Relationship between Speech Perception in Noise and Phonemic Restoration of Speech in Noise in Individuals with Normal Hearing

  • Background and Objectives: Top-down restoration of distorted speech, tapped as phonemic restoration of speech in noise, maybe a useful tool to understand robustness of perception in adverse listening situations. However, the relationship between phonemic restoration and speech perception in noise is not empirically clear. Subjects and Methods: 20 adults (40-55 years) with normal audiometric findings were part of the study. Sentence perception in noise performance was studied with various signal-to-noise ratios (SNRs) to estimate the SNR with 50% score. Performance was also measured for sentences interrupted with silence and for those interrupted by speech noise at -10, -5, 0, and 5 dB SNRs. The performance score in the noise interruption condition was subtracted by quiet interruption condition to determine the phonemic restoration magnitude. Results: Fairly robust improvements in speech intelligibility was found when the sentences were interrupted with speech noise instead of silence. Improvement with increasing noise levels was non-monotonic and reached a maximum at -10 dB SNR. Significant correlation between speech perception in noise performance and phonemic restoration of sentences interrupted with -10 dB SNR speech noise was found. Conclusions: It is possible that perception of speech in noise is associated with top-down processing of speech, tapped as phonemic restoration of interrupted speech. More research with a larger sample size is indicated since the restoration is affected by the type of speech material and noise used, age, working memory, and linguistic proficiency, and has a large individual variability.

  • Composite resin have very important roles as a esthetic fillng material. Today we are confusing by a numerous number of the polymer options in the market. Are there any complications such as tooth sensitivity? Do they have stabilization in the long term aspect? These are the reasons why many dentists don't use composite resin in my country. But the problems can be overcome by choosing the best suited materials and meticulous clinical procedures. So that we are able to expand our clinical boundary. I would like to introduce the clinical techniques of Dr.Gordon Christensen in Utah, and overall clinical cases under the base of CRA newsletter.

다양한 이종치아 골이식재를 이용한 두개골 결손부 수복: 동물 연구 (Restoration of Calvarial Defect Using a Variety of Xenogenous Tooth Bone Graft Material: Animal Study)

  • Purpose: The purpose of this study was to evaluate the histological healing process of 3 different types of xenogenic tooth bone graft material and xenogenic bone graft material. Methods: Three types of human tooth bone graft material (chips, crowns, and roots) and BioOss (Geistlich Pharma AG, Wolhausen, Switzerland) was filled at the preformed 4 round-shaped calvarial bone defects of beagle dogs. The beagles were sacrificed at 2, 4, 8, and 12 weeks, respectively, for radiological and histological evaluation. Results: Increased strength and radiopacity were detected in all graft material groups in time-dependent manner. New bone was formed and matured surrounding the graft material histologically. Also, a new bone was directly integrated with graft material. Conclusion: It was expected that newly developed tooth bone graft material would show good bone healing capacity if it was used as a graft material for the restoration of bony defect.

어린이 제 1 대구치 복합 레진 수복물의 5년 후 임상평가 (5 YEARS EVALUATION OF COMPOSITE RESIN RESTORATION ON PERMANENT FIRST MOLAR IN CHILDREN)

  • 치과용 복합레진(Resin-based composite material)의 임상적 결과는 증례의 선택, 기술적인 완성도, 술자의 숙련도등에 따라 다양하게 나타날 수 있다. 특히, 어린이의 제 1 대구치 복합레진 수복은 치아의 맹출 정도와 환아의 협조도 등에 따라 성인의 경우에 비하여 임상적으로 많은 제약을 가지게 된다. 이에 본 연구는 어린이에 시행된 복합 레진 수복물에 대한 5년 후의 평가를 목적으로 시행되었다. 2001년 1월 1일부터 1년 간 부산대학교병원 소아치과에 내원하여 제 1 대구치 복합레진 수복을 받은 16명 환자의 35개 치아에 대하여 modified USPHS criteria에 근거하여 정밀 평가를 시행한 결과, 다음과 같은 결론을 얻었다. 1. 35개 치아 중 6개(17.1%)는 재수복되어 복합 레진 수복물의 5년 후 생존율은 82.9%로 나타났다. 2. 평가기준별 관찰 결과, 색조의 조화, 마모도, 표면 조도, 민감성 혹은 불편감의 측면에서 이상적인 상태인 A등급은 각각 86.2%, 93.1%, 86.2%, 86.2%로 나타났고, 임상적으로 수용가능한 수준인 B등급은 각각 13.8%, 0%, 13.8%, 10.3%를 보였다. 변연적합성과 변연부 변색의 측면에서 A등급은 각각 13.8%, 44.8%를 보였고 B등급은 79.3%, 34.5%로 나타났으며, 이차 우식의 발생율은 20.7%를 나타내었다. 3. 69.1%인 20개 치아는 7개 항목 모두에서 임상적으로 수용 가능한 것으로 나타났다.

