류마토이드 관절염 환자 골막세포에 대한 계혈등의 면역반응 (Immunity Responses of the Spatholubus Suberectus Dunn to the Synovial Cells Isolated from Patients with Rheumatoid Arthritis)

  • This study was carried out to know the immunity responses of the Spatholubus suberectus Dunn(hereinafter referred to STSD) to the synovial cells isolated from patients with rheumatoid arthritis. Various experiments were performed in vitro to analyse the immunity effects of STSD. Gene expression and production of pro-inflammatory cytokines such as IL-1β, IL-6, TNF-α, iNOS and COX-2 were determined by RT-PCR and ELISA kit. And also the binding activity of NF-kB and AP-1 were measured by Electromobility shift assay (EMSA) and the production of ROS was measured by flow cytometry. The results were obtained as follows 1. The gene expression and production of pro-inflammatory cytokines IL-1β, IL-6, TNF-α were reduced significantly. 2. The gene expression of iNOS and COX-2 were reduced. 3. The binding activity of NF-kB and AP-1 were inhibited. 4. The production of ROS in human synovial cells was reduced significantly. Comparison of the results for this study showed that STSD had immunomodulatory effects of suppressing or enhancing. So we expect that STSD should be used as a effective drugs for not only rheumatoid arthritis but also another auto-immune disease. Therefore we have to survey continuously in looking for the effective substance and mechanism in the future.

가미대강활탕(加味大羌活湯)이 Collagen II로 유발된 관절염 생쥐모델에 미치는 면역학적 연구 (Immunogic Study on the Effect of Gamidaegang-tang on Collagen II - Induced Arthritis in Mice)

  • This study was carried out to know the immunological effect of GDGT on CIA(collagen induced arthritis) mice, a model of rheumatoid arthritis. For this purpose, GDGT was orally administerd to mice with arthritis induced by collagen II and then value of cytotoxicity on hFLSs and liver, the arthritis index, immunocyte in paw joint and DNL, rheumatoid factor (IgG and IgM), collagen II specific antibody in the serum were measured. The cytotoxicity were not shown on hFLSs and liver. The arthritis index decreased significantly after 3 week. In total cell counts of DLN and paw joint, there was a significant increase in DLN and significant decrease in paw joint. In DNL, $CD19^+$, $CD3^+$, $CD4^+$, $CD3^+/CD69^+$, $CD8^+$, $CD4^+/CD25^+$, $ CD3^+/CD49b^+$ cells increased significantly. In Paw joints, $CD3^+$, $ CD4^+$, $CD4^+/CD25^+$ cells decreased significantly. The level of serum IgG and IgM decreased significantly. The level of collagen II in the serum was decreased significantly. Marginal erosion, necrotic chodrocytes, cartilage and bone degradation were improved in histological section of paw joints. The results present significant immunological effect of GDGT on rats with arthritis induced by collgen II. So we expect that GDGT should be used as a effective drugs for not only rheumatoid arthritis but also another auto-immune disease.

  • 1. Objectives To evaluate effectiveness of one hundred thirty kinds of pharmacopuncture made of herbs used in Sasang Constitutional Medicine. 2. Methods We searched at fifteen kinds of Korean oriental medical journals websites and four kinds of Korean oriental medicine databases. '약침', '수침', 'pharmacopuncture', 'aqua acupuncture' and 'herbal acupuncture' were keywords. Searching period was March to April 2010. 3. Results One hundred eighty three articles are searched. And these are the results. 1) A lot of Soeum herb pharmacopunctures are effective in anti-tumor, brain damage, rheumatoid arthritis. 2) A lot of Soyang herb pharmacopunctures are effective in kidney injury, asthma, rheumatoid arthritis. 3) A lot of Taeeum herb pharmacopunctures are effective in obesity, asthma, anti-tumor, anti-oxidation, rheumatoid arthritis. 4) Taeyang herb pharmacopunctures are effective in rheumatoid arthritis, amyotrophia. 4. Conclusions There was similar effectiveness between oral administration and pharmacopuncture in Sasang Constitutional herbs. It suggest that pharmacopuncture could be used for Sasang constitutional disease and symptoms. Well-designed clinical studies for Sasang constitutional pharmacopuncture are needed.

한국인 류마티스 관절염의 감수성과 TSLPR 유전자 다형성의 연관성 (Association of Thymic Stromal Lymphopoietin Receptor (TSLPR) Polymorphisms with the Susceptibility of Rheumatoid Arthritis in a Korean Population)

  • 사람의 TSLPR는 염증 유발 및 알러지 반응에 중요한 역할을 한다. 우리는 TSLPR 유전자에서 11개의 유전자 다형성과 2개의 유전적 변이 부위를 발굴하였고, 이들 TSLPR 유전자 다형성이 아토피 천식의 민감성과 연관성이 있음을 확인 하였다. 이에 우리는 TSLPR 유전자 다형성과 류마티스 관절염과의 연관성에 대해서도 알아 보고자 하였다. 457명의 류마티스 관절염 환자군과 570명의 정상 대조군으로 TSLPR 유전자 다형성의 genotype과 allele frequencies를 비교 분석 해본 결과 두 그룹 간에 유의성이 없었고, 류마티스 관절염 여성 그룹에서의 비교 분석에서도 두 그룹 간에 유의성이 없었다. 또한, 류마티스 관절염 환자에서 TSLPR 유전자 다형성이 RF나 CCP levels에 영향을 미치지 않는 것으로 분석 되었다. 따라서, TSLPR 유전자 다형성이 한국인에서의 류마티스 관절염에 대한 민감성과 연관성이 없는 것으로 생각된다.

