Effect of maxillary sinus graft on the survival of endosseous implants: A 10-year retrospective study

  • Purpose: The aim of this study was to determine the survival rates of implants placed in grafted maxillary sinuses and compare the results obtained with graft materials, implant surfaces and timing of implant placement. Materials and Methods: Between January 1996 and December 2005, 391 implants were placed in 161 patients who underwent sinus grafting treatment simultaneously or separately at Ewha Womans University Hospital. According to inclusion criteria, 272 implants were placed in 102 patients with 112 sinus grafts (30 females, 72 males), aged 26 to 88 years (mean age $49.0{\pm}9.7$). The follow-up period ranged from 12 to 134 months (mean F/U $47{\pm}32$). Survival rates were evaluated according to graft material, implant surface and timing of implant placement. The Kaplan-Meier procedure and the log rank (Mantel-Cox) test were used to estimate survival rates and test for equality of survival rates between different groups of patients. Results: Ten-year cumulative survival rate for implants placed in the grafted sinuses was 90.1%. The survival rates for autogenous bone, combination and bone substitutes were 94.6%, 85.9% and 100%, respectively (p > 0.05). According to implant surface, survival rates were 84.8% in machined group and 97.5% in rough group (p < 0.05). The survival rates were 92.9% in delayed group and 86.0% in simultaneous group (p > 0.05). Conclusion: Ten-year cumulative survival rate for implants placed in the grafted sinuses was 90.1%. Rough-surfaced implants have a higher survival rate than machined-surfaced implants when placed in grafted sinuses (p < 0.05).

  • The effects of pretreatment of Co-Cr alloy, including two adhesive primers that contain either MDP or MAC-10, and silicoating on the bond The result sobtained as follows; o Strength of 4-META/MMA-TBB resin were investigated using FT-IR, SEM, and EDAX. o In the SEM observation of surface morphologies, the sandblasted specimen exibited a very rough surface, whereas the surfaces of the two groups primed with either MDP or MAC-10 were covered with a layer of primer, and the surface morphology of the silicoated specimen remained almost the same after sandblasting. o Before the thermocycling tests, the group treated with MDP demonstrated the highest mean tensile bond strength and the sandblasted group showed the lowest bond strength. o After 20,000 thermocyling, the mean tensile bond strength of the sandblasted group exhibited a 50% reduction in bond strength, while the others showed a $20\sim30%$ reduction. o Observation of the metal-resin interface revealed that in all groups the resin permeated the rough surface formed by sandblasting thereby producing a mechanical bond between the metal and the resin. It was also found that thermocycling resulted in a gap formation at the metal-resin interface of the specimens, and the sandblasted group exhibited a larger gap width than the other groups. o In fracture mode, all specimens indicated a cohesive fracture within the resin before thermocycling. However, thermocyling produced adhesive failure at the edge of the resin-metal interface in most specimens. The sandblasted group, which exhibited the lowest bond strength after thormocycling, also demonstrated the largest area of adhesive failure.

Roughness Effect on the Residual Shear Characteristics of Jumunjin Sands (거칠기 효과를 고려한 주문진 표준사의 잔류전단강도 특성 분석)

  • Residual shear strength is an important parameter in landslide dynamics and may be considered the critical factor in landslide triggering. Tests were undertaken using Jumunjin sands to examine the effects of smooth and rough surfaces on ring-shear characteristics. Under dense and drained conditions, shear velocities were recorded as 0.01, 0.1, 1, 10, 50, 100 mm s-1, with shear strength increasing with velocity and producing increasingly fine content. Particle fragmentation may thus increase landslide mobilization when the landslide body is mixed with ambient water in channelized flows.

Composition and Surface Analyses of RGP Contact Lenses (RGP 콘택트렌즈의 성분과 표면 분석)

  • Purpose: The surfaces and compositions of rigid gas permeable (RGP) contact lenses were analyzed with the consistent methods, and the basic informations for the composition design of lens materials were suggested. Methods: The bulk structures were analyzed by using Fourier infrared spectroscopy (FTIR), the compositions of surface components were observed by using x-ray photoelectron spectroscopy (XPS), the surface morphology and roughness were observed using atomic force microscopy (AFM), and the wettabilities were estimated by the surface wetting angles. The relations and trends of those results were analyzed. Results: The high oxygen permeability RGP lenses showed the trend that the fluorine decreases and the silicon increases. As the silicon and fluorine contents increased, the carbon and oxygen contents of RGP lens materials decreased at a constant ratio. The decreasing ratio of the carbon contents was three times larger than the decreasing ratio of oxygen contents. The composition of the surface treated lens was far from these tendency line. When the silicon contents increased, the rough surface was formed with the cohered particles. When the fluorine contents increased, the rough surface was formed with the deep flaws. The surface roughness increased and then wettabilities decreased as the silicon and fluorine contents increased. For the surface roughness changes, the increasing ratio of the silicon contents was two times larger than the increasing ratio of fluorine contents. The surface of RGP lens materials appeared the hydrophobic character of which the wettabilities decreased when the roughnesses increased. Conclusions: The surfaces and compositions of RGP contact lenses were measured by the same methods. Those results and relationships were compared and analysed. It is considered that these research results will be applied with the basic data for the composition design of lens materials.

