Clinical Comparison of Complications Between Esophagogastrostomy and Jejunal Free Transfer After Resection of Thoracic Esophageal Cancer (흉부식도암 절제술 후 식도-위 문합술군과 유리공장이식술군간의 조기 합병증 비교)

  • Background: Replacement of the esophagus remains a challenge for surgeons involved in esophageal disease. From 1996 to 1999, a total of 27 patients with esophageal cancer underwent free jejunal transfer(12cases) or esophagogastrostomy(15cases). To determine the results such as leakage of anastomosis site, stenosis, reflux esophagitis and operation time, respiratory complications, etc. we reviewed the 4 years experiences. Material and method: Palliative bypass surgery or esophageal prosthesis and cancers of the pharyngoesophageal or esophagogastric junction were excluded in this study. Resection was usually peformed through right thoracotomy and anastomosis was made with EEA staplers in esophagogas-trstomy. In cases of jejunal free transfer, 6cases of proximal esophagojejunostomy were stapled anastomosed and remaining 6 cases and all distal site were hand-sewn anastomosed. All reconstruction was done through posteromediastinal route. Result: There were two mortalities from thoracic esophagogastrostomy and one from jeunal free transfer. Major and minor complications(anastomosis site leakage: 3 cases, graft failure: 2cases etc) occurred in 27 cases. In 15 thoracic esophagogastrostomy cases, 11 patients had mild to moderate reflux esophagitis and 5 patients incurred stricture of the anastomosis. Operation time was about 550$\pm$280 minutes in jejunal free transfer, and about 300$\pm$ 160 minutes in esophagogastromy patients. Conclusion: Post operative reflux esophagitis and dysphagia were more frequent in Ivor-Lewis operation group than jejunal free transfer group; however, respiratory complications and operation time were significantly longer in jejunal (roe transfer group(p<0.05). To minimize the incidence of postoperative reflux esophagitis and dysphagia, patient evaluation focused on jejunal free transfer surgery is better than esophagogastrostomy followed by adequate post operative care.

  • Korea's health care delivery system is based on the Medical law and the National Health Insurance Act. In order to efficiently operate limited medical resources, it is classified to use medical institutions according to the severity of the disease. The question is whether a tertiary care hospital can terminate a medical contract for a patient, if treatment for severe diseases has already been performed at a tertiary care hospital. In the case of termination of treatment, the Korean court has both a judgment that recognizes the right to terminate and a judgment that denies the termination. Among the U.S. rulings, there are rulings that order transfer to a skilled nursing facility or nursing home or home if acute treatment is no longer needed. Considering that medical resources are limited, it is necessary to acknowledge the right to terminate the contract of the medical institution when treatment by a medical institution is completed.

QTL Analysis of Teat Number Traits in an F2 Intercross between Landrace And Korean Native Pigs

  • The aim of this study was to identify quantitative trait loci (QTLs) influencing teat number traits in an $F_2$ intercross between Landrace and Korean native pigs (KNP). Three teat number traits (left;right;and total) were measured in 1105 $F_2$ progeny. All experimental animals were genotyped with 173 informative microsatellite markers located throughout the pig genome. We detect that seven chromosomes harbored QTLs for teat number traits: genome regions on SSC1;3;7;8;10;11;and 13. Six of fourteen identified QTL reached genome-wide significance. In SSC7;we identified a major QTL affecting total teat number that accounted for 5.6 % of the phenotypic variance;which was the highest test statistic (F-ratio = 61.1 under the additive model;nominal $P=1.3{\times}10^{-14}$) observed in this study. In this region;QTL for left and right teat number were also detected with genome-wide significance. With exception of the QTL in SSC10;the allele from KNP in all 6 identified QTLs was associated with decreased phenotypic values. In conclusion;our study identified both previously reported and novel QTL affecting teat number traits. These results can play an important role in determining the genetic structure underlying the variation of teat number in pigs.

