Subjective oral dryness and stimulated salivary flow rate in medicated patients in chronic severe psychiatric patients (일부 만성 중증 정신질환자의 약물 복용에 따른 구강건조증상과 자극성 타액분비율)

  • Objectives : The purpose of the study is to investigate the xerostomia in the chronic severe psychiatric patients in Korea because there were few reports on xerostomia in the psychiatric patients. Methods : The subjects were 61 psychiatric patients in the mental hospital by convenience cluster sampling. A self-reported symptom questionnaire was filled out by the three researchers on the basis of medical records by the informed consent. The stimulated salivary flow rate of the patients was measured by saliva sampling. Results : The subjects consisted of 45.9% of male and 54.1% of female. High school graduation accounted for 40.0% and 20.0% did not attend the school. The majority of the patients were medicaid recipients. Schizophrenia accounted for 86.9% and most patients were long term care recipients. A total of 68.9% of the patients suffered from salivary dysfunction. The medication in schizophrenia seemed to decrease the stimulated salivary flow rate and made the patients difficult in chewing and swallowing due to xerostomia and low saliva secretion(p<0.05). Conclusions : Medication in schizophrenic patients caused the salivary dysfunction. So the collaboration between the psychiatry doctors and dental hygienists is very important to improve the salivary secretion in the schizophrenic patients. The continuous and long term care of the xerostomia will help the patients maintain the good oral hygiene.

Clinical Study of Delusion and Pressure Pain Threshold in Patients with Schizophrenia (정신분열병 환자의 신체망상과 압통역치에 관한 임상적 연구)

  • Objects: This study aimed to evaluate the role of somatic delusion on the pain perception in patients with schizophrenia. It was hypothesized that pressure pain thresholds would be rather higher in schizophrenic patients who had somatic delusion than patients with other delusion. Methods The subjects were consisted of 3 groups, 23 men with schizophrenia who had somatic delusion, 25 men with schizophrenia who had other delusion, and 22 normal healthy controls. By using Algometer, pressure pain thresholds were examined to subjects on three non-tender sites with 6 weeks interval. The severity of delusion was evaluated in both patient groups. Statistically, Chi-square test, One-way ANOVA, Multivariate ANOVA, and Scheffe's test were used. Results : 1) There was significant difference between somatic and other delusion groups and normal control group at initial stage. 6 weeks later even when severity of delusion was thought to be ameliorated, this finding were sustained. 2) The severity of the components of delusion, conviction and preoccupation, were significantly decreased in both somatic delusion group and other delusion group according to the time interval. The decrements of the severity of delusion seems to be related with changes in pressure pain thresholds in both patient groups. Conclusions : We re-confirmed that both schizophrenic patient groups showed higher pressure pain thresholds compared to normal healthy control. However we failed to find the role of somatic delusion on pain perception in schizophrenia. Delusion, including somatic delusion, as a whole, seems to affect the increased level of pressure pain threshold due to attention deficit and decreased motivation in patients with schizophrenia.

Association between Tardive Dyskinesia and T103C Polymorphisms of 5-$HT_{2A}$ Receptor Gene (지연성 운동장애와 5-$HT_{2A}$ 수용체 유전자 T103C 다형성과의 관계)

  • Objective:Some candidate gene polymorphisms were reported to be associated with tardive dyskinesia (TD). The aim of this study was to investigate the association of the 5-$HT_{2A}$ receptor gene polymorphisms with TD in Korean schizophrenic subjects. Method:Subjects were of 59 schizophrenic patients with TD and 60 schizophrenic patients without TD for studying of 5-$HT_{2A}$ receptor gene polymorphisms. TD was evaluated using the Abnormal Involuntary Movement Scale(AIMS). Genomic DNA was amplified by PCR and digestion with MspI and BsmI. Result:There were no statistically significant differences in the demographic variables, such as age, male to female percentage, duration of illnesses and duration of antipsychotic drug exposure between the TD group and control group. 1) T102C polymorphisms and TD Comparing the TD group and control group, the 102T/C allele was associated with a significantly increased risk for TD (${\chi}^2$=5.560, df=1, p=0.018). 2) Three AIMS categories of TD and T102C genotype. There were statistically significant differences in the three AIMS categories(${\chi}^2$=6.835, df=2, p=0.033). Conclusion:These result suggest 102T/C genotypes of the 5-$HT_{2A}$ receptor gene are related to the development of TD. The 102T/C genotypes were associated with significantly higher AIMS orofacial dyskinesia scores. These findings suggest that the 5-$HT_{2A}$ receptor gene is significantly associated with susceptibility to TD in patients with chronic schizophrenia.

