규칙적인 신체활동 참여가 여성노인의 노화과련 호르몬 농도 및 심혈관질환 위험요소에 미치는 영향 (The Effects of regular physical activities on female senior adults aging related hormone levels and a risk factor in cardiovascular disease)

  • 본 연구는 12주간의 규칙적인 신체활동 참여가 여성노인의 노화관련 호르몬(성장호르몬, DHEA-S, 에스트로겐) 및 심혈관 위험요소(아디포넥틴, hs-CRP)에 어떠한 영향을 미치는지 알아보는데 그 목적이 있다. 연구대상자는 65세-75세 여성노인 20명(운동집단 10명, 비교집단 10명)을 대상으로 하였다. 주 3회(60분씩) 12주간 한국무용 프로그램을 실시하였고 집단내 차이검증은 paired t-test로, 집단별 항목들의 변화량 차이 검증은 independent t-test를 실시하였다(p<0.05). 본 연구의 결과 에스트로겐(p=0.025), 성장호르몬(p=0.009), DHEA-S(p=0.009), 아디포넥틴(p=0.014)은 운동집단에서 운동 12주 후 유의하게 증가하였고 hs-CRP(p=0.010)는 운동 12주 후 유의하게 감소하였다. 에스트로겐(p=0.006), DHEA-S(p=0.002), 아디포넥틴(p=0.008), hs-CRP(p=0.004)는 집단간 변화량의 차이도 나타났다.

와류격자법에 의한 정상유동중의 상반회신 프로펠러 성능해석 (Analysis of Contra-Rotating Propellers in Setady Flow by a Vortex Lattice Method)

  • In this paper a Vortex Lattice Method is used to predict the performances of a contra-Rotating Propeller. Greeley and Kerwin's(1982) wake model is adopted instead of the exact trailing vortex geometry. The interaction of the two propellers is treated by the sense that the induction of one propeller upon the other propeller is averaged in the circumferential direction . Two single propellers (DTRC 4119 & DTRC 4842) are chosen and compared with the experimental and other numerical results published. Then the computational results for three CRP's (4-0-4 CRP(DTRC 3686+DTRC 3687A) 4-0-5 CRP(DTRC 3686+DTRC 3849) & DTRC CRT(DTRC 5067+DTRC 5068) are compared with the experimental and numerical results published. The interaction of both propellers by the change of inflow velocity and circulation of each propeller is investigated.

실험적 지하철 소음에서의 지적활동이 혈액생리에 미치는 영향 (Effect of a Combinacon of Mental Activity with Experimental Subway Noise on Hematology)

  • 이 연구는 지하철 소음 하에서 지적활동이 혈액생리에 어떤 영향을 미치는지 규명하기 위해 계획되었다. 56명의 건강한 지원자를 대상으로 지하철 소음노출 하에서 지적활동(암산)을 50분간 수행한 후 60분간의 휴식을 취하게 하였다. 스트레스 노출 전(기준치), 노출 50분, 노출 종료 후 60분의 휴식 등의 세 시기 때 정맥혈액을 채취하여 CBC, prothrombin time (PT), activated partial thromboplastin time (aPTT), 적혈구침강률, 섬유소원 농도, D-dimer 농도, 고감도 C-반응단백(H-CRP) 농도를 각각 분석하였다. 총 백혈구 수, 호중구 수, 림프구 수는 스트레스 종료 후 60분때는 모두 기준치보다 유의하게 상승하였다. 적혈구 수는 스트레스 노출 50분 때 기준치에 비해 유의한 증가를 보인 반면, 단구 수는 스트레스 노출 50분과 종료 후 60분 때 모두 기준치보다 유의하게 저하되었다. 혈소판 수와 적혈구침강률은 스트레스 종료 후 60분 때 기준치보다 낮았다. aPTT는 스트레스 노출 50분과 종료 후 60분 때 기준치보다 유의하게 단축되었는데 비해, PT는 스트레스 종료 후 60분때만 유의하게 단축되었다 D-dimer와 H-CRP 농도 둘 다 스트레스 50분과 종료 후 60분 때 유의한 상승을 보였다. 이 연구의 결과들은 지하철 소음과 병행된 지적활동이 백혈구수의 증가, 혈액농축, PT 및 aPTT의 단축, ESR의 감소, D-dimer 및 H-CRP 생성의 증가를 일으키며 이는 좋지 않은 지하철 환경이 잠재적으로 염증반응 및 혈전형성 반응의 발생 가능성을 유도할 수도 있음을 시사하는 것이다.

