The Current State of Food Allergy of Preschool Childcare Facilities in Hanam (하남시 영유아 보육시설의 식품알레르기 현황 조사 - 100인 미만의 어린이 급식소를 중심으로 -)

  • Objectives: The purpose of this study was to investigate how to manage food allergy of pre-school children, focusing on the current status of the food allergy in childcare facilities in Hanam which have less than 100 children. Methods: Targeting 159 preschool childcare facilities, survey was carried out for a month in March, 2015. Recovery rate was 66.7%. 106 surveys out of 159 were available for analysis using SPSS statistical program version 19.0. Results: Among 106 facilities, 58 (54.7%) reported that none of their children had a food allergy and 48 (45.3%) reported one more children had a food allergy. Total number of children having a food allergy was 71. Among them, the occurrences of food allergy in males were significantly more than that of the females (p<0.001). Further, children under 2 years of age had significantly more food allergy than the other ages (p < 0.001). The allergic inducing foods were nuts (23.3%), egg (17.8%), milk and dairy products (16.4%), fish and shellfish (13.7%), instant foods (12.3%), fruits (8.2%), soybean (4.1%), meat (2.7%), and cereals (1.4%) in order, and 6 children out of 71 were allergic to more than 2 food items. The clinical symptoms of the food allergy were a skin reaction (87.9%) and an oropharyngeal & respiratory reaction (12.1%). Majority of childcare facilities (80.3%) didn't serve alternative foods for children with food allergy. Necessity for food allergy education was significantly higher in facilities with food allergy issues than without such issues. Conclusions: The Center for Children's Foodservice Management need to educate workers of childcare facilities and parents about managing food allergy and enforce a plan to provide alternative menu to children with food allergies.

  • Artificial induction of nitrogen deficiency symptoms (leaf chlorosis) with two root respiratory inhibitors(DNP and Na$_2$S) was studied and regarded to be the same characteristic to red discoloration in rice var. Tongil seedling leaves as well as adult ones. Tongil(IR 667) was shown to be more nitrogen sensitive and have more distinctive appearance of the leaf discoloration than Punggwang(a native Japonica-type varity.) Conclusions were drawn from the present data that so-called red discoloration of Tongil under the natural field conditions is brought about either by insufficient nitrogen supply in soils or certain factors which may limit at any time the root absorption of nitrogen (low temperature, toxic gases or substances, poor drainage, around roots, etc.) in soils even with ample supply of it.

Acute Health Effects among Soldiers Involved in the Cleanup Operation Following the Hebei Spirit Oil Spill in Taean, Korea (허베이스피리트호 원유유출의 방제작업에 참여한 군인의 급성건강영향)

  • A collision between a barge and an oil tanker of Hebei Spirit caused the release of an estimated 12,547 tons of light crude oil into the Yellow Sea off the west coast of Taean-gun, Korea on December 7, 2007. This study was aimed to evaluate exposure conditions and acute health effects in soldiers participating in the Hebei Spirit oil spill cleanup activities and the association between these and the nature of the work and use of protection devices. The sample comprised 2,624 soldiers stratified by working area and number of working days. We divided working area into the coast of Taean-gun(highly polluted area) and other areas affected by Hebei Spirit oil spill. And we divided number of working days into 1-7 days, 8-14 days, 15-21 days and above 22 days. Data were obtained via a structured, self-administered questionnaire and included information on working area, type of working, number of working days, use of protective materials, and acute health effects. Acute health effects were classified into 5 groups: neurological, respiratory, dermatologic, ophthalmic and other symptoms. Data analysis was performed using unconditional logistic regression used to compute odds ratios and 95% confidence intervals. On logistic regression analysis, working on the coast of Taean-gun was significantly associated with increased risk of acute health problems: headache, dizziness, nausea, general fatigue, insomnia, flushed face, sore throat, dry throat, runny nose, cough, sputum, skin irritation, sore eyes, injection of conjunctiva, tear, and low back pain. Furthermore soldiers working more than 21 days were significantly associated with increased risk of acute health problems: insomnia, flushed face, sore throat, runny nose, cough, sputum, tear, low back pain and fever. Accordingly, the exposure to the oil and the subsequent cleanup efforts were suggested to inflict acute health problems on soldiers participating in the Hebei Spirit oil spill cleanup activities.