  • The thesis tries to compare The Structure of World History with Anti-Oedipus in the textual context, and to re-compare in the educational context. I mean by the education an event which contrasts starkly with an essence. It adopts 5W1H, a general reporting form of an accident or event, as the distinctive features at twice comparisons. The purpose of the thesis is not evaluative but interpretive comparison. In the textual context, the thesis discusses, 1) as WHAT, the use of Marx from Kant vs. Nietzsche's point of view, 2) as WHO, the actual subjects of the exchanging human vs. the productive machine, 3) as WHEN/WHERE, the society of the modes of exchange vs. the modes of inscription, 4) as HOW, the revolutionay means of the simultaneous revolution of the world vs. the schizophrenic process, 5) as WHY, the ideal subjects of the associative human vs. the non-human of liberation of desire. In the educational context, the thesis discusses, 1) in the WHAT as educational way, autonomous morality vs. active power, 2) in the WHO as the affirmity of actual subjects, that of the ideal idea vs. that of real power, 3) in the WHEN/WHERE, as the in-between time-space of education, the incommensurable communicative situation of humans vs. the conflictive of machines, 4) in the HOW, as the educational method of achieving the ideal, the involuntary restoration of the already-had ideal vs. the now-have completion and break-through of the schizophrenic process, 5) in the WHY, as the aim of education, cosmopolitan vs. overman.

Verbal Memory Function and Characteristics of Memory Process in Schizophrenia and Affective Disorder (정신분열병과 기분장애 환자의 언어적 기억능력과 기억과정의 특성에 대한 연구)

  • Objectives:This study was to compare verbal memory ability among patients with schizophrenia, bipolar manic patients and unipolar depressive patients, and to understand their charicteristics of memory process. Methods:All subjects were hospitalized patients and had been interviewed by using the Structured Clinical Interview for DSM-IV(SCID). Schizophrenic patients(N=40), bipolar manic patients(N=17), and unipolar depressive patients(N=20) were assessed with K-AVLT for verbal memory and with K-WAIS for verbal IQ. Three groups were compared regarding total immediate recall, delayed recall, delayed recognition, learning curve, memory retention, and retrieval efficiency under controlled verbal IQ. Multiple regression analysis was performed to find which clinical factors have an influence on verbal memory ability. Results:In MANCOVA, differences of verbal memory test scores among the groups were statistically significant(F=1.800, p<.05). In post hoc analysis, Patients with schizophrenia and bipolar mania showed poorer performance in immediate recall, delayed recall, delayed recognition, retrieval efficiency than unipolar depres- sive patients. And schizophrenics performed poorly in delayed recall, delayed recognition, retrieval efficiency than nonpsychotic affective disorder group, but no difference in total immediate recall, delayed recall, delayed recognition, retrieval efficiency between the schizophrenic group and the psychotic affective group. Conclusions:These results partially confirm previous reports of verbal memory ability among major psychiatric disorders. Our results showed that psychotic symptoms were related with verbal memory, and longer duration of illness was related with poorer performance in schizophrenia and unipolar depression.

A Study on the Pathological Syndromes and the Insights of the Patients in the Community Mental Health Centers (지역사회 정신보건시설 내 환자의 정신병리 증상과 병식에 관한 연구)

  • Purpose: This study is a descriptive research in which the pathological syndromes of the schizophrenic patients in the community mental health centers, the degrees of the insights, and the correlations between them were examined. Method: The subjects included 114 SPR pt. people who were enrolled in community mental health centers located in D City. The research was conducted during the period from Aug. 1st, 2002 to Jul. 30th, 2004. The instruments were PANSS developed by Kay(1987) and SUMD developed by Amador(1993). Results: Among the pathological syndromes, positives ones were $12.64{\pm}3.80$ on the average, negative ones were $32.00{\pm}5.36$, and general pathological syndromes were $30.50{\pm}6.24$. In the evaluation of the insights, the mean score was $11.81{\pm}5.09$. There were some significant correlations between positive syndromes and general pathological ones (r=.572, p=.000), and negative syndromes and general pathological ones(r=.262, p=.029), while there was no significant correlation between the insights and the sub-measures of general pathological syndromes. Conclusion: Therefore, the schizophrenic patients taking advantage of the community mental health centers have more negative syndromes than those in hospitalized, and rehabilitation programs are needed to help them continuously. And the further study of the correlations between the pathological syndromes and the degrees of the insights are required, and still, it should be analyzed what effects the insight acquirement brings about to the improvements of the pathological syndromes after the application of the insight-oriented programs.

