Proteinuria is an important risk factor for renal and cardiovascular disease. It is associated with a risk for glomerulonephritis, chronic kidney disease, and end-stage renal disease. Therefore, if persistent proteinuria is detected, kidney biopsy is considered to diagnose and treat the underlying disease. Recently, variants in the cubilin (CUBN) gene, which is associated with albuminuria, have been reported. This gene encodes cubilin, a membrane glycoprotein receptor expressed in the renal proximal tubules. Cubilin is a component of the megalin and cubilin-amnionless complex that mediates albumin reabsorption into the proximal tubules through endocytosis. A defect in cubilin leads to a reduction in albumin reuptake, resulting in albumin-dominant proteinuria. Although numerous controversies exist, several reports suggest that cubilin defects lead to proteinuria with a high portion of albuminuria but may not impair renal filtration function. If albuminuria due to reduced cubilin function is confirmed as a benign condition, we can consider using genetic studies to detect CUBN mutations in patients with proteinuria and they may not require any treatment or kidney biopsy. Here, we review recent papers on CUBN mutations and discuss the prognosis and management of individuals with this mutation.
Radiation proctitis is a common complication after radiotherapy for pelvic malignant tumors. This study was conducted to assess the efficacy of novel almagate enemas in hemorrhagic chronic radiation proctitis (CRP) and evaluate risk factors related to rectal deep ulcer or fistula secondary to CRP. All patients underwent a colonoscopy to confirm the diagnosis of CRP and symptoms were graded. Typical endoscopic and pathological images, risk factors, and quality of life were also recorded. A total of 59 patients were enrolled. Gynecological cancers composed 93.1% of the primary malignancies. Complete or obvious reduction of bleeding was observed in 90% (53/59) patients after almagate enema. The mean score of bleeding improved from 2.17 to 0.83 (P<0.001) after the enemas. The mean response time was 12 days. No adverse effects were found. Moreover, long-term successful rate in controlling bleeding was 69% and the quality of life was dramatically improved (P=0.001). The efficacy was equivalent to rectal sucralfate, but the almagate with its antacid properties acted more rapidly than sucralfate. Furthermore, we firstly found that moderate to severe anemia was the risk factor of CRP patients who developed rectal deep ulcer or fistulas (P= 0.015). We also found abnormal hyaline-like thick wall vessels, which revealed endarteritis obliterans and the fibrosis underlying this disease. These findings indicate that almagate enema is a novel effective, rapid and well-tolerated method for hemorrhagic CRP. Moderate to severe anemia is a risk factor for deep ulceration or fistula.
Abstract Infiltration of rainfall that may lead to reduce resistance force due to reduction of matric suction and to increase driving force due to increase of self weight makes the slope fail. There are many specifications to make slope stable based on factor of safety. Although result of slope stability analysis satisfy the specifications, slope failures triggered by rainfall are frequently occurred in reality because slope stability analysis cannot consider uncertainty of each soil properties. This is why conventional analysis has limitation and development of alternative method is needed. So it is suggested to adopt the reliability analysis rather than design based on factor of safety into designing safer structure. Through the evaluation of handicaps for the factor of safety based design, calculation of soil properties by site investigation, and reliability analysis considering distribution of each soil properties, distribution of failure probability in railway slope is obtained. Then, Risk assessment of slopes in Korean railway is executed from the results. Damage loss and incoming loss are considered as the loss. Using these results, it is possible to make proper countermeasure or efficient maintenance.
