진행된 위암 환자에게 발생한 범복막염 치료의 완화의학적 접근 - 대한가정의학회 완화의학연구회 세미나를 기초로 하여 - (Panperitonitis with Advanced Gastric Cancer - Based on Seminar of Korean Family Medicine Palliative Medicine Research Group -)

  • 진행된 암에서 발생하는 범복막염은 치료하지 않으면 탈수와 패혈증으로 사망할 것이 예측되고 수술적 치료 또한 높은 사망률과 합병증을 가져오며, 적극적인 수액요법과 비위관삽입, 항생제 치료 등도 아직까지 효과가 불분명하고 오히려 증상의 악화도 가져오는 것으로 알려져 있다. 이에 대한가정의학과 완화의학 연구회 세미나에서는 77세 여자 환자로 진행된 위암과 암종증으로 완화의료를 받던 중 발생한 범복막염을 수술적 치료 대신 통증조절 및 증상 완화 위중의 치료를 하여 범복막염 발생 4일 후 평화롭게 임종을 맞이하였던 증례를 보고하였고, 이 증례를 계기로 암환자에서 발생한 복막염의 치료 및 관리에 대한 문헌 고찰과 토론을 통해 다음과 같은 결론을 제시하고자 한다. 먼저 환자의 여명, 환자의 임상적 상태, 수술적 위험성 등을 고려 한 후 비수술적 완화요법을 선택할 수 있다. 통증조절을 위해서는 비경구용 진통제를 사용할 수 있고, 필요한 경우 일시적인 비위 영양관을 삽입할 수 있고 분비물이 적어지면 제거한다. 초기에 충분한 양의 비경구 수액요법이 시도될 수 있으나, 환자의 상태가 호전되지 않으면, 오히려 이로 인한 부종과 호흡곤란 등의 부작용을 최소화하기 위해 최소한의 용량을 사용하는 것을 권장한다. 항생제 사용 및 중단 여부는 환자의 자기의사결정 및 보호자와의 논의 후 임상 상태와 여명을 고려하여 결정할 수 있다.

  • 고전적 갈락토오스혈증(classical galactosemia; OMIM #230400)은 상염색체 열성 유전의 갈락토오스 대사장애로, 9번 염색체에 위치하는 GALT 유전자(OMIM *606999)로부터 전사되는 galactose-1-phosphate uridylyltransferase (GALT; E.C.2.7.7.12)의 심각한 결손으로 인해 유발되는 질환이다. GALT의 결함은 galactose-1-phosphate의 체내 축적을 일으켜, 신생아 시기부터 구토, 수유 곤란, 황달, 복수, 경련 발작, 기면 상태 등의 증상을 유발하고, 장기적으로는 백내장, 성장지연, 지능저하를 초래한다. 반면 Duarte형 갈락토오스혈증은 적혈구에서의 GALT 효소 활성도가 감소되어 있어 혈중 galactose와 galactose-1-phosphate의 농도가 증가하지만 임상적으로는 거의 증상을 보이지 않는 아형이다. 최근 신생아 선별검사의 발달과 함께, 무증상의 양성 판정 환아들이 늘고 있으며, 이들 환아들이 Duarte형 갈락토오스혈증일 가능성이 제기되고 있다. 이에 저자들은 국내에서 그 동안 드물게 보고되었던 N314D와 -119_-116delGT CA/E363K이형접합체의 Duarte형 갈락토오스혈증 일란성 쌍둥이 1례를 보고하고자 한다.

