Effects on the Respiratory Function, Lower Extremity Muscle Activity and Balance for the Wellness of Stroke Patients - Focused on Whole Body Vibration Exercise Combined with Breathing Exercise - (뇌졸중 환자의 웰니스를 위한 호흡기능, 하지근활성도 및 균형에 미치는 효과 - 호흡운동을 결합한 전신진동운동을 중심으로 -)

  • The purpose of study was to compare respiratory function and quadriceps muscle activity in stroke patients by applying inspiratory muscle training combined with whole body vibration. In addition, the purpose of study is to present an exercise method for improving the respiratory function of stroke patients and the function of the lower limb muscles of stroke patients. Totally, 21 patients with Stroke patients were randomly assigned to two groups through clinical sampling. 11 patients who applied whole body vibration combined with respiratory exercise were randomly assigned to Experiment Group I, and 10 patients who applied placebo exercise combined with breathing exercise were randomly assigned to Experiment Group II. And for 5 weeks, 4 days/week, 1 time/day, 4 sets/1 time intervention program was implemented. Before intervention, the respiratory function was measured with a maximum inspiratory pressure meter, the lower extremity muscle activity was measured using the surface EMG, and the balance ability was measured using a bug balance test. And after 5 weeks, the post-test was re-measured and analyzed in the same way as the pre-test. In the comparison of changes within the group of experimental group I, there were significant differences in the activity and balance of the respiratory muscle strength, the biceps femoris, and the anterior tibialis muscle (p<.05). In the comparison of the changes in the experimental group I, there was a significant difference in respiratory strength and balance (p<.05). In the comparison of changes between groups, there was a significant difference in the activity of the biceps femoris and anterior tibialis (p<.01). In the future, research on protocols for respiratory exercise and whole body vibration to improve neuromuscular function is considered to be necessary.

Application of Taesikbub and Lung-doyinbub in 「Dong-Ui-Bo-Gam」 as a Korean Traditional Pulmonary Rehabilitation Exercise (「동의보감」에서 제시된 태식법 및 폐장도인법의 한방호흡재활운동으로의 활용)

  • Objectives: The study is aimed to utilize Taesikbub and Lung-doyinbub described in "Dong-Ui-Bo-Gam" as a Korean Traditional Pulmonary Rehabilitation Exercise. Methods: We preliminary researched pulmonary rehabilitation exercise using domestic and international databases and old literatures of Korean Medicine. Then made a draft of Korean Traditional Pulmonary Rehabilitation Exercise based on "Dong-Ui-Bo-Gam". After organizing expert group for verifying the draft of the exercise, we developed the final Korean Traditional Pulmonary Rehabilitation Exercise. Results: The Korean Traditional Pulmonary Rehabilitation Exercise is composed of 5 actions including breathing method, flexibility exercise, percussion and hitting teeth. This exercise is recommended twice a day for at least 5 days per week under the supervision of specialist. It is expected to improve physical activity, dyspnea, health-related quality of life (HRQoL) as well as psychiatric symptoms by breathing deep and slow and exercising the thorax and upper limb muscles. Conclusion: Taesikbub, Lung-doyinbub in "Dong-Ui-Bo-Gam" may be suggested as a better pulmonary rehabilitation exercise for patients of chronic lung disease.

  • Miller Fisher syndrome is characterized by ophthalmoplegia, ataxia and areflexia and develops after respiratory tract viral infection. Other events are GI tract infection, vaccination, digitalis intoxication, insect bite and delivery. Diagnosis of Miller Fisher syndrome can be made with clinical history taking, cardinal symptoms and normal findings of CT or MRI. We have experienced a case of Miller Fisher syndrome and treated with herbal medicine, eletro-acupuncture at paralytic external ophthalmic muscles. We enforced electro-acupuncture for 10 minutes daily. We used the PG-306 electro-acupuncture products(Suzuki Iryoki Co. Japan) and applied the low consequence wave of 1-8Hz. In 3 months, all the main symptoms disappered and the patient improved in health. Based on this experience, herbal medicine and eletro-acupuncture can be applied to the Miller Fisher syndrome.

