6개월간의 저염식이 여자 대학생의 짠맛에 대한 인지와 기호, 혈압 및 뇨 나트륨 배설량에 미친 영향 (Effect of a 6-month Low Sodium Diet on the Salt Taste Perception and Pleasantness, Blood Pressure and the Urinary Sodium Excretion in Female College Students)

  • 본 연구는 여자대학생을 대상으로 장기간의 자율적 저염식 실시가 짠 맛에 대한 인지와 기호에 영향을 미쳐 나트륨 섭취나 혈압에 변화를 가져 올 수 있는지를 조사하고자 실시되었다. 14명의 실험군 대상자들은 나트륨 관련 영양교육과 정보를 제공받아 저염식을 하도록 지시 받았고 10명의 대상자는 평상시의 식생활을 유지하도록 하였다. 짠 맛의 인지와 기호는 9점 척도의 관능검사로 평가하였고 뇨 나트륨 배설량과 최적 염미 나트륨 정량 분석하였으며 혈압을 측정하였다. 주요 연구 결과는 아래와 같다. 1) 연구 시작시의 실험군과 대조군은 나트륨 관련 식행동, 신체 계측치, 혈압, 24시간 회상법에 의한 나트륨 섭취량, 6단계의 염분용액에 대한 짠 맛 인지나 기호에서 차이가 없었다. 2) 6개월의 저염식 실시 후 실험군 대상자들은 0.5% 이하 염분 용액에서 짠 맛 인지도가 높아졌고 대조군은 낮아져 저염식 실시로 낮은 농도의 염분에 적응한 듯하나 연구시작과 종료시의 paired t-test에서 통계적 유의성은 없었다. 기호도는 시작 시에는 두 군 모두 0.5% 염분 용액을 가장 선호했고 연구 종료시 대조군은 0.75%와 0.5% 염분 용액에 비슷한 기호도를 보인 반면 실험군은 0.5%, 그 다음으로 0.3% 용액에 높은 기호도를 보였으나 그 변화가 유의적 수준은 아니었다. 3) 최적 염미 나트륨 농도는 실험군에서 105.6 mmol에서 저염식 적응 후 80.7 mmol로 유의적 (p = 0.015) 감소를 보였고 대조군은 변화가 없었다. 뇨 나트륨 배설량은 대조군은 연구 시작과 종료시 차이가 없었고 실험군은 연구 종료시 1,398 mg에서 906 mg으로 유의적 감소를 하였다. 4) 수축기 혈압은 실험군 대상자에서 111 mmHg에서 106 mmHg로 유의적으로 감소하였고 이완기 혈압은 대조군, 실험군 모두에서 유의적으로 감소하였다. 혈압과 뇨 나트륨 배설량은 상관관계를 나타내지 않았다. 5) 최적 염미 나트륨 농도와 뇨 나트륨 배설량은 양의 상관관계 (r = 0.418)을 보여 짠맛에 대한 기호가 나트륨 섭취량에 영향을 미친 것으로 보인다. 최적 염미 나트륨 농도와 뇨 나트륨 배설량은 0.5% 이하의 낮은 염분 용액에 대한 짠 맛의 인지와는 음의 상관관계 (r = -0.338~-0.399)를 보여 저염식을 하는 대상자는 낮은 염도에도 잘 반응함을 보여 주었다. 본 연구의 대상자들은 6개월 간의 실험 경과 후 최적 염미도와 뇨 중 나트륨 배설량에서는 대조군과 실험군의 차이가 분명하였고 관능검사 결과는 0.5% 이하의 저염 농도에 실험군 대상자들이 짠 맛 인지와 기호도가 유의적이지는 않으나 향상하였다. 혈압은 수축기 혈압이 실험군에서만 유의적으로 감소하여 전반적으로 6개월 간의 저염식 적응이 유효한 듯하다. 그러나 본 연구결과만으로는 저염식에 적응해 가는 변화과정을 알 수 없고 분석 자료의 표준편차로 보아 개인차 커 일부 비적응자가 있었음을 알 수 있다. 앞으로 저염 적응 기간 중 개인별 반응과 함께 여러 시점에서의 관찰이 필요하다고 본다.

