A case of ARDS Overlooked Tsutstugamushi Disease that Presented as Simple Cutaneous Lesions (단순 피부병변으로 간과되어 급성 호흡곤란 증후군으로 진행된 쯔쯔가무시병 1예)

  • Tsutstugamushi disease is a major febrile disease that generally occurs in the fall in Korea with hemorrhagic fever with renal syndrome and leptospirosis. This disease is often accompanied by interstitial pneumonia, acute renal failure and liver failure. The causative agent, namely Orientia tsutsugamushi, is transmitted to humans through the bite of a laval trombiculid mite, which is commonly known as a chigger. A 78 year old man was admitted in October 2004 with intractable fever and a drowsy mentality. Two weeks earlier, he visited a private clinic complaining of a simple skin rash. He was treated with antihistamine and steroid, but his symptoms were aggravated and he was referred to our hospital. His physical examination and laboratory findings showed a septic shock status. The maculopapular rash had spread over his face, chest, abdomen and extremities. Eschar was observed in lower back area but it was too difficult to distinguish it from other skin rashes. His chest X-ray appeared as diffuse nodular patchy consolidations in the bilateral lung parenchyme. He was treated with a mechanical ventilator and doxycycline under th suspicion of Tsutstugamushi disease. However, he suffered multiorgan failure accompanied by acute respiratory distress syndrome, acute renal failure and acute hepatitis. He was treated in the intensive care unit for approximately 12 weeks and his general condition was recovered.

Survey and Phylogenetic Analysis of Rodents and Important Rodent-Borne Zoonotic Pathogens in Gedu, Bhutan

  • Rodents are well-known reservoirs and vectors of many emerging and re-emerging infectious diseases, but little is known about their role in zoonotic disease transmission in Bhutan. In this study, a cross-sectional investigation of zoonotic disease pathogens in rodents was performed in Chukha district, Bhutan, where a high incidence of scrub typhus and cases of acute undifferentiated febrile illness had been reported in people during the preceding 4-6 months. Twelve rodents were trapped alive using wire-mesh traps. Following euthanasia, liver and kidney tissues were removed and tested using PCR for Orientia tsutsugamushi and other bacterial and rickettsial pathogens causing bartonellosis, borreliosis, human monocytic ehrlichiosis, human granulocytic anaplasmosis, leptospirosis, and rickettsiosis. A phylogenetic analysis was performed on all rodent species captured and pathogens detected. Four out of the 12 rodents (33.3%) tested positive by PCR for zoonotic pathogens. Anaplasma phagocytophilum, Bartonella grahamii, and B. queenslandensis were identified for the first time in Bhutan. Leptospira interrogans was also detected for the first time from rodents in Bhutan. The findings demonstrate the presence of these zoonotic pathogens in rodents in Bhutan, which may pose a risk of disease transmission to humans.

Development of the Designs of Surgical Scrubs at General Hospitals (종합병원의 수술복 이너웨어 디자인 개발에 관한 연구)

  • In South Korea, hospital clothes for medical staff, which require high performance and functionality, are still regarded just as uniforms. Few researches have been made into the underwear for operating room staff, which is intended to protect medical staff against possible risks happening during an operation and allow them to do their job in an easy and comfortable manner. In order to present a new design of underwear for operating room staff at general hospitals in South Korea, an investigation was made into medical staff's preference for overseas brand samples, and it was found that they preferred to wear such clothes as are blue-colored, of a V-neck line, of a box style and has good absorptiveness and breathability. After developing the primary prototype and the conventional underwear for operating room staff at Hospital A, their appearances were evaluated, and it was found that the primary prototype was more favorably evaluated. After developing the secondary prototype, their preference for the colors and shapes was surveyed, and it was found that they preferred to wear No.6 blue colors and T/C materials. In conclusion, it is considered that a systematic research need be conducted into the designs of various hospital clothes, whose roles and functions are subdivided, including the underwear for operating room staff developed in this research.