Effect of Making a Hole in Zona Pellucida by Laser on Hatching of Frozen-thawed ICR Mouse Embryos (레이저를 통한 투명대내의 천공이 동결융해 ICR 마우스 수정란의 부화에 미치는 영향)

  • This study was performed to investigate the effect of laser-assisted hole in the zona pellucida (ZP) of frozen-thawed ICR mouse embryos on the process of hatching that is critical for expanded blastocysts to implant into endometrium, Vitrification medium, composed of ethylene glycol and sucrose supplemented with 7.5% (w/v) PVP, was used to freeze $2{\sim}4$ cell stage embryos recovered from oviducts of superovulated and mated female mice before storing them in $LN_2$. Right after thawing them, a laser beam was shot to make a hole in ZP followed by culturing in KSOM for $96{\sim}120\;hr$ and examining development to blastocyst and hatching every 12 hr. Laser-treated embryos showed significantly higher hatching rate compared to control (92.9% vs. 22.1%, p<0.05). From around Day 4, blastocysts developed from laser-treated embryos started hatching while the blastocysts of control group failed to hatch showing a lot of shrinkage. This study shows that a laser-assisted hole in ZP improves the hatching rate of blastocysts developed from frozen-thawed, in vitro cultured ICR mouse embryos.

A Research on TF-IDF-based Patent Recommendation Algorithm using Technology Transfer Data (기술이전 데이터를 활용한 TF-IDF기반 특허추천 알고리즘 연구)

  • The increasing number of technology transfers from public research institutes in Korea has led to a growing demand for patent recommendation platforms for SMEs. This is because selecting the right technology for commercialization is a critical factor in business success. This study developed a patent recommendation system that uses technology transfer data from the past 10 years to recommend patents that are suitable for SMEs. The system was developed in three stages. First, an item-based collaborative filtering system was developed to recommend patents based on the similarities between the patents that SMEs have previously transferred. Next, a content-based recommendation system based on TF-IDF was developed to analyze patent names and recommend patents with high similarity. Finally, a hybrid system was developed that combines the strengths of both recommendation systems. The experimental results showed that the hybrid system was able to recommend patents that were both similar and relevant to the SMEs' interests. This suggests that the system can be a valuable tool for SMEs that are looking to acquire new technologies.

Development of Auto Antigen-specific Regulatory T Cells for Diabetes Immunotherapy

  • CD4+ regulatory T cells (Tregs) are essential for normal immune surveillance, and their dysfunction can lead to the development of autoimmune diseases, such as type-1 diabetes (T1D). T1D is a T cell-mediated autoimmune disease characterized by islet b cell destruction, hypoinsulinemia, and severely altered glucose homeostasis. Tregs play a critical role in the development of T1D and participate in peripheral tolerance. Pluripotent stem cells (PSCs) can be utilized to obtain a renewable source of healthy Tregs to treat T1D as they have the ability to produce almost all cell types in the body, including Tregs. However, the right conditions for the development of antigen (Ag)-specific Tregs from PSCs (i.e., PSC-Tregs) remain undefined, especially molecular mechanisms that direct differentiation of such Tregs. Auto Ag-specific PSC-Tregs can be programmed to be tissue-associated and infiltrate to local inflamed tissue (e.g., islets) to suppress autoimmune responses after adoptive transfer, thereby avoiding potential overall immunosuppression from non-specific Tregs. Developing auto Ag-specific PSC-Tregs can reduce overall immunosuppression after adoptive transfer by accumulating inflamed islets, which drives forward the use of therapeutic PSC-Tregs for cell-based therapies in T1D.

