Association between Schizophrenia and the T102C Polymorphism of the $5-HT_{2A}$ (정신분열병과 $5-HT_{2A}$ / T102C 다형성의 관계)

  • The $5-HT_{2A}$ receptor is of great interest for research into schizophrenia and psychopharmacology in light of the observation that schizophrenic patients has 5-HT cortical-subcortical imbalance and atypical antipsychotic clozpine has $5-HT_{2A}$ antagonists properties. An significant association between schizophrenia and the T102C polymorphism of the gene for $5-HT_{2A}$receptor has been reported. In this study, we investigated an association between schizophrenia and the T102C polymorphism of the gene for $5-HT_{2A}$ receptor in Korean schizophrenic patients. The subjects consisted of 139 schizophrenic patients and 88 normal controls. Genomic DNA was amplified by PCR and digested with MsPI. The uncutt product identified allele 1(nucleotide sequence TCT) ; digested products of 216bp and 156bp identified allele 2(nucleotide sequence TCC). The allele frequencies and the genotypic distribution of $5-HT_{2A}$ receptor gene were not significantly different between schizophrenic patients and normal controls. Since allele frequencies of the T102C polymorphism may differ between individuals of different ethnic backgrounds, it needs to be conducted in an advanced research.

Relationship among Dysfunctional Attitudes, Stress Coping Strategies and Depressive Symptoms in Psychiatric Patients (정신질환자들의 역기능적 태도, 스트레스 대처 방식 및 우울증상 간의 관계)

  • This study was aimed to investigate dysfunctional attitudes, stress coping strategies and depressive symptoms in psychiatric patients. The subjects of this study consisted of 210 patients(138 schizophrenic patients, 29 depression patients, 43 alcohol dependence patients) according to DSM-IV criteria. Futhermore, the instruments were K-BDI(Beck Depression Inventory-Korean version), DAS(Dysfunctional Attitude Scale) and multidimensional coping strategy scale. The results were the following. 1) There were statistically significant correlations between depressive symptoms and dysfunctional attitudes in psychiatric patients. 2) In terms of coping strategies, there were positive correlations between depressive symptoms and focus on and venting emotions, accommodation, active forgetting, self-criticism, positive comparison, fatalism, passive withdrawal. Whereas, there was significant negative correlation between depressive symptom and active coping. 3) In terms of coping strategies, there were significant correlations between dysfunctional attitudes and focus on and venting emotions, active forgetting, self-criticism, positive comparison, fatalism, passive withdrawal. 4) Depression groups reported significantly higher BDI scores than schizophrenia groups. 5) In depression groups, DAS scores were significantly higher than those in schizophrenia groups. 6) In terms of coping strategies according to diagnosis, there were significant differences in venting emotions, active forgetting and self-criticism. As for venting emotions, alcoholic groups were scored significantly higher than schizophrenic groups. As for active forgetting, depression groups were scored significantly higher than schizophrenic groups. In self-criticism, depression groups and alcohol dependence groups reported significantly higher scores than schizophrenic groups.

Negative Association between Schizophrenia and Rheumatoid Arthritis : Epidemiology and Prolactin and Estrogen as Protective Factors (정신분열병과 류마치스 관절염 사이의 음성적 상관관계 : 역학 및 방어적 요인으로서 Prolactin과 Estrogen)

  • The purposes of this study were to test the negative association between schizophrenia and rheumatoid arthritis(RA) and to clarify the role of prolactin and estrogen as protective factors in this association. The author compared the prevalence rate of RA between 561 patients with schizophrenia and 222 patients with mood disorder. For investigating the role of estrogen and prolactin, the author checked the plasma prolactin and estradiol level in 80 patients with paranoid schizophrenia and 77 patients with RA. The results were as follows. 1) Epidemiological data The prevalence rate of RA in the schizophrenic group was 0/561 and that of RA in the mood disorder group was 2/222. To compare these results between two groups, the author applied the Binomial test using the average prevalence rate of RA(0.8%) in the general population as a reference rate. The prevalence rate of RA in the schizophrenic group was significantly lower than that of RA in the general population. However, the prevalence rate of RA in the mood disorder group was not significantly different to that of RA in the general population. 2) Comparison of plasma prolactin and estradiol level between two groups The plasma level of prolactin in the schizophrenic group was significantly higher than that of prolactin in the RA group(p=0.000). However, the plasma level of estradiol in the schizophrenic group was significantly lower than that of estradiol in the RA group(p=0.017). These results were not consistent across gender. To contrast with the results in the female group, which were consistent with the results in the total subjects, for the male group, the plasma levels of prolactin and estradiol in the schizophrenic group were significantly higher than those of prolactin and estradiol in the RA group. These results support the results of previous studies which confirm the negative association between schizophrenia and RA. These results also suggest that the elevation of plasma prolactin level in the patients with schizophrenia has a antirheumatic effect while the elevation of plasma estradiol level in the patients with RA has a anti-schizophrenic effect, and that these effects act as a possible mechanism in the negative association between two disorders. However, these results suggest that this association is specific to female patients.