  • Dermatomyositis (DM) is an idiopathic inflammatory connective tissue disorder and a systemic autonomic immune disease which shows a progressive muscle weakness and characteristic rash. It is identified by a characteristic rash accompanying, or more often preceding muscle weakness. Pathognomonic skin lesions are a blue-purple discoloration on the upper eyelids with edema (heliotropic rash), a flat red rash on the face and upper trunk, and erythema of the knuckles with a raised violaceous scaly eruption (Gottron's papule). The myopathy represents inflammatory and degenerative changes primarily affecting proximal muscles. DM often involves GI tract and respiratory system with as risk of 15-25% internal malignancy. It's managed with sun protection since muscle weakness as well as a rash could be aggravated by sun exposure. Systemic corticosteroid is an initial therapy and other immunosuppressive agent has been used as alternatives. Facial muscles are unaffected and masticatory muscles are rarely affected in DM. We present trismus close to muscle contracture in a patient with DM. Therefore, it needs continuous mouth-opening exercise to prevent progressive muscle contracture and to ensure normal mouth opening.

General Pharmacology of AS2-006A, A New Wound Healing Agent (창상치료후보물질 AS2-006A의 일반약리연구)

  • The therapeutic effect of AS2-006A, a derivative of asiaticoside, has been studied and is being developed as a new wound-healing agent. In the present study, the general pharmacological effects on 1) central nervous system, 2) autonomic nervous system, 3) respiratory system, 4) gastrointestinal system. 5) cardiovascular system. and 6) urinary system were assessed in experimental animals and in in vitro models. 1. In vivo animal study: External applications of the 1 % gel ointment of AS2-006A to rats at the doses of 200. 600 or 2000 mg/kg body weight showed no observable pharmacological effects. The effects on the central nervous system were assessed by observation of behavior, hexobarbital-induced sleeping time, pentetrazole-induced convulsion assay, body temperature measurements, and observations on spontaneous activity and catalepsy. The gel ointment exhibited no effects on the cardiovascular system (i.e. blood pressure and heart rate), renal physiology (i.e. urine volume and electrolytes excretion) and gas-trointestinal physiology (i.e. intestinal charcoal propulsion and gastric mucosal irritation). 2. In vitro experiments: The effects of AS2-006A on the physiology of smooth and cardiac muscles were assessed. Muscle contractions were isotonically and isometrically measured in organ chambers using a physiograph. Cumulative additions of AS2-006A (10$^{-9}$ -10$^{-5}$ M) induced no changes in the tension of isolated guinea pig ileum and tracheal muscles. AS2-006A only slightly increased contractility of rat atrial and papillary muscles at 10$^{-2}$ M, which was not statistically different from control. These data showed that the gel ointment of AS2-006A could be externally applied as a wound-healing agent with no potential side effects.

Two Cases of Respiratory Failure After Recovery of Cholinergic Crisis in Organophosphate Poisoning: The Intermediate Syndrome (유기인제 중독에서 콜린성 위기 회복후 급성 호흡부전을 보인 Intermediate Syndrome 2예)

  • Respiratory failure is the most serious manifestation and usual cause of death in acute organophosphate poisoning, and is common in acute cholinergic crisis. But the respiratory failure may appear suddenly in a patient who is recovering from the cholinergic crisis, even while receiving conventional therapy. These are case report of 37 years old male and 24 years old female with intermediate syndrome in organophosphate poisoning. The two patients ingested organophosphate(fenthion and mixture of DDVP with chlorpyrifos respectively) incidentally and in a sucide attempt respectively. After apparent recovery from the cholinergic crisis with a conventional therapy but before the expected onset of delayed polyneuropathy, the respiratory failure appeared suddenly with a muscular weakness, affecting predominantly the proximal limb muscles, neck flexors, territories of several motor cranial nerves. The two patients needed mechanical ventilatory support and recovery from the intermediate syndrome was complete in both patients, although one subsequently developed hypoxic encephalopathy. The clinical manifestation and electrophysiologic study support the clinical diagnosis of intermediate syndrome. The syndrome carries a risk of death. because of respiratory paralysis, if not recognized early and treated adequatedly. Prompt endotrachial intubation and mechanical ventilatory support is the cornerstone of treatment of the intermediate syndrome. Therefore, all patient should be observed in a hospital for up to 5 days after poisoning.