The Endotoxin Assay of Contaminated Titanium Implants following Various Techniques of Detoxification (오염된 임프란트 표면의 해독 방법에 따른 내독소 제거 효과에 관한 연구)

  • Peri-implantitis could be the result of biomechanical and occlusal overload as well as microbiologic invasion. The dental implant may be more susceptible to dental plaque than the natural tooth, as the predictability of a stable soft tissue attachment complex has not yet been confirmed. With the development of peri-implantitis, the implant surface would be exposed to the oral environment and becomes coated with bacteria. The objective of therapy for this condition is to regain integration of the implant with bone. Since fibroblast adherence to surfaces is impeded by endotoxin, it would seem that decontamination would be desirable to obtain maximum osseointegration. The purpose of this study was to determine whether various chemotherapeutic and mechanical treatments(distilled water, air-powder abrasive, hypersaturated citric acid, tetracycline) can detoxify contaminated titanium implant surface by means of kinetic LAL test. Experimental rough surface titanium disks were fabricated. All of them were divided into two groups(A.a group and P.g group) and each contaminated by A. actinomycetemcomitans and P. gingivalis suspension. Contaminated disks were treated with distilled water, air-powder abrasive, citric acid and tetracycline, and then all disks were placed into LPS-free water for elution. The results were as follows : 1. In A.a group, LPS elute level of all test groups were significantly lower than control group(p<0.05). 2. In A.a group, LPS elute level of test 2, test 3 and test4 groups were significantly lower than that of control group(p<0.05). But, among the test 2, test 3, test4 groups, the significant differences were not detected. 3. In P.g group, LPS elute level of test 2, test 3 and test 4 groups were lower than that of control group(p<0.05). But, among the test groups, the significant differences were not detected. From the result of this study, it would be concluded that air-powder abrasive, hypersaturated citric acid and tetracycline treatments may be effective at reducing endotoxin level on rough titanium implant surfaces, and can be clinically used. But the treatments in peri-implantitis differentially impact osseointegration making one method clinically superior. To gain this knowledges, further molecular biologic and histopathologic studies should be developed.

Surface changes of denture base resin according to two toothpastes and a kitchen detergent (일부 세치제와 주방세제 사용에 의한 의치상 레진의 표면변화)

  • Objectives : This study evaluated the changes in surface roughness of denture base resin according to the types of denture cleansers. Methods : A denture base resin(Vertex RS, Dentimax, Netherland) was used. Two toothpaste(Antiplaque, Bukwang, Korea; 2080, Aekyung, Korea) and a kitchen detergent(Trio, Aekyung, Korea) were used as a denture cleanser. The specimens were put on the V8 crossbrushing machine(Sabri enterprises, Downers grove, IL, USA) to reproduce toothbrushing and the toothbrushes were flat, round end and soft type. The surfaces of denture base resin specimens were observed by profilometer(SJ-400, MITUTOYO, Japan) and SEM(S-3000N, Hitachi Co., Ibaraki, Japan). Results : 1. According to the result of measuring surface roughness, there was statistically significant difference in Ra, Rq, and Rz(p<0.05). 2. As for Ra, Rq and Rz, Antiplaque toothpaste showed the highest roughness, and there was significant difference from other groups(p<0.01). 2080 toothpaste, Trio, and distilled water were classified as the same group. 3. According to the result of observation with the SEM, the surfaces of the Antiplaque toothpaste group after toothbrushing showed the greatest roughness, and the surfaces of 2080 toothpaste, Trio, and distilled water groups were rough in order. Trio and distilled water had the surfaces similar to those before toothbrushing. Conclusions : Studies to compare the efficacy of denture management methods and examine the effects of denture cleansers on denture materials will be helpful for dental hygienists and dentists providing patients with proper information and education. And it will be also useful for denture users' oral health.

