Controversies about Radioactive Iodine-131 Remnant Ablation in Low Risk Thyroid Cancers: Are We Near A Consensus?

  • Well differentiated thyroid cancers (WDTC), including papillary (80%) and follicular (10%) types, are the most common endocrine cancers globally. Over the last few decades most the diagnosed cases have fallen into low risk categories. Radioactive iodine-131 (RAI) has an established role in reducing recurrence and improving the survival in high risk patients. In patients with primary tumor size <1 cm, RAI is not recommended by many thyroid societies. However, low risk WDTC has been an arena of major controversies, most importantly the role and dose of adjuvant RAI for remnant ablation to minimize chances of recurrence and improving survival. This review is an attempt to update readers about the previous and existing practice based on results of non-randomized trials and evolving trends fueled by recently published randomized studies.

A Survey on the Workload Evaluation Methods and Their Applications to WMSD Work in Industries (작업평가방법론 및 현장 적용 고찰)

  • To identify and evaluate the risk factors in WMSD work, a number of ergonomic workload evaluation methods have been developed. In the legal examination of WMSD risk factors, simple observational workload evaluation methods are widely used instead of using costly measurement equipments such as EMG and motion analyzer. The simple workload evaluation methods can be categorized into three groups; risk factor checklist methods, posture observation methods, and manual material handling task evaluation methods. In terms of the categories, this survey summarized several representative workload evaluation methods and compared them each other. Then some industrial application cases referring each the workload evaluation methods were surveyed. Due to the characteristics of each method, the selection and application procedure of workload evaluation method should be appropriate for the corresponding work. Therefore, some guidelines for the selection and application procedure of workload evaluation method were suggested.

  • In general, companies operate systematically in response to financial risks such as exchange rates and liquidity, while they are vulnerable to risks in the manufacturing and sales processes. In particular, logistics refers to the activities for planning, managing and implementing efficient flows from the starting point of goods and products to the point of consumption, The purpose of this study was to develop key risks and key risk management indicators (KRIs) for risks that undermine logistics efficiency so that logistics risks can be effectively prevented and managed. As a result, 40 risk management indicators (KRIs) were developed in a total of six categories in the logistics sector, and the definition, calculation method and early warning grade of each KRI were presented so that companies could prevent risks in advance in logistics activities and contribute to enhancing efficiency of their work.

  • This study aims to establish a methodology for rational fire risk assessment for building evacuation safety in case of fire, and specifically, to propose a fire risk assessment technique using fire scenarios considering various uncertain factors in case of fire. In order to analyze the extent to which the assumed conditions can occur, that is, the probability of each accident caused by fire, the safety rate is analyzed according to the presence or absence of each factor by using fire statistics. Factors related to the fire protection performance and evacuation ability of buildings are defined as disaster factors. In this study, disaster factors were classified into the following three categories.

Combining Two Scales to Assess Risk Factors of Falling in Community-Dwelling Elderly Persons: A Preliminary Study (노인의 낙상에 영향을 주는 요인을 평가하기 위한 ABC-BBS의 적용: 사전연구)

  • The purpose of this preliminary study was to develop a measurement for assessing risk factors for falling in community-dwelling elderly persons. Rasch analysis and principal component analysis were performed to examine whether items on the Activities-Specific Balance Confidence (ABC), assessing self-efficacy, and items on the Berg Balance Scale (BBS), assessing balance function, contribute jointly to a unidimensional construct in the elderly. A total of 35 elderly persons (4 men, 31 women) participated. In this study, each item of ABC (16 items) and BBS (14 items) was scored on a 5-point ordinal rating scale from 0 to 4. The initial Rasch and principal component analysis indicated that 3 of the ABC items and 2 of the BBS items were misfit for this study. These 5 items were excluded from further study. After combining ABC and BBS, Rasch and principal component analyses were examined and finally 23 items selected; 12 items from ABC, 11 items from BBS. The 23 combined ABC-BBC items were arranged in order of difficulty. The hardest item was 'walk outside on icy sidewalks' and the easiest item was 'pivot transfer'. Although structural calibration of each 5 rating scale categories was not ordered, the other three essential criteria of Linacre's optimal rating scale were satisfied. Overall, the ABC-BBS showed sound item psychometric properties. Each of the 5 rating scale categories appeared to distinctly identify subjects at different ability levels. The findings of this study support that the new ABC-BBS scale measure balance function and self-efficacy. It will be a clinically useful assessment of risk factors for falling in the elderly. However, the number of subjects was too small to generalize our results. Further study is needed to develop a new assessment considering more risk factors of falling in elderly.

Dietary Phytochemical Index and the Risk of Breast Cancer: A Case Control Study in a Population of Iranian Women

  • In this study we assessed the dietary phytochemical index in relation to the risk of breast cancer in women. This case-control study was conducted on 100 incident breast cancer cases and 175 healthy controls. Data regarding socio-demographic factors, medical history, medications, and anthropometric measurements were collected. Dietary data were obtained using a validated food frequency questionnaire and a energy-adjusted dietary phytochemical index (PI) was calculated. The odds ratios of breast cancer were assessed across energyadjusted PI quartile categories. The mean age of participants was $46.2{\pm}8.9$ and $45.9{\pm}9.4$ years in cases and controls, respectively. The mean PI across quartile categories was $13.9{\pm}2.6$, $21.1{\pm}1.8$, $26.7{\pm}2.1$, $41.6{\pm}10.2$ in the first, second, third and fourth quartiles, respectively. After adjustment for all potential confounders, the risk of breast cancer in the forth quartile of dietary PI was significantly decreased (OR=0.08, 95%CI=0.01-0.84). Higher intake of phytochemical-rich foods is associated with lower risk of breast cancer.