Imidacloprid와 그 유도체들의 살충활성에 미치는 3N-치환기(R)의 영향 (Influence of 3-N-Substituents(R) on the Insecticidal Activities of Imidacloprid and Its Analogous)

  • Imidacloprid와 일련의 새로운 1-(6-chloro-3-pyridylmethy)-3-N-nitroimidazolidin-2-ylideneamine 유도체들을 합성하고 imidazolidine고리의 3-N-치환기(R) 변화에 따른 구조와 벼멸구(Nilaparvata lugens) 및 복숭아 혹 진딧물(Myzus persicae)에 대한 선택적인 살충활성 관계(SAR)를 검토한 바, 벼멸구에는 STERIMOL 파라미터인 B1상수$({\AA})$보다 소수성$({\pi})$, 그리고 복숭아 혹 진딧물에는 입체상수(Es)보다 극성 치환기상수$({\sigma}^{ast})$ 큰 변수로 작용하였다. 벼멸구의 경우에는 ${\pi}$상수의 적정값이 0.52이고 복숭아 혹 진딧물의 경우에는 $\sigma^{\ast}$상수의 적정값이 1.17로 이들 값에 가까운 값을 갖는 치환체일수록 큰 살충활성이 예상되었다. 두 경우, 모두 공통적으로 methyl 및 benzenesulfonyl-치환체(7 및 8)와 같은 센 전자끌게들은 살충활성에 기여하지 않았다. 그러나, 비치환(H)체(1)인 imidacloprid는 가장 큰 살충활성을 나타내었으며 H의 ${\pi}$상수와 ${\sigma}^{\ast}$상수가 이들 두 상수의 적정값에 미치지 못하지만, 양호한 살충활성을 나타낸 까닭은 2-N-nitro group의 산소원자와 3-imid group의 수소원자 사이에 형성된 분자내 회합(H-결합)에 기인함을 알았다. 그리고 pH 6.0 이하의 산성 수용액에서는 대단히 큰 잔류성을 나타내었으며 $45^{\circ}C$의 pH 7.0에서$(K_{obs};5{\times}10^8sec.^{-1})$ 반감기(t1/2)는 대략 6개월이었다.

쌀의 도정도에 따른 중금속 함량 변화 (Investigation of Heavy Metal Contents by Milling Degrees of Rice)

  • BACKGROUND: Recently, various rice by milling degree is sold for health and taste. To provide safe food to consumers, it is need to know the change of heavy metal contents according to milling degree of rice.METHODS AND RESULTS: This study was to investigate residual the levels of cadmium (Cd), lead (Pb), copper (Cu) and zinc (Zn) as stated in the milling degree of the rice contaminated Cd and Pb from 2011 to 2012 in Chungcheongnam-do. Rice samples exceeded the maximum residue limits (MRL) of Cd and Pb were milled by five degrees (0.0, 2.45, 8.02, 10.48, 15.09%). Milled rice was digested by microwave method, and analyzed heavy metal contents using ICP-OES. Recovery ratios of 4 heavy metals such as Cd, Pb, Cu and Zn were ranged for 79.7-98.9%, and limits of detection (LOD) and limits of quantitation (LOQ) were fulfilled with the normal analytical standards. Concentrations of Cd, Pb, Cu and Zn were ranged 0.416-0.433 mg/kg, 0.183-0.26 mg/kg, 3.639- 3.882 mg/kg and 16.868-19.801 mg/kg, respectively.CONCLUSION: From these results, conforming with increase of milling degree of rice, Cd, Pb, Cu, and Zn contents tended to decrease. The contents of heavy metals were decreased 3.1% in Cd, 29.3% in Pb, 6.4% in Cu and 15.1% in Zn, in according to the highest milling degree of 15.09%.

