Phytochemical compounds and quality characteristics of Aster scaber Thunb. in response to blanching conditions and treatment with solutes (블랜칭 및 용질 처리에 따른 참취(Aster scaber Thunb.)의 phytochemical 성분 및 품질특성)

  • The quality characteristics of blanched Aster scaber with the blanching condition and various solutes (non-treated, NT; soluble Ca, SC; sodium bicarbonate, SB; and magnesium sulfate, MS) were evaluated with different blanching times and solutes. The blanching process decreased the total polyphenolics, ascorbic acid contents, hardness, and cutting strength of the leaves. As for the pectinesterase and polygalacturonase, the blanching process increased their inhibitory activities, and more than 90% of them were inactivated in all the samples that were blanched. For the total counts and the number of coliform groups, the number of total aerobes at 5.92-log CFU/g before the blanching process was reduced to the approximately 2-3 log scale, and the coliform group was not detected after the blanching. The blanching also significantly decreased the total chlorophyll a and b ratios. The sensory characteristics of the Aster scaber according to the test group showed that the leaves blanched for 3 min were the most highly evaluated in terms of their overall acceptability. The phenolic compound, chlorophyll, and carotenoid contents tended to increase from before their blanching, and the Ca/Cb ratio was higher in the SC. These results showed that SC and MS treatment had greater effects on the quality characteristics and the pigmentation.

Intervention for Reducing Dental Fear and Anxiety of Dental Patients (치과내원 환자의 치과공포 감소를 위한 중재법 적용)

  • The purposes of this study were to assess dental fear and anxiety level of patients attending a dental clinic using the dental fear survey (DFS) scale, to apply interventions chosen by patients for reducing dental fear and anxiety and measure their effects. This study surveyed 34 patients who visited a dental clinic in Seoul about their self-rated health and their experience of dental fear, and measured the level of dental fear using DFS. Trained dental hygienists applied interventions desired by the patients for reducing dental fear and anxiety and, for each intervention, examined the patients' satisfaction (very helpful [5 point]~not helpful at all [1 point]). Collected data were analyzed using IBM SPSS Statistics ver. 21.0 through independent t-test or one-way ANOVA for difference in level of fear according to related characteristics, and through Wilcoxon signed rank test for comparison between before and after the intervention. The subjects' mean level of fear (DFS score) was 44.53, which was an average level, and the level of dental fear was relatively high for stimulus-response (2.72). The level of dental fear was higher in those who had experienced pains or indirect dental fear from dental treatment in the past, and those whose subjectively perceived health state was poor (p<0.05). With regard to the applied dental fear intervention, 'Helpful (3.57)' was the most common answer. Overall satisfaction before and after the application of dental fear intervention was generally high as 4.37 and 4.35, respectively, but it decreased slightly after the intervention. In order to lower the level of dental fear, it is considered effective to survey not only the level of dental fear but also patients' need of dental fear interventions and to apply a suitable intervention. It is also required to educate dental workers and to develop related manuals.

THE EFFECT OF SACROOCCIPITAL TECHNIQUE CATEGORY II BLOCKING ON ABDOMINAL MUSCLE ENDURANCE AND SHOULDER PAIN (S.O.T CATEGORY II BLOCKING이 복근지구력과 어깨 통증에 미치는 영향)

  • Objectives: The purpose of this study was to determine whether the sacrooccipital category II blocking technique may improve abdominal muscle endurance and alleviate shoulder pain caused by pelvic tilting. Methods: A total of 50 subjects diagnosed with category II sacroiliac joint instability and lower back pain were randomly assigned to the control or treatment group. The sacrooccipital technique category II blocking procedure (2-min duration) was performed 3 times a week until the category II indicator of joint instability had subsided. The control subjects were subjected to a sham procedure of equal duration and frequency. We assessed abdominal muscle endurance using the partial curl-up test and shoulder pain using a visual analogue scale, before and immediately after the intervention and 2 weeks after the intervention. Results: On two-way analysis of variance with repeated measures on time factor, significant treatment and interaction effects on muscle endurance were found. A significant interaction effect, but not treatment effect, was found for shoulder pain. Post hoc test showed that the shoulder pain was reduced immediately after intervention (treated group) and 2weeks (control and treated group) after the intervention as compared to before the intervention. Conclusions: This study suggests that sacrooccipital category II blocking can be used to alleviate shoulder pain caused by pelvic instability. The mechanisms behind the long-term benefits may include an increase in abdominal muscle endurance.

