연부 조직 종양에서 PET의 유용성: 기존의 진단법과의 비교 연구 (Diagnostic Efficacy of PET in Soft Tissue Tumors: Comparative Study with Conventional Methods)

  • 목적: 최근에는 연부 종양의 진단에 FDG-PET을 이용하기 위한 연구가 다양하게 이루어지고 있다. 그러나, 그 임상적 유용성에 대해서는 알려져 있지 않다. 이 연구의 목적은 기존의 진단법과 비교하여 FDG-PET의 유용성을 평가하는 데 있다. 대상 및 방법: 연구 대상은 2001년 3월에서 2002년 3월 사이에 연부 조직 종양으로 진단받은 29명의 환자(남자 16명, 여자 13명, 평균 47세)를 대상으로 하였다. 모든 환자군에서 기존의 검사법과 FDG-PET을 시행하였다. 타당성 검증에서 국소 병변은 모든 경우 조직검사로 판단하였고, 전이 병변은 조직 검사와 6개월 간의 추시 결과로 판단하였다. 각각의 진단은 독립적으로 시행되었으며, 진단의 정확도와 누적 비용-정확도율을 측정하였다. 결과: 국소 병변의 진단에서 MRI와 FDG-PET의 민감도, 특이도, 정확도는 각각 91%, 57%, 83% 와 95%, 43%, 83%이었다. 원격 전이의 진단에서 기존의 검사법과 FDG-PET의 민감도, 특이도, 정확도는 각각 77%, 89%, 87% 와 92%, 94%, 93%이었다. 누적 비용-정확도율은 145,000원/% 이었다. 종양의 등급별 민감도 분석에서 고등급의 종양이 가장 비용-효용성이 높았다. 결론: 국소부위의 재발과 잔존 종양의 진단에서 FDG-PET의 정확도는 MRI와 차이가 없었다. 반면, 원격 전이의 진단은 FDG-PET이 기존의 검사 보다 정확하였다. 고등급 종양의 경우 저등급 종양보다 FDG-PET의 유용성이 큰 것을 알 수 있었다.

Further Evidence of Linkage at the tva and tvc Loci in the Layer Lines and a Possibility of Polyallelism at the tvc Locus

  • Three lines of White Leghorn (WL) chickens (IWJ, IWG and IWC) maintained at Central Avian Research Institute, Izatnagar (UP), were used for chorioallantoic membrane (CAM) and liver tumour (LT) assay. Eleven-day-old embryos of each line were partitioned into three groups and inoculated with 0.2 ml of subgroup A, subgroup C and an equal mixture of subgroup A and C Rous sarcoma virus (RSV). Subgroup virus receptor on the cell surface membrane for subgroup A is coded for by tumour virus a (tva) locus and for subgroup C by tumour virus c (tvc) locus. The random association of the genes at the tva and tvc loci in IWJ and IWC line was assessed and the $x^2$-values for phenotypic classes were found to be significant, indicating the linkage between the tva and tvc loci. The linkage value was estimated to be 0.09 on pooled sex and pooled line basis. On the basis of four subclass tumour phenotypes a 4-allele model was proposed for tva locus having $a^{s1}$, $a^{s2}$, $a^{r1}$ and $a^{r2}$ alleles and the frequencies were calculated as 0.47, 0.13, 0.13 and 0.27 for IWJ line, 0.31, 0.33, 0.14 and 0.22 for IWG line and 0.44, 0.11, 0.21 and 0.24 for IWC line, respectively. Similarly, for tvc locus the frequencies of four alleles i.e. $c^{s1}$, $c^{s2}$, $c^{r1}$ and $c^{r2}$ were calculated as 0.42, 0.20, 0.21 and 0.17 for IWJ line, 0.42, 0.17, 0.27 and 0.14 for IWG line and 0.30, 0.21, 0.16 and 0.33 for IWC line, respectively. The $x^2$-values for all classes of observations were not significant (p>0.05), indicating a good fit to the 4-allele model for the occurrence of 4-subclass tumour phenotypes for tva and tvc loci. On the basis of the 2-allele model both tva and tvc locus carries three genotypes each. But, on the basis of the 4-allele model tva and tvc locus carries 10 genotypes each. The interaction between A-resistance and C-resistance (both CAM and LT death) was ascertained by taking the 10 genotypes of tva locus and 3 genotypes of tvc locus by pooling the lines and partitioning the observations into 3 classes. The $x^2$-values for the genotypic classes of CAM (-) LT (+) and CAM (-) LT (-) phenotypes to mixed virus (A+C) infection were found to be highly significant (p<0.01), indicating increased resistance, which indicates the joint segregation of $a^r$ and $c^r$ genes, suggesting the existence of close linkage between the tva and tvc loci. Therefore, an indirect selection approach using subgroup C viruses can be employed to generate stocks resistant to subgroup A LLV, obviating contamination with the most common agent causing LL in field condition.