Characteristics of Phonatory and Respiratory Control on Pitch, Loudness, Register Change in Untrained and Trained Singers (성악가와 훈련 받지 않은 일반인의 음도, 강도, 성구 변화 시 발성 및 호흡조절 특성)

  • Background and Objectives : Training of breath support and laryngeal muscles control are important components in the development of the singing voice. The purpose of this study is to compare characteristics of respiratory and phonatory control on pitch, loudness, register change with untrained males and trained male singers. Materials and Methods : The 11 untrained males and 11 trained male singers participated. Closed Quotient(CQ), fundamental frequency (fo) and relative volume contribution of the rib cage (in percentage rib cage, % RC) and relative volume contribution of abdomen (in percentage abdomen, % AB) were measured during various pitch, loudness, register tasks using /a/ vowel phonation : Legato, staccato with C3-D3-E3-F3-G3 notes and crescendo and decrescendo with C3 note as well as modal register with C3 and falsetto register with C4 note using an integrated analysis system of Respiration, EGG and Voice. Results : (1) When pitch increased with legato task, loudness also increased in untrained male group but maintained in trained male singers. CQ was also increased both untrained and trained male singers but it was not significantly different ($p>.05$). The abdomen contribution to lung volume were significantly predominant both in inhalation and exhalation in trained males singers ($p<.05$). (2) When pitch increased with staccato task, CQ was not significantly different in untrained but significantly different in trained male singers. The respiratory function of male singers were characterized by significantly predominant abdomen contribution to lung volume in exhalation except for inhalation ($p<.05$) (3) When loudness increased with crescendo, fo was significantly increased with increasing CQ in untrained males but fo was relatively consistent with increasing CQ in trained male singers. The respiratory function of male singers were characterized by significantly predominant abdomen contribution to lung volume in exhalation except for inhalation ($p<.05$). (4) Most male singers were able to change register from modal to falsetto register, but untrained males were not. Thus, CQ was significantly different between modal and falsetto register in trained male singers ($p<.05$). The respiratory function of male singers were characterized by significantly predominant abdomen contribution to lung volume in exhalation except for inhalation ($p<.05$). Conclusion : Male singers were superior to untrained males in coordination of respiratory and phonatory control on pitch, loudness, register change. Implication are offered regarding how the results might be applied to the voice therapy as well as singing training.

Extraperiosteal Paraffine Plombage Thoracoplasty for Pulmonary Tuberculosis (폐결핵에 대한 골막외 파라핀 충전술)

  • Background: From 1973 we performed EPPT for the 3,500 cases of the cavitary tuberculosis who were not responded well to the antimicrobial drugs, and we evaluated the 1,003 cases who could be followed up to more than 3 years postoperatively. Method: The key of the EPPT was that the 5-8 ribs of unilateral chest were denuded at the one stage operation and the paraffine was used as the plomb of filling the dead space out of the collapsed periosteums and intercostal muscles. Result and Conclusion: Number of patients who had unilateral EPPT was 759 and bilateral 244. The late complication were subscapular abscess (12) lump formation (42) and paraffine expectoration (26). All of those complications was cured after removal of the packed paraffine. Sputum conversion rates were 91.6% in the group with cavity less than 3 cm and 41.8% in the group with cavity bigger than 5cm.

  • Corticosteroid has been extensively used for the treatment of many medical diseases caused by immune and inflammatory response. And recently it becomes the first choice of treatment for bronchial asthma in a point of it's anti-inflammatory effects. However, this therapy has been associated with many well-known complications including truncal obesity, diabetes mellitus, excerbation of hypertension, delayed wound healing, easy bruisy, atropy of proximal muscles, psychotic symptoms, and/or osteoporosis. We report a case of patient with bronchial asthma who developed an uncommon side reaction, intractable hiccup persisting longer than 48 hours after treatment with oral corticosteroid.

  • Sparganosis is a zoonosis caused by the migrating larvae of cestode genus Spirometra. We report a rare form of sparganosis that invades the lung. A 44-year-old man presented with newly appeared pulmonary nodules that were found accidentally on regular medical checkup, and on sequential chest CT, which we checked at an interval of every 2 months, revealed that the pulmonary lesion had migrated. The patient had a medical history of having undergone surgical excisions for sparganosis in muscles and in subcutaneous tissues of the lower abdomen, perianus, thigh, right axilla, and scapula area, several times over 7 years. A right middle lobectomy was performed and the lesion was diagnosed as sparganosis based on the characteristic histological findings.