홍역 환아에서 C-반응 단백(CRP)의 의의 (Clinical Significance of C-reactive Protein in Measles)

  • 목 적 : 홍역 환아에서 CRP 값에 따라 합병증을 포함한 임상 및 검사실 소견에서 차이가 있는가를 알아보고자 하였다. 방 법 : 홍역으로 입원한 환자 253명을 대상으로 CRP 음성군 134명(0.5 mg/dL 이하, 53.0%)과 6-19 mg/L 군 85명(33.6%), 20-49 mg/L 군 27명(10.7%), 50 mg/L 이상 군 7명(2.8%), 4군으로 나누었다. 각 군에 대하여 총 발열일, 입원일, 합병증 발생 빈도, 백혈구 수와 분획, 비정상 간효소치 비율을 비교하고 통계학적으로 분석하였다. 합병증의 발생은 8일 이상 입원한 경우로 하였다. 결 과 : 전체 홍역 환아의 평균 CRP 값은 $11.1{\pm}7.5mg/L$이었다. CRP 음성군과 5-19 mg/L 군, 20-49 mg/L 군과의 비교에서 총 발열일, 입원일, 2배 이상의 간효소 치 상승 및 합병증의 빈도에서 유의한 차이가 없었다. 한편 CRP 음성군과 50 mg/L 이상 군의 비교에서는 총 발열일($4.7{\pm}1.7$일 vs $7.2{\pm}3.9$일), 입원일(5.4일${\pm}$1.4일 vs 9.4${\pm}$4.7일), 백혈구 수($5,900{\pm}2,700/mm^3$ vs $12,700{\pm}6,700/mm^3$)와 합병증의 빈도(5.2% vs 42.9%)에서 50 mg/L 이상 군에서 유의한 증가가 관찰되었다. 또한 CRP가 증가함에 따라 총 발열일, 백혈구 수 및 합병증의 빈도가 증가하는 경향을 보였으며, 간 효소치의 상승과는 상관 관계가 없었다. 결 론 : 대부분의 홍역 환아에서 CRP는 음성 또는 경도의 양성반응이 관찰되었다. CRP가 50 mg/L 이상일 경우 합병증에 의한 총 발열일과 입원일이 증가하였으며 백혈구 수가 증가하는 경향을 보였으나 간 효소 치의 상승과는 관련이 없었다.

C-reactive Protein Concentration Is Associated With a Higher Risk of Mortality in a Rural Korean Population

  • Objectives: C-reactive protein (CRP), an inflammatory biomarker, has been widely used as a preclinical marker predictive of morbidity and mortality. Although many studies have reported a positive association between CRP and mortality, uncertainty still remains about this association in various populations, especially in rural Korea. Methods: A total of 23 233 middle-aged participants (8862 men and 14 371 women) who were free from cardiovascular disease, cancer, and acute inflammation (defined by a CRP level ${\geq}10mg/L$) were drawn from 11 rural communities in Korea between 2005 and 2011. Blood CRP concentration was analyzed as a categorical variable (low: 0.0-0.9 mg/L; intermediate: 1.0-3.0 mg/L; high: 3.1-9.9 mg/L) as well as a continuous variable. Each participant's vital status through December 2013 was confirmed by death statistics from the National Statistical Office. Cox proportional hazard models were used to assess the independent association between CRP and mortality after adjusting for other risk factors. Results: The total quantity of observed person-years was 57 975 for men and 95 146 for women, and the number of deaths was 649 among men and 367 among women. Compared to the low-CRP group, the adjusted hazard ratio for all-cause mortality of the intermediate group was 1.17 (95% confidence interval [CI], 0.98 to 1.40) for men and 1.27 (95% CI, 1.01 to 1.61) for women, and the corresponding values for the high-CRP group were 1.98 (95% CI, 1.61 to 2.42) for men and 1.41 (95% CI, 1.03 to 1.95) for women. Similar trends were found for CRP evaluated as a continuous variable and for cardiovascular mortality. Conclusions: Higher CRP concentrations were associated with higher mortality in a rural Korean population, and this association was more prominent in men than in women.

Prognostic Significance of C-reactive Protein in Urological Cancers: a Systematic Review and Meta-analysis

  • Background: C-reactive protein (CRP), considered as a prototypical inflammatory cytokine, has been proposed to be involved in tumor progression through inflammation. Recent studies have indicated CRP as a progostic predictor for urological cancers, but the results remain controversial. Materials and Methods: A systematic search of Medline, Scopus and the Cochrane Library was performed to identify eligible studies published between Jan 1, 2001 and Sep 1, 2013. Outcomes of interest were collected from studies comparing overall survival (OS), cancer-specific survival (CSS) and relapse-free survival (RFS) in patients with elevated CRP levels and those having lower levels. Studies were pooled, and combined hazard ratio (HR) of CRP with its 95% confidence interval (CI) for survival were used for the effect size estimate. Results: A total of 43 studies (7,490 patients) were included in this meta-analysis (25 for RCC, 10 for UC, and 8 for PC). Our pooled results showed that elevated serum CRP level was associated with poor OS (HR: 1.26, 95%CI: 1.22-1.30) and RFS (HR: 1.38 95%CI: 1.29-1.47), respectively. For CSS the pooled HR (HR: 1.33, 95%CI: 1.28-1.39) for higher CRP expression could strongly predict poorer survival in urological cancers. Simultaneously, elevated serum CRP was also significantly associated with poor prognosis in the subgroup analysis. Conclusions: Our pooled results demonstrate that a high serum level of CRP as an inflammation biomarker denotes a poor prognosis of patients with urological cancers. Further large prospective studies should be performed to confirm whether CRP, as a biomarker of inflammation, has a prognostic role in urological cancer progression.

