The Difference between Clinical Manifestations and Feeding or Delivery Methods in Healthy Full-term Neonates and Those with Nosocomial Rotaviral Infection (수유 및 분만방법에 따른 신생아실 로타바이러스 원내감염의 증상 차이)

  • Purpose : Rotavirus is the main cause of infantile diarrheal disease worldwide. The purpose of this study is to assess the difference between clinical manifestations and feeding or delivery methods in healthy full-term neonates and those with nosocomially-acquired rotaviral infection. Methods : The study was conducted on 348 babies who were delivered in Dongguk University Kyongju Hospital from Jan 1 to June 30 in 2001. Stool specimens were collected from clinically symptomatic neonates. We studied the rate of positive stool rotazyme ELISA tests and positive symptoms. We compared the frequency of clinical manifestations according to the feeding methods [breast feeding(BF) or formula feeding(FF)] and the delivery methods[normal vaginal delivery(NVD) or Cesarean section(C/sec)]. Results : Rotavirus was detected in 152 of 348 babies. The rate of positive rotazyme ELISA test was 43.7% in healthy full-term symptomatic neonates. The most common symptom of rotaviral enteritis was mild fever; the others were watery diarrhea, abdominal distension, vomiting, delayed weight gain, irritability, poor oral intake and dehydration. Compared with FF neonates, BF neonates have lower frequency of symptoms, especially watery diarrhea, delayed weight gain and poor oral intake. The symptomatic frequency was higher in FF and NVD neonates than BF and C/sec. Conclusion : The symptoms of rotaviral enteritis were less frequent in BF or C/sec delivered neonates. BF appeared to alleviate the rotaviral enteritis but further studies are needed. The cause of the lower frequency of symptoms in C/sec delivered neonates was unknown.

Occurrence Pattern of Intussusception according to the Introduction of Rotavirus Vaccine: An Observational Study at a University Hospital (로타바이러스 백신 도입에 따른 장겹침증 발생 경향: 한 대학병원 관찰 연구)

  • Purpose: Rotavirus vaccine (RV) was introduced in Korea since 2007, and intussusception (IS) remains an important safety concern. This study investigated the trend of IS occurrence related to RV as well as the temporal relevance between vaccination and IS in children. Methods: We collected data of the patient aged ${\leq}18$ years with IS admitted to Gachon University Gil Medical Center, 2003 to 2015. For the patients that have occurred since 2008, the immunization records of RV were collected. The proportion of cases <1 year was calculated by the year and the temporal relationship between vaccination and IS occurrence was analyzed. Results: A total of 696 IS cases were noted. The cases <1 year were 30.7% (214/696). Although the incidence of all IS has increased over the 13-year period (from 74.1 in 2003 to 89.5 in 2015, linear by linear association, P=0.003), the incidence of IS <1 year has not increased (from 56.9 in 2003 to 53.3 in 2015, P=0.910), and the proportion of cases <1 year has decreased (from 35.4 in 2003 to 18.8 in 2015, P=0.000). Of 128 cases <1 year since 2008, 53.9% received RV. In the vaccinated group, 10 cases of IS occurred within 30 days, and eight cases did within 31 to 60 days. Numbers of IS after first, second, and third dose were three, 10, and five cases, respectively. Conclusions: Occurrence of IS in children <1 year of age did not increase since the introduction of RV. Further monitoring is essential for evaluation of vaccine safety.

The Effects of Exopolysaccharide Produced by Streptococcus thermophilus BODY1 on Infection of Rotavirus in MA-104 Cell (Streptococcus thermophilus BODY1이 생성하는 Exopolysaccharide가 Rotavirus의 MA-104 세포감염에 미치는 영향)

  • This study was conducted to evaluate the inhibitory effects of exopolysaccharide(EPS) produced by Streptococcus thermophilus BODY1 on rotavirus(RV). EPS was isolated from a commercial lactic acid bacteria, Str. thermophilus BODY1. The results obtained were as follows : At 0.1% of EPS, inhibitory effects of EPS on the MA-104 cell using MTT assay were, $Wa\;51.58{\pm}8.08%,\;KU \;63.09{\pm}7.58%,\;S2\;51.23{\pm}5.43%,\;YO\; 51.45{\pm}5.67%,\;K-21\;52.84{\pm}5.49%,\;NCDV\;57.50{\pm}10.85%,\;UK\;51.64{\pm}4.74%,\;KK3\;54.53{\pm}8.44%,\;JBR\;58.67{\pm}7.51%,\;S97\;50.63{\pm}5.17%,\;OSU\;55.48{\pm}5.75%,\;and\;RRV\;54.36{\pm}8.72%$, respectively. At 0.1/128%, the effects were $Wa\;5.5{\pm}6.45%,\;KU\;10.33{\pm}8.39%,\;S2\;0.98{\pm}8.39%,\;YO\;4.25{\pm}2.86%,\;K-21\;4.25{\pm}6.60%,\;NCDV\;4.01{\pm}4.12%,\;UK\;6.55{\pm}7.09%,\;KK3\;5.19{\pm}4.86%,\;JBR\;11.11{\pm}8.11%,\;S97\;6.75{\pm}6.95%,\;OSU\;10.14{\pm}8.54%,\;and\;RRV\;3.66{\pm}8.57%$, respectively. These results indicate that EPS have inhibitory effects on various serotype and sources of RV from different animals.

