During the 1990s the workplace has grown more complex and business competition has increased world-wide. All organizations, whether for-profit or non-profit have been forced to respond to market changes. More advanced information and technology, greater product diversity, shorter product life cycles, increased quality requirements, more regulation oversight, decreasing productivity, more competitors, and increasing overhead costs have motivated organizations to focus on ways to deliver products cheaper, better, and faster. Many organizations are searching for ways to reduce costs through downsizing, reengineering business processes, implementing quality management, outsourcing, and improving cost management. Support departments that provide services internal to an organization such as human resources, legal, and environmental, safety, and health (ES&H) are often the first organization targeted for cost reduction and cost control initiatives because these functions are part of a rapidly increasing overhead cost. Recently, ES&H functions are incresingly being integrated into the business of business to contribute value to organization beyond mere compliance with ES&H regulations. The discussions and development of the ISO compatible Environmental Management Standards or Occupational Safety and Health Management Standards is another impetus to integrate ES&H function into the business of business. Thus, ES&H professional need new skills to analyze the cost of their function and communicate the value of the products and services they provide. In recent years, the need for and the importance developing cost management and business skills by ES&H professionals have been emphasized in the literature. Communicating with decision makers in terms of cost and value to the organization, and by using business language and business arguments is the first step toward effectively integrating ES&H activities into the business of business. Activity-based costing (ABC) is a cost management method that measures the cost of a product or service based on the actual use of resources by activities, and based on the actual amount of activities used to produce a product or service. ABC is recommended as a tool for managers of ES&H organizations to determine the cost of developing and providing ES&H products within a for-profit firm or non-profit agency. This paper discusses the trend of integration of ES&H functions into the mainstream of business activities within an organization. The general principles of treditional cost accounting are presented as a bases for understandging why and how ABC will provide more accurate estimates of cost. The principles and concepts of ABS are presented as a tool for determining more accurately the true cost of ES&H products and services.

Backgrouds : This study was performed to compare the satisfaction levels for health examination programs provided by between the Korea National Health Insurance Corporation (KNHIC) and private healthcare organizations in Korea. We investigated factors associated with the satisfaction level for the KNHIC health examination. Also, factors explaining for the additional receipt of private healthcare organization's examination were identified. Methods : A self-administered questionnaire survey was conducted in June 2004 with 250 beneficiaries of the KNHIC industrial workers' program and 100 beneficiaries of the KNHIC selfemployed program. A total of 297 completed the questionnaire, including 213 (85.2%) and 84 (84.0%) for each insurance program. Ten questions measuring satisfaction levels for each examination program were rated on a scale ranging from 1 (strongly unsatisfied) to 5 (strongly satisfied). Results : Among the subjects receiving both examinations, the mean satisfaction level for the KNHIC examination (3.07) was significantly lower than that for the private healthcare organization's examination (3.50) (p<0.05). The KNHIC examination showed lower satisfaction for all of the 10 items than the counterpart. In particular, the KNHIC examination had the scores of lower than 3.0 for the items reflecting post-examination management services. According to the multiple regression analysis results, the satisfaction for the KNHIC examination was positively associated with the positive attitude toward health examination(${\bullet}$,=0.38, p<0.00). The logistic regression results showed that the likelihood of receiving the private healthcare organization's examination in addition to the KNHIC examination increases as the respondents were less healthier(OR=0.29, 95% CI = 0.10¢¶0.84), the satisfaction level for the KNHIC examination decreased(OR=0.46, 0.28¢¶0.75) or the attitude toward health examination was more positive (OR=2.56, 1.31¢¶5.12). Conclusion : The relatively low satisfaction level for both examination programs suggests that there's ample room for improvement of health examination services in Korea. The negative association between additional receipt of private healthcare organization's examination and satisfaction level for the KNHIC examination implies that the improvement of the satisfaction for the KNHIC examination would help to reduce the national expenditure spent on the additional examination.