Computed Tomography Manifestations of Histologic Subtypes of Retroperitoneal Liposarcoma

  • Objective: Liposarcoma (LPS) is the most common soft tissue sarcoma and accounts for approximately 20% of all mesenchymal malignancies, often occurring in deep soft tissue of retroperitoneal space. Accurate preoperative diagnosis is therefore necessary. We explored whether computed tomography (CT) could be used to differentiate between the various types of retroperitoneal liposarcoma (RPLS). Method: Forty-seven cases of RPLS, diagnosed surgically and histologically, were analyzed retrospectively. CT features were correlated with postoperative pathological appearance. Results: The study radiologist identified 29, 11, 2, 2 and 3 RPLS as atypical lipomatous tumor/well-differentiated liposarcoma (ALT/WDL), dedifferentiated liposarcoma (DDL), myxoid/round cell liposarcoma (ML/RCL), pleomorphic liposarcoma (PL) and mixed-type liposarcoma. Analysis of CT scans revealed the following typical findings of the different subtypes of RPLS: ALT/WDL was mainly visible as a well-delineated fatty hypodense tumor with uniform density and integrity margin; DDL was marked by the combination of focal nodular density and hypervascularity. ML/RCL, PL and mixed liposarcoma showed malignant biological behaviour and CT findings need further studies. Conclusions: CT scanning can reveal important details including internal components, margins and surrounding tissues. Based on CT findings, tumor type can be roughly evaluated and biopsy location and therapeutic scheme guided.

익기양음해독탕의 항암 및 항전이효과에 관한 연구 (Effect of Ikiyangeumhaedoc-tang on the tumor and metastasis)

  • Objective : Ikiyangeumhaedoc-tang(IYHT) has an effect of nourishing Yin(陰) and Jin(津), and has been used to cancer patient effectively. In order to prove the anticancer's and antimetastic effect of IYHT experimentally, studies were done. Methods : We evaluated the cytotoxic activity on HT-1080 cells as well as inhibitory effect on activity of DNA topoisomerase Ⅰ, cell adhesion, cell invasion and proliferation of HUVEC cells induced by bFGF and measured the expression of mRNA(uPA, MMP2, TIMP2), p-ERK protein, recovery effect of gap junctional intercellular communication by $H_{2}O_2$ and survival time of ICR mice bearing sacoma-180. Results : IYHT showed the inhibitory effect on DNA topoisomerase Ⅰ in the concentration of $100{\mu}g/ml,\;500{\mu}g/ml$ and the dosage-dependent inhibitory effect on the adhesion of HT-1080. The concentration of 1mg/ml of IYHT inhibited 15% of adhesion compared with control. IYHT decreased the expression of uPA, but not in MMP2, TIMP2 by RT-PCR and inhibited the expression of p-ERK effectively in the concentration of more than $500{\mu}g/ml.$ IYHT recovered the inhibited gap junctional intercellular communication by $H_{2}O_2$ to the level of 60% of normal control in the concentration of $400{\mu}g/ml$ but, did not extended the mean survival time of sarcoma 180-bearing mouse. Conclusions : It was concluded that IYHT could be applied usefully for prevention and treatment of human cancer, And also experimental study for the evaluation of molecular biological study and antimetastatic research would be recommended in the near future.

