Objectives: The objective of this study was held for prevention of coronary artery disease and improvement of health of local community residents by classifying the obesity types of their waist circumference and BMI, and appraising the coronary artery disease risk factors(CRF). Methods: We analyzed the data on the 1,914 adult cases (1,156 male and 758 female) during Nov, 2006 to Mar. 2007 on a general hospital in Daegu city, Korea. The obesity types in this research were classified into normal group, seeming obesity group, abdominal obesity group and obesity group. Also, CRF was classified by normal, mild, moderate, and severe, and each class was given the index from 0 to 3. The coronary artery disease risk factors index(CRFI) was defined as the sum of index, and we defined that if the sum is higher, higher chance of coronary artery disease risk exist. Results: According to the research, by the age group, normal group has higher percentage in age 30 to 40, and obesity group and abdominal obesity group has higher percentage in age 50 to 60. CRFI is increased by order of normal group, seeming obesity group, abdominal obesity group, and obesity group on both male and female groups, and male shows higher index than female on all four groups. CRFI will be influenced by order of obesity group, seeming obesity group, and abdominal obesity group on male, and obesity group, abdominal obesity group, and seeming obesity group on female according to the result of multiple regression between obesity type and CRFI. Conclusion: According to the result from our research, we have to have keen attention to not only seeming obesity group and obesity group, but also abdominal obesity group which has normal waist circumference influence to CRFI. So, those kinds of indexes have to be controled by controling their weight. Also, we believe that health behaviour can improve and CRF can be prevented by the early health care and early health education to those with no abnormal indication on clinical indicator but with abnormal BMI and waist circumference.
Background. Breast cancer is the most common form of cancer among Korean women. Only 14 % of urban women and 10% of rural women in Korea, however, participated in breast cancer screening behavior in 1998 (Korean Ministry of Health & Welfare, 1999). Purpose. The aim of this study was to evaluate the effect of community-based breast self-examination (BSE) education programs in Korea. Methods. First, breast cancer risk appraisals were done with 1,977 rural women. Of the 1,977 women, nearly 30% (n=494) had a higher or equal to borderline risk of developing breast cancer. This quasi-experimental study was conducted to target these women with a high or equal to borderline risk of breast cancer. The risk appraisal feedback and breast self-examination education were used as an intervention for breast cancer prevention and early detection. Results. After a 3-month follow-up, 30.5% of the women in the intervention group performed regular BSE compared to 10.2 % of women in the control group. The mean knowledge score related to breast cancer and BSE was significantly higher for the women in the intervention group than that in the control group.
Purpose: This study was undertaken to investigate bio-medical and behavioral risk factor of stroke according to the level of blood pressure among the prospective tele-health users. Method: A descriptive study was conducted using interviews with a structured questionnaire. Of the households in the village, 24 households were selected for inclusion in the study. Among all of the residents from these households, a total of 48 residents who completed all the blood tests as well as the questionnaire. Results: Among bio-medical risk factors including demographic characteristics, physiologic variables, and health history, only triglyceride(TG) was significantly different among normal, prehypertension, and hypertension groups(F=3.78, p<.05). However, regarding behavioral variables, those who were classified as prehypertension and hypertension group reported more frequent drinking, those who were in prehypertension group reported highest scores of stress and lowest frequency of exercise, and those who were in normal and hypertension group showed higher scores of interest on health and lower scores of perceived seriousness of disease and health knowledge than those who were in prehypertension group. Conclusion: Identification and implementation of modifiable risk factors of stroke according to the level of blood pressure are crucial for health care utilizing tele-medicine.
Purpose: The purpose of this study is to identify risk factors of accidental falls by age in the Korean elderly. Methods: The study subjects included 10,242 community-dwelling elders aged 65 or over. The data were analyzed by logistic regression analysis. Results: The risk factors of accidental falls in the younger old group (aged 65~74) include fear of falling (OR=16.28, 95% CI: 12.03~22.03), depression (OR=1.24, 95% CI: 1.06~1.44), and hearing impairment (OR=1.20, 95% CI: 1.01~1.43). The risk factors of accidental falls in the older old group (aged 75~84) include fear of falling (OR=11.54, 95% CI: 7.04~18.93), muscle strength (OR=1.36, 95% CI: 1.15~1.60), number of chronic disease (OR=1.65, 95% CI: 1.09~2.50), and presence of spouse (OR=1.27, 95% CI: 1.06~1.51). The risk factors of accidental falls in the oldest old group (aged 85 years old or older) include fear of falling (OR=8.05, 95% CI: 1.85~34.98) and exercise (OR=2.91, 95% CI: 1.36~6.24). Fear of falling is defined as a common factor in the all age groups. Conclusion: Different strategies should be developed for age groups to prevent elderly falls with understanding of contributing factors of falls in each group.