Effect of Fluoride Treatment after Bleaching with Hydrogen Peroxide exposed to Plasma Arc (고농도 과산화수소와 플라즈마 아크를 이용한 미백 치료에 있어서 불소의 효과)

  • This study evaluated whether fluoride treatment can affect recovery of the irregularity of enamel surface after tooth whitening with a high concentration of hydrogen peroxide (HP) activated by plasma arc light. A total of 36 bovine teeth stained with coke were used in this experiment. The specimens were classified into following three groups (two different commercial plasma arc groups and a control group without light curing source): (1) 35% HP gel only, (2): 35% HP gel and Plasma arc A, and (3) 35% HP gel and Plasma arc B. To measure color changes and surface morphologies before and after the bleaching, colorimeter and scanning electron microscopy were used, respectively. When the specimens were bleached with hydrogen peroxide and plasma arc lights, the bleaching effect was greater than when only hydrogen peroxide gels were used (Kruskal-Wallis test, p<0.05). In addition, plasma arc B showed the more color changes than plasma arc A (Bonferroni post-hoc test, p<0.05). The surfaces of the teeth treated with fluoride gel after the whitening treatment came to be smooth. Therefore, the results of this study suggested that the fluoride application for patients who got tooth whitening therapy with a high concentration of hydrogen peroxide gels activated by plasma arc light will be effective to recover rough enamel surfaces.

Ultimate lateral capacity of two dimensional plane strain rectangular pile in clay

  • This paper presents a new numerical solution of the ultimate lateral capacity of rectangular piles in clay. The two-dimensional plane strain finite element was employed to determine the limit load of this problem. A rectangular pile is subjected to purely lateral loading along either its major or minor axes. Complete parametric studies were performed for two dimensionless variables including: (1) the aspect ratios of rectangular piles were studied in the full range from plates to square piles loaded along either their major or minor axes; and (2) the adhesion factors between the soil-pile interface were studied in the complete range from smooth surfaces to rough surfaces. It was found that the dimensionless load factor of rectangular piles showed a highly non-linear function with the aspect ratio of piles and a slightly non-linear function with the adhesion factor at the soil-pile interface. In addition, the dimensionless load factor of rectangular piles loaded along the major axis was significantly higher than that loaded along the minor axis until it converged to the same value at square piles. The solutions of finite element analyses were verified with the finite element limit analysis for selected cases. The empirical equation of the dimensionless load factor of rectangular piles was also proposed based on the data of finite element analysis. Because of the plane strain condition of the top view section, results can be only applied to the full-flow failure mechanism around the pile for the prediction of limiting pressure at the deeper length of a very long pile with full tension interface that does not allow any separation at soil-pile interfaces.

MECHANICAL PROPERTIES OF LASER WELDED Co-Cr ALLOY (레이저 융합된 Co-Cr 합금의 물리적 성질에 관한 연구)

  • Statement of problem : The joints of removable partial denture alloys have failed frequently after routine usage. Purpose : The purpose of this study was to evaluate the mechanical properties of the laser welded Co-Cr alloys. Material and method : For this study 20 Co-Cr specimens were casted and 10 of them were seperated on the middle area and laser welded with Alpha laser welding machine(Siro Lasertec, Pforzheim, Germany). Rest of them which were as cast, were used as a control group. For the section of the experimental specimens, wire cutting machine was used to make a even gap of the all specimens. Laser welding was done with manufacturer's instrunction and tested each specimen by Instron Machine. Tensile strength, 0.2% yield strength and % elongations were recorded. Fractured surfaces were investigated with SEM. Results : The results were as follows : 1. The tensile strength of the laser welded group(617.7MPa) was about 75% of the as cast group(820.4MPa). It had stastically significant differences(p<0.05). 2. The % elongation of the experimental group was 6.6 which was lower than the control group(14.3). 3. Fracture of the experimental group occured in the welded surface and showed many voids. In contrast, the fracture surface of the control group was showed rough surfaces without any voids. Conclusion : The tensile strengths of the as-cast joints were higher than those for the laser welded joints, and the % elongation of the experimental group was lower than the control group. Porosity was found in laser-welded joints.

Preparation and Release Characterization of Sodium Alginate Bead Containing Phytoncide Oil (편백정유를 함유한 알지네이트 비드의 제조 및 방출 특성)

  • High molecular weight sodium alginate (HMWSA)/low molecular weight sodium alginate (LMWSA) microcapsules containing phytoncide oil were prepared with different LMWSA contents. The effects of the stirring rate and ratio of HMWSA/LMWSA on the diameter and morphology of the phytoncide/alginate beads were investigated by optical microscopy and the release behaviors of phytoncide oil from the phytoncide/alginate beads were characterized by UV/Vis. spectrophotometry. The mean particle size of the phytoncide/alginate beads decreased with increasing stirring rate and concentration of the calcium chloride solution. The surface morphology of the phytoncide/alginate beads changed from smooth surfaces to skin-like rough surfaces with increasing LMWSA content. These results were due mainly to the increased hydrophilic groups at the bead surface, resulting in an increase in the release rate of phytoncide oil in the phytoncide/alginate beads.