  • Purpose Some patients with neonatal seizures show diffuse, symmetric diffusion-restricted lesions in the cerebral white matter. The aim of this study was to describe clinical and imaging findings of patients with neonatal seizures who had diffuse, symmetric diffusion-restricted lesions without any structural or metabolic etiology. Materials and Methods A total of 56 neonates aged less than 1 week underwent brain magnetic resonance imaging (MRI) for evaluation of seizures from November 2008 to February 2017. After excluding 43 patients, 13 patients showed diffuse white matter abnormality on diffusion-weighted imaging. Initial and follow-up clinical and MRI findings were analyzed retrospectively. Results All 13 patients were born at full term. Among the ten patients who underwent a stool test for viruses, six were positive for rotavirus and one for astrovirus. MRI revealed diffuse, symmetric diffusion-restricted lesions distributed along the cerebral white matter, thalami, and midbrain variably. Conclusion Diffuse, symmetric diffusion-restricted lesions involving the cerebral white matter can be seen in patients with neonatal seizures without any structural or metabolic etiology. Rotavirus is commonly but not exclusively detected in these patients. Nevertheless, viral infection-associated encephalopathy should be considered for patients with characteristic clinical and MRI findings.

High Incidence of Staphylococcus aureus and Norovirus Gastroenteritis in Infancy: A Single-Center, 1-Year Experience

  • Purpose: The etiology of acute gastroenteritis (AGE) has changed since the introduction of the rotavirus vaccination. The aim of this study was to clarify which common pathogens, both bacterial and viral, are currently causing AGE in infants. Methods: Infants with acute diarrhea were enrolled. We tested for 10 bacterial pathogens and five viral pathogens in stool specimens collected from infants with AGE. The clinical symptoms such as vomiting, mucoid or bloody diarrhea, dehydration, irritability, and poor oral intake were recorded, and laboratory data such as white blood cell count and C-reactive protein were collected. The clinical and laboratory data for the cases with bacterial pathogens and the cases with viral pathogens were compared. Results: Of 41 total infants, 21 (51.2%) were positive for at least one pathogen. Seventeen cases (41.5%) were positive for bacterial pathogens and seven cases (17.1%) were positive for viral pathogens. Staphylococcus aureus (13 cases, 31.7%) and Clostridium perfringens (four cases, 9.8%) were common bacterial pathogens. Norovirus (five cases, 12.2%) was the most common viral pathogen. Fever and respiratory symptoms were common in the isolated viral infection group (p=0.023 and 0.044, respectively), whereas other clinical and laboratory data were indistinguishable between the groups. Conclusion: In our study, S. aureus (41.5%) and norovirus (12.2%) were the most common bacterial and viral pathogens, respectively, among infants with AGE.

Clinical Features and Role of Viral Isolates from Stool Samples of Intussuception in Children

  • Purpose: To detect major acute gastroenteritis virus (rotavirus, norovirus, astrovirus, and enteric adenovirus) and non-enteric type of adenovirus (AdV) in the stools of intussusception patients and to investigate the clinical role of detected viruses. Methods: From March 2012 to February 2013, major acute gastroenteritis virus and non-enteric type of AdV were isolated from stool samples that collected from 44 patients treated for intussusception in Chungnam National University Hospital. Patients were divided according to age and isolated virus. Results: Virus was detected in 28 (63%) stool specimens. The virus detection rate was significantly lower in patients aged under 12 months (p = 0.04). Twenty-two patients (78.6%) had non-enteric adenovirus, 4 (14.3%) had norovirus, 1 (3.6%) had sapovirus, and 1 (3.6%) had astrovirus. AdV subgroup C (AdV 1, 2, 5, and 6) comprised the majority with 20 cases (90.9%). A monthly increment-and-decrement pattern of intussusception was similar to that of viral detection in the stool samples. Enema reductions were successful in 39 patients and surgical manual reductions were performed in 5 patients. Virus was detected in 24 patients (61.5%) of enema reduction group and 4 patients (80.0%) of surgical manual reduction group. All of the detected viruses were non-enteric adenovirus subgroup C (AdV 1, 5, and 6) in surgical reduction patients. Conclusions: The virus detection rate was high in the stools of intussusception patients. The pattern of seasonal intussusception occurrence rate was parallel with seasonal these viral detection rate in the stool samples. These findings suggest that viral infection plays an important role in the development of intussusception and further research is warranted.

