Sentinel Lymph Node Imaging in Breast Cancer (유방암에서 전초림프절 영상)

  • Currently, dissection of the axillary or regional lymph nodes is considered the standard staging procedure in breast cancer. However, accumulating evidence is becoming available that the sentinel node concept may provide the same or even better staging information. In the case of melanoma, it is proven that the histological characteristics of the sentinel node reflect the histological characteristics of the distal part of the lymphatic basin. Morbidity can be reduced significantly by the use of sentinel node dissection as several authors have reported successful introduction of this technique into clinical practice. But in breast cancer patients, there are signigicant differences in practice relating to the technology, such as radiopharmaceuticals, injection sites, volume of injectate, combination with vital blue dye, preoperative lymphoscintigraphy, etc. Valuable reports on these topics appeared in recent journals. This review is a summary of those reports for nuclear physicians interested in sentinel node detection by lymphoscintigraphy in breast cancer patients.

Cancer Control Programs in East Asia: Evidence From the International Literature

  • Cancer is a major cause of mortality and morbidity throughout the world, including the countries of North-East and South-East Asia. Assessment of burden through cancer registration, determination of risk and protective factors, early detection and screening, clinical practice, interventions for example in vaccination, tobacco cessation efforts and palliative care all should be included in comprehensive cancer control programs. The degree to which this is possible naturally depends on the resources available at local, national and international levels. The present review concerns elements of cancer control programs established in China, Taiwan, Korea, and Japan in North-East Asia, Viet Nam, Thailand, Malaysia, and Indonesia as representative larger countries of South-East Asia for comparison, using the published literature as a guide. While major advances have been made, there are still areas which need more attention, especially in South-East Asia, and international cooperation is essential if standard guidelines are to be generated to allow effective cancer control efforts throughout the Far East.

  • In this article, an in-depth review is presented to explain the present status of QC and QA for air quality monitoring, especially with respect to a few pollutants of which measurement requires high standard such as VOC and heavy metals. Upon reviewing the reported results of various studies conducted previously, we attempted to suggest a future direction of QC and QA for the acquisition of the high quality data in the air quality monitoring.

  • This study used a slightly modified version of contingent valuation questionnaire designed and developed by Krupnick et al.(2000) to estimate the value of mortality risk reduction for environmental policy. Because that environmental policy, especially air pollution policy with some latency main effect on mortality risk reduction of old people with some latency period, respondents were asked about their current WTP for 5/1,000 mortality risk reduction being to take place beginning at 70 over the course of ten years. The annual mean WTP is 233,370 won (standard error 32,570 won) in spike/continuous/interval data model. The implied VSL is 466 million won. The 95% confidence interval is 339 million won~594 million won.

  • In this study, it has been attempted to build up a smart, stable, and integrated Next-generation Home Network (NHN) system capable of supporting more comfortable and convenient life in the future human housing domains, and to find out which institutional tasks are required to activate the related industries. Additionally, this study aims to review a variety of technologies suggested so far in several fields including information and communications, appliance, and building maintenance. This paper analyzes the advantages and disadvantages, acceptance structure and problems of standard technologies and it suggests the political methods and policies to embody them most effectively and integrally in the future. The said certification system is called 'Standardized Certification System for Korean-style Next-generation Home Network Buildings'(abbreviated to K-NHN) as a new system created for the purpose of developing, more effectively, NHN to be introduced in the future.

  • Kidney transplantation (KT) is the gold standard for renal replacement therapy in pediatric patients with end-stage renal disease. Recently, it has been observed that the outcome of pediatric KT is nearly identical to that in adults owing to the development and application of a variety of immunosuppressants and newer surgical techniques. However, owing to several differences in characteristics between children and adults, pediatric KT requires that additional information be learned and is associated with added concerns. These differences include post-KT complications, donor-recipient size mismatch, problems related to growth, and nonadherence to therapy, among others. This review was aimed at elucidating the clinical characteristics of pediatric KT that differ from those observed in adults.

  • Recently, ordinary mail is decreasing continuously by the development of ICT (Information and Communications Technology) including E-mail, messenger service, and etc. On the contrary, parcel post is increasing by the increase of online shopping, home shopping, and etc. Aside from these changes, the demand for various mail acceptance and delivery means is increasing with the increase of single households and dual-income couples. To overcome these environmental changes, Korea Post installs unmanned post offices which are composed of various unmanned mail acceptance and delivery postal systems. In this paper, we propose the methodological approach for the site selection of unmanned post office among the existing post offices and the installation standard of unmanned postal system considering the operation cost by the mail volume.

Maturity-onset diabetes of the young: update and perspectives on diagnosis and treatment

  • Maturity-onset diabetes of the young (MODY) is a clinically heterogeneous group of monogenic disorders characterized by ß-cell dysfunction. MODY accounts for between 2% and 5% of all diabetes cases, and distinguishing it from type 1 or type 2 diabetes is a diagnostic challenge. Recently, MODY-causing mutations have been identified in 14 different genes. Sanger DNA sequencing is the gold standard for identifying the mutations in MODY-related genes, and may facilitate the diagnosis. Despite the lower frequency among diabetes mellitus cases, a correct genetic diagnosis of MODY is important for optimizing treatment strategies. There is a discrepancy in the disease-causing locus between the Asian and Caucasian patients with MODY. Furthermore, the prevalence of the disease in Asian populations remains to be studied. In this review, the current understanding of MODY is summarized and the Asian studies of MODY are discussed in detail.

A Study on Correlation between the CMMI SPs and GPs at Maturity Levels 2 and 3

  • Assuming that the Capability Maturity Model Integration for Development v1.2 (CMMI) could be applied to an organization more effectively if the content similarities among practices were improved in terms of structure and composition, this paper presents the analysis of the correlations between the CMMI Specific Practices (SP) and Generic Practices (GP) for the Maturity Level 2 and 3 Process Areas using the Chi-square independence test. According to the analysis, a 22.2% correlation was observed. To minimize the problem of repeatedly applying similarities, 6 GPs that are highly correlated with the SPs were grouped together. Then, three different improvement plans: 1) development of a standard template-based project plan, 2) establishment of a configuration management system based on open source software to control work products and leverage experience, and 3) establishment of project assurance through an independent quality assurance-based organization and intensive review by higher-level management - were defined.

  • Incoterms are standard trade terms most commonly used in international sales contracts. ICC have updated six times to keep pace with current international trade practices. Amendments and additions were made in 1953,1967,1976,1980,1990 and presently in 2000. ICC is currently revising Incoterms 2000. The new edition, Incoterms 2010, is expected to enter into force on 1 January 2011. In this paper, I reviewed the present conditions of trade terms and the problems of Incoterms 2000 in our country. I hope this paper to contribute to revising works of Incoterms by reference of above mentioned contents, that is to say, ship's rail, expansion of seller's risk liability of FOB,CFR,CIF.