Objectives: This study was conducted to investigate the association between sarcopenia and sarcopenic obesity and cardiovascular disease risk in Korean postmenopausal women. Methods: We analyzed data of 2,019 postmenopausal women aged 50-64 years who participated in the Korea National Health and Nutrition Examination Survey in 2008-2011 and were free of cardiovascular disease history. Blood pressure, height, and weight were measured. We analyzed the serum concentrations of glucose, total cholesterol, high density lipoprotein cholesterol, low density lipoprotein cholesterol and triglyceride levels. Waist circumference was used to measure obesity. Appendicular skeletal muscle mass was measured by dual-energy X-ray absorptiometry. Sarcopenia was defined as the appendicular skeletal muscle mass/body weight<1 standard deviation below the gender-specific means for healthy young adults. The estimated 10-year risk of cardiovascular disease risk was calculated by Pooled Cohort Equation. Subjects were classified as non-sarcopenia, sarcopenia, or sarcopenic obesity based on status of waist circumference and appendicular skeletal muscle mass. Results: The prevalence of sarcopenia and sarcopenic obesity was 16.3% (n=317) and 18.3% (n=369), respectively. The 10-year risk of cardiovascular disease risk in the sarcopenic obesity group was higher ($3.82{\pm}0.22%$) than the normal group ($2.73{\pm}0.09%$) and sarcopenia group ($3.17{\pm}0.22%$) (p < 0.000). The odd ratios (ORs) for the ${\geq}7.5%$ 10-year risk of cardiovascular disease risk were significantly higher in the sarcopenic obesity group (OR 3.609, 95% CI: 2.030-6.417) compared to the sarcopenia group (OR 2.799, 95% CI: 1.463-5.352) (p for trend < 0.000) after adjusting for independent variables (i.e., exercise, period of menopausal, alcohol use disorders identification test (AUDIT) score, income, education level, calorie intake, %fat intake and hormonal replacement therapy). Conclusions: Sarcopenia and sarcopenic obesity appear to be associated with higher risk factors predicting the 10-year risks of cardiovascular disease risk in postmenopausal women. These findings imply that maintaining normal weight and muscle mass may be important for cardiovascular disease risk prevention in postmenopausal women.
The purposed of this study was to identify cardiocerebrovascular disease risk assessment and the factors that affect cardiocerebrovascular disease risk. The subjects of this study were 1340 middle age women worker in one shipyard. This study was base on worker health check up data. Data were analyzed using SPSS18.0 program. "healthy group" were 1078(80.4%), "low risk group" were 45(3.4%), "moderate risk group" were 150(11.2%), "high risk group" were 67(5.0%). Age, HDL, LDL, smoking, family history of stroke, hypertension were the factors that affected the risk cardiocerebrovascular disease. Objective : We aimed to identify clinico-radiological risk factors that may predict unfavorable neurological outcomes in traumatic brain injury (TBI), and to establish a guideline for patient selection in clinical trials that would improve neurological outcome during the early post TBI period. Methods : Initial clinico-radiological data of 115 TBI patients were collected prospectively. Regular neurological assessment after standard treatment divided the above patients into 2 groups after 6 months : the Favorable neurological outcome group (GOS : good & moderate disability, DRS : 0-6, LCFS : 8-10) and the Unfavorable group (GOS : severe disability-death, DRS : 7-29 and death, LCFS : 1-7 and death). Results : There was a higher incidence of age $\geq$35 years, low initial GCS score, at least unilateral pupil dilatation, and neurological deficit in the Unfavorable group. The presence of bilateral parenchymal lesions or lesions involving the midline structures in the initial brain CT was observed to be a radiological risk factor for unfavorable outcome. Multivariate analysis demonstrated that age and initial GCS score were independent risk factors. The majority of the Favorable group patients with at least one or more risk factors showed improvement of GCS scores within 2 months after TBI. Conclusion : Patients with the above mentioned clinico-radiological risk factors who received standard treatment, but did not demonstrate neurological improvement within 2 months after TBI were deemed at risk for unfavorable outcome. These patients may be eligible candidates for clinical trials that would improve functional outcome after TBI.
