Reinfection by Opisthorchis Viverrini after Treatment with Praziquantel

  • Background: The prevalence of infection by the liver fluke, Opisthorchis viverrini (O. viverrini), has remained high in Northeast Thailand where it is a major risk factor for the eventual development of cholangiocarcinoma (CCA). The infection is acquired by the consumption of dishes containing unsafely prepared freshwater fish, a dietary tradition which has proved resistant to change. Since many people are aware that dosing with praziquantel (PZQ) is a successful treatment for an episode of the infection, there is a risk that, to avoid the long term consequences, they will engage in a cycle of infection, dosing and reinfection. Objectives: There is a dearth of studies of reinfection by O. viverrini, and the aims of this study were to assess re-infection rates in a typical province of Northeastern Thailand where O. viverrini infection is likely and to investigate factors associated with reinfection. Materials and Methods: A total of infected 607 villagers were treated with PZQ, and those found to be no longer infected were followed up at six-monthly intervals over 12 months. Results: At the end of this period data on 457 subjects were available for analysis using descriptive statistics and logistic regression, and 50 were found to have become reinfected, giving a cumulative reinfection rate of 10.9%. The results of a multiple logistic regression analysis showed that the only factor found to be associated with reinfection was past use of PZQ. Conclusions: Recommendations are made for future larger scale and better designed reinfection studies in the light of limitations of the current study. Further efforts are needed to discourage people from eating fish dishes likely to contain viable metacariae.

The effect of the Mckenzie exercise and the williams exercise on the reduction of low back pain and on the changes of EMG (맥켄지와 윌리암스 운동 요법이 요통감소와 EMG에 미치는 영향)

  • The purpose of this study was to identify the superior exercise on the reduction of low back pain, between the Mckenzie exercise and the williams exercise. 24 chronic low back pain patients were randomly divided into 3 groups (Mckenzie exercise group = 8, williams exercise group = 8, control group = 8). Each group examined using the Borg scale in a reduction of low back pain, the EMG amplitude in a stationary sit-up position and in a stationary 1000 back extension position. The Results are as follow. 1. After the training period, the Mckenzie exercise group and the williams exercise group revealed reduction of low back pain, but the control group does not revealed it. 2. After the training period, there were no significant differences on the reduction of low back pain between the Mckenzie exercise group and the williams exercise group. 3. After the training period, no groups decreased on the abdominal muscle EMG amplitude. 4. After the training period, all groups revealed no significant differences on the abdominal muscle EMG amplitude. 5. After the training period, all groups decreased on the low back muscle EMG amplitude. 6. After the training period, all groups revealed no significant differences on the low back muscle EMG amplitude. Overall, the study suggested that the Mckenzie exercise and the williams exercise achieve the same effect on the reduction of low back pain, and the fact seems to be influenced by other factors without muscular adaptation.

A Study on the Interpretation of Amenity Structure for the Creation of Urban Landscape (쾌적한 도시환경의 창출을 위한 도시 어메니티 구조에 관한 연구)

  • A study on the method of evaluation the urban amenity structure in Pusan city was established. Finally a survey sites out of 41 regions were selected on the basis of questionnaires : Taejong-dae and Haeun-dae as a seascape, Pumosa and Daesin-park as a mountain, Daechong-park and Seongjigok-park as a mixed, and Chungryulsa, Yongdoosan-park and U. N. Cemetry as a urban type. The abstracted results of amenity elements were revealed as natural environments including convex type as beach, reservoir, valley and mountain, and plant elements including woods and flower beds which raised amenity. The elements of social surroundings including children's playing, the aged's rest, and elements of structures including historic and memorial structures and high buildings. Amenity element made up of each space by region were abstracted from the Semantic Differential method. According to the factor analysis on the ground SD scale values, Kaiser's measure of sampling adequacy for 24 variables is 08602 and very high. Four factors including pleasantness, healthiness, convenience and safety showed 54.42 percent for total variance. By means of multiple regression, the model was as follows : Y=1.6636+0.3684X4+0.1955X11+0.1614X15-0.1688X23+0.1468X24. Therefore, Y:amenity, X4:beautiful-ugly, X11:clean-dirty, X15:creative-imitative, X23:cozy-dreary, X24:free-restrained. All variables in the model were significant at 0.001 level. According to the results of regression on satisfaction, the variables of satisfaction affecting amenity are the size of green space, the condition of management and the harmony with the surroundings. I think the considerating on the above could improve amenity of each region and further Pusan city.

