Opinion Survey of Health Center Officers on Rural Health Service Improvement Project (농어촌의료(農漁村醫療)서비스 개선사업(改善事業)에 대한 보건소(保健所) 공무원(公務員)의 인식도(認識度))

  • This study was performed investigate the opinion of civil servants in Health center about Rural Health Service Improvement Project The survey by mail was carried out for 447 servants of 25 health centers in Kyungsangbuk-do and the data were collected through self-administered questionnaires to servants about need, participation, concern, and comprehension for the project and satisfied with current facility and equipment of health center. The results were as follows. Generally considered, 48.2% of the improved health center servants was satisfied with health center building and 14.0% or 24.1% of the improving or unimproved center was. About the location of health center, 37.7% of the improved health center servants was satisfied, 25.9% of the unimproved center was. Of the improved health center servants, 43% was satisfied with the medical equipment but in unimproved place, the dissatisfaction was appeared higher than any other place. 49.7% of respondents was participated in making out the Rural Health Service Improvement Project. 50.6% was interested in this project. In the improved area. 65.5% of health center servants replied that the mayor's or county executive's concern about this project was high and 46.5% in councilors but in the unimproved area. their concern was low. About the contents of the project. 24,6% of the servants in the improved center, only 15.2% in unimproved center replied that they had known well. After making out the plan, 13.6% of respondents was unsatisfied with this plan and 17.1% replied that the estimating method of selecting the project area was not good. After the improvement of institution and equipment, 86.1% of health center servants answered that the medical service provided by health center would increase but 59.2% replied that the residents' utilization rate of private medical facility would decrease. The servants of the improved health center replied that the recognition about the developing will of health service(91.2%), the efficiency(91.2%), the quality of health and medical service(93.0%), the amount of health project(91.2%) were improved. In health center which had already improved the institution and equipment, 88.5% of servants replied that the residents' utilization for health center was increased. So, this project should be continuously carried out for health center and health center must develope new project to fit region condition.

Risk Factors for Falls Among Elderly People Living in the Rural Community

  • Falling is a serious problem associated with aging. Unintentional injury, which most often results from falling, is one of the leading causes of death in elderly people. The purpose of this study is to investigate the risk factors of falls and to compare characteristics of people who fall with that of non-fallers among the rural community-dwelling elderly of Korea. A sample of 201 people, living in the community, aged 60 years and over was taken from the members of a center for seniors located in Jecheon city. The mean age of the participants was 70.5 years of age. The participants are comprised of 151 women and 50 men. Eighty four of the 201 participants (41.8%) fell during the previous year. Twenty two of the fallers (26.2%) fell down more than two times. It was found that fallers had poorer eyesight, multiple chronic diseases and a more difficult time walking than non-fallers. In the logistic regression analysis of falls, only the difficulty of walking one kilometer (OR=2.4) and chronic diseases (OR=2.5) have shown an increased risk of falls. The risk of recurrent falls is, in addition, influenced by the difficulty of walking one kilometer. The result of our study shows that the impairment of mobility was the strongest risk factor of recurrent falling.

  • As a result of the execution of the import liberalization and local autonomy with WTO system launching, centralized rural extension service converted to localized rural extension service. This advent of globalization and localization era affects our over all agricultural condition, moreover the duty and role of rural extension organization which take complete of rural extension, is compelled to diversified and specified technology. In this study, first we will investigate the change of agricultural, rural community, and local rural extension service in globalization and localization era. Second we will analyze the confronted problems of local rural extension service. With this study's outcome for basis, we will present new role and direction of local rural extension service which can contribute to local agricultural development.

Plankton Community in Weir Section of the Nakdong River and Its Relation with Selected Environmental Factors (낙동강 보 구간의 플랑크톤 군집조성과 환경요인에 의한 영향 분석)

  • Phytoplankton and zooplankton communities were investigated from 8 weir sites of the Nakdong River system to provide basic information on plankton community after weir construction and to estimate its effects on major water quality parameters. The relationship between plankton community structure and environmental factors was analysed with CCA (Canonical Correspondence Analysis). The results suggested that discharge and total phosphorus and nitrogen concentrations are important factors determining the phytoplankton species composition. For zooplankton community, the difference in discharge between September and October induced different distribution pattern of zooplankton community with more homogeneous distribution with extreme dominance of rotifers during the high discharge season. Chlorophyll a concentration representing phytoplankton biomass has been suggested as the main environmental factor affecting zooplankton community followed by COD and total nitrogen concentration.