Estimating the urinary sodium excretion in patients with chronic kidney disease is not useful in monitoring the effects of a low-salt diet

  • Background: Several epidemiologic studies have suggested that the urine sodium excretion (USE) can be estimated in lieu of performing 24-hour urine collection. However, this method has not been verified in patients with chronic kidney disease (CKD) or in an interventional study. The purpose of this study was to evaluate the usefulness of estimating USE in a prospective low-salt diet education cohort (ESPECIAL). Methods: A new formula was developed on the basis of morning fasting urine samples from 228 CKD patients in the ESPECIAL cohort. This formula was compared to the previous four formulas in the prediction of 24-hour USE after treatment with olmesartan and low-salt diet education. Results: Most previously reported formulas had low predictability of the measured USE based on the ESPECIAL cohort. Only the Tanaka formula showed a small but significant bias (9.8 mEq/day, P < 0.05) with a low correlation (r = 0.34). In contrast, a new formula showed improved bias (-0.1 mEq/day) and correlation (r = 0.569) at baseline. This formula demonstrated no significant bias (-1.2 mEq/day) with the same correlation (r = 0.571) after 8 weeks of treatment with olmesartan. Intensive low-salt diet education elicited a significant decrease in the measured USE. However, none of the formulas predicted this change in the measured urine sodium after diet adjustment. Conclusion: We developed a more reliable formula for estimating the USE in CKD patients. Although estimating USE is applicable in an interventional study, it may be unsuitable for estimating the change of individual sodium intake in a low-salt intervention study.

  • In 30 rats divided into salt, ethanol and salt plus ethanol groups, the effect of ethanol on the course of hypertension induction with the salt ingestion was studied. The results obtained from the present study are as follows. 1) In salt group mean arterial blood pressure elevated to plateau (about 140 mmHg) in two weeks and the increased blood pressure was well maintained throughout entire experimental period. 2) By four weeks after ethanol ingestion, mean arterial blood pressure of ethanol group was slightly decreased followed thereafter by slow restoration to control value. And it was believed that decline of blood pressure observed in this case probably was not resulted from cardiac depression. 3) As mean arterial pressure in salt plus ethanol group remained rather low compared with that of salt group, it was suggested that ethanol may have a dose reduction effect in the course of hypertension induction by excessive salt ingestion. It was, however, not possible from the result of present study to decide that low blood Pressure in this group was resulted whether from enhanced sodium excretion activity of ethanol or from effect on blood pressure of ethanol itself.

농촌 국민학교 아동의 혈압, 짠맛에 대한 역치, 최적 염미도, 뇨 중 배설성분 및 혈압에 관한 영양지식 (Blood Pressure, Salt Threshold, Salt Preference, Urinary Excretions and Nutrition Knowledge About Blood Pressure of Elementary School Children in Rural Area)

  • The purpose of this study was to observe the relationship of dietary factors to blood pressure in 5th and 6th grade school children. Salt threshold, salt preference and nutrition knowlege about blood pressure were tested. Twenty-four hour urines were collected for the measurements of the volume and concentrations of sodium, potassium, chloride, calcium, phosphorus, creatinine and urea nitrogen. 1) Mean systolic and diastolic blood pressure of elementary school children were 106.8/67.6mmHg in males and 108.7/69.5mmHg in females. Seven children(4%) of total subjects were found to be hypertensive. Their mean blood pressure was 130.0/86.4mmHg. 2) Urinary excretions of creatinine and urea nitrogen during twenty-four hours were 621.1mg and 1524mg, respectively. The mean daily urinary sodium excretion was 2940mg, which is equivalent to 7.37g NaCl. 3) The daily urinary excretions of other minerals were as follows; potassium was 1301mg, chloride, 4991mg, calcium, 55.7mg and phosphorus, 700.03mg. 4) Salt preference of hypertensive children90.473$\pm$0.275) was significantly higher than those of normotensive children(0.473$\pm$0.213%), but salt threshold was lower in hypertensives(0.105$\pm$0.04%) than normotensives(0.081$\pm$0.022%) (p<0.05). INdices for assessing obesity, such as body weight, triceps skinfold, weight for heigth and body surface area, were high in hypertensive children than normotensive children(p<0.05). But there was no significant difference in energy and nutrient intakes between two groups. 5) Various anthropometric measurements had positive correlations with blood pressure, but body muscle mass(%) had a negative correlation with blood pressure. And daily energy and nutrient intakes were not related to blood pressure. 6) Blood pressures-both systolic and diastolic-were significantly correlated with urinary excretions of potassium and chloride. Daily sodium excretion was related to systolic blood pressure, but no related to diastolci blood pressure. There was no relationship of salt threshold to salt preference, and neither threshold nor preference was related to blood pressure. Results of this study suggest that nutrition education program for children including the information about desirable food habits for the prevention of hypertension should be developed.