  • Objective : The purpose of this study is to investigite the more effective oriental medical treatment in pediatric diseases and its clinical applicability and chief complaints at oriental pediatrics in an area of busan and kyeungnam. Method : 884 new patients of theirs ages between 0 term 20 years who were treated at the pediatrics in Dong-Eui Oriental Medicine Busan Hospital from 1. 2000 to 29. Feb. 2001 Result : 1. The number of male cases was 514 (58%) and female cases 370 (42%) an so male cases 1.38 more than female. In age distribution, 0-1year: 242 patients(27.4%), 2-6years: 455 patients(51.4%), 7-20years: 187 patients(21.2%). 2. The Chief Complaint according to systemical division was Resparatory diseases (37.4%) for common cold, chronic cought, take a easy cold etc and the Digestive diseases(23.6%) for anorexia, diarrhea, abdominal pain etc and Nervous also Mental(11.8%) diseases for sezure, tic, etc and Allergic diseases(6.2%) for atopic dermititis, allergic rhinitis, etc and Physique diseases(5.4%) for short stature, leg pain, etc and Genaral symptoms(4.5%) for sweat profusely, headache, etc and Urinary diseases(3.3%) for bed wetting, frequently voiding, etc Discussion : 1. The Chief Complaint cannot be found a regional specificity 2. The oriental medicine treatment was still congnated as a way to improve the weakness by patients, also was widely noised about preventive medicine. In particular, Respiratory and Disestive diseases are dependent on the oriental medicine. 3. The new diseases and the new treatment method should be investigated as field of oriental medical treatment.

How the Bidi Tobacco Industry Harms Child-workers: Results From a Walk-through and Quantitative Survey

  • Objective: Bidi is an inexpensive, low-processed, hand-rolled tobacco product. In Bangladesh, especially in Rangpur, tobacco farming and bidi processing are common. In this study, we aimed to evaluate the impact of bidi making on children working in bidi factories. Methods: This qualitative(walk-through survey) and quantitative(questionnaire-based survey and a urine cotinine level quantitation) study was conducted in Rangpur, Bangladesh in 2017. Study population included child-workers aged ≤14 years who were regularly employed as bidi makers. Findings: The results of the walk-through survey revealed hazardous working environments at bidi factories. For the quantitative survey, 171 child-workers were categorized into "less-working"(<5 hours/day) and "more-working"(≥5 hours/day) groups; additionally, "forced-working"(forced into work by parents) and "voluntary-working"(working voluntarily) groups were delineated within the same population. In the logistic regression analysis, odds ratios(ORs) of respiratory symptoms and unusual absence(absent ≥5 days/month) were higher in the "more-working" group than in the "less-working" group(unusual absence: OR, 2.91; 95% confidence interval[CI], 1.43-5.94), and this association became higher in the "forced-working" group with longer working hours(OR, 5.68, 95%CI 2.30-14.00). Conclusions: Cheap, hand-rolled tobacco(bidi) is harmful to the health of bidi-making child-workers. Children in poverty bow to the demands of cheap labor and poverty and jeopardize their health and future prospects by working in bidi factories. Tobacco control policies should consider the social effects of tobacco beyond its biological effects.