Heterologous Expression of Yeast Prepro-$\alpha$-factor in Rat $GH_3$ Cells

  • Yeast pheromone a-factor is a 13-amino acid peptide hormone that is synthesized as a part of a larger precursor, prepro-$\alpha$-factor, consisting of a signal peptide and a proregion of 64 amino acids. The carboxy-terminal half of the precursor contains four tandem copies of mature $\alpha$-factor. To investigate the molecular basis of intracellular sorting, proteolytic processing, and storage of the peptide hormone, yeast prepro-$\alpha$-factor precursors were heterologously expressed in rat pituitary $GH_3 cells. When cells harboring the precursor were metabolically labeled, a species of approximately 27 kD appeared inside the cells. Digestion with peptide: N-glycosidase F (PNG-F) shifted the molecular mass to a 19 kD, suggesting that the 27 kD protein was the glycosylated form as in yeast cells. The nascent polypeptide is efficiently targeted to the ER in the $GH_3 cells, where it undergoes cleavage of its signal peptide and core glycosylation to generate glycosylated pro-a-factor. To look at the post ER intracellular processing, the pulse-labelled cells were chased up to 2 hrs. The nascent propeptides disappeared from the cells at a half life of 30 min and only 10-25% of the newly synthesized, unprocessed precursors were stored intracellularly after the 2 h chase. However, about 20% of the pulse-labeled pro-$\alpha$-factor precursors were secreted into the medium in the pro-hormone form. With increasing chase time, the intracellular level of propeptide decreased, but the amount of secreted propeptide could not account for the disappearance of intracellular propeptide completely. This disappearance was insensitive to lysosomotropic agents, but was inhibited at $16^{circ}C or 20^{\circ}C$, suggesting that the turnover of the precursors was not occurring in the secretory pathway to trans Golgi network (TGN) or dependent on acidic compartments. From these results, it is concluded that a pan of these heterologous precursors may be processed at its paired dibasic sites by prohormone processing enzymes located in TGN/secretpry vesicles producing small peptides, and that the residual unprocessed precursors may be secreted into the medium rather than degraded intracellularly.

감마선 조사된 유지 model system의 chlorophyII 제거 및 광산화 억제 (Inhibition of Photooxidation by Breakdown of ChlorophyII in Oil Model System Using Gamma Irradiation)

  • 식품내 존재하는 광감체를 제거함으로써 광산화를 억제하는 방법의 일환으로 방사선 조사시 광감체인 chlorophyII의 제거정도 및 최적 조사선량을 탐색하고 모델시스템으로서 linoleic acid 용액에 chlorophyII을 첨가하고 방사선 조사기술을 이용하여 기질의 산화없이 광감체를 제거하고자 chlorophyII b의 함량, 색도 및 과산화물가를 측정하였다. 방사선 조사선량이 증가할수록 chlorophyII b의 함량이 감소하였으며 2.517kGy이상의 선량에선 chlorophyII b 가 방사선 조사에 의해 완전히 파괴되었다. Linoleic acid 용액의 제조 직후 chlorophyII b의 함량은 2.88-2.91 ppm이었으나 20kGy의 방사선을 조사한 경우 chlorophyII b는 전혀 검출되지 않았으며 광조사에 의한 chlorophyII b의 함량은 방사선 비조사구의 경우 chlorophyII b의 함량이 광조사 시간이 경과함에 따라 약간씩 감소하였으나 방사선 조사구는 광조사 후에도 전혀 검출되지 않았다. 방사선을 조사한 후 광조사 시키는 동안 색도의 변화는 L값과 b값의 경우 비조사구에 비해 방사선 조사에 의하여 증가하였고 a값은 감소하였다. 과산화물가는 제조직후방사선 조사에 의하여 산화가 일어났으나 방사선 조사시 $N_2$ gas bubbling 처리한 경우에는 비조사구와 마찬가지로 전혀 산화가 일어나지 않았다. 또한 방사선 조사후 광조사 시킨 경우 chlorophyII b의 제거로 광산화는 일어나지 않았다.

요실금환자의 골반근육운동에 따른 자아존중감과 배뇨증상에 관한 연구 (A Study on Self-Esteem and Urinary Symptoms in Women with Urinary Incontinence)

