Associated Factors and Prevention of Upper Pole Rippling in Prepectoral Direct-to-Implant Breast Reconstruction

  • Background Despite its many advantages, prepectoral breast reconstruction also carries the risk of implant rippling. The recent introduction of partial superior implant coverage using a pectoralis muscle slip in prepectoral direct-to-implant (DTI) breast reconstruction has shown the potential to minimize upper pole rippling. The purpose of this study was to identify factors associated with rippling and the effectiveness of our surgical technique. Methods In total, 156 patients (186 breasts) who underwent prepectoral DTI breast reconstruction between August 2019 and March 2021 were identified retrospectively. Patient data were analyzed from medical records. Univariable and multivariable logistic analyses were performed to contextualize the risks associated with rippling deformity relative to demographic characteristics and other clinical factors. Retrospective propensity-matched analysis was performed to identify the relationship between rippling deformity and the reconstruction method. Results Patients with body mass index (BMI; odds ratio [OR], 0.736; p < 0.001), those with a postoperative chemotherapy history (OR, 0.324; p = 0.027) and those who received breast reconstruction via the superior coverage technique (OR, 0.2; p = 0.004), were less likely to develop rippling deformity. The median follow-up period was 64.9 weeks, and there were no significant differences between patients in types of mastectomy, implant, or acellular dermal matrix. Patients who underwent superior coverage technique-based reconstruction showed significantly reduced rippling (OR, 0.083; p = 0.017) Conclusion Patients with higher BMI and prior postoperative chemotherapy were less likely to develop rippling deformity. The superior coverage technique can be effective in minimizing upper pole rippling.

  • Radio relics in the outskirts of galaxy clusters are interpreted as synchrotron radiation due to the relativistic electrons produced via diffusive shock acceleration (DSA) in shocks with low sonic Mach numbers, Ms ≤ 3 in high beta ICM plasma. Electron injection into the DSA process at such weak shocks is one of the key elements, which has yet to be fully understood. In this study, we explore the nature of kinetic microinstabilities excited in weak quasi-perpendicular shocks through 2D particle-in-cell simulations. We find Alfven-ion cyclotron (AIC), whistler, and mirror instabilities can be triggered by ion and electron temperature anisotropy in the immediate downstream of supercritical shocks with Ms > Mcrit ~ 2.3. In particular, AIC instability causes rippling of the shock surface, which in turn generates plasma waves on multi-scales and faciliates the electron preacceleration. Our results may contribute to understanding the origins of radio relics.

Outcome of complete acellular dermal matrix wrap with polyurethane implant in immediate prepectoral breast reconstruction

  • Background Polyurethane implants have been used on and off in breast reconstruction since 1991 while prepectoral breast reconstruction has gained popularity in recent times. In this study, we present our outcomes from the use of acellular dermal matrix (ADM) complete wrap with polyurethane implants in prepectoral breast reconstruction. Methods This is a retrospective review of prospectively maintained database from 41 patients receiving complete ADM wrap with prepectoral polyurethane implants over a 3-year period. Selection criteria were adapted from a previous study (4135 Trust Clinical Audit Database) evaluating prepectoral reconstruction with Braxon matrices. Patient demographics, operative data, surgical complications, and outcomes were collected and analyzed. Results A total of 52 implant reconstructions were performed in 41 patients with a mean follow-up of 14.3 months (range, 6-36 months). The overall reported complication rates including early (less than 6 weeks) and late complications. Early complications included two patients (4.9%) with wound dehiscence. One of which had an implant loss that was salvageable. Another patient (2%) developed red-breast syndrome and two women (4.9%) developed with seroma treated conservatively. Late complications included one patient (2%) with grade II capsular contraction, 12 patients with grade I-II rippling and two patients (4.9%) with grade III rippling. Conclusions We present our experience of prepectoral polyurethane implant using complete ADM wrap. This is one of the few papers to report on the outcome of the prepectoral use of polyurethane in immediate implant-based breast reconstruction. Our early observational series show satisfactory outcome and long-term results are warranted by a large multicenter study.

