An Exploratory Study on the Construction of Retrospective Authority Data with National Authorities (국가전거를 활용한 단위 도서관의 전거데이터 소급 구축 연구)

  • In the recent cataloging environment, the function of authority data is becoming more important. In Korea, a pilot project for sharing and joint use of the national authority data has been promoted since 2019, and a plan to automatically construct retrospective authority data using national authorities is in progress. This study aims to (1) analyze the results of the pilot project for retrospective authority data jointly promoted by the National Library of Korea and KERIS in 2019, (2) explore methodological limitations and problems in the process, and (3) derive policy proposals for constructing authority data more efficiently. To this end, a university library participating in the pilot project was investigated as a case study. Based on it, the automatic generation rate and accuracy of authority data, and errors and omissions cases and causes were analyzed in detail. As a results of the study, it was found that the quality of union catalogs and bibliographic records had a great influence on automatically constructing authority data, and supplementary methods were needed to minimize these limitations.

  • The purpose of this study is to inquire into present conditions and problems of current performance test and to try to come up with a fundamental alternative of it. Eventually, the studies of performance test so far can be labelled 'prescription' as a prospective approach. On the contrary, the fundamental alternative can be labelled 'understanding' as a retrospective approach. It can be seen that these 'prescription' and 'understanding' do not separate but represent both extreme ends of problem-solving.'Prescription' exists for 'understanding' and the latter can be acquired through the former. Just under this assumption, it can be accepted that 'prescription' as a prospective approach of performance test is an inevitable measure to achieve the purpose of it. However, the ultimate purpose of 'prescription' should proceed to 'understanding' as a retrospective approach.

  • There are two types of clinical research to figure out risk factor for disease using collected data. One is prospective study to approach the subjects from the present time and the other is retrospective study to find the risk factor using the subject's information in the past. Both approached and study design are different but the purpose of the two studies is to identify a significant difference between two groups and to find out what the variables to influence groups. Especially when comparing the two groups in clinical research, we have to look at the difference between the impact clinical variables by group while controlling the influence of the baseline characteristics variables such as age and sex. However, in the retrospective study, the difference of baseline characteristic variables can occur more frequently because the past records did not randomly assign subjects into two groups. In clinical data analysis use covariates to solve this problem. Typically, the analysis method using the analysis of covariance of variance, adjusted model, and propensity score matching method. This study is introduce the way of equality adjustment between groups data analysis using covariates in retrospective clinical studies and apply it to the recurrence of gastric cancer data.

Retrospective study of porcine epidemic diarrhea in Korea by in situ hybridization (In situ hybridization에 의한 돼지 유행성 설사증의 국내발생 역추적 진단)

  • In this presented study, we established a method for diagnosis of porcine epidemic diarrhea(PED) by in situ hybridization(ISH), which made distinct progress in diagnostic pathology. We also carried out the retrospective diagnosis through ISH to assume the exact time of the first outbreak and incidence of PED in Korea. The outbreak of PED in Korea reported in 1992. However, since the end of 1980's, some researches of pig-industry have already suspected the outbreak of PED, not transmissible gastroenteritis(TGE). In this experiment, we performed the ISH using 80 formalin-fixed and paraffin-embedded tissues of porcine intestine which were requests for pathological diagnosis from 63 farms whose primary sign was diarrhea from 1984 to 1991. We prepared biotinylated cDNA probe(492base pairs) for ISH by nick translation method and carried out the ISH, using $Microprobe^{TM}$ capillary action system(Fisher $Biotech^R$). We detected PED virus in intestinal mucosa of 2 cases in 1992, 1 case in 1988, and 1 case in 1987. As a result, we assume that the outbreak of PED in Korea have already started since 1987.

Retrospective Drugs Utilization Review Study for Chronic Kidney Disease Using National Health Insurance Database (건강보험 자료를 이용한 만성신부전 환자의 신독성 약물사용 현황)

  • The purpose was to implement drug utilization review (DUR) for whom were diagnosed with chronic kidney disease (CKD) population using health insurance claim data. This study constructed drug utilization database using Health Insurance Review and Assessment Service (HIRA) database and selected contraindicated drugs with kidney based on previously developed drug utilization guide and reviewing other countries' examples. Main outcome measures were the proportion of prescription for 1 or more drugs of concern. The cohort included 115,948 subjects, who were diagnosed with chronic kidney disease. Inappropriate drugs with CKD patients was some used, and the most commonly prescribed classes were aluminum drugs. However it is difficult to find problems with inappropriate drug because claims data doesn't have laboratory data. Based on the result of retrospective drug utilization review study, more studies should be analysed drug utilization patterns and monitoring system should be developed.