부여 부소산사지 출토 치미의 재 복원을 통한 제작기법 (Restoration of the Chimi Excavated from the Busosan Temple Site in Buyeo and Study of Its Production Techniques)

  • 부여 부소산사지 출토 치미는 지난 1978년 부여박물관에서 복원하였지만, 오랜 시간이 지남에 따라 복원재료인 석고가 열화되면서 치미의 무게를 감당하지 못해 부분적으로 심하게 파손되어 있었다. 이를 보강하기 위해 응급으로 에폭시 수지를 사용하여 몸통 내부와 일부 꼬리 부분을 여러 차례 보강한 흔적이 있었다. 전시 기간에 맞춰 치미를 이동하기 위해 상태 점검을 하던 중 치미의 하부 몸통 및 날개 부분의 안정성이 매우 취약하다고 판단되어 다시 복원하기로 하였다. 재 복원하기 위해 치미를 해체하는 과정에서 당시 도공의 제작 방법을 유추해볼 수 있는 여러 제작흔적이 남아 있어 치미의 제작 방법을 연구할 수 있는 좋은 자료로 판단되어 조사하였다. 본 논문은 재 복원을 위해 해체하는 과정에서 밝혀진 부소산사지 치미의 제작기법을 정리하였다. 3차원 기술을 이용해 치미를 스캔한 후 해체 과정에서 살펴본 치미의 제작기법을 바탕으로 수직적이고 경직된 형태를 자연스러운 형태로 수정하고 복원하여 이전 재료를 교체함으로써 복원제의 내구성을 강화하였다. 모델링한 출력물을 본래 편과 접합하고 소실된 형태를 새롭게 만들어 완형으로 재 복원하였다.

Palatability and Livestock Preferences of Restored Plants in Steppe Restoration Areas, Hulunbuir, Inner Mongolia, China

  • Hulunbuir steppe, one of the four largest steppes in China, has experienced rapidly progressing desertification partly due to overgrazing by livestock. The objective of this study was to investigate the effectiveness of various vegetation recovery methods, including the selection of unpalatable plants less affected by grazing livestock. To determine livestock grazing preferences at Hulunbuir restoration sites, we used DNA barcoding methods to analyze fecal materials of horses and cattle grazing on four restored plants: two trees (Pinus sylvestris L. var. mongolica and Populus canadensis) and two shrubs (Caragana microphylla and Corethrodendron fruticosum). Neither of the two tree species were detected in livestock feces, whereas both shrub species were detected at lo w frequencies. There were no significant differences in compositions of species consumed by horses and cattle except that Asteraceae species were more often consumed by cattle. Our results showed that the four plants used for restoration may be classified as unpalatable or less palatable species in the Hulunbuir restoration area. Our results may help inform restoration strategies implemented in restoration areas, especially regarding negative effects of livestock grazing during the initial stage of restoration in Hulunbuir.

자가 경구개점막 이식을 이용한 하안검 퇴축의 교정: 2례 보고 (Correction of Lower Eyelid Retraction with Autogenous Hard Palate Mucosa: 2 Case Report)

  • Purpose: Grave's disease is an autoimmune disease with chronic and systemic features. It affects the orbital fat and muscle bringing about defect in extrinsic eye motility, diplopia, optic nerve defect and lid retraction. In patients with lagopthalmos and resulting facial deformity, treatment can be done by rectus muscle recession or filling with various material. Autogenous auricular cartilage graft is often used and synthetic material such as synthetic acellular dermis, polyethylene meshs are also used for filling of the depressed area. Nevertheless, autogenous auricular cartilage grafts are difficult to utilize and synthetic materials sometimes result in protrusion or infection. Therefore, hard palate mucosa was considered as an alternative. We report two cases of patients with lower eyelid retraction corrected with autogenous hard palate mucosa. Methods: We performed this operation in two patients of Graves' ophthalmopathy. The capsulopalpebral fascia was incised and elevated through an incision on the conjunctiva. Then, the harvested hard palate mucosa was sutured to the inferior border of the tarsus and covered with the conjunctiva. Results: The lower eyelid retraction was corrected successfully. No hypertrophy or deformation of the transplanted hard palate mucosa was noted 6 months after the surgery. Conclusions: From the results above, we may conclude that the hard palate mucosa serves as an ideal spacer for the curvature and the inner lining in lower lid retraction. Hard palate mucosa is as sturdy as the autogenous cartilage but is much easier to utilize. It can be also used for lid retraction after lower lid aesthetic surgeries or traumas.