Acute Respiratory Failure due to Fatal Acute Copper Sulfate Poisoning : A Case Report (급성 호흡부전으로 사망한 황산구리 중독 1례)

  • Copper sulfate is a copper compound used widely in the chemical and agriculture industries. Most intoxication occurs in developing countries of Southeast Asia particularly India, but rarely occurs in Western countries. The early symptoms of intoxication are nausea, vomiting, diarrhea, and abdominal cramps, and the most distinguishable clue is bluish vomiting. The clinical signs of copper sulfate intoxication can vary according to the amount ingested. A 75-year old man came to our emergency room because he had taken approximately 250 ml copper sulfate per oral. His Glasgow Coma Scale (GCS) score was 14 and vital signs were blood pressure 173/111 mmHg, pulse rate 24 bpm, respiration rate 24 bpm, and body temperature $36.1^{\circ}$ .... Arterial blood gas analysis (ABGa) showed mild hypoxemia and just improved after 2 L/min oxygen supply via nasal cannula. Other laboratory tests and chest CT scan showed no clinical significance. Three hours later, the patient's mental status showed sudden deterioration (GCS 11), and ABGa showed hypercarbia. He was arrested and his spontaneous circulation returned after 8 minutes CPR. However, 22 minutes later, he was arrested again and returned after 3 minutes CPR. The family did not want additional resuscitation, so that he died 5 hours after ED visit. In my knowledge, early deaths are the consequence of shock, while late mortality is related to renal and hepatic failure. However, as this case shows, consideration of early definite airway preservation is reasonable in a case of supposed copper sulfate intoxication, because the patients can show rapid deterioration even when serious clinical manifestation are not presented initially.

Atypical Clinical Course of after Glyphosate Herbicide Intoxication (Glyphosate 음독의 임상양상)

  • Purpose: Although glyphosate-surfactant herbicide intoxication is relatively mild toxic in humans, we encountered an atypical clinical presentation. We performed this study to understand the atypical clinical course and factors associated with severe intoxication after glyphosate ingestion. Methods: We conducted a retrospective study of 61 patients (male 43, mean age $54{\pm}18.8$ years) who were presented with glyphosate ingestion between March 1997 and March 2011. The severe intoxication group was defined as patients with systolic blood pressure less than 90 mmHg, respiratory distress needing intubation, or altered mental state. Results: Of the 61 patients, 22 patients (36.1%) had a severe clinical course, 1 patient (1.6%) had died and 1 patient (1.6%) had moribund discharge. The most common symptoms were nausea with or without vomiting which occurred in 30 patients (49.2%). Twenty-seven patients had metabolic acidosis that was the second most common medical complication. Advanced age, pH, base excess, $HCO_3$, Sat, creatinine, X-ray abnormalities and ECG abnormalities were significant factors. Hemoglobin, platelet, $pO_2$, $pCO_2$, BUN, sodium, potassium and AST levels were not different comparing the laboratory characteristics between the severe and mild intoxication groups. Conclusion: The results of this study showed that severe intoxication occurred in 22 patients (36.1%) after glyphosate intoxication. Advanced age, pH, base excess, $HCO_3$, Sat, creatinine, X-ray abnormality and ECG abnormalities were significant predictive factors for severe intoxication in patients with glyphosate surfactant herbicide poisoning.

  • A 3-month-old intact male, Labrador retriever was presented with the history of coagulopathy and anemia. The results of initial screening tests of the hemostatic system yielded a tentative diagnosis of hemophilia. Activated partial thromboplastin time (APTT) was distinctly prolonged (106 seconds) and prothrombin time (PT) was not detected due to markedly prolonged test time. Whole blood transfusions (20 me l/kg body weight) were carried out prior to assays of coagulation factor. After transfusion, the patient recovered well and hemorrhage ceased. Blood samples were assessed for coagulation factor activity. The patient showed markedly low factor IX coagulation activity (5%, reference range: 7~140%) and was diagnosed with hemophilia B. After recovery, the patient was discharged from the hospital. However, 4 months later the patient was re-hospitalized for recurrence of the initial symptoms. The owner did not want to pursue further treatment and the patient died of respiratory distress two days later.