Objective: Insulin resistance (IR) is an established risk factor for colorectal cancer (CRC). Given that CRC and IR physiologically overlap and the calpain-10 gene (CAPN10) is a candidate for IR, we explored the association between CAPN10 and CRC risk. Methods: Blood samples of 400 case-control pairs were genotyped, and the lifestyle and dietary habits of these pairs were recorded and collected. Unconditional logistic regression (LR) was used to assess the effects of CAPN10 SNP43 and SNP19, and environmental factors. Both generalized multifactor dimensionality reduction (GMDR) and the classification and regression tree (CART) were used to test gene-environment interactions for CRC risk. Results: The GA+AA genotype of SNP43 and the Del/Ins+Ins/Ins genotype of SNP19 were marginally related to CRC risk (GA+AA: OR = 1.35, 95% CI = 0.92-1.99; Del/Ins+Ins/Ins: OR = 1.31, 95% CI = 0.84-2.04). Notably, a high-order interaction was consistently identified by GMDR and CART analyses. In GMDR, the four-factor interaction model of SNP43, SNP19, red meat consumption, and smoked meat consumption was the best model, with a maximum cross-validation consistency of 10/10 and testing balance accuracy of 0.61 (P < 0.01). In LR, subjects with high red and smoked meat consumption and two risk genotypes had a 6.17-fold CRC risk (95% CI = 2.44-15.6) relative to that of subjects with low red and smoked meat consumption and null risk genotypes. In CART, individuals with high smoked and red meat consumption, SNP19 Del/Ins+Ins/Ins, and SNP43 GA+AA had higher CRC risk (OR = 4.56, 95%CI = 1.94-10.75) than those with low smoked and red meat consumption. Conclusions: Though the single loci of CAPN10 SNP43 and SNP19 are not enough to significantly increase the CRC susceptibility, the combination of SNP43, SNP19, red meat consumption, and smoked meat consumption is associated with elevated risk.

Background: Breast cancer is the most common type of cancer in women throughout the world. However, in comparison with Western women, it presents relatively early in women of Asian ethnicity. Early menarche, late menopause, use of OCP's, family history of benign or malignant breast disease, exposure to radiation and BMI in the under-weight range are well known risk factors for the development of breast cancer in premenopausal women. Early detection with the use of breast self-examination (BSE) and breast cancer screening programs can lead to a reduction in the mortality rates due to breast cancer. The aim of our study was to assess the risk factors for breast cancer among young women and to emphasize the importance of early screening among them. Materials and Methods: We conducted a cross-sectional study among women aged 18 to 25 using a self-administered questionnaire. Data was collected over a period of 6 months from June to December, 2014. A total of 300 young women selected randomly from Dow Medical College and various departments of Karachi University successfully completed the survey. Results: Respondents were 18-25 years of age (mean age=21.5). Out of the 300 young females, 90 (30%) had at least one risk factor, 90 (30%) had two, 40 (13%) had three, 8 (2.7%) had four, 2 (0.7%) had five while one female was found to have six positive risk factors for breast cancer. Some 66 women (22%) experienced symptoms of breast cancer such as non-cyclical pain and lumps. While 222 women (74%) had never performed breast self-examination, 22 (7.3%) had had a breast examination done by a health professional while 32 (10.7%) had participated in breast screening programs. A total of 223 (74.3%) women considered breast cancer screening important for young women. Conclusions: The percentage of young women with risk factors for breast cancer was found to be alarmingly high. Therefore, screening for breast cancer should start at an early age especially in high risk groups. Awareness about breast self-examination should be emphasized. Moreover, screening programs should be started to ensure early detection and reduction of mortality rates caused by breast cancer also in young Pakistani females.
Although researches have highlighted the important role of enhanced farm biosecurity to reduce the severity and prevalence of diseases in livestock, to date there has been little study in Korea on farmers' adoption of biosecurity measures to control porcine reproductive and respiratory syndrome virus (PRRSV) infection. To mitigate the risk of PRRSV infection in pigs, the risk factors by which PRRSV is introduced in pig farms must be determined. The primary aim of this study was to investigate pig producers' perceptions about on-farm biosecurity practices. We also analyzed data obtained from a cross-sectional study on 196 farrow-to-finish farms conducted between March 2013 and February 2014 to identify risk factors for PRRSV infection at farm level. Standardized questionnaires with information about basic demographical data and management practices were collected in each farm by on-site visit of trained veterinarians. Farms were classified as negative or positive through the use of infection profiles that combined data on PCR positive pigs and serological testing including antibody titer, sero-conversion pattern at each age category, and vaccination status. Data on biosecurity practices, farm management and environmental characteristics were analyzed using multivariate ordinal logistic regression. Generally, the biosecurity level in the pig farms included in this study were insufficient to reduce/prevent the risk of PRRSV infection given the high pig density areas and the considerable extent of vehicle movement. Factors associated with PRRSV infection were those where owners used on-farm vaccination programs had a lower risk of infection (OR = 0.19, 95% CI 0.06-0.61). The results from the analysis may guide to tailor biosecurity measures in the reduction or prevention of PRRS to the specific circumstances of pig farms in different localities of the world. To the best knowledge of the authors, this is the first study to report information on the biosecurity practices currently implemented on Korean pig farms.