Accuracy of various imaging methods for detecting misfit at the tooth-restoration interface in posterior teeth

  • Purpose: The present study aimed to evaluate which of the following imaging methods best assessed misfit at the tooth-restoration interface: (1) bitewing radiographs, both conventional and digital, performed using a photostimulable phosphor plate (PSP) and a charge-coupled device (CCD) system; (2) panoramic radiographs, both conventional and digital; and (3) cone-beam computed tomography (CBCT). Materials and Methods: Forty healthy human molars with class I cavities were selected and divided into 4 groups according to the restoration that was applied: composite resin, composite resin with liner material to simulate misfit, dental amalgam, and dental amalgam with liner material to simulate misfit. Radiography and tomography were performed using the various imaging methods, and the resulting images were analyzed by 2 calibrated radiologists. The true presence or absence of misfit corresponding to an area of radiolucency in regions subjacent to the esthetic and metal restorations was validated with microscopy. The data were analyzed using a receiver operating characteristic (ROC) curve, and the scores were compared using the Cohen kappa coefficient. Results: For bitewing images, the digital systems (CCD and PSP) showed a higher area under the ROC curve (AUROC) for the evaluation of resin restorations, while the conventional images exhibited a larger AUROC for the evaluation of amalgam restorations. Conventional and digital panoramic radiographs did not yield good results for the evaluation of resin and amalgam restorations (P<.05). CBCT images exhibited good results for resin restorations(P>.05), but showed no discriminatory ability for amalgam restorations(P<.05). Conclusion: Bitewing radiographs (conventional or digital) should be the method of choice when assessing dental restoration misfit.

치과용 아말감 충전의 임상적 고찰 -재충천의 이유 및 기간에 대한 조사보고- (A CLINICAL STUDY OF DENTAL AMALGAM RESTORATION -Reasons for replacement and duration of primary restoration-)

  • Ease of manipulation, adequate mechanical properties, long years of experience and economical cost are the factors which have established amalgam as the most widely used material for dental restorations. But amalgam restoration may require replacement because of secondary caries, fracture, "fall-out", dimensional change, tarnish or corrosion etc.. These failures of amalgam restorations seem to arise from failures during operations rather than from the inherent shortcomings of the material itself or of the patient's mismanagement. It is anticipated that notonly number of analgam restoration, but failures will be increase after more extensive utilization of the medical insurance which began in 1977. Then authors think that it would be helpful for the development of better treatment in daily dental practice, to know the duration of amalgam restorations and the reasons for their replacement. The data for this survey was compiled from 2, 856 out-patients of the Department of Dentistry, Ewha Woman's University Hospital from January 1975 to December 1977. 260 cases among 1,718 fillings were studied, of which 205 cases both had a single reason for replacement and recognized the date of the previous filling. The results obtained were as follows; 1. Amalgam fillings were 58. 5 percent of all dental restorative materials. Of these, 15. 13 percent of the amalgam restorations had to be replaced. 2. The first reason for replacement of amalgam restorations was secondary caries (56.10%), the second was fracture (23.80%) and the third was "fall-out" (8.78%). 3. Among those amalgms requiring replacement, 52.2 percent had been in place less than 3 years, 70.7 percent within 5 years and 89.8 percent had been in place less than 10 years. Only 10.2 percent had been in place more than 10 years.

  • Paper cultural heritages usually experienced the chemical and the physical deterioration because of various factors including preservation environment and the property of their materials. General restoration technology for paper cultural heritages is the lining. The application of the lining paper and the lining technology has varied depending on researcher's experiences. Therefore, each lining material and technology must be evaluated to develop a standard guide for preservation and restoration of paper cultural heritages. To date, various analysis techniques using lining paper, in terms of their materials, have been applied to analyses of causes and condition of damages, but it is difficult to develop a standard restoration treatment method of paper cultural heritages because it varies by their material. The Hanf, is made frrr the mulberry, is generally used for the lining in Korea.edt is required that the cr, in terms properties and general properties of Hanf, is used for lining papers, are evaluated to develop a standard guide for their restoration.ed this study, the general properties of several lining papers(Hanf), especially dimensave bility, were evaluated as part of the standardization of lining papers. The result have varied depending on the pulping chemical and the manufacturers. the basis weight, thickness and tensale strength of lining papers is bigger the paper treated by natural ash than the paper treated by sodium carbonate.ed case of formation, folding endurance, and dimensave bility, the paper treated by sodium carbonate showed better properties than those of the paper treated by natural ash.