연조직종양의 조직 성분 평가를 위한 자기공명영상: MEDIC 과 지방억제 T2 영상의 비교 (MRI Evaluation for the Histologic Components of Soft-tissue Tumors: Comparison of MEDIC and Fast SE T2-weighted Imaging)

  • 목적: 연조직종양의 조직 성분을 자기공명영상으로 평가하는데 MEDIC과 지방억제 T2 영상을 비교하여 더 나은 검사방법을 선택하고자 하였다. 대상 및 방법: 병리조직학적으로 진단된 연조직종양 10예 (신경집종 3예, 혈관종 2예, 지방종 1예, 혈관각화종 1예, 윤활막육종 1예, 지방육종 1예 그리고 악성섬유조직구종 1예)에서 25 조직 성분 (혈관 5예, 신경 4예, 섬유성 4예, 과세포성 4예, 출혈성괴사 2예, 낭성 2예, 지방성 2예, 점액버팀질 1예, 그리고 혈전 1예)을 선택하였다. 병리조직학적 조직 성분과 일치하는 동일 단면상을 가진 MEDIC과 지방억제 T2영상에 동일한 크기의 관심영역을 그려 불균질치를 얻었다. 불균질치는 영상의 불균질성을 나타내는 표준편차 값을 관심영역에서 얻은 신호강도 평균값으로 나눈 값으로 하였다. 결과: 25 조직 성분의 불균질치는 MEDIC 보다 지방억제 T2영상에서 높게 나타났다 (p < .001). 결론: MEDIC 영상은 불균질치가 현저히 낮으므로, 자기공명영상으로 연조직종양의 조직 성분을 평가하는데 지방억제 T2영상 보다 MEDIC 영상이 더 유용할 것으로 사료된다.

Damage of radioprotection and antitumor effects of water-soluble propolis

  • Some natural products are able to inhibit radiation effects and exert an antitumor effect with fewer adverse reactions; however, their antitumor effects are less than those of widely-used synthetic drugs. Propolis is a natural material that has been attracting attention, and we extracted this material with water and investigated the effect of continuous propolis administration on radioactivity-induced reduction of hemocytes, in addition to the antioxidant and antitumor effects of propolis. Following a 1-week adjustment period, water-soluble propolis was administered intraperitoneally to male ICR mice at a dose of 100 mg/kg every other day for 2 weeks. Following administration, 2 Gy whole-body irradiation was performed and the counts of leukocytes, lymphocytes, and granulocytes and monocytes in the peripheral blood were determined 1, 3, 7, 15 and 30 days after irradiation. These cells were considered since they are closely associated with immunity to radioactivity. In a second experiment, water-soluble propolis was similarly administered to the mice for 2 weeks after a 1-week adjustment period, and 2 Gy whole-body irradiation was performed. The antioxidant effects in hemocytes were then investigated using 2,2'-azobis (2-amidinopropane) dihydrochloride (AAPH), a radical generator. In a third experiment, $1\;{\times}\;10^6$ Sarcoma-180 cells were inoculated into the right thigh of mice, which were divided into four groups: control, water-soluble propolis-treated, 6 Gy irradiated and water-soluble propolis-treated + 6 Gy irradiated groups, and changes in tumor size were measured for 20 days. Statistical analysis was conducted using ANOVA for multiple groups. In the three experiments, administration of water-soluble propolis inhibited the reduction of hemocytes caused by whole-body irradiation, showed antioxidant effects against radioactivity, and inhibited tumor growth, respectively. In conclusion, our data suggest that the antioxidant effect of watersoluble propolis inhibits hemocyte reduction caused by whole-body irradiation and enhances immunological inhibition of tumor growth.

반하백출천마탕(半夏白朮天麻湯)과 반하백출천마탕가미방(半夏白朮天麻湯加味方)의 항암효과(抗癌效果)와 면역반응(免疫反應)에 관(關)한 실험적(實驗的) 연구(硏究) (Experimental studies on antitumor effects and immune responses of Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang)

