Effects of Diet of Korean Safflower(Carthamus tinctorious L.) Seed Powder on Bone Tissue in Rats during the Recovery of Rib Fracture (토종홍화씨의 급여가 실험동물의 늑골골절 회복중 골조직에 미치는 영향)

  • This study was carried out to investigate the effects of diet of Korean safflower(Carthamus tinctorious L.) seed powder on bone tissue during the recovery of rib-fracture in rats. Male Sprague-Dawley rats of 10 weeks old, weighing 370$\pm$5g, were divided into two groups including the control group(C group, AIN-76 semipurified diet) and safflower seed group(S group, AIN-76 semipurified diet+10% safflower seed powder) and were fed experimental diets for 12 days after adaptation period. After this period, the 9th right rib was fractured surgically and sham-operation was also performed. Rats were fed with experimental diets for up to 30 more days after rib-fracture. The degree of bone repair was evaluated during the recovery period at the 8th, 11th, 16th, 21st, 30th days after the surgical operation by microscopic observation of the fractured rib tissue. In callus formation, the portion of hyaline cartilage was noticably higher in S group than C group. The intracatilagenous ossification was observed at the 8th day in S group, but at 11th day in C group. The intramembranous ossification in callus was widely found over the 8th day to the 11th day in S group, but it was shown over the 11th day to the 16th day in C group. Bone resorption was also occured more rapidly in S group as indicated by large numbers of osteoclasts observed. At the 30th day, most of trabecular bones were disappeared in S group, whereas still shwon in C group over wide ranges of fractured ribs. These results imply that the supplementation of Korean safflower seed powder influences in the recovery of bone fracture by accelerating the process of bone repair.

Sudden death caused by diaphragmatic rupture following rib fracture in a female sika deer (Cervus nippon) at a zoo

  • A one-year-old female sika deer died suddenly with no preliminary signs during exhibition at a zoo. At necropsy, the carcass was emaciated and had dried fur. Examination of the thoracic cavity revealed a diaphragmatic rupture measuring 2 cm in diameter and a fracture in the middle of the right eighth rib. The liver and lungs had irregular circular discolorations caused by diaphragmatic rupture and subsequent herniation. Dark-brown-colored ascitic fluid, hydrothorax, and yellowish hydropericardium were also observed. The cause of death was determined to be diaphragmatic rupture caused by a rib fracture, which led to respiratory imbalance and circulatory disorders.

A Clinical Study of Thoracic Injuries: 190 Cases (흉부손상의 임상적 고찰: 190예)

  • 190 cases of the thoracic injuries experienced at the department of chest surgery, Chungnam National University Hospital, for 5 years from Jan. 1976 till Dec 1980, were analysed. The results are as follows; 1. The incidence rate of male to female was 5.1:1. The common age groups were 3rd, 4th and 5th decades, and the most common age group in the penetrating injury was 3rd decade. 2. The most common mode of the nonpenetrating injuries was a traffic accident [63.0%], and the most commonly used tool in the penetrating injuries is a knife [66.7%]. 3. The most common nonpenetrating injury was rib fracture [73.9%], and the common fracture sites were 5th, 6th, 7th and 8th ribs [especially, 7th rib]. The incidence rate of flail chest was 15% of the cases of the rib fractures. 4. The common associated injuries of the nonpenetrating were long bone fracture [18.3%], brain contusion [15.9%], and clavicle fracture etc.. 5. The common method of surgical treatment were closed thoracostomy [46.7%], thoracentesis, and open thoracotomy [7.4%] etc.. 6. The overall mortality was 2.8%. Surgical Correction of Pseudo-flail Chest Using Interfragmentary Wiring, Latissimus Dorsi Flap, and External Splinting in a Dog

  • A 7-year-old intact female Maltese dog presented with a history of bite wounds. Physical examination revealed labored breathing, four puncture wounds with subcutaneous emphysema of the thorax, and paradoxical respiratory movement of the right thoracic wall. On radiography, a segmental fracture of the right 7th rib and a single fracture of the 8th rib were evident on the dorsal thorax. An inward displacement of the fractured segment and contusion of the right caudal lung lobe were identified with computed tomography. A diagnosis of pseudo-flail chest was made. Exploratory thoracotomy revealed a full-thickness muscular defect, a marked discoloration of the right caudal lung lobe, a segmental fracture of the right 7th rib, and a single fracture of the right 8th rib. Necrotic tissues were removed using surgical debridement. The fractured 7th and 8th ribs were corrected using a single interfragmentary wiring technique. The thoracic wall was reconstructed using the latissimus dorsi muscle flap. Additional thoracic stabilization using a thermoplastic splint was applied to correct paradoxical respiratory movement. The external splint was removed 4 weeks postoperatively. There was no evidence of respiratory abnormalities 18 months postoperatively.

