The Results of Gastric Cancer Surgery during the Early Stage of a Training Hospital (설립 초기의 수련 병원에서의 위암 수술 성적)

  • Konkuk University Hospital (KUH), which opened in September 2005, is currently categorized as a secondary hospital. Early on after its establishment, the surgical residents and nurses were relatively inexperienced in the treatment of stomach cancer. Therefore, the quality of surgery for stomach cancer at KUH may be different from that of the existing large-scale tertiary hospitals. The purpose of this study is first to investigate the clinicopathological characteristics of the gastric cancer patients at the KUH, and second to compare our morbidity & mortality rates with those of previous studies, and we also analyzed the risk factors of morbidity at the early stage of a training hospital. Materials and Methods: This study retrospectively collected the clinicopathological characteristics and the post-operative morbidity rates and mortality rates with using the electronic medical records of all the patients who went under a gastric cancer operation at KUH from September 2005 to April 2008. Results: The total number of gastric cancer patients who underwent operation was 201. The morbidity rate and death rate at KUH were 10.4% and 0.5%, respectively. The morbidity has increased with an older age. The other variables had no influence on morbidity. Conclusion: The morbidity rate, death rate and the clinicopathological characteristics of gastric cancer patients at KUH were similar to those of the previous reports. We found that age is the main factor affecting the morbidity rate after stomach cancer surgery. For further surgical qualification of stomach cancer surgery at KUH, it is necessary to collect the survival data of patients who undergo stomach cancer surgery.

Appraisal of Guidelines for Research & Evaluation II Appraisal of Clinical Practice Guidelines for Traffic Injuries (Appraisal of Guidelines for Research & Evaluation (AGREE) II를 이용한 교통사고 상해증후군의 국내·외 기개발 임상진료지침의 평가)

  • Objectives This study was aimed to evaluate clinical practice guidelines (CPGs) of traffic injuries, which has already been developed at domestic or outside of country, and to explore the Korean medical treatments included in the CPGs. Methods Twelve electronic databases (PubMed, Cochrane library, China National Knowledge Infrastructure [CNKI {Chinese Academic Journals, CAJ}], Research Information Sharing Service [RISS], Oriental Medicine Advanced Searching Integrated System [OASIS], KoreaMed, Korean Medical Guideline Information [KoMGI), National Guideline Clearinghouse [AHRQ], Core Outcome Measures in Effectiveness Trials Initiative Website [COMET], Turning Research into Practice [TRIP], The National Institute for Health and Care Excellence [NICE], and Medical Research Information Center [MedRIC]) up to July 2021 were searched. Only systematically developed CPGs for traffic injuries were selected and appraised. The appraisal was conducted by using Appraisal of Guidelines for Research & Evaluation (AGREE) II tool. Results Six CPGs were included and evaluated. All CPGs were appraised as highly recommended because they exceeded 60% in more than 4 among 6 domains of AGREE II, including domain of 'rigor of development', and 30% in the rest. Recommendations related to Korean medicine treatments such as on manual therapy related to Chuna were given in 6 CPGs, 4 for acupuncture, 1 for Qigong and 1 for Taping. Conclusions The 6 CPGs were developed up to now through a systematic development methodology and there were many recommendations for Korean medical treatments related to manual (Chuna) treatment, acupuncture and Qigong. However, the evidence for the side effects and risk factors of Korean medical treatments was scantly reflected in CPGs. Therefore, it is considered that balanced CPG with benefits and risks should be developed, covering Korean medical diagnosis, treatment and prognosis.

  • Common femoral artery pseudoaneurysm is a potentially serious complication of peripheral angiography. There have been few prior reports of simultaneous pseudoaneurysm in both common femoral arteries after percutaneous access. Here we report the case of a 58-year-old male patient who presented with phlegmon or abscess a few days after bilateral femoral access, after which newly developed bilateral femoral pseudoaneurysm with wide neck was observed on CT angiography 2 months after infection treatment. Because the patient refused surgery for pseudoaneurysm, a stent-graft was inserted in the left side, and percutaneous thrombin injection under US guidance with balloon occlusion was performed for the right side. Most pseudoaneurysms occur immediately after the causative procedure. However, there have been some cases in which pseudoaneurysms may occur several weeks or months later; it is therefore necessary to check the risk factors and to carefully observe the hemostasis site.