Crowd Sourcing-based Social App Manufacturing environment, a small app development project by a team of anonymous virtual performed without the constraints of time and space, and manage it for the app development process need to be automated method. Effects of risk and type of the impact of risks associated with delays in the work schedule also define the characteristic function, measurement model that has been proposed, suggest the degree of influence measurement equation of risk of the project in accordance with the progressive. The advantage of this model, the project manager is able to ensure the visibility of the progress of the project. In addition, identify the project risk of work delays, and to take precautions.
There exists required safety integrity level (SIL) to assure safety in accordance with international standards for every electrical / electronics / control equipment or systems with safety related functions. The SIL is allocated from lowest level (level 0) to highest level (level 4). In order to guarantee certain safety level that is internationally acceptable, application of methodology for SIL allocation and demonstration based on related international standards is required. However, the theoretical and practical study for safety integrity level is barely under way in the domestic railway industry. This research studied not only the global process of SIL allocation to guarantee safety in accordance with international standards for safety related equipment and system, but the quantitative methodology based on international standard and the semi-quantitative methodology as alternative way for SIL allocation. Specifically, the systematic SIL allocation for platform screen door system of railway is studied applying the semi-quantitative methodology in order to save much time and effort compared to quantitative method.
Purpose: Metabolic syndrome (also known as insulin resistance syndrome) represents a constellation of hypertriglyceridemia, hypertension, impaired glucose tolerance, and obesity. Presently, the influence of various factors on metabolic syndrome was assessed in patients of a university hospital comprehensive medical examination center. Methods: Age, sex, blood pressure, height, weight, triglyceride level, high-density lipoprotein cholesterol, and glucose levels were measured in 67 people (37 males and 30 females). These factors were correlated with tobacco use, alcohol consumption, and exercise habits. Metabolic syndrome and abdominal obesity were assessed according to NCEP-ATP III criteria and the Asia-Pacific guidelines (male obesity defined as a waist circumference exceeding 90 cm), respectively. Data was analyzed using t-test, 2-test, and logistic regression. Results: Respective percentages were: tobacco use (14.9% of the 67 people), no tobacco use (85.1%), alcohol consumption (62.7%), no alcohol consumption (37.3%), regular exercise (25.4%), no regular exercise (74.6%). Logistic regression analysis revealed a gender-related odds ratio of 2.3 for metabolic syndrome and no exercise. Conclusions: Weight reduction and physical exercise may decrease the prevalence of metabolic syndrome. Early identification of metabolic syndrome and risk factor modification is prudent in cases of obesity, diabetes, hyperlipidemia, and hypertension.
To investigate the association between salt processed food and gastric cancer, a hospital based case-control study was conducted in a high risk area of China. One hundred and seven newly diagnosed cases with histological confirmation of gastric cancer and 209 controls were recruited. Information on dietary intake was collected with a validated food frequency questionnaire. Unconditional logistic regression was applied to estimate the odds ratios with adjustment for other potential confounders. Comparing the high intake group with never consumption of salt processed foods, salted meat, pickled vegetables and preserved vegetables were significantly associated with increased risk of gastric cancer. Meanwhile, salt taste preference in diet showed a dose-response relationship with gastric cancer. Our results suggest that consumption of salted meat, pickled and preserved vegetables, are positively associated with gastric cancer. Reduction of salt and salt processed food in diets might be one practical measure to preventing gastric cancer.