Rocaglamide-A Potentiates Osteoblast Differentiation by Inhibiting NF-κB Signaling

  • Rheumatoid arthritis is a chronic inflammatory disease that leads to bone and cartilage erosion. The inhibition of osteoblast differentiation by the inflammatory factor TNF-${\alpha}$ is critical for the pathogenesis of rheumatoid arthritis. To modulate TNF-${\alpha}$ mediated inhibition of osteoblast differentiation is required to improve therapeutic efficacy of rheumatoid arthritis. Here, we explored the potential role of rocaglamide-A, a component of Aglaia plant, in osteoblast differentiation. Rocaglamide-A prevented TNF-${\alpha}$ mediated inhibition of osteoblast differentiation, and promoted osteoblast differentiation directly, in both C2C12 and primary mesenchymal stromal cells. Mechanistically, Rocaglamide-A inhibited the phosphorylation of NF-${\kappa}B$ component p65 protein and the accumulation of p65 in nucleus, which resulted in the diminished NF-${\kappa}B$ responsible transcriptional activity. Oppositely, overexpression of p65 reversed rocaglamide-A's protective effects on osteoblast differentiation. Collectively, rocaglamide-A protected and stimulated osteoblast differentiation via blocking NF-${\kappa}B$ pathway. It suggests that rocaglamide-A may be a good candidate to develop as therapeutic drug for rheumatoid arthritis associated bone loss diseases.

Osseous changes in the temporomandibular joint in rheumatoid arthritis: A cone-beam computed tomography study

  • Purpose: To evaluate osseous changes of temporomandibular joint (TMJ) in patients with rheumatoid arthritis (RA) using cone-beam computed tomography (CBCT) and to correlate the imaging findings with the severity of TMJ dysfunction, clinical findings, and laboratory findings. Materials and Methods: This study consisted of 28 subjects, including 14 RA patients and 14 controls, who were scheduled to undergo CBCT imaging for the diagnosis of a complaint not related to or affecting the TMJ. The Fonseca's questionnaire was used to assess the severity of TMJ dysfunction. Rheumatoid factor (RF) and the erythrocyte sedimentation rate (ESR) were assessed in the RA patients. CBCT was then performed in all subjects and osseous TMJ abnormalities were assessed. Results: According to the Fonseca's questionnaire, 14.3% of the patients had no TMJ dysfunction, while 50%, 21.4%, and 14.3% had mild, moderate, and severe dysfunction, respectively. RF was positive in 64.3% of patients, and the ESR level was high in 100%. Imaging findings revealed a statistically significantly higher prevalence of erosion (85.7%), flattening (89.3%), osteophyte formation (32.1%), subchondral cyst (32.1%), sclerosis (64.3%), and condylar irregularities (28.6%) in the RA patients than in the controls. No correlations were found between CBCT findings and the clinical findings, the severity of TMJ dysfunction, disease duration, or laboratory results. Conclusion: RA patients might show extensive osseous abnormalities with no/mild clinical signs or symptoms of TMJ dysfunction that necessitate TMJ imaging for these patients. CBCT is a valuable and efficient modality that can assess osseous TMJ changes in RA patients.

Periodontal pathogens and the association between periodontitis and rheumatoid arthritis in Korean adults

  • Purpose: Periodontitis and rheumatoid arthritis (RA) share a similar inflammatory pathogenesis. Porphyromonas gingivalis (Pg) can induce anticyclic-citrullinated peptide autoantibodies (anti-CCP antibodies), a key factor in the development of RA. This study aimed at evaluating the relationships between the 2 diseases and identifying the clinical implications thereof, with a focus on periodontal pathogens in Korean adults. Methods: A total of 260 RA patients and 86 age- and sex-matched control patients without arthritis were enrolled in this prospective cross-sectional study. Periodontal indices and the prevalence and amount of periodontal pathogens were compared between the groups. Correlations between periodontal and RA indices were examined, as were correlations between 9 periodontal pathogens and RA indices. Results: The RA group had significantly higher values than the control group for all investigated periodontal indices (P<0.05) except the number of teeth. The gingival index (GI) was correlated with the disease activity score 28 (DAS28) (r=0.125, P=0.049), RA disease duration (r=0.253, P<0.001), erythrocyte sedimentation rate (ESR) (r=0.162, P=0.010), and anti-CCP antibody titer (r=0.205, P=0.004). Probing pocket depth (PPD) was correlated with ESR (r=0.139, P=0.027) and anti-Pg antibody titer (r=0.203, P=0.001). Bleeding on probing (BOP) was correlated with DAS28 (r=0.137, P=0.030), RA disease duration (r=0.202, P=0.001), ESR (r=0.136, P=0.030), anti-Pg antibody titer (r=0.177, P=0.005), and anti-CCP antibody titer (r=0.188, P=0.007). Clinical attachment level (CAL) and periodontitis severity were correlated with anti-Pg antibody titer (the former r=0.201, P=0.002; the latter r=0.175, P=0.006). The quantity of Pg was positively correlated with the serum anti-Pg antibody titer (r=0.148, P=0.020). Conclusions: The GI, BOP, and PPD showed positive relationships with several RA indices. The anti-Pg antibody titer had positive relationships with PPD, BOP, CAL, and periodontitis severity. Thus, increasing values of periodontal indices could be used as a risk indicator of disease development in RA patients, and an increasing anti-Pg antibody titer could be considered as a warning sign in RA patients suffering with periodontitis.