Lipoprotein and Lipid Abnormalities in Uremic Children with Maintenance Dialysis (투석 치료중인 만성 신부전 소아에서의 지질 및 지질단백 이상)

  • Purpose : Chronic renal failure is often accompanied by severe dyslipidemia, a known risk factor for cardiovascular disease. Lipoprotein(a) [Lp(a)] has recently been characterized as a risk factor for atherosclerosis and thrombosis. Cardiovascular disease is the leading cause of death in adult patients on dialysis. However, there are only limited data available concerning risk factors for atherosclerosis in uremic children. We have measured serum levels of lipids, lipoproteins, apolipoproteins and Lp(a) in uremic children with maintenance dialysis. Methods : Ten uremic children with hemodialysis (HD) and 14 with peritoneal dialysis (PD) in our dialysis unit were included in this study. The mean age of HD patients was $162{\pm}59$ months and the male to female ratio was 7:3. The mean age and sex ratio of PD patients were $123{\pm}63$ months and 6:8, respectively. The levels of cholesterol, triglyceride, lipoproteins, apolipoproteins and Lp(a) were measured from serum sampled after 14 hours of fasting. The normal control levels were cited from 2 articles presenting the normal blood lipid and lipoprotein levels of primary school and middle school children in Korea. Results : There was no difference in age, sex ratio, body mass index and duration of dialysis between the HD and the PD group. The serum concentration of the cholesterol, triglyceride and apolipoprotein B were significantly elevated in dialysis patients compared with normal subjects. The serum level of Lp(a) was significantly elevated in only PD group. The serum Lp(a) level was below 30 mg/dl in 13 and above 30 mg/dl in 11 patients. The serum albumin level was significantly decreased in high Lp(a) group than in low Lp(a) group. Conclusion : The uremic children receiving dialysis reveal abnormal serum lipid and lipoprotein profiles. These results suggest that they have a higher risk for coronary heart disease, although there has been no clinical evidence of coronary heart disease at present. A long-term follow-up study of these children to clarify the suggestion should be started now.

Seroprevalence of Dirofilaria immitis in Cats from Liaoning Province, Northeastern China

  • The present study was performed to investigate the seroprevalence and risk factors for Dirofilaria immitis infection in cats from Liaoning province, northeastern China. From October 2014 to September 2016, sera of 651 cats, including 364 domestic cats and 287 feral cats (332 females and 319 males) were assessed. They were tested for the presence of D. immitis antigen using SNAP Heartworm RT test kit. In this population, the average prevalence was 4.5%. Age and rearing conditions (feral or domestic) were found to be associated with the prevalence of D. immitis. The prevalence was significantly higher in feral cats compared with domestic cats (8.4% vs 1.4%, P<0.01). There was no significant difference between males and females (4.7% vs 4.2%, P>0.05), but older cats (${\geq}3$ years old) showed a statistically higher prevalence compared with younger cats (<3 years old) in feral populations (16.8 vs 2.4%, P<0.01), while the difference between the age groups was not statistically significant in domestic cats (2.4% vs 0.51%, P>0.05), all these results suggest that outdoor exposure time may be one of the most important factors for D. immitis prevalence in cats. Results reveal that D. immitis are prevalence in domestic and feral cats in northeastern China, which indicates that appropriate preventive measures should be taken to decrease the incidence of feline heartworm disease in Liaoning province, northeastern China.

Study on Entering Self-Employment of Young Workers (청년층의 자영업 진입에 관한 연구)

  • In recent years, the proportion of youth's self-employed is steadily increasing, and government policy also actively encourages youth to become an entrepreneur. However, most of the domestic precedent studies on the self-employed labor market focus on the middle-aged and the elderly, and previous studies on the self-employed labor market of young people are hardly ever studied. Above all, the study that examines the factors of entry into self-employment of young people is not sufficient and researchers usually utilize the study about for all ages to explain the self-employment market of young people. However, because the young and middle-aged people differ in labor market conditions, family backgrounds, and the level of accumulation of human capital, separate explanations and theories are needed. Therefore, this study explored the factors of entry into self-employment by separating the age group from 15~29 years old. The data used in the analysis was the 9th to 20th data of the Korea Labor Panel Survey. The youth unemployment rate and employment rate were referred to the Economically Active Population Survey of Statistics Korea. The analysis subject was limited to young people who are currently performing economic activities, and the analysis method used multi-level logit model. The main results are summarized as follows. First, the lower the unemployment rate and the higher the employment rate, the younger people tend to enter their own businesses on the structural level. Second on the individual level, young people who possess enough financial capital or pursuit personal aptitude or interest tend to enter self-employment. However, there are no statistical effects of human capital and entrepreneur capital.

An Empirical Analysis on the Determinants of Syndicated Investment of Korean Venture Capital (한국 벤처캐피탈의 공동투자 요인 실증분석)

  • The syndicated investment strategy of venture capital has merits such as reductionof investment risk, efficient management of investment resources, and information asymmetry mitigation. This study analyzes the investment strategy and network of venture capital by collecting and analyzing investment information of Korean venture capital on Korean start-ups for three years from 2014 to 2016. We could not find statistically significant association with the choice of syndicated investment of the stage of start-ups, the venture capital expertise (investment concentration), internal competence (number of workforce and professional manpower), and the breadth of investment network. As a result of this study, the factors that determine venture capital 's syndicated investment in Korea are only the financial factors such as the size of the investment amount and the size of the fund besides the business field of the invested company. The results suggest that the majority of venture capital firms in Korea may not customize their investment strategies in terms of their expertise or inherent business risks of invested companies.