  • Purpose: The purpose of this study was to help enhance the quality of life for women with urinary incontinence. Self-esteem, urinary symptoms, peak pressure and duration in seconds for vaginal contraction after pelvic muscles exercise were examined. Method: One-group pretest-posttest design was employed participants were 27 married women from G city. The instrument for this study were : the self-esteem scale developed by Rosenberg(1965). the Urinary Symptom Questionnaire, a subjective measurement of urinary incontinence, developed by Jackson et al.(1996) and the Perineometer used as an objective measurement of peak pressure and duration in seconds. Result: 1. Self-esteem showed significant improvement after the exercise (t=-3.832, p= .001). 2. Comparison of results before the pelvic muscles exercise and after showed that there was a statistically significant difference for several urinary symptoms including enuresis (t=2.833, p=.009), frequency of incontinence (t=2.964, p= .006), incontinence volume (t=2.280, p= .031), incontinence before getting to the restroom (t=3.035, p= .006), incontinence with no reason or feeling (t= 3.051, p= 005) burning sensation (t= 2.132, p=.043), and a sense of residual urine (t=2.267 p=.032). The mean scores showed improvement in the urinary symptoms of management of incontinence (M=$0.04{\pm}0.22$), daily frequency of incontinence management (M=$0.13{\pm}0.85$), urinary control (M=$0.15{\pm}0.86$), running to the restroom (M=$0.15{\pm}0.60$) incontinence caused by coughing or physical activities (M=$0.19{\pm}0.57$) and hesitancy (M=$0.07{\pm}0.55$). Overall urinary symptoms decreased significantly (t =3.073. p= .007). 3. Peak pressure showed an increase from a mean of $24.26{\pm}16.20mmHg$ before the exercise to a mean of $28.63{\pm} 17.79mmHg$ after (t=-2.399, p=.024). Duration in seconds also increased from a mean of $6.00{\pm}4.95sec$ to $9.15{\pm}5.83sec$ (t=-4.400, p= 000). Conclusion: These findings suggest that pelvic muscle exercise serves to decrease urinary problems, improve self-esteem and increase peak pressure and duration in seconds.

Synthesis of L-threo-3,4-Dihydroxyphenylserine(L-threo-DOPS) with Thermostabilized Low-Specific L-Threonine Aldolase from Streptomyces coelicolor A3(2)

  • Stability-enhanced mutants, H44, 11-94, 5A2-84, and F8, of L-threonine aldolase(L-TA) from Streptomyces coelicolor A3(2)(SCO1085) were isolated by an error-prone PCR followed by a high-throughput screening. Each of these mutant, had a single amino acid substitution: H177Y in the H44 mutant, A169T in the 11-94 mutant, D104N in the 5A2-84 mutant and F18I in the F8 mutant. The residual L-TA activity of the wild-type L-TA after a heat treatment for 20 min at $60^{\circ}C$ was only 10.6%. However, those in the stability-enhanced mutants were 85.7% for the H44 mutant, 58.6% for the F8 mutant, 62.1% for the 5A2-84 mutant, and 67.6% for the 11-94 mutant. Although the half-life of the wild-type L-TA at $63^{\circ}C$ was 1.3 min, those of the mutant L-TAs were longer: 14.6 min for the H44 mutant, 3.7 min for the 11-94 mutant, 5.8 min for the 5A2-84 mutant, and 5.0 min for the F8 mutant. The specific activity did not change in most of the mutants, but it was decreased by 45% in the case of mutant F8. When the aldol condensation of glycine and 3,4-dihydroxybenzaldehyde was studied by using whole cells of Escherichia coli containing the wild-type L-TA gene, L-threo-3,4-dihydroxyphenylserine(L-threo-DOPS) was successfully synthesized with a yield of 2.0 mg/ml after 20 repeated batch reactions for 100 h. However, the L-threo-DOPS synthesizing activity of the enzyme decreased with increased cycles of the batch reactions. Compared with the wild-type L-TA, H44 L-TA kept its L-threo-DOPS synthesizing activity almost constant during the 20 repeated batch reactions for 100 h, yielding 4.0 mg/ml of L-threo-DOPS. This result showed that H44 L-TA is more effective than the wild-type L-TA for the mass production of L-threo-DOPS.