Elevated Blood Urea Nitrogen/Creatinine Ratio Is Associated with Venous Thromboembolism in Patients with Acute Ischemic Stroke

  • Objective : Although venous thromboembolism (VTE) is frequently related to dehydration, the impact of dehydration on VTE in acute ischemic stroke (AIS) is not clear. This study investigated whether dehydration, as measured by blood urea nitrogen (BUN)/creatinine (Cr) ratio, influences the occurrence of VTE in patients with AIS. Methods : This is a retrospective study of patients with AIS between January 2012 and December 2013. Patients with newly diagnosed AIS who experienced prolonged hospitalization for at least 4 weeks were included in this study. Results : Of 182 patients included in this study, 17 (9.3%) suffered VTE during the follow-up period; in two cases, VTE was accompanied by deep vein thrombosis and pulmonary embolism. Patients with VTE were more frequently female and had higher National Institutes of Health Stroke Scale (NIHSS) score, more lower limb weakness, and elevated blood urea nitrogen BUN/Cr ratio on admission. In a multivariate analysis, BUN/Cr ratio >15 (odds ratio [OR] 8.75) and severe lower limb weakness (OR 4.38) were independent risk factors for VTE. Conclusion : Dehydration on admission in cases of AIS might be a significant independent risk factor for VTE.

County-Based Vulnerability Evaluation to Agricultural Drought Using Principal Component Analysis - The case of Gyeonggi-do - (주성분 분석법을 이용한 시군단위별 농업가뭄에 대한 취약성 분석에 관한 연구 - 경기도를 중심으로 -)

  • The objectives of this study were to develop an evaluation method of regional vulnerability to agricultural drought and to classify the vulnerability patterns. In order to test the method, 24 city or county areas of Gyeonggi-do were chose. First, statistic data and digital maps referred for agricultural drought were defined, and the input data of 31 items were set up from 5 categories: land use factor, water resource factor, climate factor, topographic and soil factor, and agricultural production foundation factor. Second, for simplification of the factors, principal component analysis was carried out, and eventually 4 principal components which explain about 80.8% of total variance were extracted. Each of the principal components was explained into the vulnerability components of scale factor, geographical factor, weather factor and agricultural production foundation factor. Next, DVIP (Drought Vulnerability Index for Paddy), was calculated using factor scores from principal components. Last, by means of statistical cluster analysis on the DVIP, the study area was classified as 5 patterns from A to E. The cluster A corresponds to the area where the agricultural industry is insignificant and the agricultural foundation is little equipped, and the cluster B includes typical agricultural areas where the cultivation areas are large but irrigation facilities are still insufficient. As for the cluster C, the corresponding areas are vulnerable to the climate change, and the D cluster applies to the area with extensive forests and high elevation farmlands. The last cluster I indicates the areas where the farmlands are small but most of them are irrigated as much.

Optimum Image Compression Rate Maintaining Diagnostic Image Quality of Digital Intraoral Radiographs

  • Purpose: The aims of the present study are to determine the optimum compression rate in terms of file size reduction and diagnostic quality of the images after compression and evaluate the transmission speed of original or each compressed image. Materials and Methods: The material consisted of 24 extracted human premolars and molars. The occlusal surfaces and proximal surfaces of the teeth had a clinical disease spectrum that ranged from sound to varying degrees of fissure discoloration and cavitation. The images from Digora system were exported in TIFF and the images from conventional intraoral film were scanned and digitalized in TIFF by Nikon SF-200 scanner (Nikon, Japan). And six compression factors were chosen and applied on the basis of the results from a pilot study. The total number of images to be assessed were 336. Three radiologists assessed the occlusal and proximal surfaces of the teeth with 5-rank scale. Finally diagnosed as either sound or carious lesion by one expert oral pathologist. And sensitivity, specificity and k value for diagnostic agreement was calculated. Also the area (Az) values under the ROC curve were calculated and paired t-test and oneway ANOVA test was performed. Thereafter, transmission time of the image files of the each compression level was compared with that of the original image files. Results: No significant difference was found between original and the corresponding images up to 7% (1 : 14) compression ratio for both the occlusal and proximal caries (p<0.05). JPEG3 (1 : 14) image files are transmitted fast more than 10 times, maintained diagnostic information in image, compared with original image files. Conclusion: 1 : 14 compressed image file may be used instead of the original image and reduce storage needs and transmission time.