  • This paper presents ripple-free method that can occur in multirate controller design. The conventional multirate input controller has the problem that the ripple occurs in track-following because of chattering phenomenon in control input signal. In order to resolve the problem of rippling, it was proposed to eliminate the ripple phenomenon using feedforward compensator. This paper makes explains problems in conventional ripple-tree multirate controller and introduces a multirate controller design method applying lifting technique based on current estimators in condition space. Using the ripple-tree multirate controller, we show that chattering does not occur in the control input signal through applying the final value theorem from the viewpoint of discrete-time transformation. Also, this study proves that the ripple of the proposed controller decreases with the increase of this sampling frequency and, when sampling frequency is fixed, it decreases with the increase of the control input period.

  • The paper reports an improvement method on torque ripples of multi-pole interior permanent magnet synchronous motor (IPMSM) applied to a traction motor for hybrid electric vehicles. In the case of multi-pole IPMSM, the magnetic flux generated by a permanent magnet tends to leak through the bridge of the rotor without a link with stator windings. The slit design on the rotor surface was proposed to reduce torque rippling and increase the output power by reducing the leakage flux. Two design parameters for the slit are suggested for optimal design using the response surface method. As an analysis method, the 2D finite element method (FEM) was applied to consider magnetic saturation effect.

Considerations for patient selection: Prepectoral versus subpectoral implant-based breast reconstruction

  • Background In recent years, breast implants have been frequently placed in the subcutaneous pocket, in the so-called prepectoral approach. We report our technique of prepectoral implant-based breast reconstruction (IBR), as well as its surgical and aesthetic outcomes, in comparison with subpectoral IBR. We also discuss relevant considerations and pitfalls in prepectoral IBR and suggest an algorithm for the selection of patients for IBR based on our experiences. Methods We performed 79 immediate breast reconstructions with a breast implant and an acellular dermal matrix (ADM) sling, of which 47 were subpectoral IBRs and 32 were prepectoral IBRs. Two-stage IBR was performed in 36 cases (20 subpectoral, 16 prepectoral), and direct-to-implant IBR in 43 cases (27 prepectoral, 16 subpectoral). The ADM sling supplemented the inferolateral side of the breast prosthesis in the subpectoral group and covered the entire anterior surface of the breast prosthesis in the prepectoral group. Results The postoperative pain score was much lower in the prepectoral group than in the subpectoral group (1.78 vs. 7.17). The incidence of seroma was higher in the prepectoral group (31.3% vs. 6.4%). Other postoperative complications, such as surgical site infection, flap necrosis, implant failure, and wound dehiscence, occurred at similar rates in both groups. Animation deformities developed in 8.5% of patients in the subpectoral group and rippling deformities were more common in the prepectoral group (21.9% vs. 12.8%). Conclusions The indications for prepectoral IBR include moderately-sized breasts with a thick well-vascularized mastectomy flap and concomitant bilateral breast reconstruction with prophylactic mastectomy.

An Analysis of Economic Effects of Korean Fisheries using Input, Output Analysis (산업연관분석을 이용한 수산업의 경제적 파급효과 추이 분석)

  • Today, the Korean fisheries is undergoing significant hardships, both domestically and internationally. While declining amount of catch, ascending international oil prices and others pose a compelling challenge to the fishing sector, the ever strengthening influence of international institutions related to fisheries and international trade organizations also compel to bring about myriad of changes in the realm of fishery products. Against the backdrop, this study attempted to examine the fisheries catch, aquaculture, service, processing fields in terms of its rippling effect and of how the industry has been changed by analyzing the past and present through an input-output analysis. As for research methods, 168 items of the input-output tables in 2000, 2005, 2009, and 2010 were integrated to form and classify 32 sectors (28 basic sectors + catch, aquaculture, fishery service, processed fishery products) so as to generate production inducement coefficient, sensitivity coefficient, and impact coefficient. The analysis results revealed that : though the linkage effect of fishery industry was not very sizable, the impact coefficient of the processed fishery products was high; the consumption and investment coefficient sector among production inducement coefficient was on an upturn trend ; the export coefficient was tended to decline. In the future research, it is necessary to carry out a study based on the integration of detailed classification (404 sector) and a study and analysis of fishery industry by different regions through the inter-regional input-output tables. The fishery industry is one of the crucial industries in Korea. The fishery industry is not only important in its own right but also significant as it exerts influence over other industries. Therefore, it is required that there should be more investment and supports for the development of the fishery industry, and pay efforts to ensure that the investment and development could lead to mutual growth for both the fishery and other various industries.

