A Comparative Study of Surgical Treatment in the Ruptured Achilles Tendon: Minimal incision and Open repair (아킬레스건 파열의 수술적 치료 비교: 최소 절개 봉합술과 관혈적 봉합술)

  • Purpose: The purpose of this study was to compare and analyse the clinical outcomes of minimal incision repair and open repair in ruptured Achilles tendon. Materials and Methods: We retrospectively analyzed the outcomes of 10 patients with minimal incision repair (group 1) and 19 patients with open repair (group 2) from February 2007 to June 2011. The postoperative clinical evaluations were done by Arner-Lindholm scale, AOFAS score, overall patient's satisfaction and cosmetic satisfaction of scar. Results: There was no statistical difference between two groups in Arner-Lindholm scale, AOFAS score, overall patient's satisfaction (p=1.21, 0.87, 1.07). There was statistically high rate of cosmetic satisfaction in group 1(p<0.001). There were no complications in group 1. Complications occurred in three patients (deep infection, rerupture, deep vein thrombosis) of group 2. Conclusion: Treatment of minimal incision repair in Achilles's tendon ruptures showed high rate of cosmetic satisfaction and low rate of complication's, but there were no significant differences with open repair in other clinical outcomes. The minimal incision repair could be recommended as one of the effective treatment for the Achilles's tendon ruptures.

  • Traumatic diaphragmatic rupture is an unusual finding that may occur after blunt trauma. In addition, diaphragmatic rupture occurring bilaterally is extremely rare. We experienced a 6-year-old boy with bilateral diaphragmatic rupture, whom survived after surgical treatment by open thoracotomy but, complicated with spinal cord injury discovered after surgery.

  • The injuries to the bronchi have been reported with increasing frequencies. The most common cause of such injuries is compression or crushing chest trauma. Early diagnosis and emergent repair should be done for the good prognosis. We report 3 cases who had bronchial injuries after traffic accident. Our operative procedures were a primary bronchial repair for 17 months old boy, a pneumonectomy for delayed recognition and a bronchoplasty procedure 63 days after trauma. All these bronchial ruptures were successfully treated and discharged.

  • The combined effect of increased pressure/temperature and the reduced material thicknesses act to increase the stress on the radiator componets. The design life of the radiator is influenced by the cyclic stresses and corrosion, which act to weaken the materials, radiator mechanical failure occurs when a tube or solder Joint ruptures, causing coolant loss or insufficient heat rejection. Therefore, in this study, through strain measurement of the tubes in copper/brass radiator, the strain distribution of the tubes in radiator as function of temperature and pressure is obtained.

  • Massive gastric mucosal ruptures during upper gastrointestinal endoscopy in patients with esophageal hiatal hernias are rare. However, tearing of the gastric mucosa alone, without tearing of the gastroesophageal junction, is even rarer. This study reports a case of a large mucosal rupture that occurred on the posterior wall of the upper body of the stomach during upper gastrointestinal endoscopy in an 83-year-old woman with an esophageal hiatal hernia while sedated. The patient was treated with endoclips for the rupture.

  • In this paper, an experimental study have been many studies on the joints of steel structure, for it has great influences on the safety of structures. Research on block shear rupture of the joint receiving pure tension have been done in foreign countries, but not in Korea. This study focuses on the propriety of block shear design code, according to limited state design criteria of steel structures recently established in Korea, by an experiment on the joint of angle tension members. The methods of this study were to compare other study results on block shear rupture mode and ultimate capacity, and to evaluate the propriety of the criteria design code. The result is that tension yield shear ruptures and shear yield tension ruptures happened at the joint, and the experimental rupture load was 15% higher than the capacity entered in the criteria design code. We conclude that it is necessary to revaluate the block shear design code presented by many studies on the limited state design criteria of steel structures.

  • In this paper, an experimental study have been many studies on the propriety of block shear design code. according to limited state design criteria of steel structures recently established in Korea, by an experiment on the joint of H-Beam Flange tension members. The objects of this study were to compare with the results of other studies on block shear rupture mode and ultimate capacity, and to evaluate the propriety of the design criteria. The result is that the joint happened, two types, tension yield-shear ruptures and shear yield-tension ruptures, and the experimental rupture load was 23% higher than the capacity entered in the criteria design code In this criteria, it was found that ultimate strength of block shear of H-Beam Flange was lowly estimated. Therefore, we emphasize the need of estimates on the block shear rupture by carrying out many studies in this field.

Surgical Treatment of the Ruptured Achilles Tendon: A Comparative Study between Percutaneous and Open Repair (급성 아킬레스건 파열의 수술적 치료: 경피적 봉합술과 관혈적 봉합술의 비교)

  • Purpose: The purpose of the present study was to compare and analyze the clinical outcomes of the percutaneous and open repair of acute Achilles tendon ruptures. Materials and Methods: We performed a retrospective study on 24 patients (group 1) managed with percutaneous repair, and 21 patients (group 2) managed with open repair for acute Achilles tendon rupture. The postoperative evaluations were done by an Arner-Lindholm scale and AOFAS score. Postoperative overall satisfaction and cosmetic satisfaction were also evaluated. Results: By Arner-Lindholm scale and AOFAS score, there was no difference between two groups (p<0.05). As for postoperative overall satisfaction, 5 cases were very satisfied, 16 cases were satisfied and 3 cases were fair in group 1. In group 2, 12 cases were very satisfied, 9 cases were satisfied. For postoperative cosmetic satisfaction, 13 cases were satisfied, 11 cases were fair in group 1. In group 2, 9 cases were very satisfied, 12 cases satisfied. In open repair group, a case of deep wound infection and three cases of skin necrosis were reported as complication. 2 cases of sural nerve injury were seen in percutaneous repair group and were recovered within 3 months. Conclusion: Percutaneous repair of acute Achilles tendon ruptures have high level of cosmetic satisfaction compared with open repair without any significant difference in clinical outcomes.

  • Complete ruptures of the Achilles tendon were repaired in two hunting dogs of a 3-year-old, 20 kg and a 4-year-old, 14 kg with non weight bearing lameness. Both dogs were injured by wild animals and the skin defect was showed on the caudal surface of the right limb. Surgical repair involved the attachment of the ruptured tendon with a three-loop pulley method, using nonabsorbable suture materials. After surgical procedure, a bivalved cast was applied and maintained for 6 weeks. Exercise was restricted for 2 weeks more before gradually returned to normal. Gait and standing posture of dogs were returned to normal during the follow-up of 10 weeks.