유통 중인 관행 및 인증 농산물 중 곡류와 엽채류의 잔류농약 모니터링 및 안전성 평가 (Monitoring of Pesticide Residues and Risk Assessment for Cereals and Leafy Vegetables of Certificated and General Agricultural Products)

  • 유통되고 있는 관행농산물, 유기농산물, 무농약 농산물 그리고 GAP인증농산물 가운데 곡류(쌀, 보리)와 엽채류(상추, 들깻잎)의 농약잔류실태와 안전성을 평가하기 위하여 12개의 지역의 백화점, 대형마트 및 재래시장에서 농산물을 구입하여 총 259점을 분석하였다. 분석결과 잔류농약의 검출율은 일반 농산물에서 4.6%, 유기농산물에서 1.5%, 무농약농산물에서 0% 그리고 GAP인증농산물에서 0.8%이었다. 보리 시료 63점에서는 잔류농약이 검출되지 않았고, 쌀 시료 69점에서는 isoprothiolane이 3점 검출되어 4.3%의 검출율을 보였다. 상추 시료 64점에서는 cypermethrin이 2건 검출되어 3.1%의 검출율을 보였으며 들깻잎 시료 63점에서는 dichlorvos가 13건 검출되어 20.6%의 검출율을 보였다. 검출된 농약들은 모두 잔류허용기준 미만으로 나타났으며 검출농약이 해당 농산물의 섭취로 인체에 유입될 잔류농약의 1일추정섭취량을 산출한 결과 ADI의 0.0003~0.04302% 수준이었다. 검출된 농약이 MRL 이하로 나타났거나 %ADI가 낮다고 하여 안전하다고 사료되지만 유기농산물인 들깻잎에서 농약이 검출된점 그리고 상추에서 검출된 cypermethrin은 해당작물에 대한 안전사용기준이 설정되어 있지 않기 때문에 문제가 있는 것으로 나타났다.

하우징 형태(Housing Type)로 제작된 배선 연결 커넥터의 안전성 평가에 관한 연구 (A Study on the Safety Estimation of Wiring Connection Connector Manufactured by Housing Type)

  • The purpose of this study is to evaluate the safety of a wire connector fabricated for the effective installation of a lighting fixture including its contact resistance, insulation resistance, withstanding voltage characteristics, etc., and to provide the basis for the analysis and judgment of PL(Product Liability) dispute by presenting a damage pattern due to a general flame and overcurrent. This study applied the Korean Standard (KS) for the incombustibility test of the connector using a general flame and performed an overcurrent characteristics test of the connector using PCITS (Primary Current Injection Test System). The contact resistance of the housing connector was measured using a high resistance meter and the insulation resistance was measured using a multimeter. In addition, a supply voltage of AC 1,500V for testing the withstanding voltage characteristics was applied to both ends of the connector. Measurement was performed on 5 specimens and the measured values were used as a basis for judgment. Since the connector is fabricated in the form of a housing, it can be connected and separated easily and has a structure that allows no foreign material to enter. In addition, since it has a structure that allows wires to be connected only when their polarity is identical, any misconnection that may occur during installation can be prevented. When the incombustibility test was performed by applying a general flame to the connector, it showed outstanding incombustibility characteristics and the blade and blade holder connected to the housing remained firmly secured even after the insulation sheath (PVC) was completely destroyed by fire. In addition, the mechanism of the damaged connecting wire showed a comparatively uniform carbonization pattern and it was found that some residual melted insulation material was attached to both ends. In the accelerated life test (ALT) to which approximately 500% of the rated current was applied, the connector damage proceeded in the order of white smoke generation, wire separation, spark occurrence and carbonization. That is, it could be seen that the connector damaged by overcurrent lost its own metallic color with traces of discoloration and carbonization. The contact resistance of the connector at a normal state was 2.164mV/A on average. The contact resistance measured after the high temperature test was 3.258mV/A. In addition, the insulation resistance after the temperature test was completed was greater than $10G\Omega$ and the withstanding voltage test result showed that no insulation breakdown occurred to all specimens showing stable withstanding voltage and insulation resistance characteristics.