  • Purpose: The purpose of this study was to identify predictors of obesity, serum lipids and CRP in Korean adults. The predictors of obesity and serum lipids were the subject's general characteristics, life style, eating habit and nutrients. The predictors of CRP were the subject's general characteristics, life style, eating habit, nutrients, obesity and serum lipids. Method: 115 subjects who had visited the health examination center at a hospital participated in the study. The data analysed with descriptive analysis, ANOVA, Chi-square test, Pearson correlation coefficient and multiple regression. Result: Sex, married, eating out ($4{\leq}$/w), eating out (2-3/w) and age ($61{\leq}$) were anticipated variable on BMI ($R^2$=0.488). Sex and overeating (2-3/w) were anticipated variable on body fat($R^2$=0.218). Drink (4-6/w), age (51-60), sex, vegetable fat and Systolic BP were anticipated variable on total cholesterol ($R^2$=0.217). Age (51-60), vegetable fat and unmarried were anticipated variable on LDL ($R^2$=0.180). Sex was anticipated variable on HDL and Triglyceride ($R^2$=0.054, 0.192). Breakfast (1-3/w) and meal (2/d) were anticipated variable on CRP ($R^2$=0.1268). Conclusion: It is thought that decreasing eating out and overeating might be important to prevent obesity. It is thought that decreasing drinking and fat eating might be important to improve serum lipids. It is thought that eating breakfast might be important to decrease CRP.

Factors Prognostic for Survival in Japanese Patients Treated with Sunitinib as First-line Therapy for Metastatic Clear Cell Renal Cell Cancer

  • Background: Factors predictive of survival have been identified in Western patients with metastatic clear cell renal cell carcinoma (mCCRCC) treated with sunitinib. Less is known, however, about factors predictive of survival in Japanese patients. This study evaluated factors prognostic of survival in Japanese patients with mCCRCC treated with first-line sunitinib. Materials and Methods: This retrospective study evaluated 46 consecutive Japanese mCCRCC patients treated with sunitinib as first line therapy. Clinical and biochemical markers associated with progression-free survival (PFS) were analyzed, with prognostic factors selected by uniand multivariate Cox regression analyses. Results: Univariate analysis showed that factors significantly associated with poor PFS included Memorial Sloan-Kettering Cancer Center poor risk scores, International Metastatic RCC Database Consortium poor risk and high (>0.5 mg/dl) serum C-reactive protein (CRP) concentrations (p<0.001 each). Multivariate analysis showed that high serum CRP was independently associated with poorer PFS (p=0.040). Six month disease control rate (complete response, partial response and stable disease) in response to sunitinib was significantly higher in patients with normal (${\leq}0.5mg/dl$) than elevated baseline CRP (p<0.001). Conclusions: CRP is a significant independent predictor of PFS for Japanese patients with mCCRCC treated with first-line sunitinib. Pretreatment CRP concentration may be a useful biomarker predicting response to sunitinib treatment.

Genotype CC of rs1800947 in the C-Reactive Protein Gene May Increase Susceptibility to Colorectal Cancer: a Meta-Analysis

  • Background: Single nucleotide polymorphisms of C-reactive protein (CRP) have been shown to be related to circulating CRP level, risk and prognosis in cancer patients. However, accumulating evidence of rs1800947 involvement in risk of cancer is inconsistent. Thus, a meta-analysis was performed to obtain a more precise relationship. Materials and Methods: The pooled odds ratio (OR) and its 95% confidence interval were assessed in 10 eligible articles with 12 studies containing 5,601 cancer cases and 8,669 cancer-free controls. Results: No significant association was observed overall and in subgroups in comparison of genotype GC vs GG ($P_H$=0.847, OR=0.939, 95%CI=0.810-1.087), GC/CC vs GG ($P_H$=0.941, OR=1.021, 95%CI=0.901-1.157) and allele C vs G ($P_H$=0.933, OR=1.026, 95%CI=0.909-1.159). However, statistically significance was evident in comparison of genotype CC vs GG in cancer risk ($P_H$=0.586, OR=2.854, 95%CI= 1.413-5.763), especially in colorectal cancer ($P_H$=0.481, OR=4.527, 95%CI= 1.664- 12.315). Conclusions: Genotype CC of rs1800947 in the CRP gene is strongly associated with increased cancer risk, particularly in colorectal cancer.

Study on the Contra-Rotating Propeller system design and full-scale performance prediction method

  • A ship's screw-propeller produces thrust by rotation and, at the same time, generates rotational flow behind the propeller. This rotational flow has no contribution to the generation of thrust, but instead produces energy loss. By recovering part of the lost energy in the rotational flow, therefore, it is possible to improve the propulsion efficiency. The contra-rotating propeller (CRP) system is the representing example of such devices. Unfortunately, however, neither a design method nor a full-scale performance prediction procedure for the CRP system has been well established yet. The authors have long performed studies on the CRP system, and some of the results from the authors' studies shall be presented and discussed.
