Effects of pyrimidine salvage inhibitors on uracil incorporation of Toxoplasma gondii (Toxoplasma gondii의 활성화된 uracil 도입 과정에 미치는 pyrimidine 대사 억제제의 영향)

  • Metabolic inhibitors which act in the process of pyrimidine salvage influenced on the uracil incorporation into nucleic acids of Toxoplasma. Inhibitors of dihydrofolate reductase, pyrimethamine and methotrexate, and inhibitors of thymidylate synthase, fluoro-uridine, fluoro·dUMP and fluoro-uracil, diminished isotopic uracil uptake in dose-dependent manners. Azauridine which suppresses do novo pyrimidine biosynthesis did not affect the salvage even in a relatively high dose. These results suggested that the activation of uracil salvage should be closely related with the function of TMP biosynthetic enzymes. The pattern of thymidine uptake had no differences between control HL-60 cells and Toxoplasma infected cells, which did not reject the specific proliferation of Texoplasma. It can be exploited to characterize the elects of various compounds related with the proliferation of Toxoplasma, especially its DNA synthesis.   • An optimization problem to obtain the optimal replacement interval considering the salvage values is studied. The system is minimally repaired at failure and is replaced by new one at age T(periodic replacement policy with minimal repair of Barlow and Hunter〔2〕). Our model assumes that the time horizon associated with the number of replacements is random The total expected cost considering the salvage values with random time horizon is obtained and the optimal replacement interval minimizing the cost is found by numerical methods. Comparisons between non-considered salvage values and this case are made by a numerical example.

  • On May 12th, 2007 a merchant vessel of Golden Rose (South Korea) sank into near 38 mile from Yentai in China after the vessel collided with a container vessel of Jinsung (China), leaving all 16 sailors, including seven South Koreans, missing. According to the official report. The Chinese vessel, authorities, and South Korean government did not coordination q rescue and salvage for the sailors and vessel properly as follows: 1) The Chinese vessel left the scene without rescue and salvage of the sailors, 2) Chinese authorities failed to comply with law of the sea that required them to report the collision to South Korea immediately after confirming the accident, and 3) The South Korean government is also being criticized for its slow response to the incident since the government did not set up a response team until 21 hours after the incident. In order to reduce this number of sailor missing in future incidents, this research is studied regarding assembling conferences, assigning SRR in ASEAN+3, simplicity to enter other territory waters and cooperative training and education for the SAR.

Clinical Results of the Limb Salvage Procedure in Bone Tumor (골종양 치료에 있어서 사지 구제술식의 임상적 결과)

  • During the last decade, the use of primary amputation has decreased, and a larger number of patients are being treated with resection and reconstruction with limb preservation. The advantage of chemotherapy, improvements in diagnostic imaging methods and newer techniques for reconstruction have made limb salvage an alternative to amputation for most patients with malignant bone tumor. Clinical results and functional results of the limb salvage operation of bone tumors treated at the Department of Orthopaedic Surgery, Guro Hospital, Korea university was reviewed. Eighteen bone tumors(seven giant cell tumors and eleven malignant bone tumors.) were studied over a period of five years. The limb salvage group included fourteen endoprothesis, two pasteurized autograft, one near total scapulectomy and one rotationplasty. Functional evaluation was performed according to the Enneking's modified system. The results were as followed; 1. At a mean follow-up of thirty two months (15-77 months), fourteen(83%) of the eighteen patients with limb salvage procedures had been continuously disease free. 2. There are no local recurrence but deep infection developed in two patients and three patients with distant metastasis. 3. Thirteen(87%) of the fifteen patients showed above 60% of functional results. 4. Fourteen(93%) of the fifteen patients were satisfied with the limb salvage procedures.

  • This study suggests the improvement of ROK Navy salvage and rescue diving system. ROK Navy system experiences characteristic restrictions for the environmental loading. These restrictions are known to deteriorate the efficiency of Navy salvage and rescue diving activity. In this study, the measurements were suggested to improve the efficiency. To achieve the goal, a comprehensive analysis is conducted using the published data including those of USN, NOAA and IANTD. Based on the analysis, suggestions were made. The technical diving techniques may be introduced to improve current ROK Navy diving system in limited areas. By adopting that technique, decompression procedures and underwater operation can be improved.

A Study on the Introduction and Application Plan of the Mobile Saturation Diving System for ROK Navy Salvage Operations (한국 해군의 해난구조작전을 위한 이동식 포화잠수체계 도입 및 활용방안에 관한 연구)

  • This study examined how to improve the Korean Navy diving system, particularly focused on using the mobile saturation diving system. This study determined the range of navy operations, and suggested requirements of mobile saturation diving system from the analysis of previous study and current Navy Diving system. Also, this study examined the use of military strategy in terms of its technical and applicable possibility for the enhancement of current system. In addition, the study analyzed both advantages/disadvantages of importing mobile saturation diving system products and domestic development. To review measures in installing and operating such system, this study included analysis on the effectiveness of individual platform. As the result, the study suggested requirements of mobile saturation diving system to be able to be operated by more than 6 divers for more than 17 days at more than 200m depth. And the study confirmed that there are beneficial to use mobile saturation diving system and current Navy saturation diving system together. However, it is low economical efficiency to change current Navy gas diving system to Saturation diving system. To review measures in installing and operating such system, this study suggested that second ATS-II should be built to be able to install mobile saturation diving system. Also, this study generated an utilization method of the system to use it for training when peacetime, and than it should be installed on other platform to use as additional salvage strength during wartime.