Effects of Different Exercise Intensity on FDEIA and Related Mechanisms (운동 유발성 알레르기 질환(FDEIA)에 미치는 영향과 기전분석)

  • Food-dependent exercise-induced anaphylaxis (FDEIA) is a distinct form of food allergy induced by physical exercise. It is typified by the onset of anaphylaxis during exercise, which is preceded by the ingestion of causal food allergens. Diagnosis of FDEIA is heavily dependent on clinical history. To describe the physiopathological mechanism, etiologic factors, and clinical manifestations, we evaluated the spleen index, proliferation assay of lymphocyte, ROS, ASAS, and cytokines levels in sensitized and exercise-trained mice. One-hundred mice were bred in the animal lab at D and P university under controlled conditions [$22{\pm}2^{\circ}C$, RH 45-55%, and a 12-hour photoperiod]. Animals are 7-weeks-old at the time of study and were fed a standard commercial chow diet from 09:00 to 15:00 over the 8-week study period. The mice were allowed access to distilled deionized water ad libitum. Daily food intake and weekly body gains were routinely recorded throughout the experimental period using computing scale (CAS). Mice were divided into the control group (S; control sensitized, n=25), 30 min swim training group (S30, N=25), 50 min swim training group (S50, N=25), and 80 min swim training group (S80, N=25). The results were as follows: Spleen index showed the highest level in the S80 group compared to other groups; this level was exercise-dependent. In proliferation assay of Med and OVA, the S80 group showed the highest level compared to the other groups; this level also was exercise intensity- dependent. Peritoneal ROS and IL-4 showed a statistically significant difference compared to S; however, there was no significant differences in ROS among S30, 50, and 80. From the results, we concluded that FDEIA is correlated with exercise intensity based on the levels of peritoneal ROS and cytokine profiles.

The Effect of Gamisungmagalguntang(加味升麻葛根湯) on Chronic Urticaria Identified as "Wind Heat" Pattern (풍열형(風熱型) 만성 두드러기에 대한 가미승마갈근탕(加味升麻葛根湯)의 치료효과)

  • Objective : This case series study was designed to assess the effect of Gamisungmagalguntang for chronic urticaria identified as "Wind-Heat" pattern by retrospective methods. Methods : We treated 19 patients for chronic urticaria with Gamisungmagalguntang(加味升麻葛根湯), who visited to care chronic urticaria at the department of Ophthalmology, Otolaryngology and Dermatology of Korean Medicine at Kyunghee University Korean Medical Center from 1st January 2013 to 31st August 2013. We analyzed information and body conditions of 19 patients. And by using Urticaria Activicy Score(UAS), frequency and number of wheals, itch severity and total score on a 4-point(0-3) scale were assessed in 19 patients. The change of each criterion and total scores between baseline and follow up were analyzed using paired t-test and Wilcoxon test(p<0.05). Results : 1. UAS improvement was statistically significant(p<0.001). Effects of symptom improvement were excellent(31.58%) that means symptom remission and good(42.10%) that means symptom improved over 50%. 2. Heat sensitivity as suspected provocation factor could be a marker for the Wind-Heat pattern urticaria. 3. Among the Wind-Heat pattern urticaria patients, normal digestion(52.63%) and no thirst(78.95%) were higher than abnormal conditions. Whereas about sweat item, abnormal conditions(57.89%) were higher than normal. That means abnormal sweat conditions could be a marker for a defense qi(衛氣) dysfunction of the Wind-Heat pattern urticaria. Conclusions : Our findings suggest that Gamisungmagalguntang could be effective on the Wind-Heat pattern urticaria.