Purpose: The purpose of this study was to analyze clinical outcomes from cervical cancer and stratify patients into risk groups for prognostic factors for early-stage disease. Materials and Methods: We retrospectively reviewed patients with stage IB or IIA cervical cancer treated with adjuvant radiotherapy (RT) or concurrent chemoradiotherapy (CCRT) following primary surgery at Samsung Medical Center from 2001 to 2011. Adjuvant RT was added for patients with intermediate-risk factors, and adjuvant CCRT was performed on high-risk patients after surgery. Results: We reviewed 247 patients-149 in the high-risk group and 98 in intermediate-risk group. The median follow-up was 62 months. Loco-regional failure (LRF) alone occurred in 7 patients (2.8%), distant metastasis alone in 37 patients (15.0%) and LRF with DM in 4 patients (1.6%). The 5-year disease-free survival (DFS) and overall survival (OS) rates for both groups were 79.7% and 87.6%, respectively. In the high-risk group, the 5-year DFS and OS probabilities were 72.5% and 81.9%, respectively. Histologic type, pathologic tumor size, and the number of pelvic lymph node (PLN) metastasis were significant prognostic factors for DFS and OS. We suggest a scoring system (0-3) using these prognostic factors to predict poor prognosis in high-risk patients. Using this system, patients with higher scores have higher recurrence and lower survival rates. Conclusion: In the high-risk cervical-cancer group who received primary surgery and adjuvant CCRT, non-squamous type, large tumor size and the number of PLN metastasis were significant prognostic factors, and the number of these factors was associated with survival rates.
Background: Studies on the relationship between organic solvent exposure and chronic kidney disease (CKD) have presented inconsistent results. Definition of CKD has changed in 2012, and other cohort studies have been newly published. Therefore, this study aimed to newly confirm the relationship between organic solvent exposure and CKD through an updated meta-analysis including additional studies. Methods: This systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. The search was conducted on January 2, 2023 using Embase and MEDLINE databases. Case-control and cohort studies on the relationship between organic solvent exposure and CKD were included. Two authors independently reviewed full-text. Results: Of 5,109 studies identified, a total of 19 studies (control studies: 14 and cohort studies: 5) were finally included in our meta-analysis. The pooled risk of CKD in the organic solvent exposed group was 2.44 (1.72-3.47). The risk of a low-level exposure group was 1.07 (0.77-1.49). The total risk of a high-level exposure group was 2.44 (1.19-5.00). The risk of glomerulonephritis was 2.69 (1.18-6.11). The risk was 1.46 (1.29-1.64) for worsening of renal function. The pooled risk was 2.41 (1.57-3.70) in case-control studies and 2.51 (1.34-4.70) in cohort studies. The risk of subgroup classified as 'good' by the Newcastle Ottawa scale score was 1.93 (1.43-2.61). Conclusions: This study confirmed that the risk of CKD was significantly increased in workers exposed to mixed organic solvents. Further research is needed to determine the exact mechanisms and thresholds. Surveillance for kidney damage in the group exposed to high levels of organic solvents should be conducted.

Purpose: The purpose of this study was to determine the effects of different intensity of aerobic exercise for four weeks on cardiovascular risk factors, reactive oxygen, and antioxidant enzymes in old mice. Methods: Eighteen male C57BL/6 mice age 18 months were randomly classified into the control group (n= 6), the moderate intensity exercise group (n= 6), and the low intensity exercise group (n= 6). The training groups performed the aerobic exercise twice daily for 20 minutes, five days weekly for four weeks. Data were analyzed using descriptive statistics, analysis of variance (ANOVA), the Chi-square test, and the Tukey's test with the SPSSWIN 21.0 program. Results: In this study, among the risk factors of cardiovascular disease, blood sugar (BS) (p= .023) and total cholesterol (TC) (p= .001) were significantly different between the moderate intensity exercise group and the control group. Additionally, there were significant differences in the reactive oxygen malondialdehyde (MDA) (p= .001), the antioxidant enzymes superoxide dismutase (SOD) (p< .001) and glutathione reductase (GR) (p= .015) between the moderate intensity exercise group and the control group. Conclusion: This finding suggests that moderate intensity aerobic exercise promotes the activity of antioxidant enzymes and lowers cardiovascular risk factors in older mice.