Analysis of Anisotropic Turbulent Heat Transfer in Nuclear Fuel Bundles (핵연료 집합체내의 비등방성 난류 열전달에 관한 해석적 연구)

  • The prediction of clad surface temperatures is important to the design and the safety anlaysis of nuclear reactor. The accurate prediction requires the detailed knowledge of the flow structure and heat transfer, which is complicate due to anisotropic turbulent phenomena. A two-equation model including anisotropic eddy viscosity model is applied to forecast the velocity distribution. And the temperature field is calculated with uniform wall heat flux. The Galerkin's weighted residual finite element method has been used to calculate the turbulent quantities right up to the wall. The numerical results show good agreement with available data and that turbulence anisotropy strongly affects on the mean flow and thus the temperature field. And Nu-P/D correlation is established for sodium coolant in close-packed equilateral triangular bundle in the P/D range of 1.05 to 1.30.

  • A numerical simulation on the combined natural convection and radiation is carried out in a partially open rectangular enclosure with a heater by using the finite volume and the S-8 discrete ordinate methods. The fluid inside the enclosure is considered as an absorbing, emitting and anisotropic scattering media. The heater causes a natural circulation of the fluid (10$^{5}$ $^{9}$ ) which results in significant in-flow of the ambient cold fluid through the partially open wall. Comparing the results of pure convection with those of the combined convection- radiation, the combined heat transfer results with small Planck numbers (P$_{l}$ <1.0) show much stronger circulation than those of the pure convection, and the fluid circulation is more evident for larger Rayleigh numbers. When one of three radiative properties - the medium absorption coefficient, the wall reflectivity, and the scattering albedo - increases, the fluid circulation and the heat transfer in the enclosure are reduced. The location of the heater and the open ratio of the right wall are also shown to affect the fluid circulation and heat transfer significantly. However, the anisotropy of the scattering phase function is shown to be unimportant for the fluid circulation and heat transfer within the enclosure considered in this study.

Effects of Foot Placement and Height of Bed Surface on Load of the Lumbar Spine During Transfer Activity (인체모형 옮기기 시 발의 배치와 옮기는 지면 높이가 허리척추에 미치는 영향)

  • The purpose of this study was to investigate the effect of foot placement and height of bed surface on lumbar spine load in a dummy transfer activity. Fifteen healthy male students participated in this study. All subjects were involved in four different conditions according to foot placement (11 figure and $90^{\circ}$ figure) and height of bed surface (44 cm and 66 cm) randomly. Muscular activations of the biceps brachii, rectus femoris, elector spinae using surface-EMG, vertical ground reaction using force plate, and L4/L5 compression force using 3DSSPP (3D Static Strength Prediction Program) were measured and analysed. The results showed that muscular activations were not significantly different for the various conditions except for the rectus femoris on the right side (p<.05). Futhermore, the vertical ground reaction and L4/L5 compression force were significantly different conditions (p<.05). In conclusion, it is suggested that foot placement at $90^{\circ}$ figure is safer for transfer activity compared with the 11 figure.

Price Prediction of Fractional Investment Products Using LSTM Algorithm: Focusing on Musicow (LSTM 모델을 이용한 조각투자 상품의 가격 예측: 뮤직카우를 중심으로)

  • Real estate and artworks were considered challenging investment targets for individual investors because of their relatively high average transaction price despite their long investment history. Recently, the so-called fractional investment, generally known as investing in a share of the ownership right for real-life assets, etc., and most investors perceive that they actually own a piece (fraction) of the ownership right through their investments, is gaining popularity. Founded in 2016, Musicow started the first service that allows users to invest in copyright fees related to music distribution. Using the LSTM algorithm, one of the deep learning algorithms, this research predict the price of right to participate in copyright fees traded in Musicow. In addition to variables related to claims such as transfer price, transaction volume of claims, and copyright fees, comprehensive indicators indicating the market conditions for music copyright fees participation, exchange rates reflecting economic conditions, KTB interest rates, and Korea Composite Stock Index were also used as variables. As a result, it was confirmed that the LSTM algorithm accurately predicts the transaction price even in the case of fractional investment which has a relatively low transaction volume.