A Clinical Study on Herbal Medicine Threapy Treating Nasal Polyps (비용종(鼻茸腫)(Nasal polyp) 한방(韓方) 치험예(治驗例)에 관(關)한 임상적(臨床的) 연구(硏究))

  • Background: Nasal polyps are benign mass of nasal mucose, chronic sinusitis are swelling of nasal mucose, so nasal polyps and chronic sinusitis take a place at the same time. Nasal polyps are located lateral wall of nose, middle meatus, between middle nasal concha and inferior nasal concha. Objective: To demonstrate the effect of herbal medicine therapy in the chronic sinusitis and nasal polyps before and after treatment using computed tomography (CT). To compared before and after treatment of otitis media using earscope. Materials and methods: Five patients (4 mail and 1 femail) treated in our hospital between August 1998 and July 1999 were studied. Ages ranged from 9 to 23 years (mean age:13 years). All patients had a underlying family history, and past history(allergy or sinusitis of parents or brothers). Illness period was from 1 to 36 months (mean period:10.5 month). To ascertain the efficacy of treatment, CT were checked out. In the CT, three images were obtained 2cm interval on the coronal and axial plan. We classified treatment period into the type of allergic disease. Gamihyunggyeyungyo-tang was administered mainly. Gamigwaghyangjeungki-san, Gamizwakwi-eum and Gamihyangso-san were administered for through oral route additional symptoms. Results: Treatment period was from 126 to 157 days (mean period:140.7 days). To 3 patients with chronic sinusitis and nasal polyp, treatment period was 140.7 days, to 2 patients with otitis media and chronic sinusitis and nasal polyp, treatment period was 141.5 days. So they had no significance. Not nasal polyp from middle meatus, but also polypoid mucosal change of maxillary sinus is polyps Conclusion: We know that herbal medicine therapy is the effective treatment of pediatric chronic sinusitis and otitis media using earscope and CT. Herbal medicine therapy was effective in increasing immunity, inhibiting allergic reaction and inflammation of respiratory system. we practice ECP(eosinophil cataionic protein) and MAST examination, objectify herbal medicine therapy.

A Study on Comparison between Growth Indices of Weak Children Groups via Analyzing Bone Age and Body Composition (허약아와 성장지표의 상관성에 대한 연구)

  • Objectives The purpose of this study is to investigate the relationship between weak children and growth indices. Methods One hundred twenty-six of the first and the second grade elementary school students who didn't have to develop yet their secondary sexual characteristics answered the questionnaire. Their bone age and body composition were measured. Results 1. According to the Weak Children questionnaire analysis, respiratory problem was one of the biggest problems in the weak children group (35.7%), followed by digestive problems (23.0%), psycho-neurological problems (22.2%), neuromotor problems (9.5%), and urogenital problems (8.7%). 2. From the comparison between growth indices of weak children and that of normal children are as follows: 1) The group of children who had problem in their digestive system had lower growth indices than the normal average children group. The growth indices includes mean height, weight, total body water, protein mass, mineral mass, body fat, skeletal muscle mass, body fat percentage, body mass index (BMI), and basal metabolic rate (BMR). 2) The children who had urogenital weakness had lower mean height, RH (Recent Height), RH - MPH {Recent Height(%) - Mid-Parental Height(%)}, RUS (Radius, Ulna and Short bone) score, weight, protein mass, body fat, BMI, and visceral fat than the normal children group. 3. The results of the multiple comparisons of growth indices and weak children groups are as follows: 1) Digestive weak children were the lowest in total body water, protein mass, mineral mass, skeletal muscle mass, and basal metabolic rate. 2) Urogenital weak children were the lowest in height, RH, RH - MPH, RUS score, and weight. Conclusion These results showed that children's growth is strongly correlated to their own growth problems, especially to those children who have digestive and urogenital problems. Therefore, this may be an effective way for children growth treatment in Korean medicine to treat weak symptoms.