Correlations of Cerebellar Function with Psychotic Symptoms and Cognitive Function in Schizophrenic Patients (남자 정신분열병 환자의 소뇌기능과 정신증상 및 인지기능간의 연관성)

  • Objectives:There is increasing evidence that the cerebellum plays an important role in cognition and psychiatric symptoms as well as motor coordination. The concept of cognitive dysmetria has been making cerebellar function in schizophrenia the focus of current studies. In other words, disruption in the corticocerebellum-thalamic -cortical circuit could lead to disordered cognition and clinical symptoms of schizophrenia. The purposes of this study were to determine cerebellar dysfunction in male schizophrenic patients semiquantitatively with ICARS and to investigate the clinical and cognitive correlates of ICARS in patients. Methods:We compared the scores of cerebellar neurologic sign using ICARS in 47 male patients with a DSM-IV-TR diagnosis of schizophrenia with 30 gender and age-matched healthy control subjects. The semiquantitative 100-point ICARS consists of 19 items divided into 4 unequally weighted subscores:posture and gait disturbances, kinetic functions, speech disorders and oculomotor disorders. All subjects were also assessed with cognitive function test. Cognitive functions were evaluated by Korean-Mini Mental Status Examination (K-MMSE), Verbal fluency test, and Clock drawing test. The patients were administered Korea version of Positive and Negative Symptom Scale(K-PANSS) to assess the symptom severity. Results:Schizophrenic patients had significantly higher scores on the ICARS than control subjects with posture and gait disturbances, kinetic functions, and oculomotor disorders. They also showed more significant impairments in cognitive function tests than control subjects. There was a significant correlation between ICARS and negative symptoms of patients. In cognitive function test, Clock drawing test was significantly associated with negative symptoms. In addition, Clock drawing test was negatively correlated with the total score of ICARS. Conclusion:In this study, we confirmed that schizophrenic patients have significant impairments in cognitive and cerebellar function, and that those were related with negative symptoms of schizophrenic patients. These results support a role of the cerebellum in schizophrenia. It is meaningful that we used a structured, and reliable procedure for rating neurological soft signs, ICARS. We hope that future prospective studies using a similar design help that rate of neurological sign should have been visible with the progression of illness.

Determination of free amino acids in the plasma samples of normal subjects and schizophrenic subjects in Korea by HPLC (HPLC를 이용한 한국인 정상인과 정신분열증 환자의 혈장 중의 유리 아미노산의 정량)

  • Multiple-step gradient method was used for the analysis of free amino acids in physiological fluids by high-performance liquid chromatography with diode array detector on the Amino Quant $C_{18}$ column under the condition of pH 7.2 of buffer solutions. Plasma samples of normal Korean people and abnormal Korean people who have schizophrenia were subjected to derivatization with o-phthalaldehyde in the presence of 3-mercaptopropionic acid. Quantitative analysis of amino acids in physiological fluids by internal standard method gave highly reproducible results within a relative standard deviation of less than 2~6%. And amino acids amounts of physiological fluids of Korean people gave some different results from those of foreigners. There was large differences in tyrosine amount between normal and abnormal man.

Association Analysis of Polymorphisms on Dystrobrevin Binding Protein 1(DTNBP1) Gene with Schizophrenia in the Korean Population (한국인에서 Dystrobrevin Binding Protein 1(DTNBP1) 유전자의 다형성과 정신분열병의 연합에 대한 고찰)

  • Objectives : This study was designed to investigate the association of schizophrenia and P1320, P1325, P1635, P1655, P1763 and SNP A polymorphisms on dystrobrevin binding protein 1(DTNBP1) gene in Korean patients. Methods : We analyzed P1320, P1325, P1635, P1655, P1763 and SNP A polymorphisms on DTNBP1 gene from their DNAs extracted from their blood in 388 Korean schizophrenic patients (male 198, female 190) and 372 control subjects(male 247, female 125). We compared the differences of genotype and allele distributions of the six polymorphisms on DTNBP1 gene between the Korean schizophrenic patient group and the normal control group. Results : There were no statistically significant differences of genotype and allele distributions of the P1320, P1325, P1635, P1655, P1763 and SNP A polymorphisms on DTNBP1 gene between the schizophrenic patient group and the normal control group. Conclusion : The results of this study suggest that P1320, P1325, P1635, P1655, P1763 and SNP A polymorphism on DTNBP1 gene do not have influence on the risk of the schizophnenic in the Korean population.