  • In order to investigate the effects of Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang on antitumor effects after Sarcoma-180 cells transplantation into the peritoneal cavity or left groin in mice, and immune responses in mice induced by methotrexate, the extracts of its herbal medicines were orally administered for 14 or 21 days. Experimental studies were performed for measurance of mean survival days, tumor and body weight for antitumor effects, and delayed type hypersensitivity, hemagglutinin titer, hemolysin titer, rosette forming cells, natural killer cell activity and phagocytic activity for immune responses in ICR mice. Following results were obtained : 1. Mean survival time in Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang-treated group was significantly prolonged, as compared with the control group(P<0.010, P<0.005). 2. Tumor weight in Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang-treated group was significantly depressed, as compared with the control group(P<0.050, P<0.050). 3. Body weight in Banhabaekchulcheonmatang-treated group was significantly increased (P<0.050), but in Banhabackchulcheonmatanggamibang-treated group was slightly increased with no effectivenes, as compared with the control group. 4. Delayed type hypersensitivity in Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang-treated group was significantly increased, as compared with the control group(P<0.010, P<0.050). 5. Hemagglutinin titer in Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang-treated group was significantly increased, as compared with the control group(P<0.050, P<0.050). 6. Hemolysin titer in Banhabaekchulcheonmatanggamibang-treated group was significantly increased (P<0.050), but in Banhabaekchulcheonmatanggamibang-treated group was slightly increased with no effectiveness, as compared with the control group. 7. Rosette forming cells in Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang-treated group was slightly increased with no effectiveness, as compared with the control group. 8. Natural Killer cell activity in Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang-treated group was slightly increased with no effectiveness, as compared with the control group. 9. Phagocytic activity in Banhabaekchulcheonmatanggamibang-treated group group was significantly increased (P<0.050), but in Banhabaekchulcheonmatanggamibang-treated group was increased with no effectiveness, as compared with the control group. According to the above results, it could be suggested that Banhabaekchulcheonmatang and Banhabaekchulcheonmatanggamibang have prominent antitumor effects, and enhance both cellular and humoral immunity.

K-약침(藥鍼)의 급성(急性) 아급성(亞急性) 독성실험(毒性實驗) 및 항암효과(항암효과)에 관(關)한 실험적(實驗的) 연구(硏究) (The Study on Acute Subacute Toxicity and Anti-cancer Effect of K-herbal-acupuncture)

  • Objectives : The purpose of this study was to investigate Acute$\cdot$Subacute Toxicity and Anti-cancer Effect of K-Herbal-acupuncture in mice and rats. Methods : Balb/c mice were injected intraperitoneally with K-herbal-acupuncture for $LD_{50}$ and acute toxicity test. Sprague-Dawley rats were injected intraperitoneally with K-herbal-acupuncture for subacute toxicity test. K-Herbal-acupuncture was injected on abdomen of mice with S-180 cancer cell line. Result : 1. $LD_{50}$ of K-Herbal-acupuncture was limited $4{\times}10^{-3}$ml/kg~$2{\times}10^{-3}$ml/kg by the test. 2. In acute toxicity test, all of mice were down to the moving reflex, but the weight of mice was increased in treatment group, compared with the normal group. (P<0.05) 3. In acute toxicity test of serum biochemical values of mice, glucose was increased in treatment II group, total cholesterol was increased both treatments.(P<0.05) 4. In subacute toxicity test, the clinical signs of toxication was down to the moving reflex, but it is not severe like acute toxicity test, and observed weight loss at the treatments. 5. In subacute toxicity test, liver weight was decreased compared with the normal group. (P<0.05) 6. In subacute toxicity test of complete blood count test (CBC) of rat, HCT was decreased in treatments, compared with the normal group.(P<0.05) 7. In subacute toxicity test of serum biochemical values of rat, uric acid and triglyceride were decreased, and glucose was increased in treatment groups compared with the control group. (P<0.05) 8. Median survival time was increased about $45\%$ in treatment groups compared with the control group.(P<0.05) 9. Natural killer cell activity was increased in B16F10 lung cancer model, but it was not in sarcoma-180 abdomen cancer. 10. In interleukin-2 productivity test, treatment groups didn't show significant change in lung cancer and abdomen cancer, compared with the normal group.(P<0.005) 11. In making an examination of metastatic cancer with the naked eye, melanoma metastasized in the Lung of C57BL/6 mice. The treated group showed more Melanoma than the control in the numbers and volume. Conclusion : According to the result, K-herbal-acupuncture need further study to know the function and effect in cancer.

