Solitary sclerotic fibroma on the thigh

  • Sclerotic fibroma is a rare fibrous tumor of the skin associated with Cowden's disease. In 1989, Rapini described sclerotic fibroma without Cowden's disease as solitary sclerotic fibroma of the skin. It is a solid, well-circumscribed, slow-growing nodular tumor and it looks similar to a keloid scar. Consequently, it is extremely difficult to make a differential diagnosis of solitary sclerotic fibroma with keloid scar based on clinical findings only. The authors report a case of solitary sclerotic fibroma arising at the left lateral thigh of a 25-year-old man.

  • Congenital auricular deformities may be either deformational or malformational. Malformational anomalies present with a skin or cartilage shortage. Two cases with congenital cleft earlobe were presented. A new surgical technique using a two-layered repair with front and back flaps were introduced. She waited to begin wearing earrings until six months after the surgery to prevent possible scar contracture. The patients were followed up for a period ranging from 3 to 14 months. The earlobe volume deficiency was replaced, and acceptable scar maturation was obtained.

  • Oxidation characteristics of Zirlo and Zircaloy-4 tubes, which are widely used as nuclear power fuel cladding, are studied in steam environment up to $1200^{\circ}C$. Oxidation resistances are compared in terms of the mass increase due to the absorption of oxygen. The evolution of microscopic structure accompanied with the oxidation process is investigated. Also, the influence of oxidation on the fretting wear characteristics of the tubes is studied. Piezo-electrically actuated rig is employed to fret the tubes with cross-contacting arrangement. Wear scar is observed and measured, by using microscopes and a 3D-profiler. The results of fretting wear are quantified in terms of scar size, wear volume and wear coefficient, and compared for the three different tube materials of oxidated Zirlo, virgin Zirlo and Zircaloy-4.

  • Through the observation of wear scar of two ceramic materials, microstructural wear mechanisms was investigated. As for the $Al_2O_3$-5 vol% SiC nanocomposite, the grain boundary fracture was suppressed by the presence of SiC nano-particles. The intragranular SiC particles have inhibited the extension of plastic deformation through the whole grain. Part of plastic deformation was accommodated around SiC particles, which made a cavity at the interface between SiC and matrix alumina. On the other hand, gas-pressure sintered silicon nitride showed extensive grain boundary fracture due to the thermal fatigue. The lamination of wear scar was initiated by the dissolution of grain boundary phase. These two extreme cases showed the importance of microstructures in wear behavior.

Complications of Donor Site in Latissimus Dorsi Muscle Flap (광배근 피판의 공여부에 대한 문제점의 분석)

  • Purpose : To investigate the complication of donor site in latissimus dorsi muscle flap. Materials and Methods : From April 1983 to March 1999, forty patients with latissimus dorsi muscle flap for reconstruction of extremity and with a follow-up period of more than 12 months were included. We analysed the complication, shoulder function, degree of shoulder muscle weakness, skin scar width after operation. Results After 1 year, skin scar widening in 12 cases(30%), limitation of shoulder motion in 9 cases(12.5%), muscle weakness in 14 cases(17.5%) were found. Conclusion. The rate of complication at donor site after latissimus dorsi flap operation is around 10%. To minimize the complication, avoiding axillary skin incision, minimal invasive harvesting by endoscopy, meticulous suturing of subcutaneous layer are needed.

A Clinical Report on Facial Atrophic Scars treatment with Acupotomy (도침을 이용한 안면부 위축성 흉터의 치험례)

  • Objectives: The purpose of this study is to evaluate the clinical effectiveness of acupotomy in the treatment of various facial atrophic scars except acne scars. Methods: Four patients with facial atrophic scars were treated by superficial dermal undermining with acupotomy. They were treated six times, and evaluated by comparing before and after photos, PGA(Physician's Global assessment), SESES(The Stony Brook Scar Evaluation Score) and patient satisfaction. Results and Conclusions: After observing case photos, PGA, SBSES, patient's satisfaction, it can be considered that acupotomy is an effective treatment modality for facial atrophic scars and it has no significant side effects.

  • On this study, it was applied that the method of Coastline extracting by aerial photogrammetry so as to extract the coastline using the method of RTK GPS/GLONASS. The observed area is Gwanganri beach that is located in Pusan and it was observed according to high wave of scar when the approximate highest high water and it was surveyed according to that the boundary line connecting to sea water surface at random time-zone. Observation analysis was used digital map of 1:1,000 and compared coastline that was converted tide with coastline of high tide. So this conclusions was agreed with converted coastline and high tide coastline.

  • Antisense phosphorothioate oligodeoxynucleotide(PS-ODN) against $TGF-{\beta}$ was developed as scar formation inhibitor. The scar was caused collagen deposition due to overexpression of $TGF-{\beta}$ in wounded skin. The percutaneous absorption of partially modified PS-ODN(25 mer) was investigated for the purpose of its effective delivery. Though PS-ODN has high molecular weight (MW=8,000) and polyanionic charge, it was permeated through skin. The skin permeation of PS-ODN was markedly increased by the removal of stratum corneum and dermis. Moreover, the skin permeation of PS-ODN was decreased in the following order; hairless mouse skin>rat skin>human cadaver skin. Thus, PS-ODN represents a logical candidate for further evalution due to the potential for delivery into the wounded skin.

  • The harmonious lip length, residual scar and Cupid's bow width and peak with a normal side are the aims of a unilateral cleft lip repair. Also, long term amelioration without necessary of revisional surgeries may be the ideal conditions. No one method can satisfy the wide varieties of cleft lip deformities. Recently with rearrangement of paraoral muscle and some modifications, a straight line repair technique has been concerned again. Straight scar line, simplicity, and short learning curve are the advantages of the straight line technique. Here two cases of the simple straight line technique were presented and discussed for its usefulness and reliability with short reviews of previous reports.