Effect of Metronidazole in Infants with Bowel Habit Change: Irrelative to the Clostridium difficile Colonization

  • Purpose: Clinical symptoms associated with Clostridium difficile infection (CDI) can vary widely. Carrier state without apparent symptoms is relatively common during infancy. The objective of this study was to determine the association of C. difficile colonization with bowel habit change and the effect of C. difficile colonization treatment on restoration of normal bowel habit. Methods: Between 2006 and 2014, infants at 1 to 12 months of age with diarrhea for more than 2 weeks who did not improve with conservative care were recruited from Gachon University Gil Medical Center. Infants who were followed up for at least 7 days were included. The presence or absence of C. difficile colonization, effect of metronidazole, and other medical records were reviewed. To determine the association between CDI and bowel habit change, logistic regression analysis was used. Results: Of a total of 126 infants, 74 (58.7%) were male patients. Of the 126 patients, 27 (21.4%) had C. difficile colonization. Significant (p<0.05) risk factors for C. difficile colonization included artificial milk feeding (odds ratio [OR], 4.310; 95% confidence interval [CI], 1.564-11.878), prior rotavirus vaccination (OR, 4.322; 95% CI, 1.018-18.349), and antibiotic use (OR, 4.798; 95% CI, 1.430-16.101). There was improvement in bowel habit after metronidazole therapy (OR, 0.34; 95% CI, 0.15-0.79; p<0.05), regardless of the presence or absence of C. difficile colonization, Conclusion: There was no significant correlation between bowel habit change and C. difficile colonization during infancy. However, metronidazole can be used as an optional method to manage functional gastrointestinal disorders.

Seroepidemiological studies on virus-borne diseases of cattle in Kwangju and Chonnam area (광주(光州)·전남지역내(全南地域內) 소의 바이러스성 질병(疾病)에 관한 혈청학적(血淸學的) 연구(硏究))

  • Sera from 304 Holsteins or Korean native cattle were collected from slaughterhouse in Kwangju area to study the infection of major virus-borne diseases. Serum antibody titers against infectious bovine rhinotracheitis virus(IBRV), bovine viral diarrhea virus(BVDV), parainfluenza type-3 virus(PI-3V), bovine ephemeral fever virus(BEFV), bovine Ibaraki virus(BIV), bovine Akabane virus(BAKV), bovine rotavirus(BRV), bovine coronavirus(BCV) were measured by serum neutralization tests. Results which obtained were as follows. Sera from 280 cattle(92.1%) contained antibodies against BRV which rate was the highest among the 8 viruses, and serum antibodies against BCV, BVDV, BIV, BAKV, BEFV, IBRV and PI-3V were detected from 266(87.5%), 149(49%), 108(35.5%), 94(30.9%), 80(26.3%), 32(10.5%) and 24(7.9%) cattle, respectively. Prevalence of seropositives to BVDV, BIV, BAKV, BEFV were higher among Holsteins than among the Korean native cattle(P<0.05). Prevalence of antibody titers against BVDV, BIV and BEFV in Korean native cattle were higher among females than males, while more males contained antibodies to BAKV, IBRV and PI-3V than females in their blood(P<0.05). Seropositives to BVDV, BIV, BAKV, BEFV and IBRV in Holsteins were higher among females than males(P<0.05). In Korean native cattle, serum antibody titers against IBRV and PI-3V ranged from 1:2~1:32 and 1:2~1:64, respectively, while serum antibody titers against the rest 6 viruses ranged from $1:2{\sim}1:{\geq}256$. In Holsteins, serum antibody titers against IBRV and PI-3V ranged from 1:2~1:64 and 1:2~1:32, respectively, while serum antibody titers against the rest 6 viruses ranged from $1:2{\sim}1:{\geq}256$.