A Case-Control, Restrospective Study on Tsutsugamushi Disease Occurred in Gyeongju and Pohang Provinces, Korea (경주 및 포항 지역에서 발생한 쯔쯔가무시병에 관한 연구 - 항체 유무를 중심으로 -)

  • Objectives: Tsutsugamushi disease is one of the most significant acute febrile illnesses, increasing in frequency of occurrence during the late autumn in rural areas of Korea. Methods: I have conducted a case-control study on 30 cases who had the fever as a chief complaint, then had been ruled out as having the Tsutsugamushi disease. Data was collected retrospectively by review of chart regarding their general characteristics, clinical and laboratory findings, then was analysed by chi-test. Results: Of 30 cases, 16 cases were seropositive and 14 cases were seronegative against O. tsutsugamushi. Of seropositive for tsutsugamushi disease, 56.3% were female; 37.5% were in their seventies; 50.0% were farmers; 62,6% had chances of exposure to fields or mountains. And 75.0% occurred in November. The main symptoms and signs were fever and chill(100.0%), headache(75.0%). weakness and fatigue(93.8%), and eschar(68.8%). The characteristic laboratory findings were elevated AST(50.0%), ALT(62.5%), and abnormal urinalysis(56.3%). On the other hand, of seronegative cases, 57.1% were male; 50.0% were in their fifties; 42.9% were farmers; 57.2% had chances of exposure to fields or mountains. And 71.4% occurred in November. The symptoms and signs were fever and chill(100.0%), headache(85.7%), eschar(64.3%). weakness, fatigue and skin(57.1%). The laboratory findings were elevated AST(71.4%) and ALT(64.3%), and abnormal urinalysis(42.9%). However, there were no significant differences between the seropositive and seronegative cases(P>.05). Conclusions: Acute febrile community inhabitants who have the epidemiological, clinical as well as laboratory features should be focused upon for the early diagnosis and treatment for tsutsugamushi disease whether or not possessing the serological antibody against O. tsutsugamushi.

Chigger Mite (Acari: Trombiculidae) Survey of Rodents in Shandong Province, Northern China

  • Chigger mites are parasites of rodents and other vertebrates, invertebrates, and other arthropods, and are the only vectors of scrub typhus, in addition to other zoonoses. Therefore, investigating their distribution, diversity, and seasonal abundance is important for public health. Rodent surveillance was conducted at 6 districts in Shandong Province, northern China ($114-112^{\circ}E$, $34-38^{\circ}N$), from January to December 2011. Overall, 225/286 (78.7%) rodents captured were infested with chigger mites. A total of 451 chigger mites were identified as belonging to 5 most commonly collected species and 3 genera in 1 family. Leptotrombidium scutellare and Leptotrombidium intermedia were the most commonly collected chigger mites. L. scutellare (66.2%, 36.7%, and 49.0%) was the most frequently collected chigger mite from Apodemus agrarius, Rattus norvegicus, and Microtus fortis, respectively, whereas L. intermedia (61.5% and 63.2%) was the most frequently collected chigger mite from Cricetulus triton and Mus musculus, respectively. This study demonstrated a relatively high prevalence of chigger mites that varied seasonally in Shandong Province, China.

Environmental survey on the vectors and hosts of Tsutsugamushi disease in Jeonnam province, Korea (전남지방에서 쯔쯔가무시병 숙주와 매개체의 서식환경 조사)