Comparison on Terror Risk of Large Space Structures and High-rise Buildings in Korea (국내 대공간 건축물과 고층 건축물의 테러위험도 비교)

  • Since the 1980s, the number of large space buildings in Korea has consistently been increasing due to large scale international competitions such as the Olympics and the World Cup, demands for environmental improvement, and development of structural systems. Due to these reasons, this paper conducted a comparative analysis on terrorism risk factors of large space structure and skyscrapers in Korea. The results suggest that the total risk level of high-rise and large space structure was "medium level risk" and that the terrorism risk level for large space structure was as high as that for high-rise buildings. As it relates to the risk levels depending on scenarios, terrorism risks to large space structure were higher than high-rise buildings in the "internal explosion" and "internal intrusion" categories. And the results of analyzing explosion-related scenarios except for CBR suggest that terrorism risks to large space structure were highest when it comes to Internal-Explosive followed by Internal-Intrusion and Explosive-Zone I; and the results showed a regular pattern. On the other hand, in the case of high-rise buildings, terrorism risks were highest in Internal-Explosive followed by Explosive-Zone I and Explosive-Zone II; and the results showed an irregular pattern.

Impacts of Bank-Specific and Macroeconomic Risks on Growth and Stability of Islamic and Conventional Banks: An Empirical Analysis from Pakistan

  • The implications of bank-specific risks and macroeconomic risks on the growth, profitability, and stability of Islamic and conventional banks are examined and compared in this article. The study also investigates whether corporate governance mitigates the effects of both bank-specific and macroeconomic risks on Islamic and conventional banks' development, profitability, and stability. For the period 2007-2019, we examined a panel data set of 22 banks in Pakistan, including both Islamic and conventional banks. We discovered considerable evidence that both bank-specific risks and macroeconomic risks have negative effects on the growth, profitability, and stability of Pakistani banks using a dynamic panel data estimator, the two-step Generalized Method of Moments (GMM) approach. Furthermore, the findings show that bank-specific and macroeconomic risks have different consequences in both types of banking. The impacts of liquidity risk, operational risk, capital risk, inflation risk, and exchange rate risk are higher for Islamic banks than for conventional banks. Conventional banks, on the other hand, are more vulnerable to credit risk and interest rate risk. Finally, the findings show that good corporate governance reduces the negative consequences of both categories of risks on bank development, profitability, and stability. This is true for Islamic and conventional banks alike.

A Basic Study on Assessment Criterion of the Risk Factor for the Marine Traffic Environment (해상교통환경 위험요소 평가기준 설정을 위한 기초 연구)

  • The representative risk evaluation techniques of the marine traffic environment are the FSA, PAWSA, & IWRAP. For the development of these techniques, the risk factors suitable to the marine traffic environment should be selected & the assessment criterion of the risk factors should be provided. The risk factors were selected as the factors that relate both to the frequency of casualty & to the consequence of casualty because the risk was defined as the frequency of casualty times the consequence of that casualty on the existing techniques. But, the risk factors relate to the consequence of casualty are excluded because the risk is defined as the sum of the risk factors including the frequency and the consequence by factors on this study. The 20 kinds of risk factors to compose the risk are selected and classified into 5 categories according to similar nature through the analysis of preceding study on the classification of the risk factors. Finally, as the foundation of risk assessment model's development for domestic marine traffic environment, the practical assessment criterion of the risk factors are suggested.

Korean Standard Classification of Diseases of Early Postpartum Women in a Korean Medicine Hospital (일개 한방병원에 내원한 산욕초기 산모의 한국표준질병·사인분류 분석)

  • Objectives: The purpose of this study is to collect and analyze the KCD codes applied to the treatment of 27 postpartum women who had been treated with Korean traditional medicine in a Korean medicine hospital, so that this study may be used as a basic data for setting the direction of postpartum Korean medical treatment research. Methods: It was approved by the Institutional Review Board (IRB) of ${\bigcirc}{\bigcirc}$ University medical center (IRB approval number : WSOH IRB H1708-02-01). Twenty-seven postpartum women who had been treated at ${\bigcirc}{\bigcirc}$ University medical center were received outpatient treatment for two weeks (from September 27, 2017 to January 5, 2018), and the KCD codes applied to the mothers were collected after obtaining the consent. On the day of registration of the study, the fertility, obstetric history and high-risk pregnancies were identified through an interview. Results: 1. The mean age of the 27 subjects was $33.33{\pm}3.99\;years$ old. Among the subjects, 17 mothers (63.0%) were high-risk pregnancy and 10 mothers (37.0%) were normal. 2. Among the 22 major disease categories, 8 categories were used. M code (musculoskeletal system) was used 243 times (70.85%), followed by R code (unclassified symptom) of 51 times (14.87%) and U code (special purpose code) of 23 times (6.71%). 3. The most commonly used code among the ten frequently used codes was M25.57 (joint pain, ankle and foot), a total of 47 times. Of the remaining nine codes, except for R60.1 (systemic edema) and U68.4 (The deficiency of yang in Bi), all codes were M codes (musculoskeletal system). 4. The M code (musculoskeletal system) was the most used major disease category in high-risk group, a total of 159 times. But in specific categories, the most commonly used code was R60.1 (systemic edema), a total of 28 times. 5. In normal group, the M code (musculoskeletal system) was the most used major disease category, a total of 84 times. Also, in specific categories, the most commonly used code was M25.57 (joint pain, ankle and foot), total 29 times. 6. The U code, corresponding to 'the diagnosis of childbirth and other obstetrical medical use', was used 23 times (6.71%), O code three times (0.87%) and Z code two times (0.58%), which was less than 10% of the total number of codes used. Conclusion: When analyzing KCD codes related to Korean medicine treatment for postpartum diseases, it is important to select the KCD codes that reflect the actual clinical state.