  • This study was conducted to improve the quality of National Health Policy by analysing the effects of sociodemographic characteristics, Medical institutions characteristics and characteristics of Admission-Discharge on the length of stay in hospital for schizophrenic patients from the data of Health Insurance Review & Assessment Service. The subjects were 4,692 patients who were diagnosed with schizophrenia as main diagnosis from the data records of HIRA-NIS 2016. Regression analysis was performed to evaluate the effect on the length of stay. Sociodemographic characteristics, hospital characteristics and characteristics of Admission-Discharge were used as explanatory variables and length of stay in hospital was used as a dependent variable. The result of study showed that female, age, medical aid, the kind of hospital, gangweon-do and with mental disease were found to be the factors that affect the length of stay. The significance of this study is to use the public data for evaluating factors that affect the length of stay in hospital for schizophrenic patients. But this study did not consider disease severity, so further study is needed for evaluation of the effects of factors on the length of stay according to disease severity.

A Study on Discriminant Function of KWIS Subscales in Schizophrenic Patients (정신분열증 환자에 있어서 KWIS 하위검사 판별기능에 관한 연구)

  • The purpose of this article was to determine the discriminant function analysis of the Korean Wechsler Intelligence Scale(KWIS) for 110 normal controls and 98 schizophrenics. Of special interest was to verify the clinical discriminant power of two subtests of the KWIS(Vacabulary and Digit Symbols) and Zung' s Self-rating Anxiety Scale(SAS). Four major hypotheses were postulated. The normal control group would show higher scores than the schizophrenics ; mean scores on both Vocabulary and Digit Symbol. The mean difference in Digit Symbol between the two groups would be greater than that in the Vacabulary. There would be no significant relation among Digit Symbol. Vacabulary. and Anxiety. The most powerful discriminant power would be expected from subtest of Digit Symbol. The mean discriminant scores were - 1.34425 for the control subjects. 1.34425 for the schizophrenics. The correctly discriminated percentage was 89.1% for the control subjects. 90.8% for the schizophrenics. From the findings it was concluded that both Digit Symbol and Vocabulary scales had strong diagnostic value but the former was more powerful than the latter. However. the Anxiety scales had less diagnostic value.

The Subjective Burden of Family Caregivers with Schizophrenics and Its Predictors (정신분열병 환자 가족보호자의 주관적 부담과 그 결정요소)

  • Experiences of the subjective burden and its determinants were analyzed in a sample of 110 primary family caregivers of adult schizophrenic patients. The subjects reported varying amount of subjective burden and its mean score was 17.84 that meant considerably high level of subjective burden. Reports of subjective burden were high in the items of 'pity', 'frustration and resentment', 'regret'. The result of factor analysis revealed that subjective burden consisted of four factors such as 'hopelessness', 'frustration and resentment', 'fear', and 'pity and anxiety'. The results about the determining variables of subjective burden indicated that the prognosis of patient, perceived stigma, the number of previous hospitalization, the availability of secondary caregiver, primary caregiver's age, and family income were predictive of primary caregivers' subjective burden. The result examining the multivariate relationship among subjective burden, stressors, social support, family demographic and socioeconomic characteristics revealed that the more important determinants of subjective burden were the prognosis of patient, perceived stigma, and the number of previous hospitalization, Implications for intervention to help with primary caregivers' subjective burden were discussed.

A Study of Depression in Positive and Negative Schizophrenics (양성 및 음성 정신분열증 환자의 우울에 관한 연구)

  • This study was to find out whether there were differences in the levels of depressions between positive and negative schizophrenics. This research was derived from the fact that negative schizophrenics show higher levels of depression than positive schizophrenics. This study also examined the levels of psychomotor dysfunction in positive and negative schizophrenics. For this study, there were 453 subjects. They consisted of 119 positive schizophrenics, 122 negative schizophrenics and 212 normal people. They were asked to complete Zung's Self-Rating Depression Scale(SDS) and to perform one subtest, Digit Symbol of KWIS(Korean Wechsler Intelligence Scale). Subjects' levels of depression were measured by the SDS. the level of psychomotor dysfunction was measured by Digit Symbol subtest of Korean Wechsler Intelligence Scale. ANOV A and Duncan's multiple comparison analysis were used to examine whether there were differences of depression and psychomotor dysfunction among the normal people, positive and negative schizophrenics. The results were as follows: It was found that the depression level was higher in the negative schizophrenic patients than positive schizophrenic patients. Levels of depression were significantly higher in negative schizophrenics than positive schizophrenics. Psychomotor retardation symptom was the most effective variable that discriminates between the normals and the schizophrenics. And it would be concluded that the psychomotor dysfunction was more severe in negative schizophrenics than positive schizophrenics.