분할방사선조사와 SR 4233 병용에 의한 저산소분압 종양의 반응증강 (Hypoxic Tumor Can be More Responsive to Fractionated Irradiation Combined with SR 4233 (Tirapazamine))

  • 마우스 종양인 SCCVll과 RIF-1에서 선택적 저산소세포 치사제인 SR 4233 (tirapazamine)과 분할 방사선조사를 병용하면 저산소분획이 큰 종양에서 방사선 감수성이 더욱 증강될 것인가를 확인하기 위하여 본 연구를 실시하였다. 종양세포 이식이전에 방사선조사를 받은 정상조직에서 성장한 종양의 저산소세포 분획이 크다는 사실을 이용하여 저산소세포분획이 큰 종양 모델을 수립 사용하였다. 일정 용적의(100$mm^{3}$) 종양에 대하여 1회 2 Gy를 1일 2회씩 6회 또는 1회 2.5 Gy를 1일 2회씩 8회 조사하는 등의 분할조사법을 사용하여 방사선조사를 시행하였다. SR 4233를 병용한 경우에는 매회 방사선조사 30분 전에 0.08 mmol/kg를 복강내 주사하였다. 정상조직에서 성장한 종양에 비하여 방사선 조사를 받은 조직에서는 성장한 종양은 분할 방사선조사에 대하여 약 5배의 내성을 보였으나 SR 4233 분할 치료에 대하여는 약 5배의 감수성을 나타내었다. 정상 종양 및 저산소세포 분획이 큰 종양의 방사선 감수성은 공통적으로 SR 4233의 병용후 증강되었으며 저산소세포 분획이 큰 종양에서의 증강율이 정상종양에서의 증강율과 동일하거나 더욱 높았다. 따라서 선택적 저산소세포 치사제인 SR 4233과 분할 방사선조사를 병용할 경우 저산소분획이 큰 종양에서 치사효과가 더욱 증강된 사실을 확인하였다. 이러한 결과는 높은 저산소세포 분획에 의한 방사선내성을 보이는 종양중 일부는 저산소세포치사제와 방사선치료를 병용함으로써 국소적 종양관해 실패의 요인인 저산소 세포를 극복하는 임상 적용 가능성을 시사한다.

원발성 종격동 종양의 임상적 고찰 (Clinical Analysis of Primary Mediastinal Tumors)

  • 본 인제대학교 의과대학 서을 백병원 흉부외과학교실에서는 1987년 9월 부터 1995년 12월까지 원발성 종격동 종양의 진단하에 수술을 시행한 40례를 대상으로 하여 임상적 고찰을 하였다. 관찰 대상은 남자 18례와 여자 22례로 연령은 Vll에서 68세까지 였으며 평균 )4.1세였다. 종양의 진단시 증상은 흉통(12.5%), 기침(12.5%), 호흡곤란(7.5%) 경부종괴(7.5%), 흉부불쾌감(5.0%) 등이었다. 진단은 모든 예에서 단순 흉부엑스선 사진과 흉부 전산화단층촬영을 실시하였고 5례 에서는 자기공명영상을 시행하였다. 경피적 침생검은 22례에서 시행하여 16례에서 조직학적 진단을 얻었다(민감도 72.7%). 종양의 위치별 분포는 전상부 종격동 24례(60.0%), 후부 종격동 14례(35.0%), 중부 종격동 2례(5.0%)였다. 종양의 종류는 흉선종 11례(27.5%), 신경성종양 10례(25.0%), 배아세포종 7례(17.5%), 낭종 8례(20.0%), 거대 림프 절비대(Castleman's disease) 2례(5.0%), 방추세포육종 1례(2.5%) 그리고 림프종 1례(2.5%), 였다. 악성 종양은 5례로 침습성 흉선종 3례, 방추세포육종 1례, 림프종 1례였다. 치료는 양성의 모든 예와 악성종양 3례에서 완전절제하였고, 2례는 수술이 불가능하였다. 수술후 사망은 없었고 합병증은 창상피열, 성대마비, 어깨강직이 각각 1례씩 있었다.