은침점전기자극이 $Na^+$, $K^+$ 이온과 $Ca^{2+}$ 이온변동에 미치는 효과 (Effects of Silver Spike Point Low Frequency Electrical Stimulation on Alteration of $Na^+$, $K^+$ and $Ca^{2+}$ Ions)

  • The present study examined that in vivo test is investigated in sham-operated(control group) and aldosterone-analogue deoxycorticosterone acetate (DOCA)-salt hypertensive animals(experimental group) and that the antihypertensive effect was induced by silver spike point(SSP) low frequency electrical stimulation at meridian points(CV-3, -4, Ki-12, SP-6, LR-3, BL-25, -28, -32, -52), specifically, such as diuretic action in 24 hour urine analysis from normal volunteer. The $Na^+$ and $Ca^{2+}$ ions were significantly increased in aldosterone-analogue DOCA-salt hypertensive rats than that in sham-operated rats. However, the $K^+$ ions were significantly decreased in aldosterone-analogue DOCA-salt hypertensive rats than that in sham-operated rats. The current of 1 Hz continue type of SSP low frequency electrical stimulation significantly increased in excretion of urine $Na^+$ and $K^+$ ions from normal volunteer. However, the excretion of $Ca^{2+}$ ion were significantly decreased by SSP electrical stimulation in volunteer. These results suggest that the development of aldosterone analogue-DOCA-salt hypertension is associated with changed $Na^+$, $K^+$ and $Ca^{2+}$ ions of urine. which directly affects blood pressure. Therefore, the hypertension is a risk factor on cardiovascular disease. Moreover, These results demonstrate that the SSP low frequency electrical stimulation, especially current of 1 Hz continue type, significantly regulates $Na^+$, $Ca^{2+}$ and $K^+$ ions from volunteer. Therefore, the SSP low frequency electrical stimulation is a good regulator through a diuretic action of aldosterone-induced hypertension.

임부의 염분 기호도와 식이섭취, 소디움 섭취 실태 (Salt Preference and Sodium Intake among Pregnant Women)

  • Purpose: This study was to estimate salt preference and sodium intake of pregnant women, and identify the relationship between salt preference and sodium intake. Methods: Research design was a cross sectional correlational survey with 197 pregnant women who visited outpatient clinics for antenatal care. The sodium intake levels were estimated by the amounts of sodium intake using the 24-hour recall method and sodium concentration in spot urine. The data were analyzed using descriptive statistics, t-test, ANOVA and Pearson's correlation. Results: Sodium intake using 24-hour recall method was $3,504{\pm}1,359mg$. Sodium intake levels had statistically significant differences depending on income. The average amount of sodium in spot urine was $2,882{\pm}878mg/day$. Sodium excretion levels had statistically significant differences depending on whether participants had preexisting hypertension in their family history and Body Mass Index (BMI) pre-conception. Salt preference was $62.61{\pm}20.96$ out of 180 points. Salt preference had significant differences depending on income, parity, gestational age, BMI pre-conception and showed negative correlation with sodium quantity in spot urine. Conclusion: Sodium intake in pregnant women recommended by World Health Organization recommended is 175%. Salt preference was not significantly different between sodium intake levels, however it was negatively correlated with sodium quantity in spot urine among pregnant women.