Perforator Reconstruction to Salvage the Jeopardized Flaps

  • During flap elevation, most perforators are cut except one or more perforators that are essential to flap survival. However these cutout perforators can cause deterioration of the blood circulation of the flap. To salvage the jeopardized flaps, rebuilding the perforator system is essential for flap survival. In the first case, after flap elevation, the upper abdominal flap margin was severely ischemic. To supply blood to the upper abdominal flaps, we found and used a major perforator underneath the upper abdominal flap which was cut earlier during the elevation, and we performed reanastomosis with ipsilateral deep inferior epigastric artery. Upper abdominal flap ischemic area was limited to a narrow suture area. In the second case, we performed free superficial inferior epigastric artery (SIEA) flap reconstruction. After successful anastomosis of the SIEA and superficial inferior epigastric vein (SIEV) with internal mammary artery and vein, serious venous congestion occurred immediately because of SIEV malfunction. We found the largest perforator vein under the flap, as an alternate way to drain, then connected it with the thoracoacromial vein with a vein graft harvested in the contralateral SIEV. Circulation has improved. In conclusion, perforator system reconstruction is essential in a jeopardized flap salvage.

Clinical and biochemical outcomes of men undergoing radical prostatectomy or radiation therapy for localized prostate cancer

  • Purpose: We analyzed outcomes of patients with prostate cancer undergoing either radical retropubic prostatectomy (RRP) +/- salvage radiation or definitive radiation therapy (RT) +/- androgen deprivation. Materials and Methods: From 2003-2010 there were 251 patients who underwent RRP and 469 patients who received RT (${\geq}7,560cGy$) for prostate cancer. Kaplan-Meier analysis was performed with the log-rank test to compare biochemical control (bCR), distant metastatic-free survival (DMPFS), and prostate cancer-specific survival (PCSS) between the two groups. Results: The median follow-up was 70 months and 61.3% of the men were African American. For low risk disease the 6-year bCR were 90.3% for RT and 85.6% for RRP (p = 0.23) and the 6-year post-salvage bCR were 90.3% vs. 90.9%, respectively (p = 0.84). For intermediate risk disease the 6-year bCR were 82.6% for RT and 59.7% for RRP (p < 0.001) and 82.6% vs. 74.0%, respectively, after including those salvaged with RT (p = 0.06). For high risk disease, the 6-year bCR were 67.4% for RT and 41.3% for RRP (p < 0.001) and after including those salvaged with RT was 67.4% vs. 43.1%, respectively (p < 0.001). However, there were no significant differences between the two groups in regards to DMPFS or PCSS. Conclusion: Treatment approaches utilizing RRP +/- salvage radiation or RT +/- androgen deprivation yielded equivalent DMPFS and PCSS outcomes. Biochemical control rates, using their respective definitions, appeared equivalent or better in those who received treatment with RT.

A Joint Optimization of Ordering and Replacement Policy Under Minimal Repair (최소수리가 가능한 시스템의 주문 및 교체정책 통합 최적화)

  • Maintaining a complex repairable system can be achieved by repairing, replacing, or any other activities. This paper proposes a joint optimization policy that is composed with ordering and replacing under minimal repair for the complex system. For this purpose, we derive the expected cost due to the minimal repair, ordering, downtime, inventory costs, and salvage value of units that follow generally distribution. Some properties about the optimum ordering policy that are suggested for our purpose shows that the optimum ordering policy minimizing the expected cost is either one of the two typical policies : (1) the original unit is replaced as soon as the ordered spare is delivered, or (2) the delivered spare is used as inventory part until the original unit fails.

Clinical Efficacy of Intra-Operative Cell Salvage System in Major Spinal Deformity Surgery

  • Objective : The purpose of this study was to determine the efficacy of intra-operative cell salvage system (ICS) to decrease the need for allogeneic transfusions in patients undergoing major spinal deformity surgeries. Methods : A total of 113 consecutive patients undergoing long level posterior spinal segmental instrumented fusion (${\geq}5$ levels) for spinal deformity correction were enrolled. Data including the osteotomy status, the number of fused segments, estimated blood loss, intra-operative transfusion amount by ICS (Cell $Saver^{(R)}$, $Haemonetics^{(C)}$, Baltimore, MA, USA) or allogeneic blood, postoperative transfusion amount, and operative time were collected and analyzed. Results : The number of patients was 81 in ICS group and 32 in non-ICS group. There were no significant differences in demographic data and comorbidities between the groups. Autotransfusion by ICS system was performed in 53 patients out of 81 in the ICS group (65.4%) and the amount of transfused blood by ICS was 226.7 mL in ICS group. The mean intra-operative allogeneic blood transfusion requirement was significantly lower in the ICS group than non-ICS group (2.0 vs. 2.9 units, p=0.033). The regression coefficient of ICS use was -1.036. Conclusion : ICS use could decrease the need for intra-operative allogeneic blood transfusion. Specifically, the use of ICS may reduce about one unit amount of allogeneic transfusion in major spinal deformity surgery.