  • The systematic approach to pharmacologic treatment is typically to begin with the safest, simplest, and most conservative measures. It has been realized that the more rapidly inflammation is under control, the less likely it is that there will be permanent sequelae. Nonsteroidal anti-inflammatory drugs (NSAIDs) are the mainstay of initial treatment for inflammation. In addition, the slow-acting antirheumatic drugs (SAARDs) and disease-modifying antirheumatic drugs (DMARDs) have efficacy of anti-inflammatory action in children with chronic arthritis. New therapeutic modalities for inflammation, such as etanercept and infliximab, promise even further improvements in the risk/benefit ratio of treatment. It is not typically possible at the onset of the disease to predict which children will recover and which will go on to have unremitting disease with lingering disability or enter adulthood with serious functional impairment. Therefore, the initial therapeutic approach must be vigorous in all children.

류마치스 관절염을 위한 포괄적 건강증진 프로그램이 (Comprehensive Health Promotion Program for Rheumatoid Arthritis) 건강증진 전략, 질환에 대한 지식과 건강상태에 미치는 효과 분석 (The Analysis of the Effects of Comprehensive Health Promotion Program for RA on Changes in Health Promotion Strategies, Knowledge, and Health Status)

  • This study was conducted to test whether a comprehensive health promotion program for promotion strategies and knowledge about the disease, thus leading to the improvement of health status by using repeated measure of quasi- experiment design. Eighteen RA patients who visited the RA clinic of an university hospital located in Inchon were invited to participate in the CHPPRA. According to the study results, it was shown that the CHPPRA had significant effects on the patients' health status such as pain, depression, and functional disability. Also, that the improvement of health status was achieved by a positive change in the four health promotion strategies, which consisted of goal setting, positive thinking, exercise, and knowledge about the disease. Goal setting, positive thinking, and knowledge about the disease could also affect the patients' depression. Thus it can be interpreted that the improvement of these strategies may result in a remarkable decrease of depression. In addition, alleviation of functional disability may be due to increase of exercise. However although the strategies which were directly associated with pain management were not significantly improved, pain was significantly reduced. On the other hand, the study result showed that the other health promotion strategies included in CHPPRA such as pain management, positive thinking, stress management, asking for assistance and communication were not significantly increased. although the health status such as pain, depression, and functional disability, which are final goals of the program, were significantly improved through the exposition of patients to those health promotion strategies.

진폐증 환자에서의 혈청학적 표지자의 변화 (The Changes of Serologic Markers in Pneumoconiosis of Coal Workers)

  • Background : Pneumoconiosis is a parenchymal lung disease that results from the accumulation of coal dust in the lungs and the consequent tissue reaction. To evaluate the role of various personal factors in pneumoconiosis and the significance of some serologic markers for assessing the disease activity related to pneumoconiosis, the Rheumatoid Factor(RF), ${\alpha}_1$-AT, C-Reactive Protein(CRP), ceruloplasmin and fibrinogen levels were measured. Method : All the patients were males, 45-76 years old, and the mean duration of coal dust exposure was 23.2 years. 51 patients were classified as having Simple Pneumoconiosis (SP), 59 had Progressive Massive Fibrosis (PMF). Fifty eight men with ages ranging from 26-70 years were used as normal controls. The serum RF and CRP were titrated using an Autochemistry analyzer (HITACHI 7150 : Japan) and the ${\alpha}_1$-AT and ceruloplasmin levels were measured using a Nephelometer (Behring Nephelometer : Germany) and the fibrinogen levels were estimated by using an Autoanalyzer for hematologic coagulation. Result : There was a higher RF level in the SP, and PMF groups than in the control groups but there was no statistical difference. The CRP, ${\alpha}_1$-AT, and ceruloplasmin levels were also higher in the SP, and PMF groups. However, the fibrinogen concentration was within the normal ranges in both the SP and PMF groups. Conclusion : The CWP (Ed note : Define CWP) patients had significantly higher CRP, ${\alpha}_1$-AT, and ceruloplasmin levels compared to the control group. It is believed that these serologic changes could be used as a marker of the disease activity.