Detection of diarrheagenic pathogens from feces and incidence of diarrhea in Korean calves (국내 송아지 분변 내 설사병 병원체 검출과 설사 발병율 조사)

  • Diarrhea is reported as one of the most common diseases in calves. It is thought to be a major cause of productivity and economic loss to cattle producers. The aim of this study is to provide a better understanding of well-known diarrheagenic pathogens and incidence of diarrhea in Korean calves. In this study, the relationship of calf diarrhea and pathogens were investigated from calves under 60 days of age in five areas of Korea from April to July, 2016. Of examined fecal samples, 38.3% was positive for any pathogens, and Giardia was the most common pathogen (25.5%). The incidence of diarrhea was 31% from pathogen-negative fecal samples whereas 61.1% from pathogen-positive fecal samples, suggesting high correlation between pathogenic factor and diarrhea. In addition, 80% of E. coli (K99) positive calves showed diarrhea, suggesting E. coli (K99) could be highly pathogenic. The incidence of diarrhea and infection rate increased with age. Rotavirus was revealed as a major pathogen in calves under 20 days of age, and the infection rate of Giardia increased rapidly in calves 20 to 39 days of age. The information on interconnections between clinical diarrhea and pathogens would contribute to developing strategies for treatment of calf diarrhea.

Survey on Korean-native Calves Diseases and Mortality (한우 송아지의 질병발생과 폐사율 조사)

  • From September 1996 to September 1999, 419 Korean-native calves with diseases under 6-month old collected from Kyonggi, Chungcheong, Chonlla and Kyongsang were examined by clinical, microbiological, parasitic, hematologic and histopathological mean. Among them, 124 cases were tested about the neutralization antibodies against infectious bovine rhinotracheitis virus(IBRV), Parainfluenza-3 virus(PI-3V), bovine uiral diarrhea virus(BVDV), bovine ephemeral fever virus(BEFV). In calf diseases in the survey, enteric diseases(72.8%) were most frequently involved and the following orders were taken by respiratory(17.4%) and reproductive (5.0%) disorders. In the causative pathogens associated with calf diseases and motality, 48.4% was induced by bacteria origin and also 35.6% by viral agents. Calf mortality was up to 76.3% in the cae of bacterial diseases and 55.7% in viral diseases. In bacterial diseases, frequent disorders were composed of colibacillosis(52.7%), salmonellosis(13.8%), pasteurellosis(12.8%) and campylobacteriosis(3.9%) and their mortalities showed 73.8% in colibacillosis, 73.0% in pasteurellosis, 67.9% in salmonellosis and 50.0% in campylobacteriosis (50.0%). Among the outbreaks of viral diseases, there were BVD(22.8%), bovine rotavirus infection(20.8%), bovine coronavirus infection(16.8%), bovine respiratory syncytial virus infection(15.4%), IBR(15.4%). Akabane disease(4.7%) and Chuzan diseases(3.4%). Interesting results through this studies were obtained the first isolate to Chuzan virus and Ainovirus in Korea which could be promised the development for diagnostic method and vaccines soon. Calf mortality to Akabane and Chuzan diseases was 100%. Main parasitic diseases were occupied by coccidiosis and babesiosis and their mortality of babesiosis was 20.0%. Other diseases were abomasal impaction(6.7%) and toxicosis(4.5%). The mortality of abomasal impaction was 89.3%. In some causes with malformations(1.9%) were confirmed as anasarca, derodidymus, polymelia, humerus hypoplasia, and tracheal collapse. Calf diseases had mostly been occurred in one month old grout (52.5%) and its prevalence was 25.1% in two to three month old group and 22.4% in four to six month old group. In calf mortality by age, there were 37.9% in one month old group, 18.1% in two and three month old group, and 13.8% in four to six month old group, respectively. The older the age of calf, the less the prevalence of calf enteric diseases. Respiratory diseases in calves to be tested frequently occurred in one to two month old group (41.4%). In one month old calves, the prevalence of enteric disease was 80.0%(p<0.05) and that of reproductive and respiratory disease was 9.5% and 8.2%, respectively. In two month old and four to six month old, enteric disease was 65.7% and 63.8% and respiratory disease was 28.6% and 26.6%. Seasonal prevalence and mortality of Korean-native calf diseases were not a significant difference. Prevalence of calf diseases in summer(31.5%) frequently occurred to compare that in winter(20.3%). Abortion and malformation in calves frequently occurred in spring. Hematological values in 84 calves with clinical signs showed mild to marked leukocytosis. Also, there was slight increase in hematocrit, platelet, mean corpuscular volume and mean plasma volume, but all of those were included the higher level to normal ranges. Calves with respiratory signs showed slightly erythrocytosis. One hundred seventy three calves without clinical signs were not significant different to ill cases in hematological values, but number of platelets was in higher normal range. In 125 calves, 84.8% was taken the antibody to IBRV, but 72% with the antibody had recorded the titer level lower than log$_2$5. The neutralizing antibody levels of higher than $log_{2}5$ to PI-3V and BVD virus were 60.8% and 67.2% cases, respectively. There were the cases of 57.6% had the neutralizing antibody level lower than log$_2$5 to BEFV.