Comparison of Therapeutic Effect on Carpal Tunnel Syndrome between Oriental and Western Medicine (수근관증후군 환자에 대한 한방과 양방치료효과의 비교)

  • Objectives : This study was to assess the effectiveness of Oriental medical treatment on carpal tunnel syndrome, comparing its outcome with that of Western medical treatment. Methods : After being diagnosed with carpal tunnel syndrome by electromyography, subjects were enrolled in Kyung Hee Medical Center from March 2006 to January 2007. We prescribed Kejibokryung-hwan to the Oriental medical therapy group (OM group) and NSAIDS to the Western medical therapy group (WM group). Effectiveness was assessed by degree of pain using visual analog scale (VAS) before and after 3 weeks' treatment. Adverse effects were also monitored. Results : There were 21 patients in the OM group and 19 in the WM group. No statistical significant difference was detected at the baseline assessment. After 3 weeks of medication, pain was reduced about 26% in the OM group and 46% in the WM group. These findings might be explained by that more than half of the WM group received local steroid injection, which has been known to have more rapid analgesic effect that oral medication. Although pain reduction rate was higher in the WM group than in the OM group, we suggest that Oriental medical treatment is still effective, faking into consideration the fact that completely recovered cases were found only in the OM group. No adverse effect was found in either of the groups. Conclusion : This work could help us to understand the effectiveness of Oriental medical treatment on carpal tunnel syndrome.

Economic Evaluation of Eastern, Western and Collaborative Treatments for Patients with Frozen Shoulder Pain (견비통의 한${\cdot}$양방 진료 및 협진의 경제성 평가)

  • Objectives : The purpose of this study was to evaluate and compare the cost-effectiveness of Eastern, Western, and collaborative treatments and suggest a cost-effective approach for patients with frozen shoulder pain. Methods : Using the data of fifty-two patients, treatment effectiveness was measured by CSA, SPABI, and ROM scales and changes from the baseline score were evaluated. Data source for cost estimation was based on the national health insurance (NHI) payment system. Because the price in NHI was differentiated by health care institutions, five collaborative types were considered in assessing costs. Cost-effective ratios were computed for economic evaluation. Results : Compared with Eastern treatment, collaborative and Western treatments showed better effects on CSA scale after 4 weeks' treatment. The collaborative approach was also the most effective treatment on SPADI and ROM scales. The direct cost per patient receiving Eastern treatment was less than other treatments. In general, collaborative treatment dominated Eastern and Western treatments in cost-effectiveness an analysis. However, the cost-effectiveness ratio of Eastern treatment resulted in \9,000 compared to \29,000 of collaborative treatment on SPADI. Four different indicators of ROM scales resulted in different approaches as the cost-effective treatment. Conclusions : Considering cost-effectiveness ratios, collaborative treatment was the best treatment on CSh and SPADI scales after 4 weeks' treatment. As for ROM scales, the recommended alternatives were Eastern treatment for patients with abduction and adduction disabilities, Western treatment for those with flexion disability, and collaborative approach fir those with extension disabiliry.

A Comparative Effect of Meperidine between Intravenous and Epidural Patient-Controlled Analgesia for the Postoperative Pain Relief after Cesarean Section (제왕절개 수술후 통증조절을 위해 PCA를 이용한 정맥과 경막외 Meperidine 투여효과의 비교)

  • Patient-Controlled Analgesia (PCA) has been widely used for postoperative pain relief. Meperidine is useful for PCA and has efficient analgesia, rapid onset, and low incidence of adverse effect. To compare the analgesic effect, total dose and hourly dose, side effect and neonatal status of breast feeding with meperidine via intravenous or epidural PCA for 48 hours after Cesarean Section, 40 parturient women undergoing elective Cesarean Section were randomly divided into two groups. Each respective group of 20 parturient women received meperidine via one of the intravenous PCA after general anesthesia with enflurane (IVPCA group) and the epidural PCA after general anesthesia with enflurane (IVPCA group) and the epidural PCA after epidural block with 2% lidocaine 20ml combined with general anesthesia with only $N_2O$ and $O_2$ (EpiPCA group) when they first complained of pain in recovery room. Following the administration of analgesic initial dose, parturient women of IVPCA group were allowed intravenous meperidine 10 mg every 8 minutes when they felt pain. The EpiPCA group received additional bolus dose of meperidine 2 mg and bupivacaine 0.7 mg were administered every 8 minutes as requested the patients with hourly continuous infusion of meperidine 4 mg and bupivacaine 1.4 mg. Data was collected during the 48 hours observation period including visual analog scale (VAS) pain scores, total meperidine dose, hourly dose during 48 hours and each time interval, incidence of adverse effect, satisfaction, and neonatal status with breast feeding. VAS pain scores of analgesic effect in EpiPCA group was significantly lower than in IVPCA group at 2 hours after the initial pain after Cesarean Section. Total dose and hourly dose of meperidine significantly reduced in EpiPCA group. Hourly dose of meperidine at each time interval significantly reduced during first 6 hours and from 12 hours to 24 hours in EpiPCA group. The side effects in IVPCA group were mainly sedation, nausea, and local irritation of skin. And EpiPCA group experienced numbness and itching. The degree of satisfaction of parturient women was 88.2 % in IVPCA group and 85.7 % in EpiPCA group. We did not observe any sedation, abnormal behavior, or seizure like activity in any neonates of breast feeding. From the above results we conclude that epidural PCA was more efficiently analgesic, less sedative, and consumptional, and safer for neonate than intravenous PCA, and could be an alternative method to intravenous PCA.