Outcomes after rib fractures: more complex than a single number

  • Purpose: Rib fractures are common injuries that can lead to morbidity and mortality. Methods: Data on all patients with rib fractures admitted to a single trauma center between January 1, 2008 and December 31, 2018 were reviewed. Results: A total of 1,671 admissions for rib fracture were examined. Patients' median age was 57 years, the median Injury Severity Score (ISS) was 14, and the median number of fractured ribs was three. The in-hospital mortality rate was 4%. Age, the number of rib fractures, and Charlson Comorbidity Index scores were poor predictors of mortality, while the ISS was a slightly better predictor, with area under the receiver operating characteristic curve values of 0.60, 0.55, 0.58, and 0.74, respectively. Multivariate regression showed that age, ISS, and Charlson Comorbidity Index score, but not the number of rib fractures, were associated with significantly elevated adjusted odds ratios for mortality (1.03, 1.14, and 1.28, respectively). Conclusions: Age, ISS, and comorbidities were independently associated with the risk of mortality; however, they were not accurate predictors of death. The factors associated with rib fracture mortality are complex and cannot be explained by a single variable. Interventions to improve outcomes must be multifaceted.

Evaluation of the Rib Lesions by Bone Scanning (골스캔상 늑골병변의 감별진단에 관한 관찰)

  • In order to establish criteria for distinguishing between occult fracture and metastases, we have reviewed 52 records of cancer patients and 24 of rib fracture patients referred for bone scanning. In 52 patients with biopsy-proven malignancy, multiple linear increased uptake of the ribs was found in 32 cases (61.5%), and was the most common finding. In 24 patients with rib fracture, all but 1 had fecal increased uptake of the ribs, and lesions almost always aligned in a row. By analyzing the appearance of rib lesions in total 76 patients, it was concluded that there is a high probability that rib lesions detected by bone scanning are bony metastases if 1) they are multiple linear as opposed to fecal, ad 2) they are not aligned in the same location.

  • A clinical evaluation was performed on 402 cases of chest trauma those were admitted and treated at the department of chest surgery, Chonnam National University Hospital, during the past 13 years and 7 months from Jan. 1968 to July 1980. 1. The ratio of male to female was 5.5:1 in 1968 and 3.4:1 in 1980 with progressive increase of female patient and age from 20 to 59 years is 81.0%. 2. The most common cause of chest trauma was stab wound in penetrating trauma and traffic accidents in nonpenetrating trauma. 3. The most common injury from chest trauma was hemothorax in penetrating trauma and rib fracture in nonpenetrating trauma. 4. Hemothorax was observed in 76 cases [18.9%] of total cases and rib fracture was observed in 163 cases [40.5%] of total cases. 5. Rib fracture was prevalent from 4th to 8th rib, and 1st and 2nd rib fractures were associated with major thoracic injuries and with other organ injuries. 6. Open thoracotomy was performed in 72 cases [17.9%] and others were treated with conservative nonoperative treatment. 7. Overall mortality was 4.5% [19 cases], and common causes of the death were shock and respiratory insufficiency.

  • Recently, we experienced one case of multiple ribs fracture with large chest wall defects. This patient was treated with internal fixation of ribs by use of Judet`s struts and reconstruction of chest wall defects by use of Teflon mesh. Postoperative outcome was satisfactory result and its advantages were reduced duration of operation, prevention of pulmonary herniation and reduced risk of postoperative infection.

  • This study was carried out to investigate the effects of Korean safflower(Carthamus. tinctorious L.) seed powder supplementation on bone metabolism during the recovery of rib-fracture induced by surgical operation in rats. Male Sprague-Dawley rats of 10 weeks old, weighing 370$\pm$5g, were divided into two groups after arrival : the control group(C group, AIN-76 semifurified diet) and safflower seed group(S group, AIN-76 semifurified diet+10% safflower seed powder). They were fed experimental diets for 12 days before the rib fracture operation and for 30 more days after the operation. A number 9 rib was fractured surgically and sham-operation was also performed. After the rib-fracture operation, rats were sacrificed on the 8th, 11th, 16th, 21st, and 30th day, respectively. The body weights were decreased on the 8th day after operation in both groups but recovered gradually thereafter. The fractured sections of the S group were more strongly adhered and more rapidly repaired than those of C group. Effects of safflower seed powder supplementation on indices of bone metabolism during the repairing of rib-fracture can be summarized as following. There were no differences between C and S group. In the levels of PTH and calcitonin, which regulate the concentration of plasma calcium. However, PTH level on the 8th day was significantly higher than that on the 16th day in control group. There were also no differences between groups or within groups in the levels of calcitonin. Osteocalcin levels, one of the bone-formation indices, were similar in two groups. Activities of total-ALP in plasma were highest at the 8th day in both groups and decreased thereafter, except on the 30th day in control group. The activities of bone-specific ALP that is directly related to bone calcification, showed higher levels in the S group than in the C group throughout the experimental period. They were significantly increased in the S group on the 11th, 16th, and 21st days. In comparisons of plasma calcium and phosphate levels, Ca levels were tended to be lower in the S group than in the C group and P levels showed the opposite trend. It seems that the fractured ribs were more rapidly repaired in rats supplemented with safflower seed powder than in control rats, possibly due to stimulation of bone calcification from increased activity of bone-specific ALP.   • Patients with Horner's syndrome exhibit a variety of symptoms, including miosis, palpebral ptosis, and anhidrosis. This syndrome is caused by interruptions of the sympathetic neural pathways. This paper describes two cases of patients with Horner's syndrome who experienced a first rib fracture after crushing injuries.
