예방적인 스케일링의 건강보험적용 시기에 따른 치과 의료기관 이용과 구강건강상태 전·후 차이 비교 :국민건강영양조사 제5기와 제6기 자료를 활용하여 (Comparison of the oral health status before and after preventive scaling using the provision of national health insurance: The 5th-6th Korea National Health and Nutrition Examination Survey)

  • Objectives: The purpose of this study was to compare the oral health statuses pre- and post-insurance using the $5^{th}$ and $6^{th}$ National Health and Nutrition Examination Survey data to confirm the effect of scaling insurance after a year. Methods: Data were analyzed using IBM SPSS ver. 21.0 (IBM Co., Armonk, NY, USA). The four years were integrated, and a composite sample analysis was performed. A total of 26,990 people were included in the study before applying for scaling insurance (14,343 persons) or after receiving scaling insurance (12,647 persons). A chi-squared test was performed to compare the demographic characteristics and oral health status of the subjects. The significance level of the statistical test was 0.05. Results: The proportion of patients without implants was high before the provision of scaling insurance once a year, however, the proportion of patients with one or more implants was high (p<0.05) after the provision of scaling insurance once a year. Hemorrhagic periodontal tissues and tartar formation in periodontal tissues were highly prevalent before the provision of scaling insurance once a year, however, healthy periodontal tissues and formation of periodontal pockets were highly prevalent (p<0.05) after the provision of scaling insurance once a year. The decay, missing, and filled teeth index scores were higher before the provision of scaling insurance once a year (p<0.05). Conclusions: The aforementioned results showed that scaling once a year helps prevent or treat periodontal disease. In addition, we confirmed the effect of prevention on periodontal disease and dental caries, therefore, we expect it to develop into a stable policy.

일부 치위생학과 학생들의 스케일링 실습 과정에서의 근골격계 통증과 불편감 (Musculoskeletal pain and discomfort of dental hygiene students during scaling)

  • The purpose of this study was to investigate the association between wrong postures and pain during scaling and encourage dental hygienists and students to exercise scaling in a good position. After obtaining informed consent, 107 students (3rd and 4th grade students) who had an experience with scaling practice were enrolled. The questionnaire included three general items, four items related to the posture during scaling, and nine items related to pain management (total 16 items), for which the five-point Likert scale was used. Through the questionnaire, we examined the preference of posture during scaling, posture education during scaling, pain in each part during scaling, pain management, and pain management method. In the scaling exercise, 86.3% of the subjects were instructed on the correct posture, and 87.9% of the subjects perceived the possibility of inducing musculoskeletal disorders based on the scaling posture. The percentage of subjects who responded that they performed scaling in the correct posture was 33.6% and that of subjects who answered that they bowed or turned their head by more than 15° was 64.4%. Further, 45.7% of the subjects answered that they bent their shoulders, and 29.9% of the subjects answered that their postures were not parallel to the floor. Pain during scaling was still higher when they bent their head, they bent their waist, and they bent their wrist (p<0.05). During scaling, pain was most frequent in the fingers and hands (15%), followed by the neck (14%), shoulders (11.2%), waist (9.3%), and feet and legs (2.8%). The percentage of subjects who performed regular exercise (or stretching) to prevent pain was 29.9% and that of subjects who managed pain after scaling was 12.1%. Further, exercise (24.6%) and self-massage (20.3%) were highly used as the pain management methods, and the school practice was preferred to education media for pain management (79.4%). In the scaling practice, there was a training on pain management, but the frequency of practicing in the wrong posture was high. Moreover, pain increased upon practicing in an incorrect posture. Therefore, more in-depth and systematic education on the necessity and method of musculoskeletal disease management during scaling is required.