Periareolar Augmentation Mastopexy with Interlocking Gore-Tex Suture, Retrospective Review of 50 Consecutive Patients

  • Background Periareolar Augmentation Mastopexy is one of the most challenging operations in plastic surgery. Problems with scar quality, areolar widening, and distortion are frequent problems that interfere with a predictable result. Methods A retrospective review was performed on fifty consecutive patients who underwent a periareolar augmentation mastopexy with the interlocking approach. Of the 50 patients, 30 had both preoperative and postoperative photographs and were the basis of the study. Results The age of the patients ranged from 19 to 56 years with the average age being 39 years. The postoperative follow-up averaged 9.5 months and the implants averaged 316 mL. There were no deaths, pulmonary embolism, deep vein thrombosis, or infected implants. Four patients had complications following surgery for an overall complication rate of 13%. Two patients developed an infected Gore-Tex suture. Two of these complications were treated with revision surgery. Five patients required reoperation for an overall reoperative rate of 16% (one patient was converted to a full mastopexy). Conclusions As a result of this retrospective study, we have found the interlocking approach to periareolar augmentation/mastopexy to be a safe and reliable operation.

A 10-YEAR RETROSPECTIVE CLINICAL STUDY OF $BR{\AA}NEMARK$ IMPLANTS ($Br{\aa}nemark$ 임플랜트의 10년 후향적 임상연구)

  • Statement of problems: There are few studies which reported the survival rates of the specific dental implant systems in the Korean population with the follow-up periods longer than 5 years. Purpose: This retrospective clinical study was aimed to evaluate cumulative survival rate (CSR) of $Br{\aa}nemark$ implants followed for 10 years and to determine risk factors for implant failure. Material and methods: A total of 271 $Br{\aa}nemark$ implants in 83 patients were investigated with several identified risk factors. Life table analysis was undertaken to examine the CSR. Cox regression method was conducted to assess the association between potential risk factors and overall CSR. Results: Thirty implants failed. The 10-year implant CSR was 82.5%. Cox regression analysis demonstrated a significant predictive association between overall CSR and implant length (P<.05). Conclusion: An acceptable long-term result of $Br{\aa}nemark$ implant was achieved and implant length showed a significant association with the CSR.

The Retrospective Review of 410 Shoulder Pain Inpatients in Korean Medicine Hospital (어깨 통증으로 한방병원에 입원한 환자 410명을 대상으로 한 후향적 연구)

  • Objectives The purpose of this study is to investigate the characteristics of inpatients who were admitted to Korean medicine hospital due to shoulder pain. Methods Characteristics of four hundred ten shoulder pain inpatients of single institution from 2011 to 2013 were retrospectively reviewed through medical charts. Results Patients showed different characteristics in sex, age, motives, interval between onset and admission day, first occurrence or not, related medical history before admission, usage of west medication, admission days, treatment results, radiological examinations by shoulder pain disease groups. Accordingly, Korean medical treatment such as acupuncture, moxibustion, external treatment, cupping therapy, physiotherapy, herb-medication varied with the characteristics of shoulder pain disease groups. Conclusions Through retrospective medical charts review, characteristics of 410 single institution shoulder pain inpatients showed distinctive features and versatile Korean medicine treatments by disease groups. It is expected that this study would accelerate multi-institutional and large scale characteristic review of shoulder pain patients, which would raise reconsideration and expand boundary of Korean medicine.

Diagnosis and Treatment of odontogenic cutaneous sinus tract : a retrospective study (치성 피부 누공 환자의 진단과 치료 : 후향적 연구)

  • The odontogenic cutaneous fistula in facial area is uncommon but, well defined disease. It is difficult to diagnose from the dental origin of cutaneous sinus tract. Most patients may visit to the dermatologists or general hospital without cause of disease. They usually be treated by repeated surgical excisions, biopsies, and antibiotic medications, but suffered from recurrences. We studied odontogenic cutaneous fistula through retrospective study in Jeju Special Self-Governing Province between 1 January 2009 and 12 December 2015. There were 3 males, 5 females from 14 to 78 years old with an average age of 50.4 years old. Only 2 patients felt the toothache, others didn't detect it. They suffered from recurrences and repeated treatments for 3 to 11 months with an average period of 7.9 months. They visited average 2.8 hospitals before a precise diagnosis in a dental clinic. All cases were fully healed endodontic treatment or extraction of origin teeth without recurrences. In conclusion, the cause of cutaneous fistula in facial area can be odontogenic. If dentists or doctors diagnose a patient with cutaneous fistula on face, they should check dental problems or take x-ray views for precise diagnosis. It could be helpful for differential diagnosis.