Immediate Direct-To-Implant Breast Reconstruction Using Anatomical Implants

  • Background In 2012, a new anatomic breast implant of form-stable silicone gel was introduced onto the Korean market. The intended use of this implant is in the area of aesthetic breast surgery, and many reports are promising. Thus far, however, there have been no reports on the use of this implant for breast reconstruction in Korea. We used this breast implant in breast reconstruction surgery and report our early experience. Methods From November 2012 to April 2013, the Natrelle Style 410 form-stable anatomically shaped cohesive silicone gel-filled breast implant was used in 31 breasts of 30 patients for implant breast reconstruction with an acellular dermal matrix. Patients were treated with skin-sparing mastectomies followed by immediate breast reconstruction. Results The mean breast resection volume was 240 mL (range, 83-540 mL). The mean size of the breast implants was 217 mL (range, 125-395 mL). Breast shape outcomes were considered acceptable. Infection and skin thinning occurred in one patient each, and hematoma and seroma did not occur. Three cases of wound dehiscence occurred, one requiring surgical intervention, while the others healed with conservative treatment in one month. Rippling did not occur. So far, complications such as capsular contracture and malrotation of breast implant have not yet arisen. Conclusions By using anatomic breast implants in breast reconstruction, we achieved satisfactory results with aesthetics better than those obtained with round breast implants. Therefore, we concluded that the anatomical implant is suitable for breast reconstruction.

A study on section design proposals of rural stream based on the adjacent land use and residents' preference (주변 토지이용과 주민 이용 선호도를 고려한 농촌마을 소하천 표준 횡단면 설계)

  • The purpose of this study was to propose section designs for the ecological improvement of streams in rural villages in Korea. According to the survey about ecological conditions of rural streams of 10 selected rural villages (sep., 1996), the streams could be classified into three divisions(upstream, inner village, downstream), and the average width of streams was 9m, and the height of banks 3.1m. The slopes of basin were 7.33%, 2.67%, 1.39% at upstream, inner village, downstream respectively. The survey about the residents' usage of stream and the preferred activities(jun., 1997) showed that the streams were most frequently used for the supply of agricultural water use(39%), for the washing(15%) and for the recreation and rippling(10%), etc.. Three standard designs were presented for upstream, inner village, and downstream, and three standard sections were made by each designs. For the upstream division, it was designed to be used for the recreational activities in summer and the major stream bank materials were base rock and existing boulders with least artificiality. As the streams in the residential area were expected to be frequent access and used by the residents, most facilities for the streamside activity such as waterfront, water holes, shallows, walk were concentrated in this part with some artificiality. The downstream was designed for the agricultural activities such as water supply, space for the preparation of work and some break during the work.

Surveying Visitors′ Behavior in Tokyusan National Park (덕유산 국립공원의 이용자 행태조사)

  • Visitors to Tokyusan National Park were surveyed from August 9 to 12 During this time, 443 vistors were contacted. Of those individuals, 68.7% were males. 27.5% of respondents reported that they had gone beyond high shcool, and over one-half (64.9%) had gone as far as college. 54.7% were 20 years of age. 42.9% of the survey respondents were making their frist trip to Kuch'ontong and 67.9% to Muju Resort. The largest percentage of respondents were reported that they visited Tokyusan National Park for rippling. In group type, majority were traveling with their friends or family. In activity characteristics, 83.4% were staying over nights visitors, and over 50% mentioned staying at private houses or hotels. In six types of normative violations, major reasons of littering behavior were unintentional violation and releaser-cue violation. Most respondents who witness other visitors' littering were not likely to intervene to stop their visitors' depreciative behavior(Bystander intervention Behavior). Most of visitors want to get more information from national park office.