  • Tsutsugamushi disease or scrub typhus cause by Orientia tsutsugamushi is an endemic disease in Korea. Chigger mites and field rodents play roles in transmission of the disease by the vector and host of the agent. The purpose of this study is to investigate the density of the chigger mites and field rodents due to environmental factors such as temperature, relative humidity, soil thickness and the various vegetations to the 9 field rodent collection sites. The total 62 field rodents was captured by the Sherman collapsible traps from April to October 2009 at the Jangseong of Jeonnam Province, Korea. The trapping rate of the field rodents by the different collecting sites was dominant at subside storage water (24%), bush near by dam (22%), bank around field (20%), followed by 18% of grassy field and surround cattle shed. The distribution of chigger mites by the different collecting sites was the highest at Bush near by dam (28.7%). And the sites of subside storage water, bank around field and surround cattle shed were 20.4%, 18.8%, 16.4%, respectively. On the other hand the collecting sites of stream bank and ridges between rice paddies were not collected. The temperature to the collecting sites was showed $24.1^{\circ}C$ in June and $24.2^{\circ}C$ in October which was higher than April ($10.6^{\circ}C$), whereas lower than May ($25.3^{\circ}C$) and September ($26.8^{\circ}C$). The highest number of mites was collected at $24.2^{\circ}C$ and 46.6% relative humidity in October. The chigger mites and field rodents were highly collected between 18 and 24% at the sites where are loosely in the superficial layers of the soil from 8.0 cm to 10.2 cm. Total 25 species of vegetation were distributed at the collecting sites. In the present study, strong evidence was found that bank around field and grassy field were provided for the prevalence sites of tsutsugamushi disease.

Studies on Planting Distrfbutien Status of Landscaping Plants in Korea (우리나라 조경 수목의 식재 분포에 관한 연구)

  • A total of 394 plant species in 74 families planted in home gardens, public building gardens, campuses of elementaiy, middle, and high schools, and universities, recreation areas, city parks, loeal and national ways, and highways from 1.21 local areal in Korea were surveyed- A comparison of the surveyed species to nation-widei.y growing species in those areas were studied. the results obtained were as follows : 1. The ratio of evergreen to deciduous plant species was 38.1:61,9 and that of tree:scrub:vine was 54:40.9:5.1. 2. The ratio of evergreen to deciduous plant species was 27.5:72.5 among total 276 land-scape plant species surveyed in Seoul and Kyongki-do province. The ratios were 32:68 in Kwangwon-do, 36.1:63.9 ill Chungchongbuk-do, 37:61 iii Chungchongnanl-do, 36.4:63.6 in Kyungsangbuk-do, 39.2:60.8 고 Kyungsangaaal-do, 35.7:64.5 in Jollabuk-do, 40.6:59.4 in Jollanam-do, and 43.1 :56.9 in Cheju-do. This results tell that more evergreen species are growing in the southern than in northern region in Korea. 3. Plants with 100% frequency in national planting were 8 species inclubing Forsythia koreana 91-99% were 6 species including Taxus cuspidata, 81-90% were 13 species including Magnolia liilflora, 71-80% were 10 speces including Buxus microphylla val, koreana, 61-70% were 13 species including Thuja occiduntakus, 51-60% were 15 species including Pius thunbergii, 41-50% were 15 species including Kerria japonica, 31-40% were 17 species including Cryptomeria japonica, 21-30% were 28 species including Viturnum awabuki, and below 20% were 267 species including Koelreuteria paniculata. 4 Uppermost north latitude of planting distribution of major temperature species was sum- marized as follows: Although it has been reported that Cryptomeria japonica is possible to grow in areas around -47, the result from this study shows that Cryptomeria japonica grows normally in Choonchun Seoul, Samchuck, and Kwangryeung, etc. , and is possible to grow normally in the areas with approximately -6$^{\circ}C$ in an average temperature in January, where are -2$^{\circ}C$ colder than the reported temperature, but locally warm Phyllostachys bambusoides is distributed to Mt. Kumkang and Mt. Sulak, and also planted in Seoul, Kwangryeung, and Chunchon.

The Type Classification and Function Assessment at Small Palustrine Wetland in Rural Areas (농촌지역 소규모 소택형습지의 유형분류 및 기능평가 연구)