Objectives: Although the World Health Organization (WHO) initiative "My 5 Moments for Hand Hygiene" has been lauded as effective in preventing hospital-associated infections, little is known about healthcare workers (HCWs)' hand hygiene behavior. In this study, we sought to assess knowledge and attitudes towards the concepts in this initiative, as well as associated factors, among Vietnamese HCWs at a general hospital. Methods: A structured questionnaire was administered to HCWs at a central Vietnamese general hospital in 2015. Multiple logistic regression analysis was used to identify factors associated with HCWs' knowledge and attitudes towards hand hygiene. Results: Of 120 respondents, 65.8% and 67.5% demonstrated appropriate knowledge and a positive attitude, respectively, regarding all 5 hand hygiene moments. Logistic regression indicated better knowledge of hand hygiene in workers who were over 30 years old, who were direct HCWs (rather than managers), who had frequent access to clinical information, and who received their clinical information from training. Those who worked in infectious and tropical disease wards, who had frequent access to clinical information, and who received information from training were more likely to have a positive attitude towards hand hygiene than their counterparts. Conclusions: Although many Vietnamese HCWs displayed moderate knowledge and positive attitudes towards the WHO hand hygiene guidelines, a key gap remained. Regular education and training programs are needed to increase knowledge and to improve attitudes and practices towards hand hygiene. Furthermore, a combination of multimodal strategies and locally-adapted interventions is needed for sustainable hand hygiene adherence.
This study aims to compare the healthcare status of South Korea and other member of the Organization for Economic Cooperation and Development (OECD) using the OECD health statistics 2021. We used the position value for relative comparison (PARC) index to measure the five elements of the healthcare system, demand, supply, accessibility, quality, and cost. For the statistical analysis, Mann-Kendall test was performed to examine the trend of the PARC values from 2000 to the most recent year. The results showed that supply, demand, accessibility, and quality were above median than the OECD median and the cost was below median. In sectors such as primary care, health employment and mental health care were below median average. With these result, necessary steps for a sustainable healthcare should be taken into effort by policy makers.
The purpose of this study is to analyze correlation between the view of teaching profession and the role performance of school nurse. For the achievement of this purpose 188 school nurses in Tae-Gu city and Kyung-Sang Buk-Do are randomly selected in 1992. Major findings of this study were as follows: 1) Role performance of the school nurse showed school health room, health planning, health education, disease control, environment sanitation and management of health organization, in the order named. 2) Degree of school nurse role performance showed 84.7% of the total respondents as highly. 3) Relationship between the view of teaching profession and the role performance was statistically significant correlation. 4) Role performance of school nurse was explained 29.4% by the view of teaching profession (public life, human relation & leading student) of the total variables.
Objectives: The community network is a foundation laid by the local community which has been formed historically and geographically to develop itself. This paper as a Korean way of healthy network survey for the community capacity building is an exploratory research to grasp the character of Korean society and then to organize an resident-governing partnership for that. Methods: Research objectives are CBOs(Community-Based Organizations) in S-district of Seoul. This region is a new town, however the solidarity and volunteerism of this resident is lively. The survey tool is Cho's CBOs Evaluation Questionnaire which was originated to measure the community capacity. The period of survey is from the January 2009 to the March 2009 and the study organizations which were collected by snowball sampling were 80. Results: The result shows two main networks: one is the civil society and craft union cluster, the other is welfare organization cluster. Groups of high centrality were organizations whose members are mainly women and craft organizations which were organized before 1990's. The group of high betweenness was the Association of Women's Organizations(0.188). Bi-components were six and they could be divided by organization's aim. In terms of the determinants of the participation to the health center enterprise, only the number of link(B=-0.60, p<0.04) was statistically significant. It means that when organization variables are controlled a tendency appears: the more the voluntary associations network, the less participation in the health enterprise. Conclusion: CBHOs(Community-Based Health Organizations) enhances residents' spontaneity and cohesion to increase the capability of the local community. If the surveyed healthy network, together the community health project, are used for various community development projects, the existing CBOs including CBHOs will be reorganized and furthered newly. For this it will be needed to construct an effective partnership of healthy network by restructuring the existing networks of voluntary associations.
A 34-year-old female patient was presented with leg and hip pain for 6 months as well as voiding difficulty for 1 year. Magnetic resonance imaging revealed a well-demarcated mass lesion at L2-3. The mass was hypo-intense on T1- and T2-weighted images with homogeneous gadolinium enhancement. Surgery was performed with the presumptive diagnosis of intradural extramedullary meningioma. Complete tumor removal was possible due to lack of dural adhesion of the tumor. Histologic diagnosis was clear cell meningioma, a rare and newly included World Health Organization classification of meningioma usually affecting younger patients. During postoperative 2 years, the patient has shown no evidence of recurrence. We report a rare case of cauda equina clear cell meningioma without any dural attachment.
New ways of work-shift and learning programs, which were based on the concept of 'performance improvement through people', have been introduced to healthcare organizations. I analyzed the performance of the changes and the performance differences. Data were collected through interview and survey. I discussed that modification of management practices which were developed in manufacturing organizations is important for successful implementation in healthcare organizations.