Association with oral symptom experiences by level of subjective stress recognition in the Korean adolescents (우리나라 청소년의 주관적 스트레스의 인지수준에 따른 구강증상경험과의 관련성)

  • Objectives: The purpose of this study was to investigate the relationship between the level of subjective stress recognition and oral symptom experiences including toothache, gum diseases, and oral soft tissue diseases in the Korean adolescents. Methods: The subjects were 68,043 adolescents recruited using a web-based survey, National Korean Youth Risk Behavior in 2015 by the Korean Center for Disease Control. For statistical analysis, SPSS 21.0 for Windows was used. Descriptive analysis and a Chi-square test were conducted to determine the factors associated with general characteristics, health behaviors, oral health behaviors, and level of subjective stress recognition. Finally, to investigate the relationship between the level of subjective stress recognition and oral symptom experiences, logistic regression analysis was performed. Results: Toothache related subjective stress recognition level was significantly higher in the moderate group with the score of 1.59 (95% CI; 1.49-1.68), and was greater in the high group with 2.38 (95% CI; 2.24-2.53) compared to the low group. Gum disease related subjective stress recognition level was significantly higher in the moderate group with 1.41 (95% CI; 1.32-1.51), and was greater in the high group with 1.99 (95% CI; 1.86-2.13). Oral soft tissue disease related subjective stress recognition level was significantly higher in the moderate group with 1.59 (95% CI; 1.45-1.74), and was greater in the high group with 2.55 (95% CI; 2.33-2.79). Bad breath related subjective stress recognition level was significantly higher in the moderate group with 1.48 (95% CI; 1.39-1.57), and was greater in the high group with 2.10 (95% CI; 1.97-2.25). Conclusions: Higher subjective stress recognition level was found to affect the oral symptoms experienced. Therefore, the stress management plan should be prepared through the cause identification of the main stress in the adolescents. Practical and systematic education is needed for oral health management in the schools.

Psychological Health in Residents Participating in Clean-up Works of Hebei Spirit Oil Spill (허베이스피릿호 유류유출사고 방제작업에 참여한 주민의 정신건강)

  • Objectives : Our objective was to examine and evaluate the psychological health of the residents of Taean during the cleanup of the Hebei Spirit(HS) oil spill and to review some factors associated with the results. Methods : A community survey of 71 men and women was conducted 8 weeks after the HS oil spill. Questionnaires used were the PWI(Psychological Well-being Index) scale for psychosocial distress, the CES-D(Center for Epidemiologic Studies-Depression) scale for depressive symptoms, and a questionnaire created to assess suicidal impulses. Results : The overall prevalence of high-risk psychosocial distress among the study group was 64.2%. The percentages of respondents with scores on the CES-D Scale above 16 and above 21 were 77.6% and 62.7%, respectively. The percentage of respondents categorized as having suicidal impulses was 18.3%. When compared with unexposed groups in the general population taken from various sources, the residents of Taean were 6.5 times as likely to have high stress and 9.4-9.7 times as likely to be depressed. No significant difference in the rate of suicidal impulse was found between the residents of Taean and the general population. Factors associated with high stress, depression, and suicidal impulses were age, a change in income, educational level, number of days working on the cleanup, and positive responses to questions about "affected daily activity" and "hospital visit due to work on cleanup". Conclusions : The results suggest that the HS oil spill had a significant impact on the psychological health of residents of Taean, but the comparability of the unexposed groups is a limitation of the study.

A Study on the Determinants of Liporotein(a) Level - Primary, Middle, and High School Teachers in Seoul - (혈액내 Lipoprotein(a) 농도의 결정요인에 관한 연구 - 서울시 일부 초, 중, 고교 교사들을 대상으로 -)

  • The purpose of this study was to determine the association between blood lipids, fibrinogen, fasting blood sugar, lifestyle-related factors and lipoprotein(a). This study was performed with 140 healthy adults (male:80, female:60) among the teachers in primary, middle, and high school in Seoul in November 1996. Lipoprotein(a) concentration was measured with an enzyme-linked-immunosorbents assay(ELISA), and the results showed that the distribution was highly skewed and the average concentrations of male and female were $16.94{\pm}12.70\;mg/dL\;and\;17.92{\pm}7.58\;mg/dL$, respectively. The percentage of subject with lipoprotein(a) concentrations higher than 30 mg/dL was 26.4%. Univariate analysis showed significant association between lipoprotein(a) and age, alcohol, meat, and NaCl consumption (p<0.05). In multivariate analysis, lipoprotein(a) correlated positively with age (p<0.05) and meat consumption (p<0.01). These data suggest that lifestyle-related factors may affect the lipoprotein(a) concentration regarded as a risk factor of atherosclerosis disease.