Siamese Crocodile White Blood Cell Extract Inhibits Cell Proliferation and Promotes Autophagy in Multiple Cancer Cell Lines

  • Cancer represents one of the most significant threats to human health on a global scale. Hence, the development of effective cancer prevention strategies, as well as the discovery of novel therapeutic agents against cancer, is urgently required. In light of this challenge, this research aimed to evaluate the effects of several potent bioactive peptides and proteins contained in crocodile white blood cell extract (cWBC) against LU-1, LNCaP, PC-3, MCF-7, and CaCo-2 cancer cell lines. The results demonstrate that 25, 50, 100, and $200{\mu}g/ml$ cWBC exhibits a strong cytotoxic effect against all investigated cell lines ($IC_{50}$ $70.34-101.0{\mu}g/ml$), while showing no signs of cytotoxicity towards noncancerous Vero and HaCaT cells. Specifically, cWBC treatment caused a significant reduction in the cancerous cells' colony forming ability. A remarkable suppression of cancerous cell migration was observed after treatment with cWBC, indicating potent antimetastatic properties. The mechanism involved in the cancer cell cytotoxicity of cWBC may be related to apoptosis induction, as evidenced by typical apoptotic morphology features. Moreover, certain cWBC concentrations induced significant overproduction of ROS and significantly inhibited the $S-G_2/M$ transition in the cancer cell. The molecular mechanisms of cWBC in apoptosis induction were to decrease Bcl-2 and XIAP expression levels and increase the expression levels of caspase-3, caspase-8, and p53. These led to a decrease in the expression level of the cell cycle-associated gene cyclin-B1 and the arrest of cell population growth. Consequently, these findings demonstrate the prospect of the use of cWBC for cancer therapy.

Application of Automatic Stormwater Monitoring System and SWMM Model for Estimation of Urban Pollutant Loading During Storm Events (빗물 자동모니터링장치와 SWMM 모델을 이용한 강우시 도시지역 오염부하량 예측에 관한 연구)

  • An automatic flow and water quality monitoring system was applied to estimate pollutant loads to an urban stream during storm events in DTV (Daeduk Techno Valley), Daejeon, Korea. The monitoring system consists of rainfall gage, ultrasonic water level meter, water quality sensors for DO, temperature, pH, conductivity, turbidity and automatic water sampler for further laboratory analysis. All data are transmitted through on-line system and the monitoring system is designed to be controlled manually in the field and remotely from laboratory computer. Flow rates were verified with field measurements during storm events and showed good agreements. Automatic sampler was used to collect real time samples and analyzed for BOD, COD, TN, TP, SS and other pollutant concentrations in the laboratory. SWMM (Storm Water Management Model) urban watershed model was applied and calibrated using the observed flow and water quality data for the study area. While flow modeling results showed good agreement for all events, water quality modeling results showed variable levels of agreement. These results indicate that current options in the SWMM model to predict pollutant build up and wash-off effects are not sufficient to satisfy modeling of all the rainfall events under study and thus need further modification. This study showed the automatic monitoring system can be used to provide data to assist further refinement of modeling accuracy. This automatic stormwater monitoring and modeling system can be used to develop basin scale water quality management strategies of urban streams in storm events.