  • This study was conducted to utilize as basic information for the construction of conservation and estimation system for Palustrine wetland, which was badly managed and imprudently reclaimed, through the analysis of distribution characteristics and the estimation of conservation value for sample sites (eight wetlands) in rural area. As the result of wetland type classification, these wetlands was classified by 4 types (Permanent freshwater marshes/pools, ponds, Aquaculture ponds, and Seasonally flooded agricultural land) by Ramsar system, 3 types (Emergent Wetland, Aquatic Bed, and Scrub-Shrub Wetland) by NWI (Cowardin) System, 5 types (Farm Pond Depression, Under-flow wetland, Man-made Pond Depression, Abandoned Paddy Fields Wetland, and Reservoir Shore) by National Wetland's Categorical System, and 3 types (Aquatic Bed Wetland, Emergent Wetland, and Forested Wetland) by Lee (2000) System. These results suggest us developing the new type classification system for small Palustrine wetland in Korean rural areas. The score of function assessment (The Modified RAM) for small Palustrine wetlands was high at the wetlands nearby hills and rice paddy fields, and low at those nearby upper fields, which was mainly affected by land-use and vegetation. The functions as 'Flood/Storm Water Storage', 'Runoff Attenuation', 'Water Quality Protection' were resulted by the structural difference of inflow and outlet. Some functions as 'Wetland size', 'Wetland to immediate watershed ratio', 'Presence of boat traffic', 'Maximum water depth', 'Fetch of water's body' of RAM were not appropriate in evaluation of small wetlands in rural area. Which suggest us developing the new function assessment system for small Palustirne wetland in Korean rural areas.

ROAD CROWN, TIRE, AND SUSPENSION EFFECTS ON VEHICLE STRAIGHT-AHEAD MOTION

  • During normal operating conditions, a motor vehicle is constantly subjected to a variety of forces, which can adversely affect its straight-ahead motion performance. These forces can originate both from external sources such as wind and road and from on-board sources such as tires, suspension, and chassis configuration. One of the effects of these disturbances is the phenomenon of vehicle lateral-drift during straight-ahead motion. This paper examines the effects of road crown, tires, and suspension on vehicle straight-ahead motion. The results of experimental studies into the effects of these on-board and external disturbances are extremely sensitive to small changes in test conditions and are therefore difficult to guarantee repeatability. This study was therefore conducted by means of computer simulation using a full vehicle model. The purpose of this paper is to gain further understanding of the straight-ahead maneuver from simulation results, some aspects of which may not be obtainable from experimental study. This paper also aims to clarify some of the disputable arguments on the theories of vehicle straight-ahead motion found in the literature. Tire residual aligning torque, road crown angle, scrub radius and caster angle in suspension geometry, were selected as the study variables. The effects of these variables on straight-ahead motion were evaluated from the straight-ahead motion simulation results during a 100m run in free control mode. Examination of vehicle behavior during straight-ahead motion under a fixed control mode was also carried out in order to evaluate the validity of several disputable arguments on vehicle pull theory, found in the literature. Finally, qualitative comparisons between the simulation results and the test results were made to support the validity of the simulation results.

In Vitro Study on the Artificial Plaque Removal Effect by Use of 360 Degree Rotating Head with Sonic Tooth-Brush

  • Objective: The authors have experimented for the artificial plaque removal effect of several kinds of 360 degree rotating head typed tooth-brushes with sonic vibratory actioned by using of automatic machine for horizontal scrub method in order to find the better toothbrush type for plaque removal. Methods: The experiment was conducted on three medium to 360 degree rotating head toothbrushes, a medium sized toothbrush and a medium sized toothbrush, and a flat toothbrush consisting of 30 ordinary toothbrushes. A brushing machine with horizontal scrubbing was manufactured and had variations of the end of the bristle attached to or near the surface of the teeth, a vibrating wave action force of 16,000 or 18,000 cycles per minute, and a working time of 2 or 3 minutes. The tooth removal effect was confirmed by scanning and analyzing images with a computer program after automatic brushing with the machine. The elimination rate results for each group were analyzed using the independent t-test and one-way ANOVA test. Results: It revealed the most in removal effect for the artificial plaque in such conditions as action at near the tooth surface with 18,000 cycle for 3 minutes in case of using A, B, and C tooth-brush. And it has more removal effect rate than for using the plane tooth-brush (p<0.05). Conclusion: It was recommended to develop the 360 degree rotating head and vibratory toothbrush focusing to use near the tooth surface with 18,000 cycles of vibration for 3 minutes at one site of the teeth area.