  • Amaryllis ($Hippeastrum{\times}johnsonii$) is commonly propagated by three methods: seeds, offset bulblets, and twin scaling. Since the number and size of bulblets produced by these methods are low, we conducted an experiment to evaluate different bulb cutting methods for propagation. For this purpose, bulbs (circumference of 30 - 35 cm) were sectioned or notched into 8, 12, or 16 segments or twin-scaled into 48, 72, or 96 segments for bulblet formation. Our results show that the largest number of bulblets was produced by the twin scaling method, but they take longer to grow to a final, commercial size. Sectioning and notching resulted in larger, but fewer bulblets than twin-scaling. Compared to notching, sectioning provided more space for the bulblets to grow, and is therefore the recommended method. While increasing the number of sections cut from a single bulb resulted in a larger number of bulblets, the diameter of the bulblets decreased. Therefore, sectioning the bulb into 8 segments was the best method for producing an acceptable number of vigorous bulblets.

스케일링을 이용한 다중 스케일 균열 검출 (Multi-scale Crack Detection Using Scaling)

  • 본 논문에서는 스케일링을 이용한 다중 스케일 균열 검출 방법을 제안한다. 제안하는 방법은 형태학 알고리즘, 균열 특징, 스케일링을 기반으로 한다. 사용하는 형태학 연산자는 균열의 패턴을 추출한다. 열림과 닫힘의 연산을 이용하여 균열과 배경을 구분한다. 형태학을 기반으로 하는 분할은 작은 간격의 균열을 검출하는 기존의 차분 이용 통합 방법 보다 좋은 성능을 보인다. 그러나, 형태학 방법들은 오직 하나의 구조 연산자를 사용하면 고정된 크기의 균열만을 검출할 수 있다. 따라서 스케일링 방법을 사용한다. 스케일링에 이중선형 보간법을 사용한다. 제안하는 방법은 분할된 영역의 화소 수와 최대 길이와 같은 특징들의 값들을 계산한다. 구분된 영역이 균열에 해당하는 지를 계산한 특징들의 값들에 의하여 결정한다. 실험 결과에서 제안한 다중 스케일 균열 검출 방법이 기존의 검출 방법들보다 향상된 결과를 보인다.

결빙 풍동시험을 위한 스케일링 기법 연구 (Scaling Methods for Icing Wind Tunnel Test)

  • 비행 중 결빙은 항공기 운용에 위험한 상황을 초래하게 되며, 개발 및 감항 인증 과정중 성능과 안전에 미치는 영향이 평가되어야 한다. 평가를 위한 결빙 풍동시험에서 스케일링 기법은 실제와 동등한 수집율과 결빙량이 모사되는 대체시험 조건을 결정하는 기법이다. 본 논문에서는 Olsen 및 Ruff-IV 기법과 무차원 Weber 수를 일치시켜 속도를 계산하는 방법을 적용하여 연구자들이 쉽게 이용할 수 있도록 스케일링 프로그램을 개발하였다. 동일조건에서 수행된 NASA 스케일링 결과와 비교하여 프로그램을 검증하였다. 또한 FAR Part 25 Appendix C를 적용한 스케일링 사례를 제시하고 결빙코드 FENSAP-ICE를 이용하여 스케일링 기법을 검증하였다.

Efficacy of active noise-canceling headphones in patients undergoing ultrasonic scaling

  • Purpose: Dental fear hinders patients from receiving appropriate dental treatment. In particular, the noise generated by high-speed air turbines and ultrasonic scalers can adversely affect patients. Many efforts have been made to reduce the discomfort caused by noise, but no methods are definitively recommended. The purpose of this study was to determine the efficacy of active noise-canceling (ANC) headphones in reducing the pain and discomfort associated with dental scaling. Methods: Fifty-five patients requiring scaling and root planing, aged ≥19 years and showing no auditory problems, were included. Scaling was performed for the bilateral maxillary molars and premolars while patients wore headphones, with ANC turned either on or off. The degree of noise and pain reduction in the on and off conditions were surveyed using a visual analog scale (VAS). The Wilcoxon signed-rank test was performed to compare noise-and pain-related discomfort with ANC turned on and off. Results: The sample included 28 men and 27 women with a mean age of 45.45±13.12 years. The average noise-related discomfort score was 3.84±2.12 and 2.95±1.99 when noise-canceling was turned off and on, respectively, with a statistically significant difference (P<0.05). Similarly, the average pain-related discomfort score was 3.78h±2.00 and 3.09±1.96 when noise-canceling was turned off and on, respectively, which was a statistically significant difference (P<0.05). Conclusions: The use of ANC headphones seems to reduce the discomfort caused by noise and pain in patients undergoing scaling.

