inactivity. While this study informs public health policy makers and practitioners about physical activity intervention demand for cancer survivors, future investigations should address psychosocial mediators to better inform intervention programs.
Objectives: This study aimed to investigate arterial stiffness index, physical activity, and food and nutrient intake in middle-aged adults over 40 years when the incidence of cardiovascular disease begins to increase. Methods: This study included 106 subjects (48 males and 58 females) aged between 40 and 64 years. The arterial stiffness index (brachial-ankle pulse wave velocity [baPWV], and ankle-brachial index [ABI]) were measured using a blood pressure pulse wave testing device. Physical activity was assessed using the Korean version of the Global Physical Activity Questionnaire, and food and nutrient intake was calculated using the Food Frequency Questionnaire. Results: The mean age of the subjects was 54.4 years. Although the ABI of the subjects was within the normal range, they were divided into tertiles to compare physical activity and food and nutrient intake. In males, the time spent on moderate to vigorous physical activity (MVPA) was significantly higher in T3 (600.6 min/week) than in T1 (304.4 min/week). In females, the time spent in sedentary behavior was significantly lower in T3 (294.5 min/week) than in T1 (472.1 min/week). In addition, the frequency of fish consumption was significantly higher in T3 (1.27 frequency/day) than in T1 (0.64 frequency/day) in females. Polyunsaturated fatty acid (PUFA) and ω-3 fatty acid intake, adjusted for energy intake, were significantly positively correlated with ABI (r = 0.200 and r = 0.218, respectively). Conclusions: High MVPA (in males), low sedentary behavior (in females), and PUFA and ω-3 fatty acid intake through fish consumption may be associated with low peripheral artery stiffness. Therefore, arteriosclerosis can be prevented through physical activity and proper dietary therapy.
Objectives: After many national physical activity guidelines have established, recent changes are deep and rapid. So the major features and implication to reverse worsening Korean physical activity indicator is desprate but related knowledge and informations are few. So review of recent features and implications of new physical activity guidelines have made. Methods: National physical activity guidelines of advanced countries were searched through snowballing methods. Major features were described according to the nation. Implication were drew through discussion for Korean realitiy. Results: New Australian physical activity and sedentary behaviour guideline explicitly included sedentary behaviour. The age in the guideline expanded to early years. Canada also presented 24-hour movement guidelines to early years. The second generation of the physical activity guidelines reflects the extensive amount of new knowledge. New aspects include discussions of additional health benefits related to brain health, additional cancer sites, and fall-related injuries; immediate and longer term benefits for how people feel, function, and sleep; further benefits among older adults and people with additional chronic conditions; risks of sedentary behavior and their relationship with physical activity; elimination of the requirement for physical activity benefits to occur in bouts of at least 10 minutes; and tested strategies that can be used to get the population more active. Conclusions: The most important message from the new guidelines is that the greatest health benefits accrue by moving from no, to even small amounts of, physical activity. Multiple studies demonstrate that the steepest reduction in disease risk occurs at the lowest levels of physical activity. People need to understand that even small amounts of physical activity are beneficial and that reductions in the risk of disease and disability occur by simply getting moving. So various evidence based proven strstegies are needed in Korea including workforce training.

① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는

- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