Characteristics of the Microbial Community Responding to the Vertical Distribution of TPH Concentrations in the Petroleum-Contaminated Site (유류오염지역 부지 내 TPH 수직 농도 분포에 따른 미생물 생태 특성)

  • In this study, the TPH(Total Petroleum Hydrocarbon) contamination and microbial ecological characteristics in petroleum-contaminated site were investigated through the correlation among the vertical TPH contamination distribution of the site, the geochemical characteristics, and the indigenous microbial ecology. The high TPH concentration showed in the vicinity of 3~4 m or less which is thought to be affected by vertical movement due to the impervious clay layer. In addition, the TPH concentration was found to have a positive correlation with Fe2+, TOC concentration, and the number of petroleum-degrading bacteria, and a negative correlation with the microbial community diversity. The microbial community according to the vertical distribution of TPH showed that Proteobacteria and Firmicutes at the phylum level were dominant in this study area as a whole, and they competed with each other. In particular, it was confirmed that the difference in the microbial community was different due to the difference in the degree of vertical TPH contamination. In addition, the genera Acidovorax, Leptolinea, Rugoshibacter, and Smithella appeared dominant in the samples in which TPH was detected, which is considered to be the microorganisms involved in the degradation of TPH in this study area. It is expected that this study can be used as an important data to understand the contamination characteristics and biogeochemical and microbial characteristics of these TPH-contaminated sites.

The Characteristics and its Change of Community Space in the Old Settlements of Cheongju City (도심 옛마을에 있어서 커뮤니티 공간의 특성 및 변화)

  • This study is to clarify the characteristics and its change of spatial elements for community focused on the existing 22 old settlements in urban area of Cheongju. These community spaces are mainly divided into three parts, that is central space, inner road, and blind alley(cul-de-sac). The transitional characteristics of them are as follows. Firstly, the characteristics of central spaces is lasting, but the function is influenced by changing times. The function of central spaces located at the entrance to the village had been reduced to adjustments to modern lifestyle, and currently changed into senior citizen community center, supermarket, and public area. Secondly, as the width of the existing inner roads passed through old settlements had been extended, they are changed into pedestrian and traffic road, but the shape and function of them have been maintained. When new roads passed through old settlements had been established, the shape of old inner roads is disappeared, and the function of them is changed into byway and alley. Thirdly, cul-de-sacs of old settlements have tended to create a sense of community, but new cul-de-sacs formed by lot division have been only changed to passage. When new roads are established and cul-de-sacs are changed into alleys, the community between individual households is lost.

Comparison of Medical Care Patterns of Hypertensive Patients between Rural and Urban Areas (도시와 농촌지역 고혈압 환자의 의료기관 이용 형태 비교)

  • Objectives: This study was conducted to compare the medical care patterns of hypertensive patients between rural and urban areas. Methods: We selected one rural county(Region A where there were 19 public health centers; one health center, 8 health sub-centers and 10 community health posts) and two urban districts(Region B and C where there was no health sub-center and community health post) in Daegu city. Region B had similar socioeconomic characteristics with rural county A while region C had different characteristics. The medical insurance records of 14,422 incident patients (2,501 in region A, 4,873 in region B and 7,048 in region C) with diagnostic code of hypertension from September 1998 to August 1999 were reviewed. Incident patient was defined as a patient who had no record of medical fee claim for hypertension to the national health insurance corporation in past 6 months and visited a medical facility for hypertension for the first time. The data for annual visit days, annual prescription days and annual total medical expenses were abstracted. The medical care pattern was categorized by the number of annual visit days and prescription days. The most proper care group was defined as the patient who visited 6-15 days with 240 prescription days or more in a year. Results: The type of medical facilities for the most visit was clinics, 373.% and it was followed by general hospitals, 28.2%; public health centers, 24.7%; and hospitals, 9.8% in region A(p<0.05). In region B, it was clinics, 63.1% and followed by general hospitals, 27.6%; health center, 5.2%; and hospitals, 4.1%(p<0.05). In region C, it was clinics, 53.8% and followed by general hospitals, 35.0%; health center, 6.3%; and hospitals, 4.9%(p<0.05). Annual mean total medical expenses per patient was highest in region C(won195,993) and followed by region A(won191,683) and region B(won178,713). The proportion of the most proper care group was 7.7% in region A, 5.2% in region B and 6.7% in region C(p<0.05). According to the type of medical facilities for the first visit, the proportion of the most proper care group was highest(14.7%) in the patients of public health centers, and it was followed by general hospitals, 8.8%; clinics, 3.6%; and hospitals, 2.0% in region A(p<0.05). In region B, it was highest in general hospitals, 9.7% and followed by hospitals, 4.0%; health center, 3.6%; and clinics, 3.4%(p<0.05). In region C, it was highest in general hospitals, 10.1% and followed by clinics, 5.2%; hospitals, 4.1%; and health center, 3.1%(p<0.05). Conclusions: The proportion of proper care for hypertension was higher in rural area and it was attributed to the care of health center, sub-centers and community health posts which appeared to follow patients better than hospitals and clinics.

  • This study was diagnosed the managing of educational progress of homehelper training center proceeded with a goal to improve the proper direction to grasp of the contentment of education programs. In order to attain a research objective, I surveyed 18 charge of education person in homehelper training center. Through the problem of the mananging of homehelper education, I try to show the point for growth of homehelper training. First, the course of the education and training for home helper should be classified in the field of the service activity for home helper. Second, the education centers need to establish the weekend lecture or the night lecture for volunteer with an occupation. Third, the education centers need to division the educational course according a characteristic of student. Fourth, when the course is opened, the number of persons is suitable thirty. Fifth, it needs to add the education contents to establish an attitude with volunteer for elderly people.