  • This study was conducted to examine the effects of Na, K intake and stress level on the blood pressure and urinary excretion of Na, K in the third grade lacto-ovo vegetable male high school students. Twenty-one lacto-ovo vegetarian male high school students were selected, and their physical state, stress level, dietary intake and urinary excretion of Na, K were measured followed by examining the relationship among these factors and blood pressure. The results of the study can be summarized as follows : 1. The average age of the subjects was 17.7 years, and the average score of BMI was 20.4. The average score of Rohrer and blood pressure were 119.1, 112.3/7.5mmHg, each. All of these were in a normal range. Daily calorie intake was 2676.8kcal and Na, K intakes were 152.76mEq, respectively. 2. 71.4% of subjects were susceptible to the disease associated with stress, although there was no significant difference among the salt level, stress and blood pressure. 3. There was significant difference between dietary Na intake ratio, urinary Na excretion and urinary K excretion rate (p<0.001). 4. There was significant difference between systolic blood pressure and BMI/Rohrer score(p<0.01).

Effect of sodium restriction on blood pressure of unstable or uncontrolled hypertensive patients in primary care

  • BACKGROUND/OBJECTIVES: The aims of the present study are: 1) to quantify sodium consumption of patients with unstable or uncontrolled hypertension, 2) to investigate if reduced sodium intake can lower BP in these patients, and 3), to assess the acceptability and feasibility of this approach. SUBJECTS/METHODS: This study included 25 adults (age: 50+ years) with frequently elevated BP or patients with uncontrolled, uncomplicated hypertension despite drug treatment in a general practice setting. BP and salt intake (24h urinary excretion and food records) were measured at baseline and after a sodium reduced diet. RESULTS: Mean (${\pm}SD$) systolic (SBP) over diastolic (DBP) blood pressure (mmHg) at baseline was $150.7({\pm}9.5)$/$84.149({\pm}5.6)$. Mean urinary sodium excretion was 146 mmol/24h. A reduction of 28 mmol sodium excretion decreased SBP/DBP to $135.5({\pm}13.0)$/$82.5({\pm}12.8)$ (P < 0.001). After one month of no dietary advice, only in 48%, SBP was still ${\leq}140mmHg$. CONCLUSION: Assessment of sodium intake using food records, 24h urine collections and probing questions to identify use of sodium containing supplements or drugs are essential for tailored advice targeted at sodium intake reduction. The results of the present study indicate that reduced sodium intake can lower BP after 4 weeks in unstable or uncontrolled hypertensive patients.

  • This study was conducted to find out the contents of salt in stored foods and urine of housewives. The contents of sodium in soy sauce, kochujang and kimchi was significantly higher in rural than urban area. The contents of potassium In soy sauce and kimchi was significantly higher in rural than urban area but that of soybean paste was significantly higher in urban than rural area. The level of NaCl in soy sauce, hochujang and kimchi was significantly higher in rural the urban area. The excretion amount of Na, K and NaCl in urine was significantly higher In rural than urban area. The between of blood pressure and the contents of Na in urine had a high correlation.

부산지역 산업장 근로자들의 식염 섭취량에 관한 조사 (A Study on the Salt Intake Amounts of Manufacturing Workers in Pusan Area)

  • The authors investigated estimated amounts of salt intake of workers in manufacturing industries who consisted of 1,506 males in 6 manufacturing fields and 476 females in 3 fields in Pusan area using filter paper method which was developed recently. The obtained results were as follows; 1. Mean excretion amounts of urinary electrolytes were Na 200.7 mEq/l, K 44.2 mEq/l and Cr 1.12g/l in male and Na 190.2 mEq/l, K 44.0 mEq/l and Cr 1.10g/1 in female. 2. Mean daily salt intake amounts were 19.4g in male and l5.3g in female. 3. By the fields of manufacturing industries in male, workers in metal product field had the highest salt intake as 20.0g, and the following were workers in food and tobacco field as 19.7g, workers in textile, wearing apparel and leather field as 19.6g in order. 4. In female, workers in textile, wearing apparel and leather field had the highest salt intake as l5.7g and the next were workers in chemical, coal and rubber products field as l5.0g and workers in fabricated metal products, machinery and equipments as l4.3g, respectively.