시멘트 콘크리트 포장의 스케일링 저항성 평가방법에 관한 실험적 연구 (An Experimental Study on Evaluation Methods for Scaling Resistance of Cement Concrete Pavement)

  • 적설한랭지의 경우, 동절기의 동해 및 적설에 의한 교통통행의 원활을 기하기 위하여 제설제를 사용한다. 이와 같은 제설염의 사용은 동결융해 및 염화물에 의해 콘크리트의 급속한 스케일링 피해를 야기시킨다. 이에 대한 피해로 인하여 콘크리트 포장은 콘크리트의 피복두께 감소에 따른 조기 열화 및 사용자의 안정성 등 여러 가지 문제를 발생시킨다. 이에 따라 국외적으로 이와 같은 현상에 대하여 평가를 하기 위해 다양한 시험 및 평가 방법이 제시되고 있다. 하지만 국내에서는 아직 평가방법에 대한 규정은 없으며, 관련 연구 또한 미미한 수준이다. 따라서 본 논문에서는 각 기관 및 나라에서 제시되고 있는 평가방법을 조사하고, 각각의 규정에 따라 실험을 수행하여 결과를 도출하여 비교 분석하였다. 나아가 이와 같은 실험방법과 실험결과의 고찰을 통하여 국내현장에 맞는 실험법을 제안하고자 한다.

  • The singular value decomposition and the spectral decomposition are the useful methods in the area of matrix computation for multivariate techniques such as principal component analysis and multidimensional scaling. These techniques aim to find a simpler geometric structure for the data points. The singular value decomposition and the spectral decomposition are the methods being used in these techniques for this purpose. In this paper, the singular value decomposition and the spectral decomposition are compared.

치과의료소비자의 구강건강신념이 건강보험 급여화에 따른 스케일링 행위에 영향을 미치는 요인 (The factors of oral health beliefs on scaling performance by national health insurance coverage in consumers)

  • Objectives: The purpose of this study is to investigate the factors of oral health beliefs on scaling performance by national health insurance coverage in consumers. Methods: The subjects were 353 people living in Seoul, Incheon, and Gyeonggi-do from September 25 to October 20, 2013. They filled out the self-reported questionnaire after receiving informed consents. The questionnaire included 6 questions of general characteristics, 6 questions of oral health behavior, 6 questions of health insurance coverage, and 1 question of subjective oral health recognition. The oral health belief consisted of 6 questions of seriousness, 6 questions of susceptibility, 8 questions of barriers, 5 questions of benefit, and 3 questions of self-efficacy measure by Likert 5 scale. Cronbach's alpha in the study was 0.759. Data were analyzed using SPSS version 20.0 for frequency analysis, t-test, ANOVA, post-hoc Scheffe test, Pearson's correlation coefficient, and binary logistic regression. Results: The influencing factors of oral health belief model were Seriousness(${\beta}=0.091$), Self efficacy(${\beta}=-0.471$) and age(${\beta}=0.855$)(p<0.05). Those who had highly perceived seriousness and younger age tended to have probability of scaling performance. Higher self-efficacy tended to take more chance to have scaling performance probability. Conclusions: In order to cover the scaling by national health insurance, it is very important to notice the benefit of health insurance coverage of scaling to the consumers. National health insurance coverage enables the scaling practice to be easily accessible to the people. Easy access to scaling by low cost strategy can improve the oral health behavior.