  • Rotating machines are widely used in industrial world and especially motor and generator take up much part of it. As for this kind of motor and generator, electrical loss due to eddy current is the very important factor and that is also a primary factor causes heat generation. To solve this kind of problem like the above. insulated laminating silicon steel sheet is used to prevent eddy current effect. Laminated rotor is widely used as rotating shaft of motor and generator. Due to that, electrical loss and heat problem can be solved but designer meets another problem. In general. most of the motor and generator can be normally operated under 3,600 rpm because they are designed to have the first critical speed more than that speed. But nowadays, they should be operated more than the first critical speed as usual with the trend of high speed, large scale and high precision in industrial world. The critical speed can be determined from the inertia and stillness for the rotor and bearing of rotating systems. The laminated rotor stiffness can be hardly determined because it can be derived a lot factors for instance rotor material and shape, lamination material and shape, insulation material. lamination force and so on. In this paper, the change of the natural frequency of the motor was examined with the change of the lamination force as an experimental method and design criteria will be presented for motor & generator designer, who can apply the result of numerical analysis with equivalent diameter scheme with ease.

  • SMEs manufacturing Factory, which are small-scale production systems of various types, mass-produce and sell products in order to meet customer needs. This means that the company has an excessive amount of material supply to reduce the loss due to lack of inventory and high inventory maintenance cost. And the products that fail to respond to the demand are piled up in the management warehouse, which is the reality that the storage cost is incurred. To overcome this problem, this paper uses ARIMA model, a time series analysis technique, to predict demand in terms of seasonal factors. In this way, demand forecasting model based on economic order quantity model was developed to prevent stock shortage risk. Simulation is carried out to evaluate the effectiveness of the development model and to demonstrate the effectiveness of the development model as applied to SMEs in the future.

Bankart Suture Repair for Anterior Instability of the Shoulder- Results of Arthroscopic versus Open Repair - (견관절 전방 재발성 탈구의 치료-관절경 및 관혈적 Bankart병변 수복술의 비교 -)

  • Purpose : We evaluated clinical result of arthroscopic and open Bankart repair in anterior shoulder instability to identify factors iuluencing operative result and prognosis. Materials & Methods . We reviewed 24 patients of anterior shoulder instability treated with arthroscopic Bankart repair in 16 cases and open Bankart repair in 8 cases. Average age was 26 years old and involved in dominant arm in 15 cases. Patients were suffered instability for 3.1 years before operation and mean follow-up was 2 year 9 months ( 1 you 9 months -4year 10 months). Results : Post operative pain was subsided in 2 weeks in arthroscopic surgery and 3 weeks in open surgery. The final range of motion after arthroscopic repair were flekion in 168" , external rotation in 54" , and internal rotation in 79, and after open repair 168" ,49" , and 78 respectively. In arthroscopic surgery,2 cases (13%) were redislocated, and 4 cases(25%) showed mild instability. In open case,1 case (11%) showed mild instability. According to function- al result by Rowe grading scale, satisfactory results were 12case (76%) in arthroscopic repair and 7 cases (88%) in open cases. Conclusions Both arthroscopic or open Bankart could get good results in the treatment of anterior instability of shoulder. In arthroscopic repair, perioperative morbidity was lower than open repair, but it needs careful rehabilitation program to prevent redislocation and to return to sports activity.

Nannochloris eucaryotum growth: Kinetic analysis and use of 100% CO2

  • Microalgae are receiving an increasing attention because of their potential use as $CO_2$ capture method and/or as feedstock for biofuels production. On the other hand the current microalgae-based technology is still not widespread since it is characterized by technical and economic constraints that hinder its full scale-up. In such contest the growth kinetics of Nannochloris eucaryotum (a relatively unknown marine strain) in batch and semi-batch photobioreactors is quantitatively investigated with the aim of obtaining the corresponding kinetic parameters suitable for process engineering and its optimization. In particular the maximum growth rate was evaluated to be 1.99 $10^{-3}\;h^{-1}$. Half saturation concentrations for nitrates ($K_N$) and phosphates uptake ($K_P$) were evaluated as 5.4 $10^{-4}\;g_N\;L^{-1}$ and 2.5 $10^{-5}\;g_P\;L^{-1}$, respectively. Yield factors for nitrogen ($Y_N$) and phosphorus ($Y_P$) resulted to be 5.9 $10^{-2}\;g_N\;g^{-1}$ biomass and 6.0 $10^{-3}\;g_P\;g^{-1}{_{biomass}}$, respectively. The possibility of using 100% (v/v) $CO_2$ gas as carbon source is also evaluated for the first time in the literature as far as N. eucaryotum is concerned. The strain showed a good adaptability to high concentrations of dissolved $CO_2$ as well as to low pH. The lipid content under 100% $CO_2$ is about 16.16 %wt $wt^{-1}$ and the fatty acid methyl esters composition of the extracted oil is in compliance with the European regulation for quality biodiesel.