The Relationship between the Amisulpride-Induced Hyperprolactinemia and Taq1A Polymorphism of the Dopamine D2 Receptor Gene in Schizophrenia Patients (조현병 환자에서 아미설프라이드에 의한 고프로락틴혈증과 DRD2 유전자 Taq1A 다형성의 연관성)

  • Objectives This study was aimed to investigate the association between amisulpride-induced hyperprolactinemia and the Taq1A polymorphism in the D2 dopamine receptor gene (DRD2) in schizophrenic patients. Methods The plasma concentrations of prolactin were measured before and after treatment with amisulpride in one hundred and twenty-five schizophrenic patients. The effect of the Taq1A variants of the DRD2 on the risk of amisulpride-induced hyperprolactinemia was the main the outcome measure. The genotyping for Taq1A (rs1800497) polymorphism was performed using TaqMan single nucleotide polymorphism (SNP) genotyping assay. Results There was a significant difference between the prolactin level at baseline and the 6th week after treatment with amisulpride in all the subjects. However, there were no significant correlations between ΔProlactin (the difference between prolactin level at baseline and the 6th week after treatment) and the Taq1A genotypes. Conclusions This is the first study to investigate the-correlations between the Taq1A polymorphism and the amisulpride-induced hyperprolactinemia in Korean schizophrenic patients. The current results suggested the further large-scale researches on various SNPs in the DRD2 gene will establish clear goals and provide answers to the unanswered questions described in this study.

Antipsychotics Induced Etrapyramidal Symptoms in Schizophrenics in Relation to Cytochrome P450 2D6 Genotype (정신분열병 환자에서 Cytochrome P450 2D6 유형에 따른 부작용에 대한 연구)

  • The genetically determined CYP2D6 activity may be considered to be associated with antipsychotic induced extrapyramidal side effects with interindividual variation. Genetic polymorphism of CYP2D6 was determined by polymerase chain reaction(PCR) and MspI restriction fragment length polymorphisms(RFLP) for 194 schizophrenics. Subjects with a 334bp band were classified a1a1, those with 229bp and 105bp bands a2a2, and those with all three bands a1-a2. We did not identify schizophrenic subject with poor metabolizer. 194 schizophrenic patients previously treated neuroleptic medication, were assessed by Extrapyramidal Symptom Rating Scale(ESRS).The cases were composed of 33 akathisia, 47 parkinsonism, 21 tardive dyskinesia. These results are similar to the previous understanding that the poor metabolizer is very rare in Orientals compared to Caucasians, therefore, it considered that CYP2D6 genotypes have maybe no association with schizophrenia and extrapyramidal side effects in Koreans.

The Effects of Aerobic Exercise on Chronic Schizophrenia (에어로빅 훈련이 만성정신분열병 환자에게 미치는 영향)

  • The purpose of this research was to determine whether or not a consistent program of aerobic exercise as a nursing intervention would have beneficial effect on schizophrenic patients. Nonequivalent control group pre-post test research was designed. Subjects for this study were selected from thirty-eight chronically schizophrenic patients who were hospitalized at S psychiatric hospital in Taejeon city. Of them seventeen were experimental group, twenty-one were control group. The data were collected during the period from November 1 to December 30, 1992. The instruments used in this study were Scale for Assessment of Negative Symptom developed by Andreason, Behavioral Observational Checklist (SASS) developed by Jorgensen, Harvard Step Up Test. The data were analyzed by descriptive statistics, t-test and paired t-test, using the SPSS program. The results of this study are summerized as follows : 1. The scores of SANS were a significantly difference from pre to posttesting, when the experimental group was contrasted to the control group (t=4.73, p=.00). 2. The scores of SASS were a significantly difference from pre to posttesting, when the experimental group was contrasted to the control group (t=3.42, p=.00). 3. The scores of physical fitness were a significantly difference from pre to posttesting, when experimental group was contrasted to the control group (t=-5.87, p=.00).

Effects of a Family Approach Using Rein- forcement of Functional Communication In Families With a Schizophrenic Patient (가족치료가 정실분열증 환자의 상호작용 증진에 미치는 효과 분석 -의사소통 강화를 통한 가족치료-)

  • This study investigated and evaluated a method effectively promote a communication function in families with a schizophrenic patient and the patients' interaction, by a family approach using reinforcement of functional communication between patients and their parents. The subjects of the study were patients receiving treatment in Won Gwang University Mental Hospital from July 1985 to July 1986, and their families. In the experimental group, a family approach using reinforcement of functional communication twice a week was taught. Each of the six sessions took sixty minutes, during which the interviews and questionaires were carried out. The total number of patients receiving treatment in the hospital at that time, 42, were divided by a systematic method into 20families in an experimental group and 22 in a control group. The results from testing the hypotheses established in this study, to analyse the effects of family approach through reinforcement of functional communication, are as follows; The results of the test of the first hypothesis are shown in Table 1. “patients and their parents in an experimental group receiving group training in reinforcement of functional communication will show a greater degree of interaction than those of the control group” was supported. Significant differences included Main Effects p<.001, Interactions p<.001 and Explained p<.001 in the interactions of patients, and mothers. Scores were higher for the experimental group after the treatment as well as higher than the control group.
